The effect of distraction method applied with finger puppet onpain level and emotional indicators of children during venous blood collection procedure
ÖZET
Araştırma, kan alma işlemi sırasında parmak kuklası ile uygulanan dikkati başka yöne çekmenin çocukların ağrı düzeyi ve emosyonel göstergelerine etkisini belirlemek amacıyla randomize kontrollü deneyselsel olarak gerçekleştirildi. Araştırmanın evrenini Eylül 2021 - Mart 2022 tarihleri arasında İstanbul’da bulunan bir hastanenin çocuk acil servisine başvuran 3-6 yaş aralığındaki çocuklar oluşturdu. Araştırmanın örneklemini, örneklem seçim kriterlerine uyan 80 çocuk oluşturdu (müdahale: 40, kontrol: 40). Araştırmanın verileri Çocuklara ve Ebeveynlerine Yönelik Tanıtıcı Bilgi Formu, FLACC Ağrı Değerlendirme Skalası, Çocuklarda Emosyonel Göstergeler Ölçeği (ÇEGÖ) ve parmak kuklası ile toplandı. Müdahale grubuna kan alma sırasında parmak kuklası oynatılarak çocuğun dikkati işlemden uzaklaştırıldı. Kontrol grubundaki çocuklara rutin kan alma işlemi yapıldı. Kan alma işlemi öncesi ve sırasında çocukların ağrı ve emosyonel durumu kan alma işlemini gerçekleştiren gözlemci tarafından değerlendirildi. Müdahale grubundaki çocukların ağrı puan ortalamaları ile ÇEGÖ puan ortalamalarının kontrol grubundaki çocuklara göre istatistiksel olarak anlamlı düzeyde daha düşük olduğu bulundu (p0,05). Bu araştırma sonucunda kan alma işlemi sırasında parmak kuklası ile dikkati başka yöne çekmenin hem çocukların ağrısını azaltmada, hem de emosyonel durumlarını olumlu yönde değiştirmede etkili olduğu bulundu. Pediatri kliniklerinde çalışan hemşirelerin ağrılı işlemlerin çocuklar üzerindeki olumsuz etkilerini yönetmede parmak kuklası ile dikkati başka yöne çekme yöntemini kullanmaları önerilir.ABSTRACT The research was carried out as a randomized controlled experimental study to determine the effect of distraction applied with finger puppet during the blood collection process on the pain level and emotional indicators of children. The population of the study consisted of children aged 3-6 years who applied to the pediatric emergency department of a hospital in Istanbul between September 2021 and March 2022. The sample of the study consisted of 80 children who met the sample selection criteria (experimental: 40, control: 40). The data of the study were collected with the Introductory Information Form for Children and Their Parents, the FLACC Pain Assessment Scale, the Children's Emotional Indicators Scale (CEIS) and finger puppets. The attention of child was distracted from the procedure by moving a finger puppet during blood collection to the experimental group. The children in the control group underwent routine blood collection. The pain and emotional state of the children before and during the bloodletting were evaluated by the observer who performed the bloodletting. It was found that the mean scores of pain and the CEIS of the children in the experimental group were statistically significantly lower than the children in the control group (p0,05). As a result of this research, it was found that distraction with a finger puppet during the blood collection was effective both in reducing the pain of children and in changing their emotional state in a positive way. It is recommended that nurses working in pediatric clinics use finger puppet distraction method to manage the negative effects of painful procedures on children
Does posterior approach always lead to poor functional and cosmetic outcomes in displaced pediatric supracondylar humeral fractures?
