    Utilization of Multi-Criteria Decision Making in the Selection of Mechanical Ventilators in Intensive Care Units: A Case Study

    Tıbbi ekipmanın hasta sağlığıyla doğrudan bağlantılı olduğu düşünüldüğünde, güvenilir bir tedarikçinin titizlikle seçilmesi büyük önem taşımaktadır. Yoğun bakım üniteleri bağlamında, sağlık teknolojisi değerlendirmesi ve sağlık hizmeti kararları alma süreci çeşitli kriterlere dayanmakta, kanıtlarla desteklenmekte ve ilgili paydaşların farklı bakış açılarından etkilenmektedir. Çok kriterli karar analizi (ÇKKV), bu prosedürü düzenlemek ve bir dizi bakış açısını dikkate almak için umut verici bir çerçeve sağlar. Bu araştırmanın temel amacı, yoğun bakım ünitelerindeki paydaşların sağlık müdahalelerini değerlendirirken bakış açılarını ve tercihlerini araştırmaktır. Bu incelemede, bir vaka çalışması olarak mekanik ventilatörü değerlendirmek için çok kriterli bir yaklaşım kullanılmıştır. 3. Basamak Eğitim ve Araştırma Hastanesi için ventilasyon cihazı tedarik problemi üzerine yapılan araştırmada, bulanık VIKOR ve bulanık EDAS yöntemleri kullanılarak 44 kriterle değerlendirilen 4 alternatif firma arasından en uygun olanın seçilmiştir. Sonuçlara göre, her iki yöntem de alternatif 3'ü en iyi seçenek olarak belirlemiş ve literatüre çift kriterli karmaşık varsayımlı karar verme tekniklerinin uygulanması konusunda bir katkı sunmuştur.Given that medical equipment is directly linked to patient well-being, the meticulous choice of a reliable supplier is of paramount importance. Within the context of intensive care units, health technology assessment and the process of making healthcare decisions are grounded in various criteria, substantiated by evidence, and influenced by the diverse viewpoints of involved stakeholders. Multi-criteria decision analysis (MCDA) provides a promising framework for organizing this procedure and considering a range of perspectives. The primary goals of this research were to investigate the outlooks and preferences of stakeholders in intensive care units as they evaluate healthcare interventions. This examination utilized a multi-criteria approach to assess a mechanic ventilator as a case study. In the research on the ventilation device procurement problem for the 3rd Level Training and Research Hospital, fuzzy VIKOR and fuzzy EDAS methods were used to select the most suitable one among 4 alternative companies evaluated with 44 criteria. According to the results, both methods identified alternative 3 as the best option and made a contribution to the literature on the application of dual criteria complex assumption decision making techniques

    Impact of IL-6 rs1800795 and rs1800796 polymorphisms on clinical outcomes of COVID-19: a study on severity of disease in Turkish population

    Coronavirus disease (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is exacerbated by cytokine storms, leading to severe inflammation. Interleukin-6 (IL-6) plays a critical role in this process, and variations in its promoter may influence disease severity. This study aims to investigate the relationship between IL6 promoter polymorphisms rs1800795 (G > C) and rs1800796 (G > C) and the severity of COVID-19 in the Turkish population. A total of 332 participants were included: 84 control, 80 with mild COVID-19, and 168 with severe COVID-19. IL6 polymorphisms were genotyped using the restriction fragment length polymorphism (RFLP) method. The genotypes rs1800795 GC (OR = 3.00, 95% CI: 1.669-5.398, p < 0.000), CC (OR = 7.44, 95% CI: 2.899-19.131, p < 0.000), and rs1800796 GC (OR = 2.76, 95% CI: 1.603-4.761, p < 0.000), as well as the alleles rs1800795 C (OR = 3.01, p < 0.000) and rs1800796 C (OR = 1.97, p = 0.002), may be associated with the severity of COVID-19. According to the Jonckheere-Terpstra (J-T) test, the most significant trends that vary linearly with disease severity were observed for D-dimer [J-T = 15.896, Effect size = 0.68 (0.61 to 0.76), p < 0.000] and CRP [J-T = 15.389, Effect size = 0.66 (0.59 to 0.73), p < 0.000]. The distribution of clinical parameters across genotype combinations (rs1800796/rs1800795*) showed that GC/GC* and GC/CC* were linked to a higher risk of severe inflammation, clotting, and organ damage. Additionally, it has been determined that the G-C and C-C haplotypes may be associated with increased severity of COVID-19.     Higher education students' perceptions of ChatGPT: A global study of early reactions

