Combined Cerebellar Proton MR Spectroscopy and DWI Study of Patients with Friedreich’s Ataxia
Friedreich’s ataxia (FRDA) is the commonest autosomal recessive ataxia, caused by GAA triplet expansion in the frataxin gene. Neuropathological studies in FRDA demonstrate that besides the primary neurodegeneration of the dorsal root ganglia, there is a progressive atrophy of the cerebellar dentate nucleus. Diffusion-weighted imaging (DWI) detected microstructural alterations in the cerebellum of FRDA patients. To investigate the biochemical basis of these alterations, we used both DWI and proton MR spectroscopy (1H-MRS) to study the same cerebellar volume of interest (VOI) including the dentate nucleus. DWI and 1H-MRS study of the left cerebellar hemisphere was performed in 28 genetically proven FRDA patients and 35 healthy controls. In FRDA mean diffusivity (MD) values were calculated for the same 1H-MRS VOI. Clinical severity was evaluated using the International Cooperative Ataxia Rating Scale (ICARS). FRDA patients showed a significant reduction of N-acetyl-aspartate (NAA), a neuroaxonal marker, and choline (Cho), a membrane marker, both expressed relatively to creatine (Cr), and increased MD values. In FRDA patients NAA/Cr negatively correlated with MD values (r = −0.396, p = 0.037) and with ICARS score (r = −0.669, p < 0.001). Age-normalized NAA/Cr loss correlated with the GAA expansion (r = −0.492, p = 0.008). The reduced cerebellar NAA/Cr in FRDA suggests that neuroaxonal loss is related to the microstructural changes determining higher MD values. The correlation between NAA/Cr and the severity of disability suggests that this biochemical in vivo MR parameter might be a useful biomarker to evaluate therapeutic interventions
Experimental tests about the cooling/freezing of the molten salts in the receiver tubes of a solar power plant with parabolic trough
In 2003 ENEA realized the PCS experimental Facility at Casaccia Research Centre (Rome, Italy), in order to test in real operating conditions the components of a parabolic trough solar plant, and to evaluate the technical feasibility of using the solar molten salts mixture (60% NaNO3, 40% KNO3, melting point 220÷240°C) in such a type of plant. ENEA also had the need to assess the behaviour of the solar receiver tubes during abnormal operating situations (wrong operation, pump block, power failure, etc.), when a block of the circulation of the molten salts may occur and cause the cooling or, worse, the freezing of the salts mixture inside the pipes. Some experimental tests have been performed, aimed to examine what happens in such a cases. In fact, without quick maneuvers to restart the circulation of the molten salts or to readily empty the receiver tubes, the molten salts contained in them may cool down to temperatures near/below the solidification. In this report are shown the results of the experimental tests carried out on the receiver tubes of the PCS Facility by cooling the process fluid down to temperatures near or below its freezing point. The tests show that the solidification of the salts does not damage the components of the plant but it is manageable and reversible, provided you apply the correct procedures. © 2017 Author(s)
Role of DWI assessing nodal involvement and response to neoadjuvant chemotherapy in advanced breast cancer
OBJECTIVE:
To explore the role of diffusion-weighted imaging (DWI) in the staging of axillary lymph nodes and the restaging after neoadjuvant chemotherapy (NAD) in advanced breast cancer.
PATIENTS AND METHODS:
MRI examinations of forty-two patients diagnosed with advanced breast cancer addressed to NAD and axillary lymph node dissection (ALND) were reviewed. Apparent diffusion coefficients (ADC) of each visible node in DWI in the pathologic axilla (PA) and healthy axilla (HA) were measured at the time of diagnosis (t0) and after chemotherapy (t1); mean values of the ADC were calculated. Patients were classified as responders (R), non-responders (NR), macrometastasis (MA), micrometastasis (Mi).
RESULTS:
Mean ADC was 0.92 ± 0.07 x 10-3 mm2/sec at t0 and 0.97 ± 0.06 x 10-3 mm2/sec at t1 (p = 0.284) in PA, 0.89 ± 0.06 x 10-3 mm2/sec at t0 and 0.92 ± 0.06 x 10-3 mm2/sec at t1 (p = 0.403) in HA, 0.95 ± 0.111 x 10-3 mm2/sec at t0 and 0.95 ± 0.14 x 10-3 mm2/sec at t1 (p = 0.954) in R group, 0.90 ± 0.09 x 10-3 mm2/sec at t0 and 0.97 ± 0.07 x 10-3 mm2/sec at t1 (p = 0.085) in NR group, 0.86 ± 0.10 x 10-3 mm2/sec at t0 and 0.99 ± 0.09 x 10-3 mm2/sec at t1 (p = 0.055) in MA, and 0.99 ± 0.23 x 10-3 mm2/sec at t0 and 0.95 ± 0.15 x 10-3 mm2/sec at t1 in Mi (p = 0.667).
