    Discovering optical soliton solutions in the Biswas-Milovic equation through five innovative approaches

    stract Purpose: In recent years, depending on the development of models containing highly dispersive nonlinear equations, especially in the field of nonlinear optics, it has been important to obtain the solutions of these equations with analytical methods. This study presents some effective, reliable, and easily applicable solution techniques based on the well-known extended auxiliary equation. Besides, these techniques were applied to the Biswas-Milovic equation.Methodology: First, in order to achieve the ordinary differential equation structure of the Biswas-Milovic equation, the complex wave transform was given and utilized. Then, the algorithms, constraints of each proposed approach, and key points for their implementation were presented. In addition, the relations and transitivities between the methods are also interpreted. Second, analytical optical soliton solutions were obtained by applying the proposed techniques. Lastly, graphical simulations of optical soliton types represented by some solution functions are made.Findings: It has been shown that all of the proposed methods yield effective results in solving the Biswas-Milovic equation and can be used not only for nonlinear evolution equations but also for highly dispersive nonlinear optical equations. It has been observed that the methods have the capacity to be employed easily and do not require tedious calculus and algebraic manipulations.Originality: The work in the article has not been presented before

    On the analytical soliton solutions of (1+1)-dimensional complex coupled nonlinear Higgs field model

    Abstract In this paper, the analytical solutions of the complex coupled Higgs field equation, which explains a system of conserved scalar nucleons that interact with neutral scalar mesons in particle physics, are extracted. The enhanced modified extended tanh expansion method (eMETEM) is successfully employed to solve the considered equation. Besides, the effect of problem parameters on a real scalar meson field and a complex scalar nucleon field is investigated, plotted and interpreted. Some effective results are obtained in the examination and are presented for the first time in this study. The obtained results can be applied to a wide range of nonlinear equations including ocean and shallow-water waves. 3D, 2D and contour graphical representations of the attained solutions are given using appropriate parameters. The method is efficient for gathering analytical solutions for various kinds of phenomena modeling by classical, fractional, or stochastic partial differential equations

    Effects of perfluorooctanoic acid on endoplasmic reticulum stress and lipid metabolism-related genes in human pancreatic cells

    Perfluorooctanoic acid (PFOA) is environmentally persistent and has been classified by The International Cancer Research Agency (IARC) as a possible human pancreatic carcinogen. In this study, the epigenetic alteration, the changes in the expression levels of endoplasmic reticulum stress-related and metabolism-related genes, as well as DNA methyltransferase expression were investigated using RT-PCR and ELISA assays. PFOA induced a significant increase in the methylation ratio (5-mC%), impacted DNA methylation maintenance gene expression and decreased lipid metabolism-related genes except for PPARγ (≥ 13-fold increase). While PFOA induced the expression of ATF4 (≥ 5.41-folds), CHOP (≥ 5.41-folds) genes, it inhibited the expression of ATF6 (≥ 67.2%), GRP78 (≥ 64.3%), Elf2α (≥ 95.8%), IRE1 (≥ 95.5%), and PERK (≥ 91.7%) genes. It is thought that epigenetic mechanisms together with disruption in the glucose-lipid metabolism and changes in endoplasmic reticulum stress-related genes may play a key role in PFOA-induced pancreatic toxicity

    The application of nanogels as efficient drug delivery platforms for dermal/transdermal delivery

    Abstract: The delivery of active molecules via the skin seems to be an efficient technology, given the various disadvantages of oral drug administration. Skin, which is the largest human organ of the body, has the important role of acting as a barrier for pathogens and other molecules including drugs; in fact, it serves as a primary defense system blocking any particle from entering the body. Therefore, to overcome the skin barriers and poor skin permeability, researchers implement novel carriers which can effectively carry out transdermal delivery of the molecules. Another significant issue which medical society tries to solve is the effective dermal delivery of molecules especially for topical wound delivery. The application of nanogels is only one of the available approaches offering promising results for both dermal and transdermal administration routes. Nanogels are polymer-based networks in nanoscale dimensions which have been explored as potent carriers of poorly soluble drugs, genes and vaccines. The nanogels present unique physicochemical properties, i.e., high surface area, biocompatibility, etc., and, importantly, can improve solubility. In this review, authors aimed to summarize the available applications of nanogels as possible vehicles for dermal and transdermal delivery of active pharmaceutical ingredients and discuss their future in the pharmaceutical manufacturing field