ABSTRACT
BACKGROUND: Supracondylar humerus fractures (SCHFs) are the most common types of elbow fractures in children. Closed reduction percutaneous pinning (CRPP) is the primary surgical treatment of SCHFs. In cases that cannot be managed with closed reduction, treatment with open reduction and internal fixation (ORIF) is necessary. We aimed to compare CRPP and ORIF through a posterior approach regarding clinical and functional outcomes in pediatric SCHF cases. METHODS: Patients with Gartland type III SCHF who underwent CRPP or ORIF with posterior approach at our clinic between January 2013 and December 2016 were included in this retrospective study. A total of 60 patients who underwent surgical treatment and had available data on our hospital database and no additional injuries were included in the study. We analyzed their data concerning age, gender, fracture type, neurovascular damage, and surgical treatment. In addition, we inspected the patients’ anteroposterior and lateral radiographs at 1-year follow-up visits for Baumann (humerocapitellar) angle (BA) and carrying angle (CA) and checked their goniometer assessments of elbow range of motion (ROM). The cosmetic and functional outcomes were determined using Flynn’s criteria. RESULTS: Demographic, preoperative, and post-operative data of 60 patients between the ages of 2–15 were analyzed. 46 of these patients had CRPP, and 14 had posterior ORIF. CA, Baumann angle, and lateral capitello-humeral angle were measured for fractured elbow and contralateral elbow and compared statistically. There was no statistically significant difference between the two surgical approaches in terms of CA (p=0.288), Baumann’s angle (p=0.951) and LHCA (p=0.578). At the end of 1-year follow-up, elbow ROM was measured, and there was no statistically significant difference between the two groups (p=0.190). Furthermore, there is no statistically significant difference between the two surgical approaches in terms of both cosmetic (p=0.814) and functional (p=0.319) outcomes. CONCLUSION: A comprehensive literature review of pediatric SCHF shows that surgeons do not frequently prefer posterior incisions in Gartland type III fracture that cannot be managed with closed reduction. However, posterior open reduction is a safe and effective method since it provides more control over the distal humerus, allows for a complete anatomical reduction involving both cortices, reduces the risk of ulnar nerve injury, thanks to the nerve exploration, and yields positive cosmetic and functional outcomes. Protective effects of L-carnitine and co-enzyme Q10 against axidative stress damage in hypertension
Abstract : In this study, it was aimed to investigate the effects of L-carnitine and Co-enzyme Q10 administration together with ACE Inhibitor (ACE inh.) on oxidative stress parameters in liver, brain and kidney tissues in L-NAME hypertensive rats. At the study, divided all rats into eight groups, four groups with 14 days of experiment time and four groups with 28 days of experiment time. At the end of the experiment, the rats were euthanized and their liver, brain and kidney tissues were taken. Malondialdehyde (MDA), glutathione (GSH) and nitric oxide (NO) activities were measured in tissue supernatants. While NO and MDA levels increased in all tissues, a significant decrease was observed in GSH levels (P<0.001). In conclusion, it is suggested that supplementation of L-carnitine and CoQ10 can be considered as a combination therapy strategy for patients prone to higher levels of oxidative stress and inflammation

The objective of this study is to investigate a few solutions to the nonlinear Schriidinger problem with parabolic law. The first integral and exact solutions for the reduced ODE of the model under consideration are extracted using Nucci's reduction approach. Finally, using the efficient and effective solutions technique, we display density plots and 2D, 3D plots for the suggested governing model
Optical soliton solutions of stochastic nonlinear schrödinger equation with kerr law nonlinearity
Abstract: In this study, the stochastic nonlinear Schrödinger equation with kerr law nonlinearity was examined by utilizing the Kudryashov auxiliary equation method (KAEM). The first stage of the study was to obtain the dark soliton solution of the equation, and then to observe the stochastic effect on the model obtained. For this purpose, stochastic term was added to the equation in the framework of Ito calculus, wave transformation was defined and applied for the transition from nonlinear partial differential form to nonlinear ordinary differential form. Then, stochastic optical soliton solutions were obtained by using the KAEM method. Among the solutions obtained, those that provide the main equation were taken into consideration and graphic presentations were made over them. In order to clearly observe the stochastic effect, 2D graphics and detailed analyzes were made for different values of the noise strength parameter
Relationship of Altmetric Attention Score, Twitter Performance, and Dimensions Badge Value With Traditional Metrics in Top-Cited Anterior Cruciate Ligament Research Studies
Abstract
Background: Alternative citation metrics-such as the Altmetric Attention Score (AAS), number of tweets (TN), and dimensions badge value (DBV)-are emerging as new options for assessing the value of scientific works.
Purpose: To analyze the AAS of highly cited articles on the anterior cruciate ligament (ACL) and to assess the relationship between alternative and traditional metrics such as journal rankings and article citation performance.
Study design: Cross-sectional study.
Methods: A search was conducted using the Web of Science Core Collection of databases with "anterior cruciate ligament" as the search term. Full-text articles published between 2011 and 2021 were reviewed, and the top 100 cited articles were determined. The articles were analyzed by publication year, study design, research topic, journal impact factor, journal h-index, number of total citations (TC), recent citations (RC) (ie, citations in the latest 2 years), and average citation per year (ACY), as well as AAS, TN, and DBV.
