Multidisipliner Prensiplerle Oral Diagnoz
Diş hekimliğinde multidisipliner yaklaşımların çok önemli olduğu ve bu disiplinler arası sinerjinin sağlandığı oral diagnoz, diş hekimliği ile tıp hekimliği arasında köprü görevini üstlenmektedir. Uzun yıllar bu prensipler çerçevesinde genç hekim arkadaşlarımıza, asistanlarımıza ve uzmanlık öğrencilerimize bu alanda rehber olduğumuza inanıyoruz. Bu rehberliğin oluşturduğu ekolle diş hekimliğinde semptomlar, ayırıcı tanı yöntemleri, tedavi planlamaları, sistemik hastalıklar ve acil durumlardaki değerlendirmelerde didaktik bir yaklaşımla pratik uygulamalı bir bakış açısı geliştirilmiştir. Düşüncelerimiz doğrultusunda daha önceki yıllarda editörlüğünü üstlendiğim “Oral Diagnoz”(1995) ve “Sistemik Yaklaşımlarla Oral Diagnoz” (2007) isimli mevcut kitaplarımızdan sonra gün geçtikçe baş döndürücü bir hızla değişen bilgiler ve klinik yaklaşımların diş hekimliğinde de tezahür etmesiyle güncel kalma ihtiyacı tüm gerçekliğiyle ortadadır. Oral diagnoz alanında yine editörlüğünü üstlendiğim yeni kitabım bu anlayışla yola çıkılarak düzenlenmiştir. Uzun soluklu bu yolculukta, akademik merakı ve etik sorumluluğu yüksek çok değerli hocalarımız olduğu gibi mesleğin başındaki genç hekim arkadaşlarımız da bizlere eşlik etmiştir. Kitabımızda, oral diagnoza giriş ile başlayarak tüm sistemik hastalıkları ve klinikte bu sistemik hastalıklara diş hekimliği yönünden nasıl yaklaşılması gerektiğini de içine alacak şekilde çok geniş yelpazede bilgi sunulmaktadır. Bu bilgi paylaşımları sayesinde ayırıcı tanılarda farklı bakış açıları kazandırmak amaçlanmış olup hem akademik ortamda hem de diş hekimliği pratiğinde meslektaşlarımızla olan etkileşimimiz bizi ayrıca çok mutlu etmektedir. Outcomes of chronic total occlusion percutaneous coronary ıntervention after a previous failed attempt
Abstract
The impact of a previous failure on procedural techniques and outcomes of chronic total occlusion (CTO) percutaneous coronary intervention (PCI) has received limited study. We examined the clinical and angiographic characteristics and procedural outcomes of 9,393 patients who underwent 9,560 CTO PCIs at 42 United States and non-United States centers between 2012 and 2022. A total of 1,904 CTO lesions (20%) had a previous failed PCI attempt. Patients who underwent reattempt CTO PCI were more likely to have a family history of coronary artery disease (37% vs 31%, p 30 CTO PCIs annually were more likely to achieve technical success in patients with previous failure. In conclusion, a previous failed CTO PCI attempt was associated with higher lesion complexity, longer procedure time, and lower technical success; however, the association with lower technical success did not remain significant in multivariable analysis.Abstract

What is the ideal treatment for 20–30 mm kidney stones? Comparative outcomes of 1197 patients
Background: The purpose of this study is to compare the efficacy and safety of extracorporeal shockwave lithotripsy (SWL), retrograde intrarenal surgery (RIRS), mini-percutaneous nephrolithotomy (mPNL), and standard-percutaneous nephrolithotomy (stPNL) for the treatment of 20–30 mm kidney stones. Methods: The records of 1197 patients (SWL = 149, RIRS = 205, mPNL = 525, and stPNL = 318) from 8 centers were reviewed retrospectively. Four procedures were compared for stone-free rates (SFRs), auxiliary treatment, and associated complications. Results: Initial SFRs were 43.6%, 54.6%, 86.7%, and 87.7% in SWL, RIRS, mPNL, and stPNL, respectively (P < .001), whereas the final