Genetic features of platinum-resistant ovarian cancer patients

Abstract
The authors try to decide a problem of ovarian cancer resistance to platinum drugs by the way of correlation finding between platinum-resistance of tumor and presence of gene mutations in the patient. It was shown a variety of options for BRCA gene mutations in patients with ovarian cancer: BRCA 1 (185delAG) - 64,2%, BRCA 1 (5382 insC) - 55,7%, and BRCA 2 (6174delT) - 53,8%. Authors discovered a significant positive correlation between carriage of mutations in the BRCA genes 1/2 and the sensitivity of malignant ovarian tumors to chemotherapy with platinum. Mutations in these genes occurred significantly more often in patients with platinum-sensitive ovarian cancer.

Реферат
Автор, на основании анализа результатов исследования гомеостатистических механизмов (детоксикационная функция печени, цикл оксида азота; обмен серусодержащих аминокислот) у 121 больной раком яичника и разной чувствительностью к препаратам платины, приходит к заключению, что реализация генетически определенной чувствительности рака яичника к препаратам платины связана с этими механизмами. Автор полагает, что активность цикла оксида азота влияет на уровень эндогенной интоксикации и устойчивость организма к внешним воздействиям. Уровень содержания SS-SH групп и их связь с белковыми или липопротеидными комплексами, очевидно, определяет уровень сульфид-платиновых комплексов, а состояние детоксикационной системы, возможно, определяет жизни таких комплексов и эффективность лечения, соответственно.

ROLE hOMEOSTATISTIC MECHANISMS IN THE FORMATION OF PERFORMANCE PLATINUM TREATMENT OF PATIENTS WITH OVARIAN CANCER

Abstract
The author, based on the analysis results of the study gomeostatisticheskih mechanisms (liver detoxification function, nitric oxide cycle, exchange of sulfur-containing amino acids) in 121 patients with ovarian cancer and different sensitivities to drugs of platinum, comes to the conclusion that the implementation of a genetically determined sensitivity of ovarian cancer to the drugs of platinum associated with these mechanisms. The author believes that the activity of nitric oxide cycle affect the level of endogenous intoxication and the body's resistance to external influences. The level of SS-SH groups content and their relation to protein or lipoprotein complexes, obviously determines the level of sulphide-platinum complexes, and the state of detoxification system may determine the lifetime of such systems and the effectiveness of treatment, respectively.

PATIENTS WITH THE VULVAR CANCER

Summary
Vulvar cancer is a rare malignant tumor, accounting for 3-8% of the total incidence of malignant tumors of the female reproductive system. However, in the structure of mortality, this pathology ranks fourth after cancer of the cervix, uterine body and ovaries and amounts to 18.2%. Features of the anatomical structure of the external genital organs of a woman with a developed system of blood and lymphatic vessels cause an aggressive course, a tendency to metastasis and rapid growth of a tumor of this localization. Prospects for the timely diagnosis of vulvar cancer and vaginal tumors are directly related to an increase in literacy and oncological alertness of staff in general medical institutions, in the implementation of which not only the detection of the disease in the early stages, but also work with women from risk groups to refer them to further examination and, if necessary, for treatment in a specialized institution. High mortality from these diseases is associated with the appeal of women at the late stages of the development of the oncological process, as well as with the insufficient effectiveness of the therapy, which necessitates the development of research in this area.

THE QUALITY OF LIFE OF PATIENTS WITH OVARIAN CANCER
AS AN INDICATOR OF FUNCTIONAL ASSESSING
THE EFFECTIVENESS OF ANTICANCER THERAPY

Abstract
The aim of this study was to evaluate quality of life in patients with platinumresistant ovarian cancer, depending on the pharmacoresistance and differentiated use of chemotherapy. It is shown that the output value by the subscales of the questionnaires EORTC QLQ-C30 and FACT-G in patients assigned to different clinical groups were comparable. When using a differentiated approach in the treatment of patients with ovarian cancer significantly improved performance on physical scales (PF), role (RF) and emotional functioning (EF). In addition, patients from III and IV groups had decreased intensity of nausea (NV) and general weakness (FA). With differentiated application of chemotherapeutic agents with metabolic support total score by FACT-G questionnaire was in Group III 77,8±0,9 points, and in the IV group - 77,9±0,8 points, which was significantly higher than obtained in the first and group II - 72,2±1,2 and 71,6±0,9 points. Described differences were held during all follow-up observation.

