    The most important local and regional treatment techniques on hepatocellular carcinoma and their effect over a long term overall survival

    During the last years, many local and regional techniques have been introduced, helping, together with surgery, to treat the hepatocellular carcinoma, and contributing to an important improvement of disease free survival and overall survival of patients affected by this disease. These techniques, suitable also for metastatic lesions, can be performed as exclusive ones or following surgery, and help controlling tumor progression even when it is over any possible surgical approach. Local and regional therapies can be divided into two groups: in the first one we can consider the techniques using heat to obtain their effect, in the second one we consider those using chemotherapic drugs to obtain necrosis of the neoplastic tissue. Necrosis can be obtained through the energy produced by radiofrequency probes, through the alcohol or acetic acid injection or through the injection of embolyzing substances, also together with chemotherapic drugs, into the hepatic artery. Last but not least, the injection of yttrium labeled microspheres is available. These are injected into the hepatic artery and are able to cytoreduce the tumor through a local irradiation

    [Italiano]: Questo volume rappresenta la finalizzazione di una iniziativa nata, condivisa ed interamente realizzata all'interno del gruppo social dei "Prof in cucina e in Cantina", che ha inteso raccogliere ricette e descrizioni aneddotiche di singoli Autori, appartenenti ai ruoli del personale docente e tecnico-amministrativo-bibliotecario dell'Ateneo federiciano, il più antico Ateneo pubblico al mondo. Aneddoti e ricette forniscono spaccati di vita quotidiana e ricostruzioni di eventi che hanno accompagnato gli Autori nei diversi percorsi formativi, sociali e lavorativi, e che non escludono riferimenti al recente periodo emergenziale della pandemia COVID-19. Ad introdurre il Volume è un menù federiciano, con una proposta di 3 pietanze che attualizzano ricette ed abitubini alimentari dell'epoca, e che hanno come riferimento il Liber de coquina, edito a corte di Federico II. Gli 8 Capitoli del volume raggruppano ricette e aneddoti per categorie, e ciascuno di essi è poi preceduto da un breve inserto con ulteriori riferimenti storici all'alimentazione a Corte di Federico II. Here, the several Authors, represented by the teaching and the technical/administrative/librarian staff of the Federico II University in Naples, the oldest public University in the world, propose recipes and anecdotal descriptions of their educational, social and work-related paths, including some episodes from the recent COVID-19 pandemic. A menu inspired to Frederick II introduces the book, with 3 recipes inspired to the Liber de coquina, a cooking manual compiled at the Emperor's court. Each of the following 8 thematic chapters of the book is also anticipated by a brief insert reporting historical reconstructions of the alimentary habits at the court of Frederick II. Thus, old and new atmospheres, accompanied by the scent of aromas and suggestions inspired by the experience of each Author, come together to offer Readers fragments of everyday life at the court of Frederick II, in the 21st century

    Oncoplastic surgery and cancer relapse: cosmetic and oncological results in 489 patients

    During the past 20 years, breast conservation has become the preferred treatment modality for breast carcinoma, and in recent times there is an increased expectation from breast cancer patients to retain their "normal breast appearance". For large tumor-to-breast ratio excision, the subspecialty of oncoplastic surgery is born, helping to achieve a good oncologic and esthetic result. In our study we have considered 767 patients undergone a mastectomy or quadrantectomy, and especially 489 undergone quadrantectomy. We have used our protocol for breast reshaping and analyzed our data in terms of oncologic safety and esthetic results. Considering the lesions, they were placed like this: 214 (44%) in the SEQ, 58 lesions (12%) in the SIQ, 54 lesions (11%) in the IEQ, 24 lesions (5%) in the IIQ, 45 lesions (9%) respectively in the CQ and between the SQ, 39 lesions (8%) between the EQ, 5 lesions (1%) respectively between the internal quadrants and between the inferior quadrants. We have chosen simple breast reshapings in case of operations on the superior quadrants, while, in case of operations on the inferior quadrants, we have chosen complex techniques, like reshapings according to a "key hole" reductive mammaplasty, which requires also a contralateral reshaping. We have done simple and monolateral reshapings respectively in 372 (76%) and 296 (60.5%) cases. We have had early complications in 98 (20%) cases: 12 infections (2.4%), 10 hematomas (2%), 11 seromas (2.2%), 65 liponecrosis. As late complications, we have found scar retractions and minus areas in 20 cases (4.08%), while we have found asymmetries and bigger deformities in 34 cases (6.95%). We have not found any cancer relapse after one year of follow up, while we have had 3 cases of relapse (0.6%) after 5 years of follow up, respectively after 5, 4 and 2 years. This result has to be attributed to our preoperatory project of surgery derived from many factors, among which stands out the MRI done in all the cases. We think that an immediate breast reshaping following quadrantectomy is the best esthetic and psychologic option for breast cancer patients

    The paper analyses two bilingual (Neapolitan-Italian) sectoral repertoires by Federico Gusumpaur: the Vocabolario ornitologico napolitano-italiano (1874) and the Vocabolario botanico napolitano (1887). After some remarks on the author, whose basic biographical data are unknown, I will describe the main characteristics of the two repertoires: the criteria used, the glossing system, the probable sources, and the destination. I will also focus on the differences between the two vocabularies, which consist in the presence of two alphabetical indices (Latin-Neapolitan and Italian-Neapolitan) in the Vocabolario botanico napolitano and the non-merely local destination of this second repertoire.Il contributo si concentra su due repertori settoriali bilingui (napoletano-italiano) redatti da Federico Gusumpaur: il Vocabolario ornitologico napolitano-italiano (1874) e il Vocabolario botanico napolitano (1887). Dopo alcuni cenni sull’autore, del quale non sono noti i dati biografici fondamentali, si presentano le caratteristiche principali dei due repertori: i criteri adottati, il sistema di glossatura, le probabili fonti, la destinazione. Si mettono inoltre a fuoco le differenze tra i due vocabolari, che consistono nella presenza di due indici alfabetici (latino-napoletano e italiano-napoletano) nel Vocabolario botanico napolitano e nella destinazione non solo locale di questo secondo repertorio