    Obtaining the soliton solutions of local M-fractional magneto-electro-elastic media

    In this research paper, the generalized projective Riccati equations method (GPREM) is applied successfully to procure the soliton solutions of the local M-fractional longitudinal wave equation (LWE) arising in mathematical physics with dispersion caused by the transverse Poisson’s effect in a magneto-electro-elastic circular rod (MEECR). Applying a wave transformation to the local M-fractional LWE, the equation can be turned into a set of algebraic equations. Solving the algebraic equation system, we procure the soliton solutions of the local M-fractional LWE. Both the obtained solution functions in the study and the graphical simulations depicted for these functions. It will assist researchers working in this field in the physical interpretation of this equation. Moreover, the reported solutions propose a rich platform to examine the local M-fractional LWE

    Uniportal video-assisted thoracoscopic surgery esophagectomy outcomes in 40 consecutive patients

    Abstract Objectives: Minimally invasive esophagectomy has improved over time becoming faster and less invasive. We have changed our technical approach from multiportal to uniportal video-assisted thoracoscopic surgery (VATS) esophagectomy over the years. In this study, we analysed our results with uniportal VATS esophagectomy technique. Methods: This study was a retrospective analysis of 40 consecutive patients with the intent to perform uniportal VATS esophagectomy for esophageal cancer between July 2017 and August 2021. Demographic criteria, comorbidities, neoadjuvant therapy, intraoperative data, complications, length of stay, pathological data, 30- and 90-day mortality and 2-year survival data were recorded. Results: Forty patients (21 female) were operated (median age 62.9 [53.5-70.25]). Eighteen patients (45%) received neoadjuvant chemoradiation. The chest part of all cases was started with uniportal VATS and 31 (77.5%) was completed uniportally (34 Ivor Lewis, 6 McKeown). The median thoracic operation time in minimally invasive Ivor Lewis esophagectomy was 90 min (77.5-100). The median time for uniportal side-to-side anastomosis was 12 min (11-16). Five (12.5%) patients had leak, and 4 were intrathoracic. Twenty-eight (70%) patients had squamous cell carcinoma, 11 adenocarcinoma and 1 squamous cell carcinoma with sarcomatoid differentiation. Thirty-seven (92.5%) patients had R0 resection. The mean number of lymph nodes dissected was 24 ± 9.5. Thirty- and ninety-day mortality was 2.5% (n = 1). The mean follow-up was 44 ± 2.8 months. Two-year survival was 80%. Conclusions: Uniportal VATS esophagectomy is a safe, fast and feasible alternative to other minimally invasive and open approaches. Comparable results to contemporary series are observed in perioperative and oncologic outcomes

