Intravenous versus combined (intravenous and intra-arterial) thrombolysis in acute ischemic stroke a transcranial color-coded duplex sonography-guided pilot study
Background and Purpose—Determine feasibility and safety of intravenous (IV) versus combined (IV-IA [intra-arterial])
thrombolysis guided by transcranial color-coded duplex sonography (TCCD).
Methods—Thirty-three patients eligible for IV thrombolysis, within 3 hours of onset of symptoms, with occlusion in
middle cerebral artery territory (TCCD monitoring, thrombolysis in brain ischemia [TIBI] flow grade [0 –3]), underwent
IV thrombolysis (tissue plasminogen activator, 0.9 mg/kg). In case of recanalization (modification of TIBI score 1)
after 30 minutes IV thrombolysis was continued over 1 hour; otherwise, it was discontinued, with subsequent IA
thrombolysis. Recanalization was determined by TIBI (TCCD) and angiographically by thrombolysis in myocardial
infarction (TIMI) flow grades. Clinical outcome measures were assessed at baseline, 24 hours (NIHSS) and 3 months
(modified Rankin Scale).
Results—In the IV group, 10/17 patients (59%) with complete or partial recanalization after 30 minutes had a favorable
outcome at 3 months (modified Rankin Scale 0 to 2). TIBI flow grades 3 to 5 after 30 minutes of IV thrombolysis
predicted a good prognosis compared with TIBI grades 1 to 2 (P0.05). In the combined IV/IA therapy group (no
recanalization after 30 minutes), 9/16 patients (56%) had a favorable outcome at 3 months. One symptomatic
intracerebral hemorrhage occurred in each group.
Conclusions—Combined IV-IA versus IV thrombolysis guided by TCCD was feasible and safe. Recanalization after 30
minutes of IV thrombolysis led to a favorable outcome in 59% of the patients, provided TIBI flow grades were of 3 to
5. In the absence of early recanalization during IV thrombolysis, there was clinical benefit to proceed to IA therapy for
a significative proportion of patients (56%). (Stroke. 2006;37:1805-1809.
The Humanist Reception of St. Basil's Homily In Illud: Attende Tibi Ipsi In the XV–XVI cent
This paper deals with the humanist reception of St. Basil’s homily In illud: attende tibi ipsi up to 1532. In the XV cent., three new Latin translations were made in the circle of cardinal Bessarion: by Bessarion himself, by his protégé Athanasius Chalkeopulos, and by an anonymous author, probably Pietro Balbi. The translation of Franciscus Maturantius was published as a separate edition in 1522, and that of Rafaelle Maff ei appeared in the first Latin Opera of Basil in 1515. A review of these translations and of the dedicatory epistles shows that not only the humanistic program or theological views of Basil were of interest for the humanists. Attende tibi is valued as an example of biblical exegesis and because of its moral and ascetic content. Although, on the whole, reception centers in this period tend to distance from the Church, all our translators, except for one, are associated with the Roman Catholic Church. The comparison of the biblical «give heed to thyself» with the Delphic «know thyself», found in Maturantius’ dedicatory letter and in Maffei’s marginalia, aims at demonstrating the superiority of Christian wisdom, not at promoting the study of philosophy. Only two of the discussed translations were published, and a more or less large-scale dissemination of Basilius Latinus starts no earlier than in the 20s. of XVI cent., when the translation of Maff ei was reissued in Paris (1520 and 1523), Cologne (in 1523 and 1531) and Basel (1523)
Max Weber e il dibattito attuale sulle migrazioni
Max Weber and the Current Debate on Migration. Drawing on Max Weber’s famous Antrittsvorlesung (1895), the author develops an ideal type framework to categorize various perspectives on the migration issue. Based on two contrasting attitudes – the axiological-differential and the universal-relativistic – the author critically examines the positions of Thilo Sarrazin, Bassam Tibi, and Aladdin El-Mafaalani
Islamic marketing : insights from a critical perspective
This paper seeks to encourage a critical dialogue within the realm of Journal of Islamic Marketing. It invites marketing scholars and practitioners working on various topics related to Islam and Muslim societies to adopt fresh theoretical and methodological positions that would enhance our understanding of multiple marketing and market dynamics in Muslim societies. The author suggests that the advancement of knowledge in the area of Islamic marketing requires reflexivity and self-critique. The paper highlights the constructive value of critical approach to the development of marketing theory and practice. This paper reflects the author’s personal viewpoint on the production of knowledge and improving practice in the realm of Islamic marketing

