    Pregnancy complications in women with polycystic ovary syndrome

    Background: The great majority of studies performed so far concerning women diagnosed with polycystic ovary syndrome (PCOS) have focused on diagnosis, menstrual cycle abnormalities, hirsutism and infertility. Although progress has been made in developing methods for achieving a pregnancy and reducing multiple gestations in women with PCOS, little attention has been paid to pregnancy complications and subsequent child outcomes. This review aims to summarize current knowledge regarding the clinical and pathophysiological features of pregnancy and children in women with PCOS. Methods: A literature search up to April 2015 was performed in PubMed, Medline, the Cochrane Library andWeb of Science without language restriction. All articles were initially screened for title and abstract and full texts of eligible articles were subsequently selected. Systematic reviewswith meta-analysis were initially included for each specific subject. Recent randomised controlled trials (RCTs), which were not included in the systematic reviews, were also included. In addition to evidence from meta-analyses or RCTs, we used non-randomized prospective, uncontrolled prospective, retrospective andexperimental studies.Whenspecific data for patientswithPCOSwere lacking, results fromgeneral populationstudieswere reported. Results: Women withPCOSexhibit a clinically significant increased risk of pregnancycomplicationscompared with controls.Data whichwere not adjusted for BMI or otherconfounders demonstrated inPCOSa 3-4-fold increased risk of pregnancy-induced hypertension and pre-eclampsia, a 3-fold increased risk of gestational diabetes and 2-fold higher chance for premature delivery. Features characteristic of PCOS, such as hyperandrogenism, obesity, insulin resistance and metabolic abnormalities, may contribute to the increased risk of obstetric and neonatal complications. Limited available data suggest that offspring ofwomen with PCOS have an increased risk for futuremetabolic and reproductive dysfunction. Underlying pathophysiological mechanisms of pregnancy complications along with its association with health of offspring remain uncertain. To date, the strategies for prevention and management of pregnancy complications in women with PCOS, and whether long-term health of these women is influenced, and to what extent, by pregnancy and/or pregnancy complications, remain to be elucidated. Conclusions: Women with PCOS show an increased risk of pregnancy complications. Heterogeneous aetiological factors involved in PCOS and associated co-morbidities may all be involved in compromised pregnancy and child outcomes. In women with PCOS, a possible relationship with genetic, environmental, clinical and biochemical factors involved in this complex condition, as well as with pregnancy complications and long-term health for both mother and child, remains to be established

    Sir, We thank Dr Galazis for his interest in our review (Palomba et al., 2015) and for the issues raised about the increased risk of pregnancy complications in women with polycystic ovary syndrome (PCOS). Our comprehensive review demonstrated that women with PCOS have a 2-fold increased risk of gestational diabetes mellitus (GDM) and a 3–4-fold increased risk of pregnancy-induced hypertension (PIH) and preeclampsia (PE) when compared with non-PCOS controls (Palomba et al., 2015)

    The modernist angel: Art at the Limits of the Human in D. H. Lawrence, H. D. and Mina Loy

    PhDThe subject of this thesis is a figure that might provisionally be called the *modemist angel'. Focusing on modernist literature, and more particularly on the work of D. H. Lawrence, H. D. and Mina Loy, it aims to isolate from the many angels found in all periods and all types of art a historically specific and intellectually coherent paradigm: an angel of and for its modernist times. A figure of precisely this type could be said to exist in the form of Walter Benjamin's 'angel of history'. Critics who address the question of the modern angel in texts by Franz Kafka and Rainer Maria Rilke often do so in conjunction with the problem posed by the angel of history. Beginning with a chapter on Benjamin, this thesis nevertheless follows a different trajectory. Over five chapters, it explores a modernist landscape formed not only by Lawrence, H. D. and Loy, but also by European and American writers such as A. R. Orage, Allen Upward, Ezra Pound, Wallace Stevens, Havelock Ellis, Edward Carpenter, Sigmund Freud and Friedrich Nietzsche. Although the angel that emerges from this investigation might, in some respects, be said to anticipate Benjamin's later version, this figure is also very different, standing for a project that is distinctively, and recognisably, modernist in nature. He/she (the sex of the modernist angel is often open to question) represents an attempt to reconcile the divine responsibilities of the artist with the material and gendered conditions of being, specifically of being human, in the modem world. This thesis looks again at the clash of intellectual paradigms in the early-twentieth century - notably, the confrontation of the Romantic view of art as a superhuman or sacred undertaking with the psychoanalytical or evolutionary idea that all human endeavour is underpinned by sub-human motives - and suggests the angel as a new and instructive figure through which to think the perilous limits between the human and the divine in modernist literature