    The paper presents the most comprehensive and large-scale global study to date on how higher education students perceived the use of ChatGPT in early 2024. With a sample of 23,218 students from 109 countries and territories, the study reveals that students primarily used ChatGPT for brainstorming, summarizing texts, and finding research articles, with a few using it for professional and creative writing. They found it useful for simplifying complex information and summarizing content, but less reliable for providing information and supporting classroom learning, though some considered its information clearer than that from peers and teachers. Moreover, students agreed on the need for AI regulations at all levels due to concerns about ChatGPT promoting cheating, plagiarism, and social isolation. However, they believed ChatGPT could potentially enhance their access to knowledge and improve their learning experience, study efficiency, and chances of achieving good grades. While ChatGPT was perceived as effective in potentially improving AI literacy, digital communication, and content creation skills, it was less useful for interpersonal communication, decision-making, numeracy, native language proficiency, and the development of critical thinking skills. Students also felt that ChatGPT would boost demand for AI-related skills and facilitate remote work without significantly impacting unemployment. Emotionally, students mostly felt positive using ChatGPT, with curiosity and calmness being the most common emotions. Further examinations reveal variations in students' perceptions across different socio-demographic and geographic factors, with key factors influencing students' use of ChatGPT also being identified. Higher education institutions' managers and teachers may benefit from these findings while formulating the curricula and instructions/regulations for ChatGPT use, as well as when designing the teaching methods and assessment tools.     Ekran Süresinin İşlenmiş Gıda Tüketimine Etkisi

    Today, with technological developments, many technological devices have gained an important place in the vital activities of individuals. In addition to these technological devices, especially devices such as mobile phones, televisions, computers and tablets have been associated with the concept of screen time. Individuals have started to spend a long time on many screen types such as various applications, media, video games. This screen time may cause changes in individuals' eating behaviours. The aim of this study is to determine the effect of screen time on processed food consumption. For this purpose, interview method, which is one of the qualitative research techniques, was used. The sample group of this study consists of university students. In order to ensure sufficient diversity of views, volunteer students from the first to the fourth year of the university from different study disciplines were preferred. The interviews (n = 10) were conducted between 20.01.2025-22.01.2025 using a semi-structured form. Each interview lasted approximately 30 minutes. With the interviews, thematic categories related to the foods consumed by university students during the screen were determined. The notes taken during the interviews were analyzed in accordance with qualitative research methods to identify emerging themes. As a result of the data obtained, thematic analysis was carried out and 3 different themes were determined. These themes are the feeling of sharing, the idea of saving and the idea of dependency. In addition, as a result of the findings obtained from the study, it was determined that the participants spent most of their screen time on mobile phones and consumed processed foods in most of these periods.Günümüzde teknolojik gelişmelerle birlikte birçok teknolojik alet bireylerin yaşamsal faaliyetlerinde önemli bir yer edinmiştir. Bu teknolojik aletlerin yanı sıra özellikle cep telefonu, televizyon, bilgisayar, tablet gibi aletler ekran süresi kavramı ile ilişkilendirilmiştir. Bireyler çeşitli uygulamalar, medyalar, video oyunları gibi birçok ekran türlerinde uzun süre vakit geçirmeye başlamıştır. Bu geçirilen ekran süresi bireylerin yeme davranışlarında değişikliğe sebep olabilmektedir. Bu çalışmanın amacı ekran süresinin işlenmiş gıda tüketimine etkisinin belirlenmesidir. Bu amaç doğrultusunda nitel araştırma tekniklerinden olan görüşme metodundan yararlanılmıştır. Bu çalışmanın örneklem grubunu üniversite öğrencileri oluşturmaktadır. Yeterli görüş çeşitliliğini sağlamak için, farklı çalışma disiplinlerinden üniversitenin birinci sınıfından dördüncü sınıfına kadar olan gönüllü öğrenciler tercih edilmiştir. Araştırma kapsamındaki görüşmeler (n = 10) 20.01.2025-22.01.2025 tarihleri arasında yarı yapılandırılmış form ile gerçekleştirilmiştir. Her bir görüşme yaklaşık 30 dakika sürmüştür. Yapılan görüşmelerle birlikte üniversite öğrencilerinin ekran süresince tükettikleri yiyeceklere ilişkin tematik kategoriler belirlenmiştir. Görüşmeler sırasında tutulan notlar, ortaya çıkan temaları belirlemek amacıyla nitel araştırma yöntemlerine uygun şekilde analiz edilmiştir. Elde edilen veriler neticesinde tematik analiz gerçekleştirilmiş ve 3 farklı tema belirlenmiştir. Bu temalar paylaşım hissi, tasarruf düşüncesi ve bağımlılık düşüncesidir. Ayrıca çalışmadan elde edilen bulgular neticesinde katılımcıların ekran sürelerini en çok cep telefonlarında geçirdikleri, bu sürelerin büyük bir kısmında da işlenmiş gıdalar tükettikleri belirlenmiştir

    Hidden threats: Brucellosis diagnosis and co-infection patterns in Crimean-Congo Hemorrhagic Fever suspects