Results: For articles in the top 100 list, the median TC was 160 (interquartile range [IQR], 117-561) and the median AAS was 24.50 (IQR, 1-730). A higher AAS score was achieved by articles on return to sports and anterolateral ligament (P < .05). The AAS and TN were significantly and positively correlated with the RC (r = 0.459 and P = .001; r = 0.438 and P = .001, respectively) and ACY (r = 0.363 and P = .001; r = 0.393 and P = .001, respectively).
Conclusion: Alternative metrics were linked to traditional metrics but were not a direct representation of bibliometrics. The AAS was not correlated with TC numbers in the ACL research. Higher AAS and Twitter popularity of an article were related to receiving high-volume RC performance. The DBV represented a stronger correlation with traditional metrics than the AAS
Antioxidant and neuroprotective effects of dexpanthenol in rats induced with traumatic brain injury
Trauma-induced primary damage is followed by secondary damage, exacerbating traumatic brain injury (TBI). Dexpanthenol has been shown to protect tissues against oxidative damage in various inflammation models. This study aimed to investigate possible antioxidant and neuroprotective effects of dexpanthenol in TBI. Wistar albino male rats were randomly assigned to control (n = 16), trauma (n = 16) and dexpanthenol (500 mg/kg; n = 14) groups. TBI was induced under anesthesia by dropping a 300 g weight from 70-cm height onto the skulls of the rats. Twenty-four hours after the trauma, the rats were decapitated and myeloperoxidase (MPO) levels, luminol- and lucigenin-enhanced chemiluminescence (CL), malondialdehyde (MDA) levels, superoxide dismutase (SOD) levels, and catalase (CAT) and caspase-3 activities were measured in brain tissues. Following transcardiac paraformaldehyde perfusion, histopathological damage was graded on hematoxylin-eosin-stained brain tissues. In the trauma group, MPO level, caspase-3 activity and luminol-lucigenin CL levels were elevated (p < 0.05–0.001) when compared to controls; meanwhile in the dexpanthenol group these increases were not seen (p < 0.05–0.001) and MDA levels were decreased (p < 0.05). Decreased SOD and CAT activities (p < 0.01) in the vehicle-treated TBI group were increased above control levels in the dexpanthenol group (p < 0.05–0.001). in the dexpanthenol group there was relatively less neuronal damage observed microscopically in the cortices after TBI. Dexpanthenol reduced oxidative damage, suppressed apoptosis by stimulating antioxidant systems and alleviated brain damage caused by TBI. Further experimental and clinical investigations are needed to confirm that dexpanthenol can be administered in the early stages of TBI
Novel fluorescein isothiocyanate (FITC) cored PAMAM dendrimers as drug delivery agent
Abstract
Fluorescent dyes, or fluorophores, enable researchers to visualize specific biological molecules by fluorescence microscopy. Herein, a novel fluorescent FITC-poly(amidoamine) (FITC-PAMAM) dendrimers have been synthesized via divergent approach. The molecular structure of the FITC-PAMAM dendrimers (G-0 to G5) was characterized by H-1 NMR, C-13 NMR spectroscopy and by FTIR. In order to emphasize the structural efficacy of FITC-PAMAM dendrimer, the drug delivery behavior has been evaluated by using 5-fluorouracil (5-FU) as a model cancer therapy drug. Anti-proliferative activity of FITC-cored PAMAM dendrimers in all generations (G0-G5) with or without 5-FU were analyzed on human gastric carcinoma cells (AGS) by WST-1 cell proliferation assay. Initially, the nontoxic dose of unloaded FITC-PAMAM dendrimers was assessed for further drug loading experiments. Loading of 5-FU into FITC-PAMAM dendrimers, regardless of generation, increased the toxicity of the drug. Besides, 5-FU loaded FITC-PAMAM dendrimers induced a generation-based gradual inhibition of cell proliferation. FITC and phase contrast merged images indicated that FITC-PAMAM dendrimers facilitate the entry of the drug into the cells, in a generation-dependent manner. The FITC- dendrimers may serve as a promising intracellular drug delivery agent for 5-FU by increasing cell uptake into cells and thereby, enhancing antiproliferative activity on AGS cells in a generation-dependent manner
On solitary wave solutions for the extended nonlinear Schrödinger equation via the modified F-expansion method