Programa de Pós-Graduação em Psicologia.Esta pesquisa tem como objetivo verificar o impacto de uma exposição científica nas representações sociais sobre meio ambiente dos alunos do ensino médio. Desde a sua origem, nos anos 60, a teoria das representações sociais analisa a difusão da ciência pelos meios de comunicação, ocupando uma posição central nos estudos da divulgação científica e permeando tanto a organização de meios de divulgação como de conteúdos necessários a sua consecução. Uma dimensão importante e presente nas exposições científicas contemporâneas é a abordagem social da ciência e tecnologia. Nessa dimensão, um tema que tem sido privilegiado pelos museus de ciências e professores diz respeito ao meio ambiente. A construção dessa exposição levou em conta a noção de paradigmas ambientais e foi construída de forma a explicitar o contraste entre o homem como excluído ou como parte integrante do meio ambiente. Participaram dessa pesquisa 285 alunos da segunda série do ensino médio de quatro escolas de Florianópolis. A pesquisa foi feita nas escolas e em uma sala de exposições da UFSC. Pode-se dizer que houve um impacto sobre as representações sociais de meio ambiente dos alunos que visitaram a exposição através do surgimento de esquemas estranhos (referentes ao conteúdo da exposição) nessas representações, apontando um crescimento informativo e cognitivo desses alunos. Espera-se poder contribuir com o nível de alfabetização científica dos cidadãos catarinenses, bem como verificar a contribuição da atividade de divulgação científica para a teoria das representações sociais, além de contribuir para o desenvolvimento do campo de pesquisa dessa teoria

    Informe al Virrey Antonio María de Bucareli y Ursúa sobre el viaje y la llegada a la bahía de San Carlos de Monterey de una fragata real enviada para nuevas exploraciones. El Padre Crespí y el Padre Peña viajaron en el barco y llegaron sanos y salvos a la misión al día siguiente. El autor señaló las discrepancias entre los relatos de los soldados y los religiosos sobre cómo transcurrió la expedición, por lo que está presentando los diarios de los religiosos al virrey como relatos fieles del viaje. El informe menciona los desafíos enfrentados durante la expedición y el encuentro providencial con los pueblos indígenas, que llegaron en numerosas canoas, desmintiendo las concepciones erróneas anteriores sobre la inhabitabilidad del área. —— Report to Viceroy Antonio Maria de Bucareli y Ursua on the voyage and arrival in the San Carlos de Monterey port of a royal frigate sent for new explorations.     The effectiveness of interventions to treat severe acute malnutrition in young children: a systematic review

