Elevating Palliative Care in the Emergency Department: Increasing Direct Admissions and Documented Goals of Care
The project aimed to streamline admissions from the ED to the Palliative Care Unit (PCU) and increase documented Goals of Care (GoC) discussions by October 2025. Key interventions included assigning a dedicated Palliative Care attending physician to the ED for early GoC discussions, utilizing a Hospice Nurse Liaison, and enabling direct transfers to the PCU. As a result, the unit saw an increase in direct ED-to-PCU admissions (totaling 125) and improved GoC documentation in the ED from a baseline of 26% to 31% by the target deadline
Reducing Hypo- and Hyperglycemia in Diabetic Patients with Strategic Meal Delivery Times
Uncontrolled meal times led to increased glycemic events in diabetic patients who were eleigible for Conceierge Dining Services. To address this, the unit collaborated with Dietary Services to establish standardized meal delivery times (8:30 am, 12:15 pm, 5:30 pm) for diabetic patients, along with daily patient tracking and staff education. As of September 2025, the unit is on track to meet its goal of a 5-10% reduction, showing significant decreases from 5 to 1 monthly hypoglycemia events and from 11 to 7 monthly hyperglycemia events
