Leaf mineral composition of grafted eggplant grown in soil infested with Verticillium and root-knot nematodes [Composição mineral de folhas de berinjela enxertada e cultivada em solos infestados com Verticillium e nematóide-das-galhas]

The objective of this work was to determine differences in leaf mineral composition between ungrafted and grafted onto (Solanum torvum) eggplant (Solanum melongena), cultivars 'Faselis' and 'Pala', grown in a soil infested with Verticillium dahliae and Meloidogyne incognita, or in a noninfested soil. Grafting increased leaf P and Mn concentrations, and decreased N concentrations, in both soils. Grafting also enhanced leaf Ca concentration of 'Pala', but it did not affect that of 'Faselis' depending on the cropping year. Leaf Mg concentration of grafted plants in infested soil was lower than that of ungrafted ones in noninfested soil. Results showed that, under the same fertilization program, the grafted 'Faselis' plants used the nutrients more efficiently than the 'Pala' ones. Use of S. torvum as a rootstock for 'Faselis' resulted in an effective protection against multiple pathogen infestation. Fertilization may be necessary when grafted 'Faselis' plants are grown in a soil infested with the pathogens, since grafting and infestation generally decrease leaf N, Mg, Ca and Fe concentrations, either by reducing the nutrient concentrations directly or by increasing leaf Mn concentration

© 2020 The Authors. ESC Heart Failure published by John Wiley & Sons Ltd on behalf of European Society of Cardiology Aims: Hypokalaemia is a risk factor for ventricular arrhythmias and sudden death in ambulatory patients with chronic heart failure (HF). The objective of this study was to examine the association between hypokalaemia and outcomes in hospitalized patients with decompensated HF in whom sudden death is less common. Methods and results: Of the 5881 hospitalized patients with HF, 1052 had consistent hypokalaemia (both admission and discharge serum potassium \u3c4.0 mmol/L), and 2538 had consistent normokalaemia (both admission and discharge serum potassium 4.0–5.0 mmol/L). Propensity scores for consistent hypokalaemia, estimated for each of 3590 (1052 + 2538) patients, were used to assemble a matched cohort of 971 pairs of patients with consistent hypokalaemia vs. consistent normokalaemia, balanced on 54 baseline characteristics (mean age, 75 years; 60% women; 28% African American). We repeated the above process to assemble 2327 pairs of patients with discharge potassium \u3c4.0 vs. 4.0–5.0 mmol/L and 449 pairs of patients with discharge serum potassium \u3c3.5 vs. 4.0–5.0 mmol/L. Hazard ratios (HR) and 95% confidence intervals (CIs) associated with hypokalaemia were estimated in matched cohorts. 30 day all-cause mortality occurred in 5% and 4% of patients with consistent normokalaemia vs. consistent hypokalaemia, respectively (HR, 0.78; 95% CI, 0.52–1.18; P = 0.241). HRs (95% CI) for 30 day mortality associated with discharge serum potassium \u3c4.0 and \u3c3.5 mmol/L were 0.90 (0.70–1.16; P = 0.419) and 1.69 (0.94–3.04; P = 0.078), respectively. Hypokalaemia (\u3c4.0 or \u3c3.5 mmol/L) had no association with long-term mortality or other outcomes. Conclusions: In hospitalized older patients with HF, compared with normokalaemia (serum potassium 4.0–5.0 mmol/L), hypokalaemia (\u3c4.0 or \u3c3.5 mmol/L) had no significant associations with outcomes

Common Secondary Causes of Resistant Hypertension and Rational for Treatment

    No full text
Resistant hypertension is defined as uncontrolled blood pressure despite the use of three antihypertensive drugs, including a diuretic, in optimal doses. Treatment resistance can be attributed to poor adherence to antihypertensive drugs, excessive salt intake, physician inertia, inappropriate or inadequate medication, and secondary hypertension. Drug-induced hypertension, obstructive sleep apnoea, primary aldosteronism, and chronic kidney disease represent the most common secondary causes of resistant hypertension. Several drugs can induce or exacerbate pre-existing hypertension, with non-steroidal anti-inflammatory drugs being the most common due to their wide use. Obstructive sleep apnoea and primary aldosteronism are frequently encountered in patients with resistant hypertension and require expert management. Hypertension is commonly found in patients with chronic kidney disease and is frequently resistant to treatment, while the management of renovascular hypertension remains controversial. A step-by-step approach of patients with resistant hypertension is proposed at the end of this review paper

The reflective writing class blog: Using technology to promote reflection and professional development

    No full text
INTRODUCTION: The hidden (informal) curriculum is blamed for its negative effects on students\u27 humanism and professional development. To combat this, educational initiatives employing mentored reflective practice, faculty role-modeling, and feedback have been advocated. AIM: Promote reflection on professional development using collaborative, web-based technology. SETTING: Four-week basic medicine clerkship rotation at an academic institution over a one-year period. PROGRAM DESCRIPTION: Students were asked to contribute two reflective postings to a class web log (blog) during their rotation. They were able to read each other\u27s postings and leave feedback in a comment section. An instructor provided feedback on entries, aimed to stimulate further reflection. Students could choose anonymous names if desired. PROGRAM EVALUATION: Ninety-one students wrote 177 posts. One-third of students left feedback comments. The majority of students enjoyed the activity and found the instructor\u27s feedback helpful. Assessment of the posts revealed reflections on experience, heavily concerned with behavior and affect. A minority were not reflective. In some cases, the instructor\u27s feedback stimulated additional reflection. Certain posts provided insight to the hidden curriculum. DISCUSSION: We have discovered that blogs can promote reflection, uncover elements of the hidden curriculum, and provide opportunities to promote professional development. © 2008 Society of General Internal Medicine
