    Impact of one-year treatment with biotechnological drugs on work ability in patients with rheumatoid arthritis in Italy: a prospective real-life study

    Objectives: We aimed to evaluate the impact of biologic therapy on work productivity outcomes in an Italian real-life cohort of biologic-naïve patients with active rheumatoid arthritis (RA). Methods: This observational prospective multicentre study enrolled RA patients in working age with an active disease who started their first biologic agent. Every patient completed the RA-specific Work Productivity Survey (WPS-RA) at each clinical evaluation (baseline, 6 and 12 months). The primary outcome of the study was the productivity gain at 12 months from the beginning of the biologic treatment, compared to baseline, assessed in terms of absenteeism and presenteeism reduction, both for employed and unemployed subjects. Linear regression analyses were performed to assess the impact of patient- and disease-related variables on productivity gain. Results: Overall, 100 patients were enrolled and 85 completed the study. All indexes of disease activity and functional ability were significantly improved from baseline already at 6 months. At 12 months, the 55 employed subjects showed a significant reduction in the mean number of days of work missed (absenteeism) and of reduced productivity (presenteeism). A significant reduction in the mean number of days of household work missed was observed for all patients. At multivariate analysis, functional disability had a significant negative impact on all parameters of household work productivity, while the achievement of a low disease activity or remission was inversely correlated with presenteeism. Conclusions: One year of treatment with a biological drug significantly impacts on the disease activity and work ability of RA patients and allows economic gains due to productivity improvement

    Il riconoscimento dell'identità di genere tra sport e non discriminazione: la vicenda di Caster Semenya

    Caster Semenya è una atleta sudafricana diventata nota per aver vinto delle medaglie olimpiche nelle gare atletiche di mezzofondo, la sua specialità. Recentemente è stata protagonista di una importante decisione del CAS (Court of Arbitration for Sport), il tribunale arbitrale per lo sport. La questione in discussione riguarda il fatto che il corpo di Caster produce naturalmente, e non attraverso il doping, una quantità di testosterone che, secondo le altre atlete e la IAAF, le fa guadagnare un vantaggio sleale nelle competizioni atletiche. Siffatta produzione ormonale è dovuta alla sindrome che caratterizza Caster dalla sua nascita, ovvero il 46XY. Al fine di equiparare le "forze" sulla pista atletica, la IAAF ha limitato la tolleranza di 5 nanomoli di testosterone per litro di sangue per le atlete con sindrome 46XY. Caster ha sfidato questa disposizione davanti al CAS, mentre la IAAF ne ha sospeso l'applicazione. Il 1. maggio 2019, la Corte Arbitrale per lo Sport ha rigettato l'istanza e Caster nel frattempo ha mutato i suoi obiettivi sportivi. L'interesse di questa decisione va oltre la questione sportiva perché impone una conformità di genere anche a scapito della salute e delle qualità individuali che dovrebbero rendere unica ogni persona. Lo scopo di questo contributo è dimostrare perché la decisione del CAF non riguarda solo la salute degli atleti, ma consiste nella violazione di una barriera che deve rimanere insormontabile: il rispetto della dignità (e quindi delle caratteristiche) della persona.Caster Semenya is a South African athlete famous for winning Olympic medals in the middle distance race, her specialty. She was recently the subject of a decision by the Court of Arbitration for Sport. Caster's body naturally produces, and not through doping, a quantity of testosterone which according to her competitors and the IAAF makes her to gain an unfair advantage in athletic competitions. This hormonal production is due to the syndrome that characterizes Caster from her birth, namely the 46XY. In order to "equalize" the "forces" on the athletic track, the IAAF has set a limit of 5 nanomoles of testosterone per liter of blood on the runners who had the 46XY syndrome. Caster challenged this provision before the CAS and the IAAF suspended it for a certain period. On May 1st 2019, the Court of Arbitration for Sport rejected her claim, and in the meantime Caster changed her sport goals.The interest of this decision goes beyond the athletics matter because it imposes a gender conformity even to the detriment of the health and of individual qualities that should make each person unique. The purpose of this paper is to demonstrate why the CAF decision does not only concern the health of the athletes, but it consists of the violation of a barrier that must remain insurmountable: the respect for the dignity (and therefore for the characteristics) of the person

    The impact of PrEP: results from a multicenter health technology assessment into the Italian setting

    Introduction: The use of oral FTC/TDF for pre-exposure prophylaxis (PrEP) among high-risk people without HIV is emerging as an innovative strategy to decrease HIV epidemic. The study aims at evaluating the implications related to PrEP introduction, from a multidimensional point of view, with particular attention to the sustainability of its implementation. Methods: To achieve this objective a Health Technology Assessment was conducted, within 35 Italian Infectious Disease Departments. Data were collected from literature review and using validated questionnaires and literature evidence. The introduction of PrEP (applied as "add-on" and "substitute" prevention strategy) into the clinical practice was compared with a baseline scenario (use of condoms among men who have sex with men and serodiscordant couples, and the use of Needle Syringe Programme among injection drugs users). Results: Despite the improvement in patients’ quality of life, PrEP would generate a decrease of patient safety, and an increase in staff workflow, with investment in medical supplies and training courses. PrEP would lead to significant economic investments both for the NHS, and for citizens (40% and 2,377% respectively) if used as an add-on strategy, assuming FTC/TDF patent cost. With the off-patent drug, the NHS would benefit from an advantage (37%) and a shrink of the patients' expenditure emerged (+682%). More economic resources are required if PrEP is applied as a substitute strategy, considering both the patent (NHS: 212%; citizens: 3,423%) and the off-patent drug (NHS: 73%; citizens: 1,077%). Conclusions: The most cost-containing strategy would be the use of PrEP, as an add-on strategy, assuming the off-patent drugs, even if safety and organisational aspects would be deeply considered

    Circular business models in the European manufacturing industry: a multiple case study analysis

    Circular economy has recently emerged as an alternative industrial paradigm to the traditional “take, make, dispose” economic model, with the aim to promote more sustainable resource consumption patterns and production processes. However, more empirical research is needed to address the topic of how circular economy is adopted in practice by companies. In this study, we mainly adopt the lenses of circular business models and analyze three European case studies operating in the manufacturing industry that embraced circular economy principles in their business. In particular, the paper investigates and presents peculiar managerial practices to create and capture value in circular business models, and highlights the need to conceive a systemic perspective on the implementation of these practices, especially for companies operating in the manufacturing industry. Moreover, the paper shows to managers willing to embrace circular economy principles how the implementation of managerial practices for circularity can support the design of the manufacturing companies’ business model within which they operate. Finally, the paper argues about the role of a peculiar external condition, i.e., the environmental regulation, in influencing the way companies implement the managerial practices for circularity