SFRs were 71.8%, 80%, 90.5%, and 89.6% (P < .001). The rate of auxiliary treatment in the groups was 38.3%, 26.8%, 5%, and 4.4%, respectively (P < .001). The initial and final SFRs in the mPNL and stPNL groups were higher than those in SWL and RIRS groups (P < .001). The rate for auxiliary treatment was lower in the mPNL and stPNL groups (P < .001). The operation time was longer in the RIRS group (P = .005). According to the Clavien–Dindo classification, the complication rate in the SWL group was lower than that in the surgical approaches (P < .001); however, no statistical difference was detected between RIRS, mPNL, and stPNL groups. mPNL and stPNL had a higher success rate than RIRS or SWL for treating 20–30 mm kidney stones. Conclusion: In the treatment of 2–3 cm renal stones, RIRS and PNL were more effective than SWL to obtain a better SFR and less auxiliary treatment rate. Compared with RIRS, mPNL and stPNL provided a higher SFR with similar complication rates
Multiple soliton and M-lump waves to a generalized B-type Kadomtsev-Petviashvili equation
Abstract
In this study, we focus on the (3+1)-dimensional generalized B-type Kadomtsev-Petviashvili (gBKP) equation in fluid dynamics, which is useful for modeling weakly dispersive waves transmitted in quasi media and fluid mechanics. As a general matter, this paper examines the gBKP equation including variable coefficients of time that are widely employed in plasma physics, marine engineering, ocean physics, and nonlinear sciences to explain shallow water waves. Using Hirota's bilinear approach, one-, two, and three-soliton solutions to the problem are constructed. By employing a long-wave method, 1-M-, 2-M, and 3-M-lump solutions are derived. In addition, interaction phenomena of one-, and two-soliton solutions with one-M-lump wave are revealed. Moreover, an interaction solution between a two-M-lump wave and a one-soliton solution is also offered. The planes that M-lump waves travel among them are derived. We believe that our findings will help improve the dynamical properties of (3+1)-dimensional BKP-type equation
Investigation of the effect of music on the transitional phase and after the birth process
ÖZET
Araştırma, müziğin geçiş fazı ve sonrası doğum sürecine etkisinin incelenmesi amacıyla
Kasım 2021- Nisan 2022 tarihleri arasında randomize kontrollü deneysel nitelikte yapıldı.
Araştırmanın evreni, İstanbul’da bir kamu hastanesine doğum yapmak için başvuran
kadınlardan oluştu. Güç analizi sonucunda örneklem büyüklüğü, “Müzik Uygulama
Grubu” 43 gebe ve “Kontrol Grubu” 43 gebe olmak üzere toplam 86 gebeden oluştu.
Araştırma araçları olarak “Gönüllü Onam Formu”, “Tanıtıcı Bilgi Formu”, “Doğum Takip
Formu”, “Visuel Analog Skala”, “Durumluluk Anksiyete Ölçeği” ve “Doğum Memnuniyet
Ölçeği-Revize” formları kullanıldı. Araştırmada elde edilen veriler SPSS (Statistical
Package for Social Sciences) for Windows 22.0 programı kullanılarak analiz edildi.
Kadınların, müzik dinlemeyi sevdiği, doğumdan sonra başkalarına müzik dinlemeyi
önerdiği, sonraki doğumunda müzik dinlemeyi istedikleri, travay sürecinde müzik
dinlemekten memnun kaldığı, müzik dinlemenin kadınları rahatlattığı görüldü. Kadınların
servikal dilatasyon 8cm iken ve doğumun dördüncü evresinin 2. saatinde Durumluluk
Anksiyete Ölçeği puanları gruplara göre karşılaştırıldığında istatistiksel olarak anlamlı bir
fark olmadığı saptandı (p>0.05). Doğum memnuniyeti toplam puanları gruplara göre
karşılaştırıldığında istatistiksel olarak anlamlı bir fark olmadığı saptandı (p>0.05).