    Purpose: This study aims to investigate the frequency of brucellosis in patients with Crimean-Congo Hemorrhagic Fever (CCHF). Method: In this study, 1231 patients were evaluated retrospectively, including 14 patients with CCHF and brucellosis coinfection and 25 patients with brucellosis alone. Statistical methods such as the Mann-Whitney U test and Fisher's exact test were used. Result: Of the patients with a preliminary diagnosis of Crimean-Congo Hemorrhagic Fever (CCHF), 3.2% were found to have brucellosis. In the group co-infected with brucellosis and CCHF, the time to hospital admission was shorter. Additionally, tick exposure and headache were observed more frequently in this group. Creatine kinase levels were found to be higher in the co-infected group, while lymphocyte counts and fibrinogen levels were lower compared to the group with brucellosis alone. Conclusion: Our study revealed a 3.2% rate of brucellosis in patients with a preliminary diagnosis of CCHF. Due to the similarity of symptoms and common risk factors, the differential diagnosis of brucellosis should be considered in patients diagnosed with CCHF. Additionally, the possibility of brucellosis coexisting with CCHF should always be considered. Even in patients diagnosed with CCHF, an evaluation for brucellosis must be conducted

    Urease immobilization on alginate-based composite micro-beads

    The aim of this research is to investigate the contribution of incorporating Aloe Vera (Aloe barbadensis) (Av), sepiolite (Sp) and carrageenan (Cr) to alginate beads in immobilizing urease enzyme. For this purpose, only alginate (A), AAv, ASp and ACr composite beads were synthesized. Simultaneously, the enzyme-immobilized beads (A-U, AAv-U, ASp-U and ACr-U) were formed by co-synthesis in the presence of urease. All prepared beads were characterized by FTIR analysis and SEM images. The optimum pH and temperature values of the immobilized enzyme activity were determined and compared with those of the free enzyme. The optimum pHs were 7 for free enzyme, AAv-U, Asp-U enzymes, 8 for ACr, and 4.5 for A-U. While the optimum temperature for free enzyme was around 20 °C, immobilized enzymes showed high activity at higher temperatures. In addition, the re-usability and storage stability of the enzyme-immobilized beads were investigated at two different temperatures and in two different environments.     Bu çalışma, Erzincan ilinde görev yapan ortaöğretim müzik öğretmenlerinin görüşleri doğrultusunda, 2018 yılında güncellenmiş ve mevcut olarak kullanılan ortaöğretim müzik öğretim programının Türk müziği içeriklerinin uygulanabilirliğini tespit etmeyi amaçlamaktadır. Araştırmanın çalışma grubunda 2023-2024 eğitim öğretim yılında Millî Eğitim Bakanlığı'na bağlı Erzincan ilinde ortaöğretim kurumlarından görev yapan 9 müzik öğretmeninin görüşlerine yer verilmiştir. Çalışmada yöntem olarak nitel araştırma yöntemi kullanılmış, verileri toplama aracı olarak ise araştırmacı tarafından oluşturulan yarı yapılandırılmış görüşme formu katılımcılara uygulanmıştır. Katılımcılara halihazırda kullanılan öğretim programı ve bu programda yer alan Türk müziği içerikleriyle ilgili 17 soru sorulmuş, sorular alt problemlere göre sınıflandırılmıştır. Verilerin analizinde içerik analizi yöntemi kullanılarak frekans ve yüzde değerleri verilmiştir. Elde edilen veriler doğrultusunda; öğrencilerin hazır bulunuşluk düzeylerinin yetersiz olduğu ve Türk müziğine ilgi göstermedikleri, müzik öğretmenlerinin Türk müziği konusundaki bilgilerinin yetersiz olduğu ve Türk müziği kazanımlarını anlatırken zorluk yaşadıkları, müzik dersinin tüm sınıflarda işlenmemesinin programın uygulanmasında sorunlar yarattığı ve Türk müziği kazanımlarını uygularken fiziki ortam ile materyallerin yetersiz olduğu dair sonuçlara ulaşılmıştır. Öğretmenlerin görüşme sorularına vermiş olduğu cevaplardan yola çıkarak, ortaöğretim programının Türk müziği içeriklerinin uygulanabilir olmadığı tespit edilmiştir. Anahtar Sözcükler: Müzik öğretim programı, Öğretmen görüşü, Türk müziği, Kazanım.This study aims to evaluate the applicability of the Turkish music content within the updated and currently used secondary education music curriculum of 2018, based on the opinions of secondary education music teachers working in Erzincan province. The study group includes the opinions of 9 music teachers from secondary education institutions in Erzincan province, affiliated with the Ministry of National Education, during the 2023-2024 academic year. The study was designed using qualitative research methods. A semi-structured interview form, created by the researcher, was used as the data collection tool and administered to the participants. A total of 17 questions related to the currently used curriculum and the Turkish music content within this program were asked to the participants. The questions were categorized according to sub-problems. Content analysis was used to analyze the data, and frequency and percentage values were provided. Based on the obtained data, it was determined that the preparedness levels of students were insufficient, students showed no interest in Turkish music, music teachers' knowledge of Turkish music was inadequate, music teachers faced difficulties in explaining the Turkish music achievements included in the program, the fact that music lessons are not taught in every class created problems in implementing the program, and physical environment and materials were inadequate when applying the achievements. Based on the responses of the teachers to the interview questions, it was concluded that the Turkish music content in the current secondary education curriculum is not applicable

