    Salivary IL-33 and sST2 levels in relation to TLR2 rs111200466 polymorphism and periodontitis

    Abstract Objectives Toll-like receptor-2 (TLR2) signalling pathway is involved in the regulation of interleukin (IL)-33 and its receptor suppression of tumorigenicity-2 (ST2). This study aimed to compare salivary IL-33 and soluble ST2 (sST2) levels of periodontitis patients with those of periodontally healthy individuals in relation to their TLR2 rs111200466 23-bp insertion/deletion polymorphism within the promoter region.Materials and Methods Unstimulated saliva samples were collected, and periodontal parameters were recorded from 35 periodontally healthy individuals and 44 periodontitis patients. Non-surgical treatments were applied to periodontitis patients, and sample collections and clinical measurements were repeated 3 months following therapy. Salivary IL-33 and sST2 levels were measured with enzyme-linked immunosorbent assay kits, and TLR2 rs111200466 polymorphism was detected by polymerase chain reaction.Results Elevated salivary IL-33 (p = 0.007) and sST2 (p = 0.020) levels were observed in periodontitis patients, in comparison to controls. sST2 levels declined 3-months following treatment (p < 0.001). Increased salivary IL-33 and sST2 levels were found to be associated with periodontitis, with no significant relation to the TLR2 polymorphism.Conclusion Periodontitis, but not TLR2 rs111200466 polymorphism, is associated with elevated salivary sST2 and possibly IL-33 levels, and periodontal treatment is effective in reducing salivary sST2 levels

    Toxic Leadership Scale Development in Hospitality Sector and Scale Item-Bias Analysis

    Abstract The aim of this study is to develop a valid and reliable toxic leadership scale for the hospitality sector. The research was carried out on two sample groups of employees working in four- and five-star hotels in Alanya, Turkey. Both groups have similar characteristics. There were 336 participants in the first sample group and 623 participants in the second sample group. In order to test the construct validity of the toxic leadership scale, an exploratory factor analysis and 27% sub-upper group comparisons were performed on the first sample group's data. A confirmatory factor analysis and a differential item functioning analysis were performed on the second sample group's data. The reliability of the scale was tested. From the analysis of the psychometric properties of the toxic leadership scale it was determined that the scale has high construct validity and reliability

    Angioedema after favipiravir treatment: Two cases

    Drug-induced angioedema often affects elderly patients with chronic drug use. Proper diagnosis and prompt with effective treatment reduce mortality. With the increasing use of favipiravir, many side effects, especially increased alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels due to liver toxicity, and also skin lesions are reported. First patient oral favipiravir treatment on the second day was ad-mitted to the hospital with rash and swelling on the eyelids. Second patient suffered from significant swelling on the upper lip and displayed angioedema. In this cases, angioedema findings regressed after rapid diagnosis and parenterally administered antihistamine and steroid treatment. Although there is no effective drug therapy in the treatment of COVID-19, favipiravir is also included in the treatment protocols in many countries. Clinicians should be careful about the side effects and possible skin manifestations, especially including angioedema, related to the use of favipiravir

    Demographic and microbiological characteristics of tuberculous lymphadenitis and other extrapulmonary tuberculosis cases

    ABSTRACT Objective: Tuberculosis (TB) is a significant public health problem that remains important worldwide. This study aimed to examine the demographic characteristics and clinical features of patients with extrapulmonary tuberculosis (EPTB) and also identify the comorbidities of EPTB. Our secondary aim was to compare tuberculous lymphadenitis (TBL) with other forms of EPTB. Patients and Methods: This single-center, retrospective, observational study was conducted on patients diagnosed with EPTB in a training hospital in Turkey between January 1, 2011 and December 31, 2020. The demographic characteristics and clinical features of the patients were examined. TBL cases were compared with other forms of EPTB cases. Results: The most common clinical form was TBL. Fever, night sweats, and accelerated erythrocyte sedimentation rate were significantly more common in the other forms of the EPTB group compared to TBL. The rate of tuberculin skin test positivity was significantly higher in patients with TBL. The complication rate was 3%, and mortality rate was 4% among all patients. Conclusions: Extrapulmonary tuberculosis should be considered in the differential diagnosis by evaluating the medical history of patients who present with organ-specific symptoms, especially in TB endemic countries. The clinic may differ according to the organ involvement. TBL may be seen with milder symptoms compared to other clinical forms

