Meanings and More...: Policy Brief of the ICCA Consortium no. 7
In 2018 the Council of the ICCA Consortium decided to develop a lexicon of meaningful, and at times complex, concepts and terms frequently used in its work, policies and relations with its Members and Partners. A few specific papers had been commissioned and prepared before, but no attempt had been made to collate working definitions of frequent use, while many felt a need for such a reference compendium. This need was evident also because the Consortium has highlighted and adopted new ways of referring to phenomena that, historically, had not been conceptually analysed. First among these phenomena are the very ICCAs—territories of life at the heart of the Consortium’s work. This document is the result of the Council’s decision. It is a rich beginning, expected to evolve and be further integrated and enriched in the years to come. We decided to publish it as part of our Policy Brief Series, as many of the concepts and terms collected here are crucial in the policies of the Members, Partners and international institutions that the Consortium is hoping to positively influence through its work. Most of all, this document is meant to stimulate thinking, debate, joint analysis, learning and action within the Consortium’s membership. It is only as a result of these collective discussions and concrete initiatives that the ‘meanings’ of our collective language will emerge (and evolve). The concepts and terms collated here are relatively few. We thus decided to list them not in alphabetical order but in our suggested order of reading or glancing through (one of the many possible ones). Anteriormente se habían encargado y preparado algunos documentos específicos, pero no se había hecho ningún intento de recopilar definiciones de trabajo de uso frecuente, mientras que muchos sentían la necesidad de un compendio de referencia de este tipo. Esta necesidad era evidente también porque el Consorcio ha puesto de relieve y adoptado nuevas formas de referirse a fenómenos que, históricamente, no se habían analizado conceptualmente. El primero de estos fenómenos son las propias ICCA-territorios de vida que constituyen el núcleo del trabajo del Consorcio. Este documento es el resultado de la decisión del Consejo. Es un rico comienzo, que se espera evolucione y se integre y enriquezca aún más en los años venideros. Decidimos publicarlo como parte de nuestra serie Policy Brief, ya que muchos de los conceptos y términos aquí recogidos son cruciales en las políticas de los Miembros, Socios e instituciones internacionales en las que el Consorcio espera influir positivamente a través de su trabajo. Artificial intelligence enhanced ultrasound (AI-US) in a severe obese parturient: a case report
BACKGROUND: Neuraxial anesthesia in obese parturients can be challenging due to anatomical and physiological modifications secondary to pregnancy; this led to growing popularity of spine ultrasound in this population for easing landmark identification and procedure execution. Integration of Artificial Intelligence with ultrasound (AI-US) for image enhancement and analysis has increased clinicians' ability to localize vertebral structures in patients with challenging anatomical conformation. CASE PRESENTATION: We present the case of a parturient with extremely severe obesity, with a Body Mass Index (BMI) = 64.5 kg/m(2), in which the AI-Enabled Image Recognition allowed a successful placing of an epidural catheter. CONCLUSIONS: Benefits gained from AI-US implementation are multiple: immediate recognition of anatomical structures leads to increased first-attempt success rate, making easier the process of spinal anesthesia execution compared to traditional palpation methods, reducing needle placement time for spinal anesthesia and predicting best needle direction and target structure depth in peridural anesthesia

Abstract
SiC/SiC ceramic matrix composites (SiCf/SiC CMCs) are regarded as the new materials for the hot-section components of aircraft gas turbine engines, since they have one-third of the density of metallic superalloys, a higher temperature capability, good mechanical strength, and excellent thermal shock resistance. However, high-temperature water-vapor-rich combustion gases can induce severe surface recession phenomena in SiC/SiC leading to component failure. For this reason, it is necessary to design protective coatings, i.e., environmental barrier coatings (EBCs), able to protect the SiC/SiC surface in combustion environments. In the present work, ytterbium monosilicate (Yb2SiO5), stable when exposed to water vapor at high temperatures, and ytterbium disilicate (Yb2Si2O7), characterized by a thermal expansion coefficient closer to that of the substrate, were selected for a multilayer EBC system. EBCs were processed using the atmospheric plasma spray (APS) technique. A set of deposition parameters were tested, varying the power of the torch, and the composition and microstructure of the deposited coatings were studied in terms of porosity, crack density, and post-deposition phase retention by performing SEM, EDS, and XRD analysis. The results allow for the definition of the influence of deposition parameters on the final properties of multilayer EBC coatings

