Endoscopic management for delayed diagnosis of a foreign body penetrating the esophagus into the lung

    No full text
A 31-year-old male presented with chest pain started after eating chicken about 2 weeks earlier. Upper endoscopy and Computed tomography scan of the chest revealed a sharp chicken bone penetrating the esophageal wall into the right lung. The foreign body was removed endoscopically using a rat-tooth forceps, followed by prophylactic placement of a metal stent across the esophageal perforation site. Foreign body-induced perforation is one of the common etiologies of benign esophageal perforations. Although the primary treatment is surgery, endoscopic therapy may be appropriate in individualized cases like our patient

Indica-se, ainda, que a discussão se desenvolveu a partir do método dedutivo, com embasamento doutrinário e jurisprudencial, e que a matéria deste trabalho monográfico está situada precipuamente nas esferas do Direito Constitucional e do Direito TributárioThis work has the scope to define the place of payment of Service Tax of any kind (ISSQN), in view of the existing fuss about it, some understanding that this tribute focuses on local service provision and other tax that is due to the City, or the Federal District, which is located on the property provider. Thus, to achieve such purpose, shall be trodden down of the field of tax incidence of ISSQN, starting with superficial approaches to the nature and object of the Tax Law and its conformation in the Brazilian Tax System, through the verification of the importance of constitutional norms, while fundamentals of this system, and the evolution of the Brazilian Constitutional System, marking the main features of the legal order in force, entering in sequence, to study specific Service Tax (ISS), which will study the historical conjuncture, the taxing power to institute it, and their limits, the playing field by providing complementary laws on conflicts of jurisdiction, classification and criteria of its rule-array of tax incidence. Finally, the deeper and the light of the primacy of the Constitution, especially considering the principles of local autonomy and territoriality and the playing field of complementary law will be investigated fixation, the Charter of the Republic of 1988, the spatial aspect of the ISS, adequacy of provisions of national law which aims to secure it, the limits of the action of the legislature and municipal treatment determined by the jurisprudence of the Superior Courts to the question of the place of payment of Service Tax. Done this way, defending the supremacy of the Constitution, the author will present his findings. It is stated further that discussion grew out of the deductive method, with doctrinal and jurisprudential foundation, and that the subject of this monograph lies is situated mainly in the spheres of Constitutional Law and Tax La

The classification of species in trophic guilds facilitates studies on the ecology interactions. However, the large number of definitions of terms used and the overlap has hampered the standardization of trophic guilds, especially bottom-feeder fishes. In this context, this study aimed to quantify, by a scientometrics analysis, the research involving species of bottom-feeder fishes and trophic guilds in which they are classified, and discuss the definitions used for each trophic guild. A survey in the Scopus and Web of Knowledge was performed and the publications were analyzed according to the type of trophic guild, publication year, the first author ´s country, the journal of publication, approach used, climate, the type of environment, subject and trophic guilds definitions. The trophic guild of the bottom-feeder fishes\u27s fall among the least studied, however, we observed an increased number of publications since 1990. The climatic regions with the highest number of publications were the Tropical and Temperate hot climate, especially the United States, Brazil and Holland in the number of published papers. The publications, mainly observational, were developed mainly in freshwater environments, with emphasis on dietary studies. We found the occurrence of different definitions to bottom-feeder fishes, and the classification criteria were based mainly on food resource and feeding site. Considering the great diversity of food resource present in the sediment of aquatic environments, the classification of the bottom-feeder fishes in various trophic guilds is justified. However, there needs to be a standardization of the criteria for classification, thus it will generate fewer conflicts regarding the nomenclature used to determine the trophic guilds of fish. The classification of species in trophic guilds facilitates studies on the ecology interactions. Regaining the Connection between River and People: A History of the Pima, Their Traditions, and the Gila River

abstract: Pima was not the original name for the tribe of the Native Americans that lived along the Gila River, but their name is only one of many changes this tribe has faced due to the influence of outside sources. Pima was a name given to this tribe by the Spaniards during the 1600s. Throughout history the Pima have fought not only for their rights as a nation, but also for the rights of their beloved river, the Gila River. The relationship between the Pima tribe and Gila River is a very strong and deep connection. In fact, the Pima call themselves A-a'tam, which means "the people," and they identify with the Papago, a historical name for the Indians in the Sonoran Desert. A'kimult, which means "river" was added to their name as well. Being known as the River People may fit perfectly with this tribe due to their close knit and respectful relationship with the river over the decades (Russell, "The Pima Indians, 1975). Today the Pima call the Gila River Indian Community their main Tribal headquarters, which is located south of Phoenix. The Gila River Indian Community (GRIC) is part of both the Pinal and Maricopa counties and has a land area of 583.749 square miles ("Brownfields", 2010). Water deprivation and a bleak agricultural economy are challenges facing the Pima, which historically were strong in both areas. The Gila River has gone from a naturally flowing river to a restricted and nearly dried up waterbed. This research paper will examine the changes that both the Pima and the Gila River have undergone since settlers began to claim land in the late 1800's. It is my goal to look into the natural history and ecology of the Gila River and explain how this change has affected the Pima and their sustainability as a tribe. The ultimate goal is to have information easily accessible for reference for future research projects and to ] provide background information to help implement new programs and projects that will benefit the GRIC. Understanding the relationship between the Gila Valley and Pima will present areas where sustainable projects can improve the economy and society as a whole. The Native Americans who will be specifically addressed during this research are the tribes on the GRIC, which comprise the Akimel O-odham (Pima) and Pee Posh (Maricopa). These communities have a resident population of 15,084 and are located in the Maricopa and Pinal counties of central Arizona, to the south and east of metropolitan Phoenix ("Brownfields", 2010). Today the community is 372,000 acres and varies with an elevation of 935 feet to 1,450 feet ("Gila River", 2012). Literature will be the primary area of research along with informal discussions with employees of the GRIC. The Pimas' beliefs, interests, and practices will be addressed and researched; and review of the literature that deals with each problem they have faced as a result of the changing economy and society. By researching the relationship and proposing new ideas to help maintain the GRIC it will assure that the interests of the Pima are the priority. The potential that this research project can offer must encompass cultural sustainability, which is "developing, renewing and maintaining human cultures that create positive, enduring relationships with other peoples and the natural world" (Hawkes, 2001)
