Functionalised DNA - introducing and applying a versatile porphyrin molecular ruler
Porphyrin moieties were rigidly attached to DNA to generate an accurate molecular ruler. Molecular ruler analysis was conducted using steady-state fluorescence, circular dichroism and small angle X-ray scattering spectroscopic techniques, in an attempt to analyse the FRET, exciton coupling and scattering intensity between different porphyrin-porphyrin labelled DNA combinations. A 21-mer test sequence was labelled with a porphyrin in one position on one strand, and seven different positions on seven complementary strands, to overall give seven porphyrin-porphyrin inter-strand combinations. Steady-state fluorescence and circular dichroism spectroscopic analysis of the Soret band revealed individual Watson-Crick bases pair molecular ruler sensitivity. Small angle X-ray scattering attempts between metallated-porphyrin entities did not reveal sufficient scattering at low concentrations, in contrast, an iodinated analogue of the porphyrin system did displayed scattering correlating to different iodine iodine distances. After calibration of the porphyrin system, the moieties were applied to study protein-DNA interactions between Tus, a 36 KDa DNA binding protein, and Ter, a specific 21-mer DNA sequence. Molecular ruler nalysis of the complex required an extended version of the Ter DNA sequence to which modifications were attached. Established FRET pairs FAM and TAMRA were applied to investigate protein-DNA complexation. Native PAGE analysis revealed Tus binds to the extended DNA via a sliding mechanism. Fluorescence analysis of the established FRET pairs identified changes in fluorescence not correlating to changes in FRET, and instead was attributed to emission quenching upon protein binding. Applying the zinc and free base porphyrin version displayed subtle changes in the Soret band circular dichrosim upon complexation, indicating small DNA helical change upon complexation. A 45-mer DNA sequence was designed to form multiple hairpin-duplex conformations with the addition of an appropriate complementary strand. Attaching FRET pairs to the extremes of the DNA sequence enabled multiple DNA conformations, and hence FRET distances to be obtained from one doubly modified DNA sequence. The combinations were characterised by UV-Vis, fluorescence and circular dichroism spectroscopy. Finally, terpyridine labelled DNA sequences selectively formed DNA nanotubes through orthogonal hydrogen bonding and metal complexation interactions. Short DNA strands were designed to self-assemble into long duplexes through a sticky-end approach. Addition of weakly binding metals such as zinc induced the formation of tubular arrays consisting of DNA bundles 50-200 nm wide and 2-50 nm high. Factors Influencing Provision of Care to Hospitalized Peadiatrics Burn Patients: A Qualitative Study Among Nurses in Muhimbili National Hospital Dar es salaam, Tanzania
The increase number of burn patients‘ admission in Muhimbili National Hospital indicated that burn injury is still a problem in our setting. In July 2011 up to June 2012 a total of 6135 paediatrics patients were admitted in MNH due to various diseases of which 319 patients were admitted due to burn injury. This is equal to 5.2% of all paediatrics admission (unpublished report). However hospital management made good coordination and availability of working equipments in order to archive optimal care for paediatric burn patients. Even though Health care providers especially nurse were facing some difficulties in provision of burn care. The main objective of this study was to explore nurses‘ perceptions on factors influencing provision of care to pediatric burn patients among nurses at Muhimbili National Hospital. Five in depth interviews were conducted in order to explore nurses‘ perception on factors that may influence provision of nursing care to hospitalized pediatric burn patients. This study was conducted from May 2012 to June 2012 in MNH. Simple observation method was used to complement data obtained through in-depth interviews. The study informants were sampled by using purposive sampling procedure. Data was analyzed by using content analysis approach. Findings The present study revealed two major categories which include motivating factors and barriers in provision of burn care. Participants described the use of closed method of wound dressing as an important skill that accelerates healing, decrease risk of wound contamination and decreased number of contractures. Presence of team work in burn care, facilitated patients recovery through reviewed and discussed management of patients as a team. Availability of equipments helped nurses to perform nursing care smoothly. Furthermore, participants felt gratified and increased work performance when they saw patients who sustained very severe burn injury recovering and going back home. Despite the fact that burn care was found somehow successfully in Muhimbili National Hospital (MNH), participants revealed several factors that hindered provision of care. These factors include limited resources, patient workload, and