    Optical solitons for a concatenation model by trial equation approach

    Abstract: This paper addresses the newly proposed concatenation model by the usage of trial equation approach. The concatenation is a chain model that is a combination of the nonlinear Schrodinger’s equation, Lakshmanan–Porsezian–Daniel model as well as the Sasa–Satsuma equation. The recovered solutions are displayed in terms of dark solitons, singular solitons, cnoidal waves and singular periodic waves. The trial equation approach enables to recover a wide spectrum of solutions to the governing model. The numerical schemes give a visual perspective to the solutions derived analytically

    Consensus on technical procedures for simulation-based training in thoracic surgery: An international needs assessment

    Objective: To identify and prioritize technical procedures for simulation-based training to be integrated into the thoracic surgical curriculum. Methods: A three-round Delphi survey was conducted from February 2022 to June 2022 among 34 key opinion leaders in thoracic surgery from 14 countries worldwide. The first round was a brainstorming phase to identify technical procedures that a newly qualified thoracic surgeon should be able to perform. All the suggested procedures were categorized, qualitatively analyzed, and sent to the second round. The second round investigated: the frequency of the identified procedure at each institution, the number of thoracic surgeons that should be able to perform these procedures, the degree of risk to the patient if the procedure is performed by a non-competent thoracic surgeon, and the feasibility of simulation-based education. In the third round, elimination, and re-ranking of the procedures from the second round were performed. Results: Response rates in the three iterative rounds were 80% (28 out of 34), 89% (25 out of 28) and 100% (25 out of 25) in the first, second, and third round, respectively. Seventeen technical procedures were included for simulation-based training in the final prioritized list. The top five procedures were VATS lobectomy, VATS segmentectomy, VATS mediastinal lymph node dissection, diagnostic flexible bronchoscopy, and RATS port placement, RATS docking and undocking. Conclusion: The prioritized list of procedures represents a consensus of key thoracic surgeons worldwide. These procedures are suitable for simulation-based training and should be integrated in the thoracic surgical curriculum

    A cross-sectional analysis of paediatric urologists’ current practices, opinions and areas of perceived importance in the delivery of adolescent & transitional care

    Abstract Introduction: Complex urological anomalies often require continued care as patients reach adulthood. Adequate transition for adolescents with ongoing urological care needs is critical to allow for seamless care in adult hospitals. Studies have shown that this can lead to improved patient and parental satisfaction, and lower utilisation of unplanned inpatient beds and emergency department visits. There is currently no ESPU-EAU consensus on the adequate mechanism and very few individual papers examining the role of urological transition for these patients in a European setting. This study aimed to identify current practice patterns in paediatric urologists providing adolescent/transitional care, to assess their opinions towards formal transition and to look for variations in care. This has implications for long-term patient health and specialist care. Methods: An 18-item cross-sectional survey was compiled and pre-approved through the EAU-EWPU and ESPU board offices prior to dissemination to all registered ordinary members affiliated with the ESPU. This was created using a mini-Delphi method through the EWPU research meetings to provide current semi-quantitative data relating to current opinions and attitudes of this cohort. Results: A total of 172 respondents (55% paediatric general surgery; 45% urology) across 28 countries completed the survey. The majority of respondents were in practice >10 years and spent >80% time in paediatric urology. There was no formal transition process according to 50% respondents and over half of those that did have less than 1/month, with 70% units had no designated corresponding adult service. Furthermore, 93% paediatric believe a formal transition service to be very important, using a multidisciplinary framework. A pareto chart demonstrated 10 specific conditions to be of most interest in transition to adulthood. Conclusion: This is the first study to assess the requirements of paediatric urologists for adequate transitional care, however due to the nature of the survey's distribution, this was a non-scientific poll based on a convenience sample of respondents. It is critical that dual-trained or adult-trained urologists with a specific interest in paediatric urology work with current paediatric urologists in a multidisciplinary fashion to facilitate early transition based on the adolescent's developmental and biopsychosocial requirements. National urological and paediatric surgical societies need to make transitional urology a priority. The ESPU and EAU should collaboratively consider developing transitional urology guidelines to allow a framework by which this can occur

