    Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Anaplastic lymphoma kinase-positive large B-cell lymphoma: Description of a case with an unexpected clinical outcome

    Anaplastic lymphoma kinase-positive (ALK-positive) large B-cell lymphoma is a rare and aggressive variant of large B-cell lymphoma (LBCL), first reported by Delsol et al in 1997, showing distinctive morphologic, immunophenotypic and cytogenetic features. The latest 2008 World Health Organization Classification of Tumours of Haematopoietic and Lymphoid tissues recognizes ALK-positive LBCL as a separate entity. Here, we report a case of ALK-positive large B-cell lymphoma in a 53-year-old man with diffuse abdominal and mediastinal lymph-nodes involvement. According to the Ann Arbor staging system, the patient had a stage IIIB lymphoma. The age-adjusted International Prognostic Index was 2 (stage III and elevated lactate dehydrogenase), so the disease was considered high risk. The patient underwent chemotherapy, radiotherapy, and an autologous stem cell transplantation. The patient is alive and free of disease 35 months after diagnosi

    Caregivers of Patients with Hematological Malignancies within Home Care: A Phenomenological Study

    The role of caregivers in homecare settings is relevant to the patient’s wellbeing and quality of life. This phenomenon is well described in the literature for the oncological setting but not specifically for that of hematological malignancies. The aim of this study was to explore the experience of primary caregivers of patients with hematological malignancies within home care. We conducted a phenomenological study based on interviews with 17 primary caregivers of hematological patients. Analysis of the contents led to the identification of five main themes. Perhaps, the innovative aspects of this study can be summarized in three points: This service was demonstrated to fulfil the ethical aspects of providing the patient with a dignified accompaniment to the end of life. Secondly, the efficiency of the service and the benefit are directly dependent on the caregivers’ wellbeing, so knowledge of the dynamics and emotions involved can lead to the development and implementation of programs for hematological malignancies. Lastly, a collaborative caregivers–professionals relationship can improve a sense of accomplishment for all parties involved, lessening the family’s frustration related to not having done their best. Home care brings significant benefits for both the patient and the caregivers and fulfils the ethical obligation of providing the patient dignified end-of-life care

    Home care of acute leukaemia patients: From active therapy to end-of-life and palliative care. The 3-year experience of a single centre

    Purpose: The aim of the study was to evaluate the feasibility and the potential effects of the Haematological Home Care (HHC) programme for acute leukaemia (AL) patients, either in active chemotherapy or in the terminal phase of disease. Methods: We retrospectively assessed a group of AL patients assisted at home in terms of number of hospitalisations, accesses to emergency department and place of death. We also used historical data to evaluate potential effects of HHC. Results: The study group consisted of 44 patients, 36 of whom (82%) required palliative treatment, and eight (18%) had ongoing active chemotherapy. The mean number of hospitalisations was 0.64 (range 0-7) per patient, and the number of emergency department (ED) visits was 0.82 (range 0-4) per patient. Place of death was at home for 51.4% of patients and in hospital for 40.5%. Considering a historical group of 17 patients assisted at home the rate of hospitalisations and ED visits were 2.53 (range 0-9) and one (range 0-3), respectively. Place of death was home and hospital in 6% and 65%, respectively. Conclusions: Haematological Home Care for AL patients is feasible and has potential positive effects in terms rate of hospitalisations and place of death

    The privilege deed granted by Isabella of Aragon to the Jews of Bari This paper  is the edition of a notarial deed relating to an agreement entered into between the universitas of Bari and the local Jewish community. Among other inserts, the deed contains an unpublished privilege granted in 1522 to the Jews of Bari by its feudal lord, Isabella of Aragon, concerning the safety of the Jewish families, their general assets, and the methods of exercising the activity of moneylending. The author provides a close analysis of the manuscript and a biographical note on Isabella of Aragon which explains the context in which the deed was granted, comparing its articles with similar measures previously issued in favour of Jews living in the Kingdom of Naples.In questo lavoro si presenta l’edizione di un atto notarile relativo a un accordo stipulato tra l’Universitas di Bari e la locale comunità ebraica. L’atto contiene, tra gli inserti, un inedito privilegio concesso nel 1522 agli ebrei di Bari dalla feudataria della città Isabella d’Aragona e relativo alla sicurezza delle loro famiglie, dei loro beni e alle modalità di esercizio dell'attività di prestito. Dopo una attenta analisi del manoscritto, l’autore fornisce cenni biografici su Isabella d’Aragona e si sofferma ad analizzare gli articoli che compongono il privilegio, confrontandoli con provvedimenti simili già emessi in precedenza a favore degli ebrei del regno di Napoli.
