    Procedural time and outcomes of chronic total occlusion percutaneous coronary intervention

    Abstracat : Chronic total occlusion (CTO) percutaneous coronary interventions (PCIs) can be lengthy procedures. We sought to investigate the effect of procedural time on CTO PCI outcomes. We examined the procedural time required for the various steps of CTO PCI in 6,442 CTO PCIs at 40 US and non-US centers between 2012 and 2022. The mean and median procedure times were 129 § 76 and 112 minutes, respectively, with no significant change over time. The median times from access to wire insertion, guidewire manipulation time, and post crossing were 20, 32, and 53 minutes, respectively. Lesions crossed in <30 minutes were less complex, as reflected by lower Japanese CTO score (1.89 § 1.19, p <0.001) than lesions that were not successfully crossed (2.88 § 1.22) and lesions that were crossed in ≥30 minutes (2.85 § 1.13). The likelihood of successful crossing if crossing was not achieved after 30, 90, and 180 minutes were a 76.7%, 60.7%, and 42.7%, respectively. The parameters independently associated with ≥30 minutes guidewire manipulation time in patients with a primary antegrade approach included left anterior descending target ves- sel, proximal cap ambiguity, blunt/no stump, occlusion length, previous failed attempt, medium/severe calcification, and medium/severe tortuosity. The mean duration of CTO PCI is approximately 2 hours (»20% of time for access to wire insertion, »30% wire manipulation time, and »50% postwiring time). Guidewire crossing time was shorter in less complex lesions and in cases without complications

    Impact of lesion length on the outcomes of chronic total occlusion percutaneous coronary intervention: Insights from the PROGRESS‐CTO registry

    Background: The impact of occlusion length on the procedural techniques and outcomes of chronic total occlusion (CTO) percutaneous coronary intervention (PCI) has received limited study. Methods: We examined the clinical and angiographic characteristics and procedural outcomes of 10,335 CTO PCIs at 42 US and non-US centers between 2012 and 2022. The cohort was divided into two groups based on lesion length (≥20 mm vs. <20 mm). Results: Long lesions were present in 7208 (70%) patients. Comorbidities were more common in patients with long CTOs. Compared with short lesions, long lesions had higher J-CTO score (2.8 ± 1.1 vs. 1.3 ± 1; p < 0.001) and retrograde wiring was more often the initial (15.5% vs. 4.0%; p < 0.001) and successful (22.8% vs. 8.2%; p < 0.001) crossing strategy. Long lesions were more likely to require longer procedure (123 vs. 91 min; p < 0.001) and fluoroscopy (47.1 vs. 32.2 min; p < 0.001) time, larger contrast volume (218 vs. 200 mL; p < 0.001) and higher air kerma radiation dose (2.4 vs. 1.7 Gy; p < 0.001). After adjusting for potential confounders, long lesions were associated with lower technical success (odds ratio [OR]: 0.91 per 10 mm increase; 95% confidence interval [CI]: 0.88, 0.94) and higher major adverse cardiovascular events (MACE) (OR: 1.08 per 10 mm increase; 95% CI: 1.02, 1.15). Conclusions: CTO PCI of long occlusions is independently associated with lower rates of technical success and higher rates of in-hospital MACE

    Investigation of the relationship between COVID-19 disease and semen parameters in idiopathic male infertility patients

    OBJECTIVE: Recent studies have shown that there may be a deterioration in sperm parameters in patients who had recovered from COVID-19 disease. We aimed to investigate the relationship between COVID-19 disease and semen parameters in idiopathic male infertility patients. PATIENTS AND METHODS: The study was conducted among male patients who applied with infertility between June 2021 and February 2022 following the approval of the Ethics Committee. Idiopathic infertility patients who could give semen analysis were included in the study. Detailed medical history of all patients was obtained. The presence of detectable causes of infertility was defined as exclusion criteria. The patients who had COVID-19 disease history (Reverse Transcriptase-PCR or Computed Tomography findings) in the last year were divided into two groups COVID-19 (+) and COVID-19 (-). The semen samples obtained from patients after a 3 day sexual abstinence in accordance with the WHO 2021 criteria were recorded. RESULTS: A total of 42 male idiopathic infertility patients who met the criteria were included in the study. It was analyzed that both groups were similar in terms of sociodemographic characteristics, comorbidities, and habits (p>0.05). It was determined that 40.4% (n=17) had COVID-19 disease. The mean duration time after COVID-19 was 9.6 (4-17) months. Mean sperm concentration was found to be statistically significantly lower than the COVID-19 (-) group (41.59±17.4 vs. 58.8±21.9; p=0.021). Semen volume (3.05±0.7 vs. 3.32±0.6 mL; p>0.05), progressive sperm motility (34.05±20.96 vs. 43.00±16.94; p=0.12) and normal sperm morphology (3.47±1.42 vs. 3.08±1.41; p=0.41) were similar in both groups. The mean sperm concentration of the patients who recovered in the last 6 months (25.37±9.07 vs. 56.03±29.67 million/ml; p=0.013) compared to patients with >6 months after recovery (n=9) was found to be significantly lower. CONCLUSIONS: The COVID-19 disease can cause a significant decrease in sperm concentration in idiopathic infertility patients, especially in the first 6 months, and the rates of oligospermia and asthenospermia are higher