    Pharmacophore-based virtual screening: Identification of selective sirtuin 2 inhibitors

    ABSTRACT: The Class III histone deacetylases protein Sirt2 has been implicated in the pathogenesis of several agerelated diseases such as inflammation, cancer, and type II diabetes and is considered an attractive novel therapeutic target. High-quality small-molecule inhibitors of Sirt2 are vital as chemical probes for target validation and potential starting points for new therapeutics. We applied an iterative virtual screening campaign including structure-based pharmacophore generation, ensemble docking, and protein-ligand interaction fingerprint analysis, to identify potential Sirt2 inhibitors from commercially available chemical libraries. Several hit molecules were determined to make exceptional interactions both with the catalytic (C) pocket and selective extended C pocket (EXC) pocket at the same time which indicated that these compounds represent promising lead structures for the development of selective and potent Sirt2 inhibitors

    Plication for diaphragm paralysis after paediatric cardiac surgery: A single-centre experience

    Abstract Objective:Diaphragm paralysis is a well-known complication following surgery for CHDs, which increases morbidity, mortality, and length of hospital stay as well as costs. Herein, we present our experience with diaphragm plication following paralysis of the phrenic nerve encountered after paediatric cardiac surgery. Methods:This study retrospectively reviewed the medical records of 23 diaphragm plications in 20 patients who underwent paediatric cardiac surgery between January 2012 and January 2022. The patients were carefully selected based on aetiology and a combination of clinical manifestation and chest imaging characteristics including chest X-ray, ultrasonography, and fluoroscopy. Results:Twenty-three successful plications were performed in 20 patients (15 males and 5 females) out of a total of 1938 operations performed in our centre. Mean age and body weight were 18.2 +/- 17.1 months and 8.3 +/- 3.7 kg, respectively. The period between the cardiac surgery and diaphragmatic plication was 18.7 +/- 15.1 days. The highest incidence of diaphragm paralysis was encountered in systemic to pulmonary artery shunt patients with 7 out of 152 patients (4.6%). Any mortality was not encountered during a mean follow-up period of 4.3 +/- 2.6 years. Conclusions:Early results of plication of the diaphragm following phrenic nerve palsy in symptomatic patients who underwent paediatric cardiac surgery are encouraging. Evaluation of the diaphragmatic function should be a routine part of post-operative echocardiography. Diaphragm paralysis may be a consequence of dissection, contusion, stretching, and thermal injury both in terms of hypothermia and hyperthermia

    A novel approach for ultrasensitive amperometric determination of NADH via graphene based electrode obtained by electrochemical intercalation of tetraalkylammonium ions

    Abstract The reduced β-nicotinamide adenine dinucleotide (NADH) is an important coenzyme in many biological systems. In this work, a new electrochemical sensor platform prepared using tetraoctylammonium ions (TOA+) as an intercalant for the intercalation with graphite electrode is reported to determine NADH efficiently. While NADH detection was performed using amperometry at 0.55 V, the characterization of the developed sensor was examined using scanning electron microscopy (SEM), energy dispersive X-ray spectroscopy (EDX), electrochemical impedance spectroscopy (EIS), Fourier transform infrared spectroscopy (FT-IR), atomic force microscopy (AFM), X-ray photoelectron spectroscopy (XPS), Raman spectroscopy, and cyclic voltammetry (CV) techniques. The manufacture of the modified electrode is easy, and the repeatability of the signal process is good enough for NADH determination. The developed NADH sensor showed excellence in the low detection and quantitation limit, high sensitivity, good stability, and specificity. The proposed TOA+/PGE-based electrochemical sensor displayed good analytical performance and LOD and LOQ values were calculated as 0.077 μM and 0.257 μM, respectively. In addition, the effect of TOA+/PGE on the analysis of species that may show interference effects found in real samples was examined. It proved that the sensor could successfully detect the NADH level in the human blood sample. Graphical Abstract: [Figure not available: see fulltext