    Özet : Prematüre yenidoğanlarda gavajla beslenmeden total oral beslenmeye geçişin iyi bakım uygulamaları ile desteklenmesi, oral beslenme becerisinin etkin ve güvenli bir şekilde başlatılmasını sağlar. Bu araştırma, orafarengeal kolostrum uygulanan prematüre yenidoğanların tam enteral beslenmeye geçişinde oluşturulan gece gündüz döngüsünün etkisini belirlemek amacıyla yapıldı. Mayıs–Aralık 2022 tarihleri arasında Randomize kontrollü deneysel araştırma deseninde yapılan araştırmanın verileri, Sağlık Bilimleri Üniversitesi Bağcılar Eğitim ve Araştırma Hastanesi Yenidoğan Yoğun Bakım Ünitesinde yatan, 28-32 gestasyon haftasında olan, araştırmaya dahil edilme kriterlerini karşılayan 30 deney 30 kontrol olmak üzere 60 prematüre yenidoğanla etik izinler alındıktan sonra gerçekleştirildi. Her iki grup yenidoğanların ağız boşluğuna 0.1 ml'lik kolostrum sağ ve sol bukkala 14 gün süreyle, 3 saat arayla uygulandı. Deney grubunda gündüz zemini beyaz, gece koyu renkli kuvöz örtüsü ile 12 saatlik gece-gündüz döngüsü oluşturuldu. Kontrol grubunda gece -gündüz döngüsü ünitede rutin kullanılan küvöz örtüsüyle (zemini beyaz) sağlandı. Hasta tanılama formu ve hasta izlem çizelgeleri ile elde edilen veriler, SPSS (Statistical Package for Social Sciences) for Windows 22.0 programı ve analitik-tanımlayıcı istatsitiksel yöntemler ile p<0,05 anlamlılık düzeyinde değerlendirildi. Kontrol ve deney grubunun tanıtıcı özellikler açısından istatistiksel olarak benzer olduğu, gece-gündüz döngüsü oluşturan deney grubunun kontrol grubuna göre daha erken tam enteral beslenmeye geçtiği, daha fazla kilo artışına sahip olduğu saptandı (p<0,05). Araştırmada; orafarengeal kolostrum uygulanan prematüre yenidoğanlarda tam enteral beslenmeye geçiş süresine gece gündüz döngüsünün etkili olduğu bulundu

    ÖZET : Bitkisel ürünler, geçmişten günümüze kadar birçok kültürde beslenmenin önemli bir parçası olmuştur. Ülkemizde bitkisel ürünler oldukça yaygın olarak kullanılmaktadır. Bitkisel ürünler konusunda sağlık çalışanlarının görüşlerinin ne olduğuna dair yeterli çalışma bulunmamaktadır. Bu çalışma sağlık çalışanlarının bitkisel ürünleri güvenli kullanımına yönelik tutumlarını değerlendirilmesi amacıyla yürütülmüştür. İstanbul Biruni Üniversitesi Hastanesi'nde çalışan 160 sağlık çalışanı çalışmaya dahil edilmiştir. Veriler yüz yüze toplanmıştır. Çalışmanın veri toplama araçları olarak Kişisel Bilgi Formu ve Bitkisel Ürünlerin Güvenli Kullanımına Yönelik Tutum Ölçeği Anketi kullanılmıştır. Verilerin analizinde SPSS-15.0 programı kullanılmıştır. Çalışmada ölçekten alınan toplam puana göre meslekler arası istatiksel açıdan fark bulunmuştur (p<0.05). En yüksek puanı sağlık lisansiyeri grubu (diyetisyen, fizyoterapist, eczacı, odyolog) almıştır. Düzenli bitkisel ürün kullanan sağlık çalışanlarının ortalama puanları daha yüksek bulunmuştur ve istatiksel açıdan önemli bulunmuştur (p<0.05). Düzenli bitkisel ürün kullanan sağlık çalışanları ve sağlık lisansiyeri meslek grubunun bitkisel ürünlerin güvenli kullanımına yönelik daha doğru bilgiye sahip olduğu ve daha olumlu tutum gösterdikleri sonucu bulunmuştur. Ölçek sonuçları ayrıca 4 alt faktörde incelenmiştir. Bu faktörler meslek ve eğitim durumlarına göre değerlendirilmiştir. Eğitim açısından sonuçlara bakıldığında faktör 2'de kadınlarda "Ön lisans – lisansüstü" arasında ve "lisans – lisansüstü" arasında önemli fark bulunmuştur (p<0.05). Faktör 3'te ise kadınlar arasında "Ön lisans – lisansüstü" grupları arasında önemli fark bulunmuştur (p<0.05). Bu sonuçlara göre lisansüstü eğitimi almış kadın sağlık çalışanlarının faktör 2 ve faktör 3 konularında daha doğru bilgiye sahip olduğu sonucuna ulaşılmıştır. Meslekler açısından sonuçlar incelendiğinde, faktör 2 ve faktör 3 açısından önemli farklılık bulunmuştur (p<0.05). İstatiksel farklılığın kaynağına göre sağlık lisansiyeri grubu faktör 2 ve faktör 3 konularında daha yüksek bilgiye sahip olduğu ve diğer mesleklere göre daha pozitif bir tutum gösterdikleri sonucu bulunmuştur.ABSTRACT : Herbal products have been an important part of nutrition in many cultures from past to present. Herbal products are widely used in our country. There are not enough studies on the opinions of health professionals about herbal products. This study was conducted to evaluate the attitudes of healthcare professionals towards the safe use of herbal products. 160 healthcare professionals working at Istanbul Biruni University Hospital were included in the study. Data were collected face to face. The Personal Information Form and the Attitude Scale for the Safe Use of Herbal Products Questionnaire were used as the data collection tools of the study. SPSS-15.0 program was used in the analysis of the data. In the study, a statistical difference was found between occupations according to the total score obtained from the scale (p<0.05). The highest score was received by the health licensee group (dietitian, physiotherapist, pharmacist, audiologist). The mean scores of healthcare professionals using regular herbal products were found to be higher and statistically significant (p<0.05). It was found that the health professionals and health licensors who regularly use herbal products have more accurate information about the safe use of herbal products and have more positive attitudes. Scale results were also analyzed in 4 sub-factors. These factors were evaluated according to their occupation and education status. Considering the results in terms of education, a significant difference was found between "Associate degree - graduate" and "undergraduate - graduate" among women in factor 2 (p<0.05). In factor 3, a significant difference was found between the "Associate degree - postgraduate" groups among women (p<0.05). According to these results, it was concluded that female health workers who received postgraduate education had more accurate information on factor 2 and factor 3. When the results were examined in terms of occupations, a significant difference was found in terms of factor 2 and factor 3 (p<0.05). According to the source of the statistical difference, it was found that the health license group had higher knowledge on factor 2 and factor 3 and showed a more positive attitude than other professions