    Severe acute malnutrition (SAM) arises as a consequence of a sudden period of food shortage and is associated with loss of a person’s body fat and wasting of their skeletal muscle. Many of those affected are already undernourished and are often susceptible to disease. Infants and young children are the most vulnerable as they require extra nutrition for growth and development, have comparatively limited energy reserves and depend on others. Undernutrition can have drastic and wide-ranging consequences for the child’s development and survival in the short and long term. Despite efforts made to treat SAM through different interventions and programmes, it continues to cause unacceptably high levels of mortality and morbidity. Uncertainty remains as to the most effective methods to treat severe acute malnutrition in young children.ObjectivesTo evaluate the effectiveness of interventions to treat infants and children aged &lt; 5 years who have SAM.Data sourcesEight databases (MEDLINE, EMBASE, MEDLINE In-Process &amp; Other Non-Indexed Citations, CAB Abstracts Ovid, Bioline, Centre for Reviews and Dissemination, EconLit EBSCO and The Cochrane Library) were searched to 2010. Bibliographies of included articles and grey literature sources were also searched. The project expert advisory group was asked to identify additional published and unpublished references.Review methodsPrior to the systematic review, a Delphi process involving international experts prioritised the research questions. Searches were conducted and two reviewers independently screened titles and abstracts for eligibility. Inclusion criteria were applied to the full texts of retrieved papers by one reviewer and checked independently by a second. Included studies were mapped to the research questions. Data extraction and quality assessment were undertaken by one reviewer and checked by a second reviewer. Differences in opinion were resolved through discussion at each stage. Studies were synthesised through a narrative review with tabulation of the results.ResultsA total of 8954 records were screened, 224 full-text articles were retrieved, and 74 articles (describing 68 studies) met the inclusion criteria and were mapped. No evidence focused on treatment of children with SAM who were human immunodeficiency virus sero-positive, and no good-quality or adequately reported studies assessed treatments for SAM among infants &lt; 6 months old. One randomised controlled trial investigated fluid resuscitation solutions for shock, with none adequately treating shock. Children with acute diarrhoea benefited from the use of hypo-osmolar oral rehydration solution (H-ORS) compared with the standard World Health Organization-oral rehydration solution (WHO-ORS). WHO-ORS was not significantly different from rehydration solution for malnutrition (ReSoMal), but the safety of ReSoMal was uncertain. A rice-based ORS was more beneficial than glucose-based ORSs, and provision of zinc plus a WHO-ORS had a favourable impact on diarrhoea and need for ORS. Comparisons of different diets in children with persistent diarrhoea produced conflicting findings. For treating infection, comparison of amoxicillin with ceftriaxone during inpatient therapy, and routine provision of antibiotics for 7 days versus no antibiotics during outpatient therapy of uncomplicated SAM, found that neither had a significant effect on recovery at the end of follow-up. No evidence mapped to the next three questions on factors that affect sustainability of programmes, long-term survival and readmission rates, the clinical effectiveness of management strategies for treating children with comorbidities such as tuberculosis and Helicobacter pylori infection and the factors that limit the full implementation of treatment programmes. Comparison of treatment for SAM in different settings showed that children receiving inpatient care appear to do as well as those in ambulatory or home settings on anthropometric measures and response time to treatment. Longer-term follow-up showed limited differences between the different settings. The majority of evidence on methods for correcting micronutrient deficiencies considered zinc supplements; however, trials were heterogeneous and a firm conclusion about zinc was not reached. There was limited evidence on either supplementary potassium or nicotinic acid (each produced some benefits), and nucleotides (not associated with benefits). Evidence was identified for four of the five remaining questions, but not assessed because of resource limitation.LimitationsThe systematic review focused on key questions prioritised through a Delphi study and, as a consequence, did not encompass all elements in the management of SAM. In focusing on evidence from controlled studies with the most rigorous designs that were published in the English language, the systematic review may have excluded other forms of evidence. The systematic review identified several limitations in the evidence base for assessing the effectiveness of interventions for treating young children with severe acute malnutrition, including a lack of studies assessing the different interventions; limited details of study methods used; short follow-up post intervention or discharge; and heterogeneity in participants, interventions, settings, and outcome measures affecting generalisability.ConclusionsFor many of the most highly ranked questions evidence was lacking or inconclusive. More research is needed on a range of topic areas concerning the treatment of infants and children with SAM. Further research is required on most aspects of the management of SAM in children &lt; 5 years, including intravenous resuscitation regimens for shock, management of subgroups (e.g. infants &lt; 6 months old, infants and children with SAM who are human immunodeficiency virus sero-positive) and on the use of antibiotics.FundingThe National Institute for Health Research Technology Assessment programme.<br/

    Antoni Malczewski’s Late Debut and the Issue of Underestimating the Poem Maria

    The article discusses problems related to the debut of the outstanding author of Polish Romanticism. The author of the article describes the reasons for Malczewski’s late debut. She draws attention to the artistic, ideological, existential and biographical sources of the poem. She also writes about the circumstances of publishing the poem and the first reviews. Then she moves on to discuss the texts of Maurycy Mochnacki and Adam Mickiewicz, who appreciated the artistry of Maria. The researcher shows that the issue of underestimating Maria is a reception myth.
