Sofosbuvir–daclatasvir improves hepatitis C virus–induced mixed cryoglobulinemia: Upper Egypt experience

The present study aimed to evaluate the effect of sofosbuvir–­daclatasvir therapy on symptomatic HCV-related MC and sustained virologic response (SVR) achievement. Patients and methods: This prospective cohort study was carried out on 120 patients with chronic HCV infection, clinically suspected to have MC, but only 63 of whom were positive for cryoglobulins. HCV-MC patients were treated with sofosbuvir 400 mg and daclatasvir 60 mg once daily for 3 months. The serum cryoglobulins levels, complement 3 (C3), complement 4 (C4) (using ELISA assay kits) and rheumatoid factor (RF) (using immunoturbidimetric assay kit), were measured in the included HCV infected patients (to confirm HCV-MC diagnosis), in addition to quantitave HCV-RNA assays, using real time PCR. All these measurements have been done before stating therapy and 12, 24 weeks post-therapy for assessments of immunological recovery, viral load and SVR. Results: Significant increase in the serum cryoglobulin levels and RF with significant decrease in C3 and C4 serum levels were detected in only 63 out of 120 included HCV infected patients, upon whom the study has been completed. They showed significant decrease in their mean cryoglobulin levels from 41.47 µg/mL ±12.32 SD to 5.12 µg/mL ±3.59 SD then to 5.09 µg/mL ±3.02 SD, 12 to 24 weeks post-therapy respectively (p<0.001), with significant decline in RF concentrations and rise in C3 and C4 serum levels approaching the normal values. There were improvements in the presenting HCV-MC clinical manifestations in variable degrees, ranging from 5 (71.42%) in patients with glomerulonephritis to 62 (98.4%) in patients with purpura. Eighty-seven percent of the included patients showed complete response (clinical, virological and immunological recovery) and 13% showed partial response (virological and immunological recovery without clinical improvement of cryoglobulinemia associated manifestations). Conclusion: A combined therapy of sofosbuvir 400 mg and daclatasvir 60 mg once daily for 3 months was associated with a significant decrease in serum cryoglobulin levels and appears as a reasonable treatment option for HCV-associated MC. International Seminar on ‘Islam in Myanmar’
A day-long international Seminar on Islam in Myanmar on the theme “Democratization in Myanmar: Opportunities and Challenges for Its Muslim Community” was held Conference Hall, IAIS Malaysia on April 29, 2012. International Institute of Advanced Islamic Studies (IAIS) Malaysia and Myanmar Muslim Intellectual Forum (MMIF) jointly organized the seminar in collaboration with International Institute of Islamic Thought (IIIT) East Asia, Institute for the Study of the Ummah and Global Understanding (ISUGU) and Regional Islamic Da\u27wah Council of Southeast Asia and the Pacific (RISEAP). Emeritus Professor Datuk Dr. Osman Bakar, Program Chairman and Deputy CEO, IAIS Malaysia and Dr Nay Aung Kyaw @ Mohammed Ali Hassan, Vice President, MMIF broke the ice with their welcoming addresses. Then Founding Chairman and CEO, IAIS Malaysia Professor Dr Hashim Kamali gave his opening speech while officiating address was delivered by Tun Abdullah Haji Ahmad Badawi, Patron of IAIS Malaysia and Former Prime Minister of Malaysia
Analisis dakwah dalam cerpen-cerpen terpilih Rejab F.I.
Makalah ini bertujuan untuk membincangkan persoalan dakwah yang berpaksikan kehidupan orang-orang Melayu yang kaya dengan akal budi dan tata susila hidup. Kaedah analisis kandungan digunakan dengan memfokuskan terhadap persoalan dakwah dalam kumpulan cerpen Papaverine oleh Rejab F.I. Data dideskripsi dan dibincangkan berpandukan pendekatan dakwah yang terangkum dalam Teori Pengkaedahan Melayu yang diperkenalkan oleh Hashim Awang secara deskriptif iaitu dihuraikan secara terperinci sesuai dengan ruang lingkup kajian. Hasil analisis menunjukkan cerpen-cerpen terpilih Rejab F.I berkait rapat dengan aspek kemanusiaan terisi dengan kepelbagaian rencah kehidupan yang padat dan kompleks. Daripada analisis yang menjurus kepada watak dan perwatakan, kajian ini mendapati empat tema yang terungkap dari Pendekatan Dakwah iaitu takwa, keadilan, ilmu dan taubat. Penemuan tema takwa, keadilan dan ilmu yang terungkap dalam cerpen-cerpen terpilih oleh Rejab F.I tersebut merupakan gambaran landskap pemikiran masyarakat Melayu berdasar pada nilai hidup semasa. Semua ini terjalin mengikut prinsip kehidupan orang-orang Melayu yang berpegang teguh kepada ajaran dan suruhan Allah SWT
Publication productivity of Malaysian researchers in the field of Computer Science and Information Technology
Searches made in the CD-ROM databases, COMPENDEX (1987-1999), IEL (IEE/IEEE Electronic Library) (1988-1999) and INSPEC (1990-1998) revealed a total of 389 publications contributed by Malaysian researchers in the field of computer science and information technology. The trend in output indicates rapid growth that is expected to continue in future. A total of 458 unique Malaysian authors contributed to the 389 publications. Collaboration between two authors was the dominant authorship pattern. Single-authored or more than 3-authored works were rare. The active authors were affiliated to a few institutions, with the Universiti Teknologi Malaysia, Universiti Sains Malaysia and Universiti Malaya accounting for the highest number of publications, either in the form of journalarticles or papers in conference proceedings. The most active research areas include simulation system, control engineering, computer-assisted instruction, programming techniques, expert systems, asynchronous transfer mode, image processing, software engineering and digital signal processing and applications

