Clinical experience with insulin detemir, biphasic insulin aspart and insulin aspart in people with type 2 diabetes: Results from the Casablanca cohort of the A 1 chieve study
Background: The A 1 chieve, a multicentric (28 countries), 24-week, non-interventional study evaluated the safety and effectiveness of insulin detemir, biphasic insulin aspart and insulin aspart in people with T2DM (n = 66,726) in routine clinical care across four continents. Materials and Methods: Data was collected at baseline, at 12 weeks and at 24 weeks. This short communication presents the results for patients enrolled from Casablanca, Morocco. Results: A total of 495 patients were enrolled in the study. Four different insulin analogue regimens were used in the study. Study patients had started on or were switched to biphasic insulin aspart (n = 231), insulin detemir (n = 151), insulin aspart (n = 19), basal insulin plus insulin aspart (n = 53) and other insulin combinations (n = 41). At baseline glycaemic control was poor for both insulin naïve (mean HbA 1 c: 10.2%) and insulin user (mean HbA 1 c: 9.4%) groups. After 24 weeks of treatment, both groups showed improvement in HbA 1 c (insulin naïve: −2.3%, insulin users: −1.8%). Major hypoglycaemia was observed in the insulin naïve group after 24 weeks. SADRs were reported in 1.2% of insulin naïve and 2.1% of insulin user groups. Conclusion: Starting or switching to insulin analogues was associated with improvement in glycaemic control with a low rate of hypoglycaemia

Artworks of Post-Revolutionary Iranian Female Photographers: Identification and Analysis
The Iranian Revolution and the Imposed War (Iran-Iraq War 1980-1988) and the years that followed led to inception of various cultural and social conditions in Iran. Following the formation of new intellectual and ideological movements, the art movements underwent many transformations. Consequently, arts universities began to teach various fields of arts, and a new chapter of artistic photography was begun. As the arts universities and institutions developed, people started to view the art movements differently, and this allowed women to play a more active role in this field. The present paper aims to identify Post-Revolution Iranian female photographers and analyze their artworks, investigate the position and direction of the modern photography in Iran, identify the most prominent Post-Revolutionary female photographers, and study their perspective on objects. The author also aims to investigate the subjects most popular among these photographers, examine the effects of Iran's Post-Revolution culture on their artworks, and identify its signs. The artwork collection of five photographers, namely Maryam Zandi, Rana Javadi, Shadi Ghadirian, Azadeh Akhlaghi, and Newsha Tavakolian were considered in this text
Management of diabetes in Morocco: results of the International Diabetes Management Practices Study (IDMPS) – wave 5
Objectives: The International Diabetes Mellitus Practice Study (IDMPS) is a 5-year survey documenting changes in diabetes treatment practices in developing countries. The primary objective of this survey was to assess the therapeutic management of type 2 diabetes mellitus (T2DM) in real-life medical practice. The secondary objectives were to evaluate the clinical management of type 1 diabetes mellitus (T1DM) and to assess the proportion of all diabetic patients failing to reach the glycated haemoglobin (HbA1c) <7% target. Methods: Data were analysed for 738 patients (240 with T1DM and 498 with T2DM) included in wave 5 of the IDMPS in Morocco in 2011. Results: Nearly two-thirds (61%) of T2DM patients were treated with oral glucose-lowering drugs (OGLDs) alone, 13.1% were treated with insulin alone and 23.3% were treated with OGLDs plus insulin. Insulin use was less frequent, was initiated later and involved a greater use of premixes versus basal/prandial schedules compared to other populations evaluated in the IDMPS. The majority (92.5%) of T1DM patients were treated with insulin alone and the remainder received insulin plus an OGLD. Insulin protocols included basal + prandial dosing (37.5%) and premix preparations (41.3%). The recommended target of HbA1c <7% was achieved by only 22.2% of T1DM patients and 26.8% of T2DM patients. More macrovascular but fewer microvascular complications were reported in T2DM compared to T1DM patients. Late complications increased with disease duration so that 20 years after diagnosis, 75.7% of T2DM patients were found to have at least one late complication. Conclusions: The clinical burden of diabetes is high in Morocco and the majority of patients do not achieve the recommended glycaemia target, suggesting that there is a huge gap between evidence-based diabetic management and real-life practice. Better education of patients and improved compliance with international recommendations are necessary to deliver a better quality of diabetic care