    The effect of professional belonging in midwives on their attitudes towards medical errors

    Özet Mesleki aidiyet, çalışan bireylerin mesleğine yönelik düşünce ve davranışlarının belirlenmesinde iş verimini pozitif yönde etkileyen bir kavramdır. Bu araştırma, ebelerin mesleki aidiyeti ve tıbbi hataları arasındaki ilişkiyi belirlemek amacıyla tanımlayıcı ve ilişki arayıcı tipte Ağustos 2022-Ocak 2023 tarihleri arasında kamu hastanelerinde çalışan ve çalışmayı kabul eden 475 ebenin katılımı ile oluşturuldu. Katılımcılara "Tanıtıcı Bilgi Formu", "Ebelik Aidiyet Ölçeği" ve "Tıbbi Hatalarda Tutum Ölçeği" uygulandı. Ebelerin yaş ortalaması, 27.58±5.49'dur. Ebelerin %70.4'ünün lisans mezunu, %97.4'ünün lisans ve üzeri eğitimlerinin olduğu, %64.5'inin evli olduğu, %93.6'sının çekirdek aile tipinde olduğu ve %24.4'ünün de gelirinin giderinden düşük olduğu saptandı. Evli olan ebelerin eşlerinin; %35.5'inin lisans ve üzeri eğitimleri olduğu ve %64.5'inin çalıştığı saptandı. Ebelerin aidiyet toplam, duygusal aidiyet, mesleki rol ve sorumlulukları yerine getirme, mesleki gelişme ve olanakları değerlendirme, meslekte görev ve yetki sınırı puanlarında, gebelik sayısı, düşük sayısı, doğum sayısı ve yaşayan çocuk sayısına göre istatistiksel olarak anlamlı fark saptanmadı (p>0.05). Ebelerin, tıbbi hataya yaklaşım puanlarının; meslekten memnun olanların puanları (x=3.70), meslekten memnun olmayanların puanlarından (x=3.86) istatistiksel olarak anlamlı düzeyde düşük bulundu (t=-2.28; p=0.02<0.05; d=0.23; η2=0.01). Sonuç olarak ebelerin mesleki aidiyet düzeylerinin yüksek olduğu, duygusal aidiyet, mesleki rol ve sorumlulukları yerine getirme, mesleki gelişme ve olanakları yerine getirme ve mesleki aidiyet duygusunun anlamlı bir şekilde arttığı bulundu. Sonuçların ebelerin mesleki aidiyet düzeyleri ve tıbbi hatalarla ilgili farkındalığı arttırmak amacıyla daha fazla çalışma ile desteklenmesi önerilmektedir. Abstract Professional belonging is a concept that positively affects work efficiency in determining the thoughts and behaviors of working individuals toward their profession. This descriptive and relationship-seeking study was conducted to determine the relationship between midwives' professional affiliation and medical errors, with the participation of 475 midwives who worked in public hospitals between August 2022 and January 2023 and agreed to participate. "Introductory Information Form", "Midwifery Belonging Scale" and "Attitude Scale towards Medical Errors" were applied to the participants. The average age of midwives is 27.58±5.49. It was determined that 70.4% of the midwives had a bachelor's degree, 97.4% had a bachelor's degree or higher, 64.5% were married, 93.6% were in a nuclear family type and 24.4% had an income lower than their expenses. Spouses of married midwives; It was determined that 35.5% had a bachelor's degree or higher and 64.5% were working. No statistically significant difference was found in midwives' total belonging, emotional belonging, fulfilling professional roles and responsibilities, professional development and opportunities evaluation, duty and authority limit scores in the profession, according to the number of pregnancies, number of miscarriages, number of births and number of living children (p>0.05). Midwives' approach to medical errors scores; the scores of those who were satisfied with the profession (x=3.70) were found to be statistically significantly lower than the scores of those who were dissatisfied with the profession (x=3.86) (t=-2.28; p=0.02<0.05; d=0.23; η2=0.01). As a result, it was found that midwives' professional belonging levels were high, emotional belonging, fulfilling professional roles and responsibilities, professional development and opportunities, and the sense of professional belonging increased significantly. It is recommended that the results be supported by further studies to raise awareness about midwives' professional commitment levels and medical errors