Evaluation of time-dependent pathways in an acute ischemic stroke protocol that incorporates CT perfusion: A tertiary referral center experience
Background and Objective: Intravenous thrombolysis service for stroke was introduced at the Universiti Kebangsaan Malaysia Medical Centre (UKMMC) in 2009, based on the recommendations of a multidisciplinary team of clinicians. We report the experience at our center in establishing a stroke protocol incorporating computed tomography perfusion (CTP) of the brain, to assess the feasibility of incorporating CTP in the stroke protocol.
Methods: A retrospective review of all patients who had a CTP between January 2010 and December 2011 was performed. Results: Of 272 patients who were admitted with acute ischemic stroke, 44 (16.2%) arrived within 4.5 hours from symptom onset and had a CTP performed with the intention to treat. The median time for symptom-to-door, symptom-to-scan and door-to-scan was 90.0 minutes (62.5 – 146.3), 211.0 minutes (165.5 – 273.5) and 85.0 minutes (48.0 – 144.8) respectively. Eight patients (2.9%) were thrombolysed of whom five received IV thrombolysis and three underwent mechanical thrombolysis. The median symptom-to-needle and door-to-needle times were 290.5 minutes (261.3 – 405.0) and 225.0 minutes (172.5 – 316.8) respectively. Four patients were thrombolysed despite being outside the window of treatment based on the CTP findings. Six of the thrombolysed patients had a Modified Rankin Score (MRS) of 1-2 at 5 months post procedure.
Conclusions: CTP provides a benefit to management decisions and subsequent patient outcome. It is feasible to incorporate CTP as a standard imaging modality in a stroke protocol. The delays in the time-dependent pathways are due to our work flow and organisational process rather than performing the CTP per se
Review an assessment of electromagnetic wave propagation in sand and dust storms at microwave and millimeter wave band- part 1
Electromagnetic wave propagation in arid and semi-arid regions is influenced by sand
and dust storms. Meager information has hitherto been reported as to the effect of storms on
telecommunication systems operating in such regions. This paper presents a survey of current
understanding of the wave propagation during storms. In this first part of the review—Part I, detailed
parametric assessment of some electrical and mechanical properties affecting wave propagation in sand
and dust storms is given. The second part of the review—Part II describes the principle of approach
and technology adopted for the investigation highlighting both strengths and drawbacks. The results
demonstrate that most authors have calculated signal attenuation effect, revealing that it is not very
significant unless during severe storms. A few papers indicate the possibility of more significant cross
polarisation. Part I explicitly gives an account of the sand and dust storms’ phenomenon, reviews
the storms’ parameters affecting electromagnetic wave propagations and discusses the microwave and
millimeter wave bands

Suspended particles in the atmosphere during sand and dust storms have numerous
consequences on electromagnetic wave propagation in arid regions. The electromagnetic wave signal
may suffer attenuation and cross polarization upon encounter with the suspended particles. However,
meager information has hitherto been reported about effect of storms on the telecommunication systems
operating in such regions. This paper presents a survey of current understanding of the electromagnetic
wave propagation in sand and dust storms. A review of the literature covering electromagnetic scattering
theory and applications is given. The review describes the principle of approach and technology adopted
for the investigation highlighting both strengths and drawbacks. Detailed parametric assessment of the
effects of storms on wave propagation as it concerns signal attenuation and cross polarization is also
carried out. The results demonstrate that most authors have calculated the attenuation effect, revealing
that it is not very significant unless very high suspended dust densities are assumed (i.e., during severe
sand and dust storms). A few papers indicate the possibility of more significant cross polarisation. The
obvious gap in knowledge of this field is finally also clearly established

This article attempts to look at the impact of Islamization process on the Sultanate of Malacca during the 15th century. Islam has offered civilizational life for the Malays. Malacca grew from an unknown place during the pre-Islamic period to become one of the well-known centres of Islamic religion and culture in the region. Islam has changed the status of Malacca after reducing its pre-Islamic customs and ways of life. The importance of the Malay Sultanate of Malacca has been well-documented and much has been written about it by many authors either by Malaysians such as Buyong Adi1, Kernal Singh Sandhu, Mohd Jamil Mukmin, Mohd Taib Osman, Muhammad Yusoff Hashim, Abu Hassan Sham, Khoo Kay Kim, and Joginder Singh Jessy, and Zubir Usman, or by the non-Malaysians including R. O. Winstedt, R. J. Wilkinson, Walter William Skeat, C. O. Blagden, Paul Wheatley, D. G. Hall, F. J. Moorhead, J. Bantin and R. Roolvink, J. Kennedy, John Bastin, Liang Liji, M. B. Hooker, Nicholas Tarling, Paul Wheatley, Sarnia Hayes Hoyt, T. W. Arnold, W. P. Groeneveldt, Christoper H. Wake, P. E. de Josselin de Jong and H. L. A van Wijk, Robert W. McRoberts, and Wang Gangwu. They wrote on various genres of literature and culture of the Malays as well as the history of the Sultanate. However, some of them especially the orientalists, because of their adoption of various Western social theories, this application of such theory to the sultanate or the Malay society yield untenable results. We shall argue some of whom tend to regard Islam as unimportant in shaping the Malay worldview, society and identity. This article will emphasize the civilizational significance of the Islamic impact by looking at the system of political authority and the concept of government of the Sultanate