    Danışman: Prof. Dr. Arzu ÖzdinçlerBu çalışma Konnektif Doku Masajı ve Klasik Masajın migren hastalarında atak sıklığı, atak süresi, atak şiddeti, uyku, depresyon ve yaşam kalitesi üzerindeki etkinliğini gözlemlemek ve iki tedavi yöntemini karşılaştırmak amacıyla planlandı. 18 yaş üzeri 30 migren tanılı hasta randomize bir şekilde, Konnektif Doku Masajı grubu (KDM) ve Klasik Masaj grubu (KM) olarak iki gruba ayrıldı. Her iki gruptaki hastalara 8 hafta süreyle haftada 2 seans olmak üzere uygulama yapıldı. Hasta Bilgi Formu, Vizüel Analog Skala (VAS), Migren Özürlülük Değerlendirilme Ölçeği (MİDAS), Migrende 24 Saatlik Yaşam Kalitesi Ölçeği, Pittsburgh Uyku Kalitesi İndeksi (PUKİ), Beck Depresyon Envanteri (BDÖ) ve Global Değişim Ölçeği (GDÖ) kullanıldı. Çalışmanın sonuçlarına göre; atak sıklığı ile atak süresinde, müdahale öncesi ve sonrası gruplar arası karşılaştırmada ve grup içi karşılaştırmalarında anlamlı farklılık bulunamadı (p>0,05). Atak şiddeti için kullanılan VAS skorlarında ve özürlülük değerlendirmesi için kullanılan MIDAS skorlarında müdahale öncesi ve sonrasında; gruplar arası karşılaştırmada anlamlı bir farklılık bulunmazken grup içi karşılaştırmalarda her iki grupta da anlamlı farklılık bulundu (p0,05). GDÖ skorlarında anlamlı derecede memnuniyet elde edilirken iki grup arasında anlamlı bir farklılık bulunamadı (p>0,05). Migren hastalarında Konnektif Doku Masajı ile Klasik Masajın etkin bir tedavi yöntemi olduğu ancak birbirine üstünlüğü olmadığı görüşündeyizThis study was planned to examine the effects of Connective Tissue Massage and Classical Massage on migraine patients, to observe their effectiveness on attack frequency, attack duration, attack severity, sleep, depression and quality of life, and to compare the two treatment methods. Thirty migraine patients over 18 years of age were randomly divided into two groups as the Connective Tissue Massage group (CTM) and the Classic Massage group (CM). The patients in both groups were treated for 8 weeks, 2 sessions per week. Patient Information Form, Visual Analog Scale (VAS), Migraine Disability Assessment Scale (MIDAS), The 24-Hour Migraine Quality of Life Questionnaire, Pittsburgh Sleep Quality Index (PUKI), Beck Depression Inventory (BDI) and Global Rating Scale (GRS) were used. According to the results of the study; No significant difference was found between the frequency of attacks and the duration of attacks, before and after the intervention, and in-group comparisons (p>0.05). In the VAS scores used for attack severity and MIDAS scores used for disability assessment, before and after the intervention; While there was no significant difference in the comparison between groups, a significant difference was found in both groups in the in-group comparisons (p0.05). While significant satisfaction was obtained in GRS scores, no significant difference was found between the two groups (p>0.05). We can conclude that Connective Tissue Massage and Classical Massage are effective treatment methods in migraine patients, but they are not superior to each other

    Biruni index as a novel diagnostic tool for the early prediction of mortality in critical patients with COVID-19: A cohort studyn

    Background: The aim of this study was to find out the potential risk factors associated with mortality in severe coronavirus disease 2019 (COVID-19) patients hospitalized due to viral bronchopneumonia, and to establish a novel COVID-19 mortality index for daily use. Methods: The study included 431 quantitative real-time polymerase chain reaction (qRT-PCR)-confirmed COVID-19-positive patients admitted to the intensive care unit in a tertiary care hospital. Patients were divided into training and validation cohorts at random (n= 285 and n= 130, respectively). Biruni Index was developed by multivariate logistic regression analysis for predicting COVID-19-related mortality. Results: In univariate logistic regression analysis, age, systolic and diastolic blood pressures, respiratory and pulse rates per minute, D-dimer, pH, urea, ferritin, and lactate dehydrogenase levels at first admission were statistically significant factors for the prediction of mortality in the training cohort. By using multivariate logistic regression analysis, all of these statistically significant parameters were used to produce Biruni Index. Statistically significant differences in Biruni Index were observed between ex and non-ex groups in both training and validation cohorts (P < 0.001 for both comparisons). Areas under receiver operating characteristic (ROC) curve for Biruni Index were 0.901 (95CI%: 0.864-0.938, P < 0.001) and 0.860 (95CI%: 0.795-0.926, P < 0.001) in training and validation cohorts, respectively. Conclusion: As a pioneering clinical study, Biruni Index may be a useful diagnostic tool for clinicians to predict the mortality in critically ill patients with COVID-19 hospitalized due to severe viral bronchopneumonia. However, Biruni Index should be validated with larger series of multicenter prospective clinical studies