CONCLUSIONS:
Mean ADC between PA and HA, R and NR, MA and Mi did not significantly differ at t0 and t1 (p > 0.05). Variation in mean ADC between t0 and t1 was not significant in all groups (p > 0.05), except for a trend toward significance (p = 0.055) in MA. DWI has a potential role in restaging of macrometastatic axillary nodes after NAD
Ius soli: qualche precisazione di metodo in materia di diritti di cittadinanza e diritti della cittadinanza
The Author analyses, among many possible meanings of citizenship, two types which had a part in the process of modern State building: citizenship is sometimes a relationship between the individual and the sovereign, sometimes consciousness of a collective membership. Both meanings have a link with the sovereignty and the process of modern State building. After the “great Neutralisation” of the Nineteenth Century, some reflections are developed about the relationship between citizenship and sovereignty in the Twentieth Century. Finally, the Author emphasizes the necessity of a statute that sets the requirements needed to become a citizen in order to pacify the conflicts of multicultural societies
Role of diffusion-weighted magnetic resonance imaging in prostate cancer evaluation
Purpose: The authors analysed the role of diffusion-weighted imaging (DWI) as an additional tool in magnetic resonance (MR) evaluation of prostate cancer.
Materials and methods: Forty-one patients with suspected prostate cancer underwent MR imaging (1.5 Tesla). A DWI sequence was added to the standard morphological protocol, with a maximum b value of 1,000 s/mm2. Diffusion maps were obtained, and the apparent diffusion coefficient (ADC) was calculated by drawing a region of interest (ROI) over healthy tissue and areas suspicious for malignancy. Histology was considered the gold standard.
Results: The areas correctly classified by MR imaging (42/51) had a low signal intensity on T2-weighted imaging and low ADC value (0.99±0.15 mm2/s; p<0.01) compared with the healthy peripheral zone (PZ) (1.73±0.27 mm2/s; p<0.01). Nine areas classified as suspicious for malignancy on T2-weighted sequences showed high ADC (1.44±0.06 mm2/s; p<0.01) and were confirmed to be disease free by subsequent histological examination. The accuracy of morphofunctional MR imaging was 81.6% compared with 73.7% of the morphological analysis alone.
Conclusions: The addition of DWI to the standard protocol increases the overall diagnostic performance of MR imaging in detecting prostatic cancer. Thus, DWI can help the clinician determine the most appropriate management strategy for the patient
The Italian Supreme Court Joint Sections set forth the interpretative underpinnings of the "Gelli-Bianco" law: varying degrees of guilt aimed at limiting medical liability, article 2236 C.C. makes a comeback
The article’s author has sought to summarize the regulatory and legal evolution of accidental criminal liability of doctors starting from earliest positions up until the enactment of the Gelli-Bianco law. An in-depth analysis is laid out based on the Italian Supreme Court Joint Sections ruling n. 8770/2018 (so-called Mariotti decision). The author has also elaborated upon the notion of varying degrees of guilt, which was taken out of the law’s wording, to be later reintroduced as a concept by judicial interpretation. It is worth noting that Article 3 of the Balduzzi decree, article 590 sexies of the Italian Criminal Code and the reference to article 2236 of the Civil Code reflect an awareness on the part of legislators that medical liability needs to be limited. Clearly, the approach based on lawfulness alone, which protects from liability physicians that have adhered to guidelines, has been dismissed, superseded by the notion of minor fault. Nonetheless, the new legislation, in the author’s estimation, constitutes a standard particularly ill-suited to modern medical practice, which has a high degree of complexity. The author concludes that it might be worth considering a more balanced alternative: getting back to the notion of fault, considering minor fault relevant, rather than major fault
The Author dwells on the comparability of the d.P.C.m. taken to implement the containment measures and to manage the COVID-19 health emergency to the urgent and needed acts. The Author also highlights the only partially satisfactory nature of the arguments set out in the judgment that rejected the issues of constitutionality raised against the decree-laws adopted in order to regulate such measures