Neuropeptide Y is an essential in vivo developmental regulator of cardiac I-Ca,I-L
Cell culture studies demonstrate an increase in cardiac L-type Ca2+ current (I-Ca,I-L) density on sympathetic innervation in vitro and suggest the effect depends on neurally released neuropeptide Y (NPY). To determine if a similar mechanism contributes to the postnatal increase in I-Ca,I-L in vivo, we prepared isolated ventricular myocytes from neonatal and adult mice with targeted deletion of the NPY gene (Npy(-/-)) and matched controls (Npy(+/+)). Whole-cell voltage clamp demonstrates I-Ca,I-L density increases postnatally in Npy(+/+) (by 56%), but is unchanged in Npy(-/-). Both I-Ca,I-L density and action potential duration are significantly greater in adult Npy(+/+) than Npy(-/-) myocytes, whereas I-Ca,I-L density is equivalent in neonatal Npy(+/+) and Npy(-/-) myocytes. These data indicate NPY does not influence ICa,L prenatally, but the postnatal increase in I-Ca,I-L density is entirely NPY-dependent. In contrast, there is a similar postnatal negative voltage shift in the I-V relation in Npy(+/+) and Npy(-/-), indicating NPY does not influence the developmental change in I-Ca,I-L voltage-dependence. Immunoblot analyses and measurements of maximally activated I-Ca,I-L (in presence of forskolin or BayK 8644) show that the differences in current density between Npy(+/+) and Npy(-/-) cannot be attributed to altered Ca2+ channel alpha(1C) subunit protein expression. Rather, these results suggest that the in vivo NPY-dependent postnatal increase in I-Ca,I-L density in cardiac myocytes results from regulation I-Ca,I-L properties by NPY
Current ideas about molecular genetic subtypes of ovarian cancer: a personalized approach and a new platform for further research
Highly malignant ovarian cancers are a histopathological diagnosis, but can be multiple diseases at the molecular level. Research aimed at identifying molecular genetic subtypes of ovarian cancer is being conducted to find an answer to the question: can different molecular subgroups influence the choice of treatment? One of the achievements of this direction is the recognition of the dualistic theory of the origin of ovarian carcinomas with their division into High-grade and Low-grade subtypes. However, the data of sequencing of the tumor genome suggest the existence of 6 subtypes of carcinoma, including two LG and
four HG subtypes. Patients of subtype C1 are characterized by a high stromal response and have the lowest survival, tumors of C2 and C4 subtypes have a higher rate of intratumoral CD3 + cells, lower stroma gene expression and better survival than C1. The mesenchymal subtype C5 is widely represented by mesenchymal cells, characterized by overexpression of N-cadherins and P-cadherins, low expression of differentiation markers and lower survival
than C2 and C4. The use of a consensus algorithm to determine the subtype allows the identification of only a minority of ovarian cancers (approximately 25%). In this regard, the practical significance of this classification still requires additional research, and today it is permissible to talk about the existence of only 2-3 reproducible subtypes. It is thought that it makes sense to randomize tumors into groups with altered expression of angiogenic genes and with overexpression of immune response genes, as in the angiogenic group there is a comparison of the advantage in survival (prescribing bevacizumab improves it, and in the
immune group even increases bevacizumab). Molecular subtypes with poorer survival rates (proliferative and mesenchymal) also benefit most from bevacizumab treatment. The review focuses on some advances in understanding molecular, cellular, and genetic changes related to ovarian cancers with the results achieved so far in describing molecular subtypes of ovarian cancer. The available information is the basis for planning further research
High-Q resonances with low azimuthal indices in all-dielectric high-index nanoparticles
Recently, a novel class of high-Q optical resonators based on all-dielectric subwavelength nanoparticles with high refractive index has been proposed [M. V. Rybin, et al, arXiv:1706.02099, 2017]. Here we study a complex spectrum of such nanoscale resonators by means of the resonant-state expansion, treating the problem as a nonHermitian eigenproblem. We show that the high-Q features can be described within the mechanism of external coupling of open channels via the continuum.
