    Questo mio scritto è in onore del fu Bruno Schettini, amico e collega degli ultimi 5 anni, ora che siamo ancora in attesa di ulteriori sviluppi da parte della Comunità Europea nel campo dell’istruzione permanente ben dieci anni dopo la pubblicazione del Memorandum su questo argomento. Bisogna ancora vedere se nuove strade si dipartiranno dall‟opinione dominante dello scorso decennio e se siano state recepite le lezioni che scaturiscono dalle critiche pubblicate da un certo numero di scrittori, incluso un libro scritto da Ettore Gelpi che Bruno Schettini aiutò a pubblicare in italiano e per il quale scrisse una introduzione molto perspicace. In questo intervento intendo fornire un riassunto del concetto di lifelong learning e di come l‟OECD abbia mutato le sue origini nate in seno all‟UNESCO, ma più specificamente dalle mutazione dovute ai 6 messaggi emanati dalla Comunità Europea e contenuti nel Memorandum on Lifelong Learning . Nello stesso tempo cercherò di spiegare come il discorso su questo argomento possa essere portato avanti fino a diventare un vero e proprio contributo al rafforzamento della democrazia.peer-reviewe

    AUTOMATIC SAGITTAL-TRANSVERSAL MAXILLARY EXPANSION: ORTHOGNATHODONTIC EVOLUTION

    The outcome of daily clinical practice in interceptive orthognathodontics is influenced by the severity of the pathology. It also heavily depends on the paediatric patient's ability to cooperate. The need to choose between dentoalveolar and skeletal expansion using fixed appliances arises from the necessity to achieve a predictable result in the shortest time possible and grant optimal patient growth, even in the absence of cooperation. However, the transposition of the typical three-way steel screw from removable appliances to fixed ones complicates expansion treatments, especially for the patient's parents, forcing the dental professional to perform frequent checks relative to expansion’s activations. The introduction of an automatic one-way nickel-titanium expansion screw in the shape of a crossbow, to be intended as an evolution of the one-way steel screw present in traditional rapid expanders, has paved the way for a new interceptive orthognathodontics paradigm. Daily clinical practice makes skeletal transversal maxillary expansion substantially independent of patient cooperation. In this paper, a new device is presented, with the aim to further refine the concept of independence present in contemporary one-way crossbow-shaped expansion screw appliances, simply introducing three undersized crossbows made from nickel-titanium. This design can exert efficient expansion simultaneously in three directions, making maxillary expansion automatic both sagittally and transversally, minimizing the need for patient cooperation, and reducing parents’ concern

    Immediate Implant Placement in Infected Sites: A Case Series

    BACKGROUND: Immediate implant placement has several advantages, such a reduction in the number of surgical treatments and reduction of the time between tooth extraction and the placement of the definitive prosthesis. However, there are still some situations which could jeopardize the success of the aforesaid therapy, such as the presence of an infection caused by periodontal disease or peri-apical lesions. The aim of this study was to evaluate the clinical success of implants placed in fresh extraction sockets which showed clinical signs of periodontal disease. MATERIALS AND METHODS: Twenty patients (13 males and 7 females) aged 24 to 65 were included in this study. After the initial examination and the treatment planning, all of the patients selected for the study underwent the periodontal treatment deemed necessary to facilitate wound healing. All the 20 teeth were extracted due to an infection. The second stage surgery was performed 4 months after the initial procedure. The following clinical parameters were evaluated for each patient at the time of implant placement and at the end of the 12 month follow-up period: Attachment level (AL), presence or absence of mobility, presence or absence of pain, presence or absence of suppuration. The bone level was measured as the distance from the implant shoulder to the first bone implant contact (Distance Bone Implant) (DIB) by periapical radiographs. The stability and health of the soft tissue was clinically evaluated by means of the plaque score. RESULTS: The healing period was uneventful for all the patients. All the implants were osseointegrated. At the end of the 12 month follow-up period, patients were asymptomatic and showed no signs of infection or bleeding when probed. The mean attachment level (AL) at the mid-buccal location per implant was 0.8 mm at the baseline and 0.9 mm at the end of the follow-up. The mean width of KM measured at the mid-buccal location per implant at the baseline and 1-year visits was 3.7+\-0.4 mm and 3.3+\-0.5 mm respectively. The mean Attachment level measured at the mid-buccal location per implant was 0.8 mm at the baseline and 0.9 mm at the end of the follow-up. The pariapical radiographs, obtained in a standardized manner, revealed a mean increase of 1 mm in the DIB value. At the 12-month-follow-up, the presence of plaque was observed in 44 of the 80 sites analyzed. CONCLUSIONS: On the basis of this study implants placed in fresh extraction sockets - which were affected by an infection- may be a valid operative technique, which leads to predictable results, if adequate pre- and postoperative care is taken