    The relationship of hematological parameters and C-reactive protein (CRP) with disease presence, severity, and response to systemic therapy in patients with psoriasis

    Abstract Objectives: Systemic inflammation has an important role in psoriasis, which is a chronic disease with an increasing prevalence and is associated with comorbidity. Our aim is to investigate the relationship of hematological parameters and C-reactive protein (CRP) with the presence and severity of the disease in patients with psoriasis. It is also to investigate whether it can be used as a biomarker in monitoring the response to systemic treatment. Materials and methods: This retrospective study was conducted with the participation of 139 psoriasis patients receiving biological therapy (BT) and conventional therapy (CT) and 140 healthy controls. Demographic, clinical, and laboratory data of patients and controls were examined and all parameters were compared with the psoriasis area severity index (PASI) score. In addition, the changes in these parameters before the treatment and in the third month of the treatment were examined in the patient groups who received BT and CT. Results: White blood cell (WBC), neutrophil, monocytes, platelet (PLT), plateletcrit, red blood cell, neutrophil-lymphocyte ratio (NLR), and platelet-lymphocyte ratio (PLR) monocyte-lymphocyte ratio (MLR), red cell distribution width (RDW), CRP and erythrocyte sedimentation (ESR) levels were higher compared to the healthy control group in psoriasis patients (p<0.05). Baseline PASI values were positively correlated with WBC, neutrophils, monocytes, NLR, MLR, and CRP. WBC, neutrophil, NLR, CRP, and ESR levels decreased in all patients in the third month of treatment (p<0.05). WBC, PLT, neutrophil, and NLR in patients receiving BT; while WBC, neutrophil, NLR, CRP, and ESR levels decreased in patients receiving CT, RDW levels increased (p<0.05). Adalimumab; NLR and basophil, methotrexate; WBC, NLR, neutrophil, and ESR levels caused a significant decrease (p<0.05). Conclusion: The fact that increased WBC, neutrophils, monocytes, NLR, MLR, and CRP levels are associated with the severity of psoriasis indicates that these parameters reflect systemic inflammation in psoriasis. In addition, the decrease in these parameters after BT and CT suggests that they can be considered simple and reliable markers that can be used as a complement to the PASI score in assessing disease severity and response to treatmen
