    We thank Chiappa et al. (4,5) for commending our work (2,3,7), which we reciprocate in light of their thought provoking research that sparked the ensuing trans-Atlantic debate on the effects of inspiratory muscle loading on lactate clearance after exercise. Specifically, despite using similar methodologies, Chiappa et al. (4,5) have twice shown accelerated lactate clearance with inspiratory loading, whereas we have twice shown no effect (2,7). We hypothesized that these discrepancies may be due to interstudy differences in participant endurance training status, as evidenced by higher V˙O2peak and faster blood lactate recovery kinetics in our participants. We were thus intrigued by the authors’ unpublished data showing, in sedentary individuals, no effect of inspiratory loading on lactate clearance. In their accompanying figure, the authors also present novel data showing a significant correlation between maximal inspiratory pressure (MIP) and changes in the area under the blood [Laj] curve with inspiratory loading. This observation informed their hypothesis that the efficacy of inspiratory loading is influenced by inspiratory muscle mass rather than training status

    When local food is resourced by restaurants that establish good relationships with other local stakeholders, this can enhance local food production, shape the place identity and image of a destination. While some of these aspects are beginning to receive scholarly attention, there is still too little research that examines the attitude of restaurants towards using and marketing local food as a way of promoting local identity and reinforcing the competitiveness of a destination. This is particularly the case in Italy where, to our best knowledge, there have been no studies published on this topic. This study aims at investigating to what extent restaurant managers are leveraging on local and typical food and what are the main barriers or constraints that, in their view, do not allow them to exploit fully local/typical food products as a positive means of attracting tourists or indeed make the most of opportunities to network with other local stakeholders. To achieve this, the present study draws on quantitative research carried out on a sample of restaurants selected in two Italian regions (Molise and Sardinia).     In environments where endoscopic retrograde cholangio-pancreatogram (ERCP) services may at time be limited or unavailable, the surgeon must also decide between the insertion of a conventional T-tube or primary choledochorrhaphy when it is operatively perceived that the common bile duct (CBD) has been adequately cleared. Where the hospital stay of the patients with a T-tube in situ is prolonged, there is a higher incidence of postoperative bacteraemia with a moderate morbidity from biliary leakage after T-tube withdrawal. We report here a case as well as methods designed to prevent tube dislocation and alternatives to T-tube insertion after choledochotomy, whether performed open or laparoscopically
