Un docente che voglia preparare del materiale computerizzato volto all’apprendimento di una lingua straniera può farlo facilmente grazie alle varie lingue autore e ai più semplici programmi autore che sono disponibili sul mercato. Nonostante le indiscutibili potenzialità di questi strumenti, sono pochi i docenti che si cimentano nel loro sfruttamento. Perché? Il presente lavoro si propone di offrire alcune risposte a questo interrogativo. Parte da un’analisi del rapporto tra docenti e nuove tecnologie, offre una distinzione fra linguaggi e sistemi autore analizzandone finalità e potenzialità per poi sviluppare un’analisi contrastiva fra i dieci software didattici più usati al momento dai docenti di lingue straniere. Si sofferma sul programma autore T.E.A.C.H. Monitor (Nordra 1989) in dettaglio, individuandone caratteristiche, pregi, difetti ed applicazioni didattiche. Termina con una serie di considerazioni pratiche che potranno essere di aiuto a coloro che si accingono alla creazione di materiale computerizzato volto all’apprendimento di una lingua straniera
Risk stratification and prevention of sudden death in hypertrophic cardiomyopathy.
Sudden cardiac death is the most devastating manifestation of hypertrophic cardiomyopathy (HCM) and often occurs in young and previously asymptomatic patients. Therefore, risk stratification for sudden death has a major role in the management of HCM and has acquired even greater relevance since the implantable cardioverter-defibrillator (ICD) has proved to be highly effective in preventing sudden death in this disease. The ICD is definitely indicated for secondary prevention of sudden death in patients with HCM who have survived a cardiac arrest with documented ventricular fibrillation, or experienced one or more episodes of sustained ventricular tachycardia. However, uncertainties persist regarding the precise selection of patients for primary prophylactic ICD implantation. A number of risk markers are used to assess the magnitude of risk, including family history of premature sudden death; extreme left ventricular (LV) hypertrophy (> 30 mm) in young patients; nonsustained ventricular tachycardia on Holter electrocardiographic recording; unexplained (not neurally mediated) syncope, particularly in young patients; and blood pressure decrease or inadequate increase during upright exercise. Multiple risk factors convey a definite increase in risk. However, a single risk factor such as family history of multiple sudden deaths, massive LV hypertrophy in a young patient, or frequent and/or prolonged runs of nonsustained ventricular tachycardia on Holter, may also justify consideration of a prophylactic ICD
Pharmaco-toxicological aspects of herbal drugs used in domestic animals
Herbal drugs are more and more used both in human and veterinary medicine to mitigate and prevent minor diseases and to support conventional medicine using allopathic drugs. Nevertheless, ‘natural product’ does not mean lack of adverse effects, and many people and veterinarians do not know enough about the adverse reactions that can occur following the administration of such drugs in domestic animals. Moreover, herbal products can interact with each other when administered concomitantly or
can agonize or antagonize the effects of synthetic drugs administered as primary therapy. The use of non-conventional
medicines (NCM) should be considered as a veterinary practise. In this paper, the herbal drugs most utilized in domestic animals, both pets and large animals, are reviewed, as their use is increasing, despite the prejudices of the academic world and some of the adverse effects and interactions that can occur in domestic animals

Update on acute bone and joint infections in paediatrics: A narrative review on the most recent evidence-based recommendations and appropriate antinfective therapy
Acute bone and joint infections (BJIs) in children may clinically occur as osteomyelitis (OM) or septic arthritis (SA). In clinical practice, one-third of cases present a combination of both conditions. BJIs are usually caused by the haematogenous dissemination of septic emboli carried to the terminal blood vessels of bone and joints from distant infectious processes during transient bacteraemia. Early diagnosis is the cornerstone for the successful management of BJI, but it is still a challenge for paediatricians, particularly due to its nonspecific clinical presentation and to the poor specificity of the laboratory and imaging first-line tests that are available in emergency departments. Moreover, microbiological diagnosis is often difficult to achieve with common blood cultures, and further investigations require invasive procedures. The aim of this narrative review is to provide the most recent evidence-based recommendations on appropriate antinfective therapy in BJI in children. We conducted a review of recent literature by examining the MEDLINE (Medical Literature Analysis and Retrieval System Online) database using the search engines PubMed and Google Scholar. The keywords used were “osteomyelitis”, OR “bone infection”, OR “septic arthritis”, AND “p(a)ediatric” OR “children”. When BJI diagnosis is clinically suspected or radiologically confirmed, empiric antibiotic therapy should be started as soon as possible. The choice of empiric antimicrobial therapy is based on the most likely causative pathogens according to patient age, immunisation status, underlying disease, and other clinical and epidemiological considerations, including the local prevalence of virulent pathogens, antibiotic bioavailability and bone penetration. Empiric antibiotic treatment consists of a short intravenous cycle based on anti-staphylococcal penicillin or a cephalosporin in children aged over 3 months with the addition of gentamicin in infants aged under 3 months. An oral regimen may be an option depending on the bioavailability of antibiotic chosen and clinical and laboratory data. Strict clinical and laboratory follow-up should be scheduled for the following 3–5 weeks. Further studies on the optimal therapeutic approach are needed in order to understand the best first-line regimen, the utility of biomarkers for the definition of therapy duration and treatment of complications