    A mathematical model of the ongoing coronavirus disease (COVID-19) pandemic: A case study in Turkey

    Turkey reported the first case of COVID-19 on 11 March 2020 since the outbreak of the deadly coronavirus pandemic. COVID-19 spread rapidly in Turkey, where about a total of 3,208,173 cases of infected persons were registered by 29 March 2021 with 2,957,093 cases of recovered persons and 31,076 reported deaths. A new mathematical COVID-19 model containing six classes is presented. Also, the positive invariant region of the solutions, basic reproductive number, disease-free equilibrium, and its stability are highlighted. Afterward, the disease-free equilibrium is locally asymptotically stable when R0 < 1. Moreover, the proposed model was further generalized to the fractional-order derivative in the AtanganaBaleanu (ABC) context for a more successful realization. Besides, the existence and uniqueness of solutions via techniques of Schaefer’s and Banach fixed point theorems were established. Based on the publicly recorded number of infected people from 1-31 July 2020 in Turkey and least-squares curve fitting techniques with fminsearch function the fractionalorders model has been validated and can better fit the data compared with the integer-order model. Also, using the Atangana-Toufik scheme, numerical solutions, as well as simulations, are presented for different values of fractional order

    Danışman: Prof. Dr. Nezihe Kızılkaya BejiBu araştırmanın amacı; üniversite öğrencilerinin Prekonsepsiyonel bilgi ve tutumlarının değerlendirilmesidir. Araştırma Biruni Üniversitesi'nde Ocak 2020-Nisan 2020 tarihleri arasında yürütülmüş tanımlayıcı türde bir çalışmadır. Araştırmanın evrenini, Biruni Üniversitesi Sağlık Bilimleri Fakültesi 3. ve 4. sınıfta okuyan kadın öğrenciler oluşturmuştur. Çalışmada 195 öğrenciye ulaşılabilmiştir. Çalışma verileri Tanıtıcı Bilgi formu ve Prekonsepsiyonel Bilgi ve Tutum Ölçeği kullanılarak elde edilmiştir. Veriler Google Forms üzerinden online olarak toplanmıştır. Veriler SPSS 22.0 programı ile analiz edilmiştir. Analizler parametrik olmayan testler (ikili gruplarda Mann-Whitney U, ikiden fazla gruplarda Krusskal-Wallis testi) ile yapılmıştır. Öğrencilerin Prekonsepsiyonel Bilgi ve Tutum Ölçeği ve alt boyutlarından aldıkları ortalama ve standart sapma puanı 173.47±6.21; alt boyut 1'den alınan ortalama ve standart sapma puanı 40.34±1.22; alt boyut 2'den 28.91±1.71; alt boyut 3'ten, 20.47±1.44; alt boyut 4'ten, 32.04±1.94; alt boyut 5'ten, 20.1±1.73; alt boyut 6'dan, 19.5±1.91; alt boyut 7'den ise 12.04±0.74 olarak saptandı. Araştırmada; öğrencilerin %46,5'inin aile planlamasını gebelikten korunma olarak tanımladıkları, düzenli ilaç kullanan öğrencilerin Prekonsepsiyonel puan ortalamalarının (174.06±5.95) ilaç kullanmayanların ortalama puanlarından (170.17±6.71) daha yüksek olduğu belirlendi. Kronik hastalığı olmayan öğrencilerin puan ortalamaları (12.08±0.71) kronik hastalığı olan öğrencilerin puan ortalamalarından (11.75±0.84) daha yüksek bulundu. Düzenli ilaç kullanmayan gruptaki öğrencilerin ölçek, alt boyut-4 ve alt Boyut-7 puan ortalamaları (174.06±5.95, 32.23±1.81, 12.12±0.51) düzenli ilaç kullanan öğrencilerin puan ortalamalarından (170.17±6.71, 31±2.32, 11.62±1,4) daha yüksek bulundu. Öğrencilerin Prekonsepsiyonel Bilgi ve Tutum Ölçeği puanlarının yüksek olduğu ve prekonsepsiyonel bilgi ve tutumlarının da olumlu yönde olduğu belirlendi.The purpose of this research; evaluation of preconceptional knowledge and attitudes of university students. The research is a descriptive study conducted at Biruni University between January 2020 and April 2020. The universe of the research consisted of female students studying in the 3rd and 4th grades of the Faculty of Health Sciences of Biruni University. In the study, 195 students could be reached. Study data were obtained using the Introductory Information Form and the Preconceptional Knowledge and Attitude Scale. Data were collected online via Google Forms. The data were analyzed with the SPSS 22.0 program. Analyzes were made with non-parametric tests (Mann-Whitney U in paired groups, Krusskal-Wallis test in groups of more than two). The mean and standard deviation scores of the students from the Preconceptional Knowledge and Attitude Scale and its sub-dimensions were 173.47±6.21; mean and standard deviation score from sub-dimension 1 was 40.34±1.22; 28.91±1.71 from sub-dimension 2; from sub-dimension 3, 20.47±1.44; from sub-dimension 4, 32.04±1.94; from sub-dimension 5, 20.1±1.73; from sub-dimension 6, 19.5±1.91; it was determined as 12.04±0.74 in sub-dimension 7. In the research; It was determined that 46.5% of the students defined family planning as contraception, and the preconceptional mean scores of the students who regularly use drugs (174.06±5.95) were higher than the mean scores of those who did not use drugs (170.17±6.71). The mean score of the students without chronic disease (12.08±0.71) was found to be higher than the mean score of the students with chronic disease (11.75±0.84). Scale, sub-dimension-4 and sub-dimension-7 mean scores of the students in the group who do not regularly use drugs (174.06±5.95, 32.23±1.81, 12.12±0.51) 170.17±6.71, 31±2.32, 11.62±1.4) were found to be higher. It was determined that the Preconceptional Knowledge and Attitude Scale scores of the students were high and their preconceptional knowledge and attitudes were also positive