Kadınların, servikal dilatasyon 8cm’de VAS, servikal dilatasyon 10cm’de VAS, bebek
doğduktan hemen sonra VAS, doğumun dördüncü evresinin 2. saati VAS ölçümleri
gruplara göre istatistiksel olarak anlamlı düzeyde bir fark göstermedi (p>0.05). Sonuç
olarak, travayın geçiş sürecinde müziğin annenin anksiyetesinde, doğum memnuniyetinde
ve doğum ağrısı üzerinde etkili olmadığı görüldü. Ebe/hemşirelerin gebenin takip ve
bakımında destek olması için loş, sessiz ve tek kişilik bir odada müzik dinletilerinden
yararlanmaları önerilmektedir.ABSTRACT
The research was conducted in a randomized controlled experimental manner between
November 2021 and April 2022 in order to examine the effect of music on the transitional
phase and postpartum birth process. The universe of the research consisted of women who
applied to a public hospital in Istanbul to give birth. As a result of the power analysis, the
minimal sample size consisted of 86 pregnant women, 43 of whom were in the "Music
Practice Group" and 43 in the "Control Group". "Volunteer Consent Form", "Descriptive
Information Form", "Birth Follow-up Form", "Visual Analog Scale", "Static Anxiety Scale"
and "Birth Satisfaction Scale-Revised" forms were used as research tools. The data obtained
in the study were analyzed using the SPSS (Statistical Package for Social Sciences) for
Windows 22.0 program. It was seen that the women liked to listen to music, suggested
listening to music after giving birth to others, wanted to listen to music in the next delivery,
were satisfied with listening to music during labor, and listening to music was relaxing.
When the cervical dilatation of the women was 8cm and the scores of the State Anxiety
Scale at the 2nd hour of the fourth stage of labor were compared according to the groups,
there was no statistically significant difference (p>0.05). When the total scores of birth
satisfaction were compared according to the groups, no statistically significant difference
was found (p>0.05). The women's cervical dilatation VAS at 8cm, cervical dilatation VAS
at 10cm, VAS immediately after the baby was born, VAS measurements at the 2nd hour of
the fourth stage of labor did not show a statistically significant difference compared to the
groups (p>0.05). As a result, it was observed that music did not affect the mother's anxiety,
labor satisfaction and labor pain during the transition period of labor. It is recommended
that midwives/nurses benefit from music concerts in a dim, quiet and single room to support
the follow-up and care of the pregnant woman
Analyzing the Performance of ChatGPT About Osteoporosis
Abstract
Introduction: This study evaluates the knowledge of ChatGPT about osteoporosis.
Methods: Osteoporosis-related frequently asked questions (FAQs) created by examining the websites
frequently visited by patients, the official websites of hospitals, and social media. Questions based on these
scientific data have been prepared in accordance with National Osteoporosis Guideline Group guides. Rater
scored all ChatGPT answers between 1 and 4 (1 stated that the information was completely correct, 2 stated
that the information was correct but insufficient, 3 stated that although some of the information was
correct, there was incorrect information in the answer, and 4 stated that the answer consisted of completely
incorrect information). The reproducibility of ChatGPT responses on osteoporosis was assessed by asking
each question twice. The repeatability of the ChatGPT answer was considered as getting the same score
twice.
Results: ChatGPT responded to 72 FAQs with an accuracy rate of 80.6%. The highest accuracy in ChatGPT's
answers about osteoporosis was in the prevention category, 91.7%, and in the general knowledge category,
85.8%. Only 19 of the 31 (61.3%) questions prepared according to the National Osteoporosis Guideline Group
guidelines were answered correctly by ChatGPT, and two answers (6.4%) were categorized as grade 4. The
reproducibility rate of ChatGPT answers on 72 FAQs was 86.1% and the reproducibility rate of ChatGPT
answers on National Osteoporosis Guideline Group guidelines was 83.9%.