    Green bonds and financial markets: Interdependence across different market situations

    This study examines the multifaceted return interdependence between green bonds and various assets and markets (conventional bonds, Islamic and conventional stocks, Bitcoin, Ethereum, crude oil, and gold), covering various market conditions. Daily data are from January 11, 2016 to February 12, 2024, and the methods employed include quantile-on-quantile return connectedness (QQC), cross-quantilogram (CQ), and Granger causality in quantiles (GC), which allow us to make inferences about the safe-haven ability of green bonds. The QQC results establish that the connectedness at directly related quantiles is more pronounced than those of inversely related quantiles for all cases. The degree of return connectedness is highest with conventional bonds, followed by gold and stocks. Green bonds act as the net transmitter of spillover shocks to Bitcoin and gold markets, whereas it serves as the receiver of shocks from the remaining markets. The CQ results indicate that positive returns in green bonds significantly follow negative returns in conventional bonds 5 days later, suggesting that green bonds have a one-week lagged safe-haven ability. The GC from green bonds is bidirectional and mostly intensified in extreme quantiles with oil, cryptocurrencies, and conventional stocks, but unidirectional and visible with Islamic stocks during market downturns

    Cationic dye biosorption from aqueous solution using sunflower tray and process optimization

    In recent years, the level of water resources around the world has been decreasing due to global warming, climate change and increasing consumption. In addition, the developing industry pollutes existing water resources. This situation negatively affects all living things on earth. In this study, the biosorption process of cationic dye methylene blue (MB), which was selected as a model dye compound from residual water, and sunflower tray biosorbent was discussed. It was seen that the amorphous structure of the sunflower tray was similar to the hexagonal structure. The process parameters were investigated to reach the highest biosorption efficiency. The highest efficiency (99%) was achieved at pH 6, biosorbent dose 10 g/L, dye concentration 10 mg/L and temperature 20 degrees C. It was determined that the biosorption process with an exothermic reaction was compatible with the Langmuir isotherm (R2 = 0.96) and the pseudo-second-order (PSO) kinetic model (R2 = 0.99). qmax was calculated as 117.6 mg/g.     The role of intrinsic spirituality, emotion dysregulation, and resilience on post-traumatic stress disorder symptoms in university students who survived earthquake

    Purpose: The study aimed to explore the relationship between intrinsic spirituality, emotion dysregulation, resilience, and PTSD in Turkish university students who were earthquake survivors. In addition, the validity and reliability of the Brief Emotion Dysregulation Scale (BEDS) were evaluated among Turkish university students who were earthquake survivors. Methods: A sample of 651 Turkish university students who were earthquake survivors participated in an online survey distributed via WhatsApp, completing the Intrinsic Spirituality Scale (ISS), the Brief Emotion Dysregulation Scale (BEDS), the Brief Resilience Scale (BRS), and the Post-Traumatic Stress Disorder Checklist for DSM-5. Structural equation modeling and mediation analysis were employed to explore the predictive relationships between these psychological variables and PTSD symptoms. Results: The findings confirmed that the BEDS is a valid and reliable scale for assessing emotion dysregulation levels in Turkish university students who were earthquake survivors. Intrinsic spirituality had a negative correlation between emotion dysregulation and PTSD, indicating that higher levels of PTSD are associated with lower intrinsic spirituality. A positive correlation was found between intrinsic spirituality and resilience, suggesting that increased intrinsic spirituality is linked to higher levels of resilience. Furthermore, emotion dysregulation and resilience were found to mediate the relationship between intrinsic spirituality and PTSD. Moreover, emotion dysregulation has a mediating effect in the relationship between resilience and PTSD. Conclusions: The study highlights the determinant effects of intrinsic spirituality on emotional dysregulation, resilience and PTSD in Turkish university students who survived the earthquake. Interventions and support programs designed to reduce PTSD symptoms should benefit from the results of this research, and future research should include longitudinal studies to assess the long-term effects of intrinsic spirituality on PTSD. It is also crucial to examine the effects on various demographic groups, including differences in age, education level, and socio-economic status