    Eustachian tube dysfunction in children with adenoid hypertrophy: The effect of ıntranasal azelastine-fluticasone spray treatment on middle ear ventilation and adenoid tissue

    Objectives: Eustachian tube dysfunction (ETD) is frequent in children with adenoid hypertrophy (AH). Although the most common treatment of AH is surgical removal of adenoid tissue, numerous studies have reported the efficacy of intranasal steroids. The effects of the intranasal steroid and azelastine combination on AH and ETD have not been reported before. In this study, we tried to determine the effects of 3-month intranasal Azelastine-Fluticasone dipropionate combination (Aze-Flu) treatment in children with ETD and AH. Materials and Methods: 100 children who had open mouth sleep, snoring, and sleep apnea and were diagnosed with AH and ETD participated in this study. The mean age was 7.73 ± 2.37 (4–14 years). The rates of adenoid tissue hypertrophy and choanal occlusion were evaluated using a rigid pediatric nasal endoscope and reassessed after 3 months of Aze-Flu nasal spray treatment. The function of the Eustachian tube (ET) was evaluated before and after medical treatment using the Eustachian tube score, the Eustachian dysfunction test-7 (ETS-7) and tubomanometry (TMM). Results: The results were evaluated in 100 patients with AH and ETD. The adenoid tissue to choana rate was 82% before treatment and decreased to 37% after treatment. The ETS-7 test score was 6.36 before treatment and increased to 9.72 at the end of 3 months. Both the regression of the adenoid tissue and the improvement in the Eustachian function scores were statistically significant (p < 0.05). Conclusions: AH significantly increases the frequency of ETD. In this study, it was observed that Aze-Flu treatment was significantly effective in both regression of the adenoid tissue and Eustachian tube dysfunction. We believe that it can be applied as an initial therapy in children with AH and associated ETD

    Extended exp (-φ (ξ))-expansion method for some exact solutions of (2+1) and (3+1)-dimensional constant coefficients KdV equations

    We gained some exact solutions of the (2+1)- and (3+1)-dimensional constant coefficients KdV equations by using extended exp(−φ(ξ))-expansion method. These solutions we gained are hyperbolic, trigonometric, exponential and rational solutions. In addition, we provided both two-dimensional and three-dimensional graphics of the solutions we found in −10≤x≤10 and −1≤x≤1 intervals. Then, we observed, via Mathematica 11.2, that the solutions prove the equations. This method has been used for obtaining exact travelling wave solutions of nonlinear partial differential equations (NLPDEs)