    Retrieval of optical solitons with anti-cubic nonlinearity

    Abstract: Purpose: In this article, two main subjects are discussed. First, the nonlinear Schrödinger equation (NLSE) with an anti-cubic (AC) nonlinearity equation is examined, which has a great working area, importance and popularity among the study areas of soliton behavior in optical fibers, by using the enhanced modified extended tanh expansion method and a wide range of optical soliton solutions is obtained. Second, the effects of AC parameters on soliton behavior are examined for each obtained soliton type. Methodology: In order to apply the method, the non-linear ordinary differential equation form (NLODE) of the investigated NLSE-AC is obtained by applying the defined wave transformation. Then, with the help of the proposed algorithm for the NLODE form, polynomial form, an algebraic equation system is obtained by setting the coefficients of this form to zero, and the solution of this system is also obtained. After determining the suitable solution set, the optical soliton solution of the investigated problem is obtained with the help of the serial form of the proposed method, a Riccati solution and wave transform. After checking that the solution satisfies the investigated problem, 3D and 2D graphics are obtained for the special parameter values and the necessary comments are made in the relevant sections. Findings: With the proposed method, many optical soliton solutions, such as topological, anti-peaked, combined peaked-bright, combined antipeaked dark, singular, combined singular-anti peaked, periodic singular, composite kink anti-peaked, kink, periodic and periodic, with different amplitudes are obtained, and 3D and 2D representations have been made. Then, the effect of AC parameters on the soliton behavior in each case has been successfully studied. It has been shown that AC parameters have a significant effect on the soliton behavior, and this effect changes depending on the soliton shape and the parameters. Moreover, providing and maintaining the delicate balance between the soliton shape and the parameters and the interaction of the parameters with each other involves great difficulties. Originality: Although some soliton types of the NLSE-AC equation have been presented for the first time in this study, there is no study in the literature showing the effect of AC parameters on soliton behavior, especially for the NLSE-AC equation

    ÖZET Bu çalışma akademisyenlere egzersiz programı ile verilen bel sağlığı eğitiminin fiziksel fonksiyon ve vücut farkındalığına etkisini incelemek üzere planlanmıştır. Çalışma 25-65 yaş aralığında yaş ortalamaları 43,22±10,924 yıl olan, 49’u kadın 45’i erkek toplam 94 gönüllü akademisyen (çalışma grubu n=47, kontrol grubu n=47) ile tamamlanmıştır. Her iki grubun katılımcıları 6 haftalık eğitim öncesinde ve sonrasında değerlendirilmiştir. Katılımcıların fiziksel fonksiyonlarını değerlendirmek için Tek Ayak Üzerinde Durma Testi (gözler açık ve kapalı), Yarı Çömelme Testi (Half Squat), Mekik Testi (Curl-Up) ve Otur Uzan Testi (Sit and Reach) kullanılmıştır. Vücut farkındalığını değerlendirmek için Vücut Farkındalığı Anketi (VFA) kullanılmıştır. Çalışma grubu katılımcıları sınıf içi ortamda egzersiz programı ile verilen bel sağlığı eğitimi (Bel Okulu) almışlardır. Bel okulu eğitim programı sonrasında ise 6 hafta boyunca (haftada bir gün) egzersiz eğitimine katılmışlardır. Kontrol grubuna ise sadece bel sağlığı eğitimi konusunda bilgilendirici broşür verilmiş ve okumaları istenmiştir. Çalışmanın sonuçlarına göre fiziksel fonksiyon açısından tüm parametrelerde her iki grupta da anlamlı artışlar gözlemlenmiştir (p ≤0,05). Ancak gruplar birbiriyle karşılaştırıldığında, farkın çalışma grubu lehine istatistiksel olarak anlamlı olduğu saptanmıştır (p≤0,05). Vücut farkındalığı skorlarında her iki grupta da 6 hafta sonunda artış gözlemlenmiştir (p≤0,05). Ancak çalışma ve kontrol grupları karşılaştırıldığında, vücut tepkileri tahmini (VTT) alt skoru (p≤0,05) hariç gruplar arasında fark bulunamamıştır (p≥ 0,05). Bel sağlığı eğitimi akademisyenlerin fiziksel fonksiyonlarını ve vücut farkındalıklarını arttırmıştır. Ancak, egzersiz yapmanın fiziksel fonksiyonları arttırmadaki etkisinin daha fazla olduğu sonucuna ulaşılmıştır