L'ampia analisi proposta tocca tutte le opere dell'autrice evidenziandone i legami intertestuali, anche tra poesia e narrativa, e i progressivi sviluppi in un arco cronologico esteso, tra anni Sessanta e primi anni Duemila. Lo studio coglie anche l'importanza dei riferimenti alle arti visive, in particolare alla fotografia, che Giulia Niccolai ha praticato in prima persona negli anni della Neoavanguardia, e alla pittura americana.The essay places the narrator and poet Giulia Niccolai in the canonical twentieth century Italian literary discussing her poetics and stylistic figures. The wide analysis proposed touches all the works of the author highlighting the intertextual links, also between poetry and narrative, and the progressive developments in an extended chronological period, between the Sixties and early Twenties. BDC-Decomposition for global influence analysis
In biochemical networks, the steady-state input-output influence is the sign of the output steady-state variation due to a persistent positive input perturbation; if the sign does not depend on the value of the strictly positive system parameters, the influence is structural. As recently shown for small perturbations, when the linearized system approximation is valid, steady-state input-output influences can be structurally assessed, for biochemical networks with m unknown parameters, by means of a vertex algorithm with complexity 2m. This letter shows that the structural input-output influence of a biochemical network is a global property, which does not require any small-perturbation assumption. It also shows that, using a new algorithm, the complexity can be reduced down to 2m-n , where n is the system order, thus drastically reducing the computation time. Finally, when the uncertain parameters belong to known intervals, non-conservative bounds are given for the steady-state ratio between output and input, allowing for sensitivity analysis.Accepted Author ManuscriptTeam Tamas Keviczk
Utilizzo ed efficacia della ventilazione meccanica non invasiva in pazienti con trauma toracico puro: studio retrospettivo del biennio 2017-2018
Introduzione: Il trauma è la terza causa di morte nella popolazione adulta mondiale ed in particolare il 25-35% di queste morti sono da attribuire al trauma toracico. Il motivo per cui le lesioni toraciche destano grande preoccupazione è che possono compromettere la respirazione e la funzione cardiovascolare. La gravità del trauma toracico influenza i tempi e la priorità di intervento; tale gravità è espressa all’interno di sistemi di valutazione dei traumi globali o specifici per il torace, come il Thoracic Trauma Severity Score (TTSS).
Prima dell’avvento della ventilazione non invasiva (NIV), i pazienti con trauma toracico potevano essere ventilati esclusivamente con metodiche invasive; a partire dagli anni ‘80 sono state pubblicate alcune revisioni sistematiche e linee-guida che affrontato il tema della NIV nel trauma toracico. L’uso della NIV o CPAP, quando non controindicato, è da prendere in considerazione perché non gravato dalle complicazioni della ventilazione invasiva.
Scopo dello studio: A nostro giudizio, il TTSS potrebbe essere di ausilio al clinico come indicatore per l’utilizzo dei diversi supporti ventilatori (ossigenoterapia, CPAP, NIV, ventilazione invasiva) in funzione della gravità del quadro clinico.
Materiali e metodi: Sono stati arruolati nello studio 78 pazienti ricoverati presso la U.O. Anestesia e Rianimazione 6 – P.S. nel periodo compreso fra il 01/01/2017 e il 31/12/2018, con diagnosi di trauma toracico puro o prevalente in politrauma. I suddetti pazienti sono stati divisi in 4 gruppi (A: solo NIV; B: NIV e successiva ventilazione invasiva; C: solo ventilazione invasiva; D: solo ossigenoterapia), sulla base del tipo di supporto ventilatorio ricevuto durante il ricovero. Per tutti i pazienti, inoltre, è stato anche registrato il Glasgow Coma Scale (GCS).
Risultati: Solo 69 pazienti sono risultati eleggibili per lo studio. Per ognuno di questi sono riportate le caratteristiche generali, il valore del TTSS, scomposto nelle sue diverse componenti, e il valore di pressione positiva di fine espirazione (PEEP) utilizzato.
Discussione e conclusioni: Nel gruppo C la scelta di avviare sin da subito la ventilazione invasiva risulta congrua al quadro clinico generale e di insufficienza respiratoria di tali pazienti, sebbene presentino un valore più basso di TTSS (10,67 ± 4,16), hanno però anche un valore più basso di GCS (11,86 ± 3,68). Il gruppo D è il campione con il miglior quadro respiratorio e ha coerentemente beneficiato solo dell’ossigenoterapia (TTSS: 9,64 ± 2,98 e GCS: 14,52 ± 1,05). Nei gruppi A e B, pur essendo il valore globale del TTSS simile: 11,87 ± 2,1 (A) e 11,625 ± 2,7 (B), c’è differenza nelle sue componenti, presentando il gruppo A maggior gravità per le fratture costali e il gruppo B maggior gravità per contusione e scambio gassoso e questo spiegherebbe lo shift verso la ventilazione invasiva. Inoltre, il GCS risulta peggiore, segno di maggiore compromissione neurologica (A:14,8 ± 0,77 vs B: 13 ± 2,67).
Infine, solamente 2 sono i pazienti deceduti, entrambi appartenenti al gruppo B, quello relativo al fallimento della NIV, probabilmente a causa di un errore di valutazione da parte del clinico nell’aver protratto troppo a lungo il tentativo con NIV.
Concludendo, emerge un particolare interesse verso quei malati in cui l’evidenza clinica sottolinea prevalentemente un quadro contusivo; in tal senso, ci proponiamo di ampliare il campione al fine sia di individuare una possibile migliore gestione del trattamento sia di presentare dati statisticamente significativi