Conclusion: Present study outcomes for the first time showed that ChatGPT provided adequate answers to
more than 80% of FAQs about osteoporosis. However, the accuracy of ChatGPT’s answers to inquiries based
on National Osteoporosis Guideline Group guidelines was decreased to 61.3%
Genetic deviation associated with photodynamic therapy in HeLa cell
Photodynamic therapy (PDT) is a method that is used in cancer treatment. The main therapeutic effect is the production of singlet oxygen (1O2). Phthalocyanines for PDT produce high singlet oxygen with absorbers of about 600-700 nm. Aim: It is aimed to analyze cancer cell pathways by flow cytometry analysis and cancer-related genes with q-PCR device by applying phthalocyanine L1ZnPC, which we use as photosensitizer in photodynamic therapy, in HELA cell line. In this study, we investigate the molecular basis of L1ZnPC's anti-cancer activity. Material method: The cytotoxic effects of L1ZnPC, a phthalocyanine obtained from our previous study, in HELA cells were evaluated and it was determined that it led to a high rate of death as a result. The result of photodynamic therapy was analyzed using q-PCR. From the data received at the conclusion of this investigation, gene expression values were calculated, and expression levels were assessed using the 2-∆∆Ct method to examine the relative changes in these values. Cell death pathways were interpreted with the FLOW cytometer device. One-Way Analysis of Variance (ANOVA) and the Tukey-Kramer Multiple Comparison Test with Post-hoc Test were used for the statistical analysis. Conclusion: In our study, it was observed that HELA cancer cells underwent apoptosis at a rate of 80% with drug application plus photodynamic therapy by flow cytometry method. According to q-PCR results, CT values of eight out of eighty-four genes were found to be significant and their association with cancer was evaluated. L1ZnPC is a new phthalocyanine used in this study and our findings should be supported by further studies. For this reason, different analyses are needed to be performed with this drug in different cancer cell lines. In conclusion, according to our results, this drug looks promising but still needs to be analyzed through new studies. It is necessary to examine in detail which signaling pathways they use and their mechanism of action. For this, additional experiments are required
Efficiency of middle vault reconstruction with dorsal septal t flap in achieving ıdeal dorsal aesthetic lines and regularity
Background Dorsal aesthetic lines and irregularities are
the most critical points that can be manipulated in rhinoplasty operations. Although spreader grafts and flaps have been used for this purpose in the reconstruction of the keystone area for a long time, dorsal irregularity is often inevitable. This study aims to evaluate the effectiveness of dorsal septal T flap and nasal dorsum reconstruction in achieving ideal dorsal aesthetic lines and regularity.
Methods Thirty-four patients (30 women, 4 men) who underwent closed technique rhinoplasty between 2019 and 2021 were included in the study. The mean age of the patients was 25.4 years. Patients with crooked nose, severe deviation, and severe dorsal hump were also excluded. All patients included in the study underwent Rhinoplasty Outcome Evaluation (ROE) questionnaires preoperative and postoperative 1st year.Results The mean age of the patients was 25.4 years (range 18–40 years). A total of 34 patients, including 30 women and four men, were operated. The mean follow-up period was 13.2 months, ranging from 12 to 14 months. In the physical examinations performed at the control visits, no
functional problem was encountered in any patients. Preoperative and postoperative mean ROE scores were 60.9 and 92.7, respectively. Conclusions With the technique described in the study,
dorsal hump recurrence is minimized compared to other preservation techniques. Since the keystone area transition can be fine-tuned, continuity is ensured, and ideal dorsal aesthetic lines are created. The stability of the upper lateral cartilages is provided, thus eliminating the need for spreader grafts.

Consistent solitons in the plasma and optical fiber for complex Hirota-dynamical model
Abstract
In this paper, the nonlinear complex Hirota-dynamical model is explored using the unified auxiliary equation method, which is the most powerful method for studying exact solutions to nonlinear models. This model is one of the important standards of the nonlinear Schrodinger equation in which the third derivative term represents the self-interaction in the high-frequency subsystem. Typically, in plasma, this term is isomorphic to the so known self-focusing effect. The aforementioned equation is crucial to plasma physics due to the agreements between the self-interaction at high frequency and the well-known self-focusing effect in plasma. Different structures of solutions are successfully investigated for this model. Consequently, we obtain some solutions like dark-bright, bright, dark, singular, periodic, trigonometric function, Jacobi elliptic function, and exponential solutions. We simulate the 2D, 3D, and counter plots of the constructed solutions by choosing the suitable values of the parameters involved
Novel exact solutions to a coupled Schrodinger-KdV equations in the interactions of capillary-gravity waves
Abstract
This paper explores the interactions of capillary-gravity waves by presenting novel exact solutions to a coupled Schrodinger-KdV equation. The authors introduce a reduction method based on Nucci's approach to derive these exact solutions. The results provide a new approach to solving the Schrodinger-KdV equation, offering potential applications in various fields such as mathematical physics, hydrodynamics, and soliton theory. Effectiveness and versatility of utilized reduction technique make it a valuable tool for researchers in various fields, including mathematical physics, fluid mechanics, and nonlinear dynamics. Overall, this research contributes to the understanding of the complex behavior of capillary-gravity waves and provides a useful tool for modeling and analyzing such phenomena