Stationary analysis of the shortest queue problem
International audienceA simple analytical solution is proposed for the stationary loss system of two parallel queues with finite capacity K, in which new customers join the shortest queue, or one of the two with equal probability if their lengths are equal. The arrival process is Poisson, service times at each queue have exponential distributions with the same parameter, and both queues have equal capacity. Using standard generating function arguments, a simple expression for the blocking probability is derived, which as far as we know is original. Using coupling arguments and explicit formulas, comparisons with related loss systems are then provided. Bounds are similarly obtained for the average total number of customers, with the stationary distribution explicitly determined on {K,…,2K}, and elsewhere upper bounded. Furthermore, from the balance equations, all stationary probabilities are obtained as explicit combinations of their values at states (0,k) for 0≤k≤K. These expressions extend to the infinite capacity and asymmetric cases, i.e., when the queues have different service rates. For the initial symmetric finite capacity model, the stationary probabilities of states (0,k) can be obtained recursively from the blocking probability. In the other cases, they are implicitly determined through a functional equation that characterizes their generating function. The whole approach shows that the stationary distribution of the infinite capacity symmetric process is the limit of the corresponding finite capacity distributions. For the infinite capacity symmetric model, we provide an elementary proof of a result by Cohen which gives the solution of the functional equation in terms of an infinite product with explicit zeroes and poles
Real-time validation of transcranial doppler criteria in assessing recanalization during intra-arterial procedures for acute ischemic stroke an international, multicenter study
BACKGROUND AND PURPOSE - : We sought to evaluate the diagnostic accuracy of ultrasound criteria for recanalization during real-time transcranial Doppler monitoring of intra-arterial reperfusion procedures in acute ischemic stroke patients in an international, multicenter study. METHODS - : Consecutive acute ischemic stroke patients with proximal intracranial occlusions underwent intra-arterial reperfusion procedures with simultaneous real-time transcranial Doppler monitoring at 3 tertiary-care stroke centers. Residual flow signals at the site of angiographically confirmed occlusions were monitored at a constant transtemporal insonation angle using a standard head-frame. Recanalization was assessed simultaneously by digital subtraction angiography and ultrasound using thrombolysis in myocardial infarction and thrombolysis in brain ischemia (TIBI) criteria, respectively. Independent readers blinded to digital subtraction angiography performed validation of TIBI flow grades. The interrater reliability for assessment of TIBI grades was investigated. RESULTS - : We evaluated time-linked real-time digital subtraction angiography transcranial Doppler images from 96 diagnostic digital subtraction angiography runs during intra-arterial reperfusion procedures in 62 acute ischemic stroke patients (mean age, 59±17 years; 58% men; median baseline National Institutes of Health Stroke Scale score, 18 [interquartile range 12-21]; median time from symptom onset to intra-arterial procedure initiation, 240 minutes [interquartile range 163-308]). The interrater reliability for evaluation of TIBI grades and assessment of recanalization was good (Cohen κ: 0.838 and 0.874, respectively; P<0.001). Compared with angiography, transcranial Doppler had the following accuracy parameters for detection of complete recanalization (TIBI 4 and 5 versus thrombolysis in myocardial infarction 3, flow grades): sensitivity, 88% (95% confidence interval, 72%-96%); specificity, 89% (79%-95%); positive predictive value, 81% (65%-91%); negative predictive value, 93% (84%-98%); and overall accuracy 89% (80%-94%). CONCLUSIONS - : At laboratories with high-interrater reliability, TIBI criteria can accurately predict brain recanalization in real time as compared with thrombolysis in myocardial infarction angiographic scores. © 2013 American Heart Association, Inc