    Single vs. multiple operators for chronic total occlusion percutaneous coronary interventions: From the PROGRESS‐CTO Registry

    Background: There is limited data on the impact of a second attending operator on chronic total occlusion (CTO) percutaneous coronary intervention (PCI) outcomes. Methods: We analyzed the association between multiple operators (MOs) (>1 attending operator) and procedural outcomes of 9296 CTO PCIs performed between 2012 and 2021 at 37 centers. Results: CTO PCI was performed by a single operator (SO) in 85% of the cases and by MOs in 15%. Mean patient age was 64.4 ± 10 years and 81% were men. SO cases were more complex with higher Japan-CTO (2.38 ± 1.29 vs. 2.28 ± 1.20, p = 0.005) and Prospective Global Registry for the Study of Chronic Total Occlusion Intervention scores (1.13 ± 1.01 vs. 0.97 ± 0.93, p < 0.001) compared with MO cases. Procedural time (131 [87, 181] vs. 112 [72, 167] min, p < 0.001), fluoroscopy time (49 [31, 76] vs. 42 [25, 68] min, p < 0.001), air kerma radiation dose (2.32 vs. 2.10, p < 0.001), and contrast volume (230 vs. 210, p < 0.001) were higher in MO cases. Cases performed by MOs and SO had similar technical (86% vs. 86%, p = 0.9) and procedural success rates (84% vs. 85%, p = 0.7), as well as major adverse complication event rates (MACE 2.17% vs. 2.42%, p = 0.6). On multivariable analyses, MOs were not associated with higher technical success or lower MACE rates. Conclusion: In a contemporary, multicenter registry, 15% of CTO PCI cases were performed by multiple operators. Despite being more complex, SO cases had lower procedural and fluoroscopy times, and similar technical and procedural success and risk of complications compared with MO cases

    Immunomodulatory activities of novel carbon quantum dots on in vitro activated mammalian macrophages

    Abstract Quantum dots have been utilized in medicine and biology due to their biocompatibility as well as fluorescent properties. In medicinal and biological applications, they have been widely used for their in vivo imaging capacities as well as drug delivery and depending on the functional groups sometimes as treatment agents. In most of the studies, their possible immunomodulatory roles are omitted and in our study, we aimed to generate a series of novel carbon quantum dots that can be used as drug delivery vehicle or as active ingredient against inflammation. These novel carbon quantum dots were tested on the in vitro activated mammalian macrophages to examine their effects on the production of the inflammatory cytokines by ELISA method. Our results suggest that depending on the functional groups, there were differential immunomodulatory activities of these novel carbon quantum dots

    Hydrazone compounds are frequently prepared for the utilization of biological activities and coordination abilities [1, 2]. Additionally, the hydrazone functional group plays a critical role in the in the context of supramolecular chemistry [3, 4]. Among the biological properties of hydrazide-hydrazones based compounds, the antimicrobial activity is the most frequently encountered one in scientific literature [5, 6]. Plenty of Schiff bases and their complexes have been studied for their important properties, such as dyes and pigments [7], catalysts [8], colorimetric and fluorescent chemosensors [9, 10], proton-induced spectral switches [11], intermediates in organic synthesis [12]. Schiff base and their metal complexes continue to serve as one of the most important models to mimic active sites of metalloenzymes [13]. Salen and o-hydroxy derivatives of Schiff bases (azines) have been extensively studied due to their well-defined complexation behaviors [14-19]. In this work, a title Schiff base derivative, which is a new polymorphic modification of the red pigment known as Para red or Pigment Red 1 [20-24], has been synthesized.