Abstract Because the importance of the optical wave propagation in fbers, we seek for the optical travelling wave solutions and efect of the third-order dispersion parameter for the extended nonlinear Schrödinger equation (NLSE) which is used to describe the femtosecond(fs) pulse propagation in optical fber. We have used the modifed F-expansion method because it is efective, sufcient and ofers more solutions for this kind of problems. First, we obtain the nonlinear ordinary diferential form (NODE) by using the wave transformation on the investigated extended nonlinear Schrödinger equation. Then, we propose the modifed F-expansion method to get solution of the equation as the NODE form, and by calculating the balancing constant over the NODE form. By substituting the proposed solution function and its derivatives in the NODE form, we obtain an algebraic equation system over this NODE. With the solution of this system, we obtain the appropriate solution sets for the undefned parameters, and then, using the appropriate sets, the proposed solution function, the F-expansion solution functions, and the wave transform, together, we obtain the solution functions of the investigated problem. In order to contribute to the physical interpretation of the problem, 3D, 2D and contour graphs of the obtained solution functions were plotted and interpreted. The problem has not been examined by considering two diferent situations with the efect of third order dispersion parameter as in this study, before. In this respect, the results obtained in the study are new, and it is believed that they will contribute to the studies in this feld
Çocuk sağlığı ve hastalıkları kliniğinde çocuğu yatarak tedavi gören ebeveynlerin ateş yönetim davranış düzeyleri ve ilişkili faktörler
ÖZET Vücut ısısının yükselmesi olarak tanımlanan ateş, aileleri en çok endişelendiren ve en yaygın görülen durumlarından biridir. Sağlık profesyonelleri, aileler ve çocuklara bakan kişiler ateş ve ateş yönetim davranışları hakkında bilgi sahibi olmalıdır. Araştırmalar, ateş bilgilerinin sürekli güncellenmesi, ateşi ölçmek için farklı yöntemlerin kullanılması ve ateş tedavisine ilişkin farklı algılar nedeniyle aileler ve sağlık uzmanları arasında ateş korkusunun devam ettiğini göstermektedir. Bu çalışma 18 yaş altı çocukların ebeveynlerinin ateş yönetim davranış düzeylerini ve ilişkili olduğu faktörleri tespit etmek amacıyla yapıldı. Araştırmanın evrenini, Ocak 2022-Nisan 2022 tarihleri arasında İstanbul da bulunan bir kamu hastanesi çocuk sağlığı ve hastalıkları kliniğinde çocuğu yatarak tedavi gören 510 hasta yakını olmak üzere örneklem büyüklüğü minimum 274 hasta yakını olarak hesaplandı. Bu bağlamda, çalışmaya katılmayı kabul eden ve veri toplama formlarını eksiksiz dolduran toplam 281 ebeveyn örneklemi oluşturdu. Veriler “Tanıtıcı Bilgi Formu” ve “Ebeveyn Ateş Yönetim Ölçeği Türkçe Versiyon / PFMS-TR (Parent’s Fever Management Scale)” kullanılarak toplandı. Verilerin değerlendirilmesi için SPSS v.24 programı kullanıldı. Araştırma sonucunda, ebeveynlerin ateş varlığını %52,66 oranında ateş ölçerle ölçerek tespit ettikleri, ölçüm aracı olarak genel anlamda %65,12 oranında dijital termometre kullandıkları, ateş ölçümü için genel olarak koltuk altından ölçüm aldıkları görülmüştür. Araştırma sonucunda ebeveynlerin %72,24’ünün çocuğun ateşlenmesi durumunda en fazla çocuğun havale geçirmesinden endişelendiklerini ifade ettikleri belirlendi.ABSTRACT Fever, defined as an increase in body temperature, is one of the most common childhood conditions that worries families the most. Health professionals, families, and caregivers should be knowledgeable about fever and fever management behaviors. Studies show that there is still fear of fever among families and health professionals due to the continuous updating of fever information, the use of different methods to measure fever, and different perceptions of fever treatment. This study aimed to determine the fever management behaviors of parents of children under 18 years of age and the factors related to these behaviors. The universe of the study was assumed to be a minimum of 274 caregivers, with a sample size of 510 caregivers of children who were admitted for treatment at the Pediatric Clinic of Hospital in Istanbul between January 2022 and April 2022. In this context, a total of 281 caregivers who agreed to participate in the study and completed the data collection forms comprised the sample. The data was collected using the "Introductory Information Form" and the "Parent's Fever Management Scale (PFMS-TR) Turkish Version." SPSS v.24 was used for data evaluation. As a result of the study, it was found that parents identified the presence of fever by measuring it with a thermometer at a rate of 52.66%, using a digital thermometer at a rate of 65.12% in general as a measuring tool, and generally taking measurements from under the armpit. As a result of the study, it was seen that 72.24% of parents were most concerned about their child passing out when they have a fever