Desde 1951 sua produção literária tem crescentemente estimulado debates críticos e controvérsias, devido à sua poética intrinsecamente relacionada a movimentos de libertação das mulheres, pacifismo, anti-semitismo e outras questões sociais. Este estudo tem por objetivo analisar as relações entre poesia e política na poesia de Rich de 1951 a 1999, tentando acompanhar seu desenvolvimento. Para tanto, o primeiro capítulo discute as idéias de Mikhail Bakhtin e Theodor Adorno sobre lírica e sociedade em relação às de Rich sobre o assunto. Os capítulos posteriores analisam os poemas dela em diálogo com esse embasamento teórico, entrevistas e ensaios de Rich, assim como críticas sobre a obra dela

    Total Conometric Implant Prothesis Rehabilitation with Peek Secondary Caps: A Case Report

      This Case Report presents the rehabilitation of a 66-year-old female patient using a telescopic retention system with Polyetheretherketone (PEEK) secondary caps. The treatment utilized PEEK-based conometric retention for a partial upper-arch prosthesis. The report highlights the advantages and disadvantages of this approach compared to traditional retention systems. The patient's initial concerns about removability and aesthetic functionality were alleviated, and her satisfaction was underscored by significant improvements in her social activities and oral comfort. The case illustrates the feasibility of integrating PEEK into telescopic systems, addressing limitations of traditional materials and telescopic coupling while providing a cost-effective. Further clinical evaluation and research into surface modifications are essential to fully optimize this approach for long-term success

    Hybrid Surgery to Manage Aortic Arch Pathology

    Background and Objectives: Aortic arch disease is still a high-risk surgical challenge despite major advances both in surgical and anesthesiological management. A combined surgical and endovascular approach has been proposed for aortic arch disease treatment to avoid hypothermia and circulatory arrest in high-risk patients. Materials and Methods: Between June 2004 and June 2021, 112 patients were referred to our department for aortic arch surgery; 38 (33.9%) patients underwent supra-aortic debranching and endovascular treatment. Of these, 21 (55%) patients underwent type I aortic arch hybrid debranching procedure and in 17 (45%) patients a type II aortic arch hybrid debranching procedure was performed. None of the patients were emergent. Results: No intra-operative deaths were recorded. In the type I aortic arch hybrid debranching patients’ group, one patient died at home waiting the endovascular step, one developed ascending aortic dissection and another one developed a pseudoaneurysm at the site of the debranching at follow-up. In the type II aortic arch hybrid debranching patients’ group, left carotid artery branch closure was detected at follow-up in one patient. Thirty day/in-hospital rates of adverse neurological events for both the surgical and endovascular procedures were 3% for minor stroke, with no permanent neurological deficit and 0% for permanent paraplegia/paraparesis. In 100% of the cases, the endovascular step succeeded and the type Ia endoleak rate was 0%. Conclusions: Hybrid arch surgery is a valuable option for aortic arch aneurysm treatment in patients with high surgical risk. The choice of aortic arch debranching between type I or type II is crucial and depends on anatomic and clinical patient characteristics. Further larger scale studies are needed to better define the advantages of these techniques

    Posttraumatic Use of Dental Implants Immediately after Tooth Extraction: Clinical Study

    PURPOSE: Root fracture is a combined injury of cementum, dentin, and pulp. Many of these traumas remain untreated, mistreated, or overtreated. It leads to a more complicated treatment in case of tooth loss. Many different treatment procedures, with a very changeable success rate, have been proposed for years to treat teeth with root fractures. The objective of the following clinical studies was to evaluate the clinical effectiveness of implants placed in fresh extraction sites to treat teeth with horizontal root fracture. METHODS: The study group included 25 patients (15 men and 10 women) between the ages of 20 and 65 years. After an initial examination and a treatment planning, all of the patients underwent periodontal treatment, which was deemed necessary to favor wound healing.All the 25 teeth were extracted because of horizontal root fracture located at the level of the middle third. The second-stage surgery was performed 6 months after the initial procedure. The following clinical parameters, presence or absence of mobility, presence or absence of pain, and presence or absence of suppuration, were evaluated in each patient at 6 and 12 months after implant placement. Radiographs were taken using the standard method to evaluate the marginal bone loss. RESULTS: The healing period was uneventful for all patients. All implants had osseointegrated. After 12 months, patients were asymptomatic and showed no signs of infection or bleeding when probed. CONCLUSIONS: On the basis of this study, implants placed right after tooth extraction are a valid treatment procedure, which induces predictable results as treatment of fractured teeth. © 2014 Mutaz B. A través de sus influencias reconocibles en su libro La escenografía cinematográfica,recorremos el film Los tallos amargos (Fernando Ayala, 1956) en busca de sus contribuciones a la labordel director de arte. Estudiaremos en escenas particulares del film las posibles deconstrucciones de lailusión de realidad del cine clásico, mediante el análisis espacial planteado por Éric Rohmer sobre lapelícula Fausto de F. W. Murnau, el uso de metáforas orientacionales y las nociones de espacio clásico yposclásico según Santiago Vila