Common infections acquired in intensive care: Microbiological aspects and risk factors
Nosocomial infections are a serious health problem resulting in an enormous burden of morbidity and mortality rates, and high health care costs. The various microorganisms implicated in nosocomial infections were not known for causing recalcitrant nosocomial infections, they are opportunistic pathogens and hence pose a challenge to patients especially those with immunocompromised conditions. Patients at the intensive care unit are the most at risk of these hospital-acquired infections The infections usually encountered in intensive care unit (ICU) include urinary tract infection, pneumonia, tuberculosis, gastroenteritis. The main risk factors for these infections can be divided into three key groups: those related to patient characteristics and underlying diseases, those related to the acute disease process, and those related to the use of invasive diagnostic or therapeutic procedures. Incidence of ICU-acquired infections vary between hospitals and according to the type of population studied, being highest in burn units and surgical and trauma ICUs and lowest in coronary care units.The major preventive effort to reducing the risk of nosocomial infections should be focused on hospitals and other health care facilities. The responsible health authority should develop a national (or regional) programme to support hospitals. Such programmes must assess and promote good health care, appropriate isolation, sterilization, and other practices, staff training, and epidemiological surveillance should be developed

Chemical composition, antioxidant activity, and diuretic effect of Moroccan fresh bee pollen in rats

Aim: This study investigated the chemical composition, antioxidant activity, and diuretic effect of Moroccan aqueous extract of fresh bee pollen (AEFBP) in normal rats. Materials and Methods: The chemical composition of the extracted bioactive compounds was assessed using liquid chromatography with diode array detection coupled to electrospray ionization (ESI) tandem mass spectrometry (LC/DAD/ ESI-MSn). 2,2-diphenyl-1-picrylhydrazyl and the reducing power were used to assess the antioxidant properties of the extract, together with the determination of total phenols and flavonoids. To assess the diuretic effect, 20 normal rats were divided into five groups: The first was a control group administered by distilled water (10 mL/kg body weight), the second group received furosemide (10 mg/kg body weight), the third group received 100 mg/kg body weight of AEFBP, the fourth group received 250 mg/kg body weight of AEFBP, and the fifth group received 500 mg/kg body weight of AEFBP for 30 days. Toward the end of this experiment, urine output was measured, and plasma and urine were sampled to analyze creatinine, potassium, chloride, and sodium levels. Results: N1,N5,N10-tri-p-coumaroylspermidine is a spermidine derivative and was the main compound in this sample, in a total of 19 compounds identified, including flavonoids, glucoside flavonoids, and methylated derivatives. Force feeding with the AEFBP induced a significant increase in urine output and urinary electrolyte levels with a dependent dose-effect without changes in plasma electrolytes, whereas furosemide decreased plasma potassium. Conclusion: Moroccan fresh bee pollen extract contains flavonols and spermidines that induce a potential antioxidant activity related to significant diuretic effect without changes in plasma composition.The authors are grateful to the Foundation for

Wild strawberry (Arbutus unedo): Phytochemical screening and antioxidant properties of fruits collected in northern Morocco
