Infection in Indonesian Population
We measured the accuracy of the urine test (RAPIRUN) for detection of Helicobacter pylori infection in Indonesia (Jakarta, Pontianak, and Jayapura) using histology confirmed by immunohistochemistry and/or culture as gold standards. We also used immunohistochemistry to identify CagA phenotype and analyzed H. pylori CagA diversity in Indonesia. The overall prevalence of H. pylori infection in 88 consecutive dyspeptic patients based on the urine test was 15.9 (14/88), 38.1 for patients in Jayapura that had higher prevalence of H. pylori infection than that in Jakarta (9.7, P = 0.02) and Pontianak (8.3, P = 0.006). Overall sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of RAPIRUN were 83.3, 94.7, 71.4, 97.3, and 93.2, respectively. All of the H. pylori-positive patients were immunoreactive for anti-CagA antibody but not immunoreactive for East Asian specific anti-CagA antibody in all H. pylori-positive subjects. We confirmed the high accuracy of RAPIRUN in Indonesian population. In general, we found less virulent type of H. pylori in Indonesia, which partly explained the low incidence gastric cancer in Indonesia. © 2015 Ari Fahrial Syam et al

Recurrent Acute Pancreatitis as A Manifestation of Sphincter of Oddi Dysfunction
Recurrent acute pancreatitis (RAP) is defined as two or more occurance of acute pancreatitis with no evidence of underlying chronic pancreatitis. Prevalence of RAP varied from 10-30%. One of the postulated mechanism of this condition is sphincter of Oddi dysfunction (SOD) which is a clinical biliary pain syndrome or acute pancreatitis (AP) due to pancreatobiliary obstruction at the level of sphincter of Oddi. We reported a 29-year-old female patient who came to Cipto Mangunkusomo Hospital regarding upper quadrant abdominal pain with previously well documented history of AP in the last six months before admission. Laboratory findings showed elevated pancreatic enzyme level which was consistent with AP. The patient underwent magnetic resonance cholangio-pancreatography (MRCP) and endoscopic ultrasound (EUS) examination and both of the results showed dilatation of pancreatic duct which suggested SOD. Due to the lack of further diagnostic modality, manometry was not performed on this patient. However, after excluding other possible etiology of SOD, the patient underwent endoscopic retrograde cholangio-pamcreatograhy (ERCP) and sphincterotomy was performed. The signs and symptoms of AP was relieved after sphincterotomy and not yet recurred.</jats:p
Management of Recurrent Cholangitis in Patient with Iatrogenic Bile Duct Stricture
Iatrogenic bile duct stricture is the most common causes of benign bile duct stricture. Several studies reported that approximately 80% of benign strictures occur following injury during a cholecystectomy. Strictures of the biliary tract have a broad spectrum of manifestations, ranging from mild elevation of liver enzymes to life-threatening infections such as cholangitis, liver abscess, and biliary cirrhosis. Moreover, due to its indolent course with subtle clinical manifestations, diagnosis is often delayed and most patients present with a protracted, complicated course. We reported a case of 26 years old female with recurrent cholangitis due to iatrogenic bile duct stricture. Biliary drainage through endoscopic approach followed by surgical procedure was selected as strategic management for the patient.</jats:p
Diagnosis dan Tata Laksana Enteropati akibat Obat Anti Inflamasi Non Steroid (OAINS)
Obat anti inflamasi non steroid (OAINS) merupakan salah satu golongan obat yang paling banyak dan paling sering diresepkan di Indonesia. Meskipun mampu mengurangi gejala inflamasi, penggunaan OAINS juga menimbulkan gangguan dalam mekanisme pertahanan mukosa saluran pencernaan. Kerusakan mukosa saluran pencernaan akibat OAINS tidak hanya dialami lambung, namun juga dialami oleh usus halus yang disebut enteropati. Namun demikian, enteropati akibat OAINS masih belum mendapat perhatian yang besar.</jats:p
Nutritional Management in Acute Pancreatitis
Acute pancreatitis induce a catabolic stress that increase systemic inflammatory response with worsening nutritional status. Current approach in acute pancreatitis therapy was still symptomatic because of no definitive therapy yet to prevent any inflammatory and proteolytic cascade. One of the most important thing to consider in acute pancreatitis therapy was nutritional management. “Pancreatic rest” concept that formerly used have been known to increase cost, sepsis incidence due to catheter use, and also metabolic and electrolyte disorder. Nowadays, “gut rousing” concept was preferable compared to “pancreatic rest” concept, support that nutritional management was needed to stimulate and generate intestinal function. Enteral nutrition administration have to consider patient’s hemodynamic status. Necrosis incidence, respiratory failure, intensive care, and mortality was found to be lower in patients given enteral nutrition in first 48 hours compared to after 48 hours. Nutrition administration via nasogastric tube or nasojejunal tube was still in doubt while several studies showed that nasogastric tube administration was safe and tolerated, otherwise could be evaluated in larger population sample study. Nutrition and metabolic monitoring was also an important part to reach nutritional goals and reduce complications.</jats:p
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.