lack of standard skills. Participants described that there were limited human and non human resources, such as shortage of staffing which resulted into overworking, physical and emotional exhaustion among nurses, lack of water which is the mainstay of infection prevention control, lack of specimen equipment like pus swab. Since burn patient are at risk of infection therefore they need to take pus swab frequently. Nevertheless, participants expressed that they lack standard skills on burn care since there was no special training on burn care. Instead they were teaching themselves in the sense that experienced nurses in burn unit were teaching new nurses who came to work in burn unit. The finding of this study revealed that there are both positively and negatively factors that influence provision of burn care. Positive factor (motivation) needed to be maintained but action is required to be taken in order to reduce negative factors. More public health enlightenment is needed on prevention and initial intervention for burns in children. Community need to be aware that prevention of burn should be a priority since caring for burnt patient is very expensive. Further studies are needed in large population since this information is not conclusive to factors influence burn care in Tanzania.\ud

Gender differences in self-reported late effects, quality of life and satisfaction with clinic in survivors of lymphoma
Objectives: gender differences in perceived vulnerability to late effects and views about follow-up among cancer survivors have received little attention. As lymphoma affects both genders similarly, we compared the consequences of cancer (late effects, perceived vulnerability and quality of life (health-related quality of life (HRQoL)), and satisfaction with clinic visits between genders.Methods: a cohort of 115 younger adults (18–45 years, >5 years disease-free survival), who had been treated for lymphoma participated. Questionnaires (n = 91) were completed before and after (n = 62) routine consultant-led appointments. Survivors (n = 24) without appointments were recruited by post. Questionnaires included HRQoL, late effects, perceived vulnerability, issues survivors wanted to discuss and reported discussing in clinic, time waiting in clinic and consultation satisfaction.Results: there were no gender differences in number of self-reported late effects or perceived vulnerability. Men with more late effects reported worse psychological HRQoL (r = 0.50, p<0.001). While men wanted to discuss more topics than they did, women were able to discuss the topics they wanted (ANOVA, p = 0.01). Multiple regression analyses showed a shorter wait in clinic (r = ?0.46, p = 0.009) and discussing more topics (r = 0.34, p = 0.06) explained 30.6% of the variance in consultation satisfaction for men.Conclusions: issues surrounding follow-up provision are increasingly important given the length of survival in young adults following treatment for lymphoma. Men may experience poor psychological well-being due to distress about unanswered concerns. Consideration of their concerns should be prioritised, given that satisfaction and ultimately continued attendance at clinic and HRQoL may be dependent on the extent to which follow-up meets survivors' expectation
Na-ca Exchange And Ca Fluxes During Contraction And Relaxation In Mammalian Ventricular Muscle
There are four cellular Ca transport systems which compete to remove Ca from the myoplasm in mammalian ventricular myocytes. These are 1) the SR Ca-ATPase, 2) the sarcolemmal Na-Ca exchange, 3) the sarcolemmal Ca-ATPase and 4) the mitochondrial Ca uniporter. Using multiple experimental approaches we have evaluated the dynamic interaction of these systems during the normal cardiac contraction-relaxation cycle. The SR Ca-ATPase and Na-Ca exchange are clearly the most important, quantitatively; however, the relative roles vary in a species-dependent manner. In particular, the SR is much more strongly dominant in rat ventricular myocytes, where ~ 92% of Ca removal is via SR Ca-ATPase and only 7% via Na-Ca exchange during a twitch. In other species (rabbit, ferret, cat, and guinea pig) the balance is more in the range of 70% SR Ca-ATPase and 25-30% Na-Ca exchange. Ferret ventricular myocytes also exhibit an unusually strong sarcolemmal Ca-ATPase. During the steady state the same amount of Ca must leave the cell as enters over a cardiac cycle. Beyond Lesson Studies and Design Experiments: Using theoretical tools in practice and finding out how they work
This paper aims to illustrate how fruitful insights into the link between school teaching practice and student learning outcomes can be theoretically grounded by the variation theory from the field of phenomenography; and from this framework demonstrate how a 'pedagogy of awareness' can be implemented in the classroom. In this study, five teachers and 162 students at Primary Four level of school education in Hong Kong participated and the practice of the 'learning study' was adopted. By comparing the results of pre- and posttests, a significant gain was observed in the students learning outcomes.