    Sphenoid sinus volumes of patients with unilateral cleft lip and palate

    Abstract : Objective There is only one study in the literature in which 3D evaluation of sphenoid sinus volume in patients with cleft lip and palate was used. This shows that this issue has not been adequately evaluated. The present study aimed to compare the volume of the sphenoid sinuses in patients with cleft lip and palate on cone-beam computed tomography images, and to compare them with healthy controls. The high variability of sphenoid sinus volumes poses a risk for iatrogenic lesions during surgical procedures. Because the degree of pneumatization due to increased volume increases the likelihood of exposure of vital neurovascular structures such as the internal carotid artery, maxillary nerves, and optic nerves. Design Retrospective cohort study. Setting University Hospital. Patients A total of 64 patients (32 cleft palate, 32 controls) over 18 years of age who had sphenoid sinuses in the image area were included in our study. Outcomes Sphenoid Sinus Volume. Results When the groups were compared in terms of total sinus volume, there was a statistically significant difference between them. Total sinus volume was significantly mean 2.806,87 mm lower in patients with cleft lip and palate compared to the healthy group (P = .015). Conclusion To be aware this lower volume of the sphenoid sinus in cleft lip and palate patients will be useful for the surgical approach to sphenoid sinus. However, the literature on the subject is insufficient, more comprehensive clinical and radiological studies are needed on a larger sample

    Optical wave propagation to a nonlinear phenomenon with pulses in optical fiber

    Abstract We examine the three-component coupled nonlinear Schrodinger equation that is used for the propagation of pulses to the nonlinear optical fiber. Multi-component NLSE equations have gained popularity because they can be used to demonstrate a vast array of complex observable systems as well as more kinetic patterns of localized wave solutions. The solutions are obtained by using the generalized exponential rational function method, a relatively new integration tool. We extract various optical solitons in different forms. Moreover, exponential, periodic solutions and solutions of the hyperbolic type are guaranteed. In addition to providing previously extracted solutions, the used approach also extracts new exact solutions and is beneficial for elucidating nonlinear partial differential equations. The graphs of different shapes are sketched for the attained solutions and some physical properties- are raised. The reported solutions in this work are new as they are compared to earlier similar studies. The results of this paper show that the used method is effective at improving the nonlinear dynamical behavior of a system. The findings show that the computational approach taken is successful, simple, and applicable even to complicated phenomena

    Investigation of the use of accidentally exposed to light x-ray films with artificial solarization method as a duplicate

    Abstract Aim Observing the outcomes of the artificial solarization method in the use of blue light-sensitive extraoral films as duplicates due to accidental light exposure. Material and method Two groups were formed using two different extraoral films (Kodak T Mat G and Agfa Curix) and these films were exposed to light. Then the films were prepared by placing them under the master copy films. Some of the films in each group were exposed to sunlight, while others were exposed to ultraviolet light as an artificial solarization method (Feket printing equipment). These duplicate films were then processed. Results Duplicate films obtained from sunlight produced slightly better results than artificial solarization, and Agfa films produced a better image than Kodak films. Conclusion The film method, which was duplicated by artificial solarization, is not a useful method for radiology clinics

    Abstract The main purpose of this study is to introduce symmetry analysis and Nucci's reduction method for revealing the exact solutions of the periodic Hunter-Suxon equation, which is parameterized by the speed of the Galilean frame. Under one-point similarity transformations, we obtained a set of generators that transformed the given equation into an ordinary differential equation, which can be conveniently taken into consideration for exact solutions. Under special choices, we got three separate families of vector fields out of the problem and constructed solutions for every family. In each case, the Nucci reduction technique generated a solution with a first integral. 3D, density, and contour graphics are given for a better understanding of the solutions

    Radiotherapy increases ammp-8-levels and neutrophil/lymphocyte ratio rapidly in head and neck cancer patients: A pilot study

    Abstract Radiotherapy for head and neck carcinoma (HNC) has both curative and palliative purposes. This study investigated mouthrinse aMMP-8 levels, molecular forms of MMP-8, blood neutrophil counts and neurophil/lymphocyte ratios before and 3 weeks after HNC radiotherapy started. Thirteen HNC patients undergoing radiotherapy were included. Mouthrinse samples (before and 3 weeks after HNC radiotherapy had started) were assayed quantitatively by aMMP-8 point-of-care-kit (PerioSafe®/ORALyzer®) and by western immunoblot. Total neutrophil counts and neutrophil/lymphocyte ratios were evaluated in the hemogram results. Three weeks after HNC radiotherapy started, significant increases in aMMP-8 levels and neutrophil/lymphocyte ratios were observed. No significant difference was found in total neutrophil counts. Elevations of the activated and fragmented MMP-8 levels after HNC radiotherapy application were observed on western immunoblot analysis. The increase in the aMMP-8 levels and neutrophil/lymphocyte ratios indicate inflammation both locally and systemically suggesting increased risk for periodontitis due to the HNC radiotherapy