The aim of this study was to determine the polyphenolic compounds and the antioxidant ability of Arbutus unedo fruits, collected from three regions of northern Morocco, using high-performance liquid chromatography coupled to diode array and electrospray ionization mass spectrometry detection. The proper extraction method has been selected to achieve this objective. After delipidation, the three harvests were extracted by sonication using two solvents with increased polarity ethyl acetate and MeOH:water, 80:20 (v/v). Total polyphenols, flavonoids, tannins and anthocyanins were respectively: 108.41 +/- 9.29 mg GAE/g (w/w) dry weight (DW), 101.07 +/- 5.6 mg QE/g (w/w) (DW), 0.45 +/- 0.48 mg EC/g (w/w) (DW) and 0.35 +/- 0.48 mg Pg-3-glu/g (w/w) (DW). EC50 values for reducing power and DPPH radical scavenging activities were between 1.37 +/- 0.2 and 17.82 +/- 0.12 mg/mL (w/v). A total of 75 compounds were tentatively identified and some of these had never been found until nowadays in Arbutus unedo. The average amount of antioxidant compounds obtained by semi-quantitative analyses was 120.35 +/- 32.05 mg/100 g (w/w) (DW). The attained results clearly highlight the potential of A. unedo as a source of healthy compounds, which could be advantageously added to the daily diet, making it a potential candidate for the cure for many emerging diseases. Thyroid Ectopia Revealed in the Adulthood: About One Case
Thyroid ectopia is defined as the presence of thyroid tissue outside its normal pre-tracheal location. It’s may be discovered at any age. It’s clinical impacts are highly variable, precocious or late, depending on whether the thyroid is functional or not. We describe from this case an ectopic thyroid revealed in the adulthood. It is about a 44 years old woman with a medical history of goiter under supervision in her sister also followed for a chronic nasolacrimal duct obstruction. She benefitted from orbital and sinus computer tomography that showed a tongue’s base lesion process of with a strong contrast. Tongue MRI objectified a tongue base process measuring 43x37 mm with heterogeneous enhancement after gadolinium injection with total thyroid atrophy. Thyroid scintigraphy 131 confirmed the diagnosis of an ectopic thyroid: There was a rounded hyperfixation, a retro-mandibular median projecting as sub-lingual, without visualing uptake tracer on the thyroid lodge level. Physical examination and hormonal exploration were without abnormalities. In front of the uncomplicated asymptomatic sub-lingual thyroid, regular supervision was recommended based on thyroid hormones dosage and a scintigraphy or MRI
Barriers and facilitators to implementing workplace interventions to promote mental health: qualitative evidence synthesis
Background: Despite growing interest in workplace mental health interventions, evidence of their effectiveness is mixed. Implementation science offers a valuable lens to investigate the factors influencing successful implementation. However, evidence synthesis is lacking, especially for small-to-medium-sized enterprises (SMEs) and for specific work sectors. The objectives of this review are to establish the scope of research with explicit analysis of implementation aspects of workplace mental health interventions and to identify barriers and facilitators to implementation in general and within SMEs and selected sectors. Methods: A systematic scoping review and meta-synthesis of mixed methods process evaluation research from 11 databases, with the evaluation of methodological quality (MMAT) and confidence in findings (CERQual), was conducted. We selected information-rich studies and synthesised them using domains within the Nielsen and Randall implementation framework: context, intervention activities, implementation; and mental models. Results: We included 43 studies published between 2009 and 2022, of which 22 were rated as information-rich to be analysed for barriers and facilitators. Most studies were conducted in healthcare. Facilitators reflecting 'high confidence' included: relevant and tailored content, continuous and pro-active leadership buy-in and support, internal or external change agents/champions, assistance from managers and peers, resources, and senior-level experience and awareness of mental health issues. Healthcare sector-specific facilitators included: easy accessibility with time provided, fostering relationships, clear communication, and perceptions of the intervention. Stigma and confidentiality issues were reported as barriers overall. Due to the small number of studies within SMEs reported findings did not reach 'high confidence'. A lack of studies in construction and Information and Communication Technology meant separate analyses were not possible. Conclusions: There is dependable evidence of key factors for the implementation of workplace mental health interventions which should be used to improve implementation. However, there is a lack of studies in SMEs and in a larger variety of sectors.
