Mefenamic Acid Caplet Stability in Storage Rooms with Different Temperature and Humidity
Mefenamic acid is an unstable drug to light and humidity can cause decomposition. This research aim to get stability data of mefenamic acid stored in West Jakarta health centers as consideration to good drug storage. The research was done by determining 7 health centers using simple random sampling from 74 health centers in West Jakarta, then the drugs depository condition were observed by observation method during 6 months, and the evaluation of mefenamic acid caplet quality were observed by experiment method according to the 4th edition of The Indonesia Pharmacopoeia. The result of this research indicate that five of the depository condition were up to standard, the room temperature of these health centers range 23-29 οC with humidity of 58-67% and mefenamic caplet in those health centers also up to standard during depository 6 months. There were two health centers were the depository room with lam light, no air conditioning and no ventilation, room temperature of 30-35 οC and humidity of 68-70%, in which the color and content of mefenamic acid deteriorated
Work System Improvement of Evacuation Process Conducted by Emergency Medical Technicians Using Virtual Environment Modeling
The work of emergency medical technicians (EMT) during patient evacuation involves lifting task in an emergency situation, which results in the increasing risk of musculoskeletal disorders such as low back pain. The purpose of this research was to investigate the workplace and ergonomic aspect that influence work posture of the EMT using simulation approach in a virtual environment. Biomechanic model (mannequin) of the EMT had been simulated and analyzed by using LBA and OWAS method. The mannequin was given an improvement based on ergonomic principle of manual lifting task and then was reanalyzed. Improvement that can be used for the work system of the evacuation process conducted by EMT considering its nature situation is the work posture of personnel during the process of lifting the patient into the stretcher. The technique that can be used is the proper lifting techniques. The results of this research can be used as a recommendation to the work system of the EMT
Organophosphate Pesticide Exposure Effects toward Inhibition of Blood Cholinesterase Activity among Vegetable Farmers. Organophosphate pesticides can inhibit blood cholinesterase in human body
This study aimed to find relationship between length of exposure of organophosphate pesticides with cholinesterase enzyme activity in the farmers’ blood. The study was conducted at the Joint Farmers Group in Kelurahan Campang year 2009 using crosssectional study design and sampling by the simple random sampling method (56 respondents). Data collection was carried out by interview and blood cholinesterase was measured using the Livibond Cholinesterase Test Kit AF267. Results showed that 100% farmers were poisoned, with 71.4% suffer from light-over-exposure and 28.6% moderateover exposure. Bivariate test analysis using chi-square test showed that there are no statistically significant relationship between the length of exposure (year of working as pesticide farmer, spraying time per week, and the last time spraying) with poisoning level (over-exposure probable and serious-over exposure). It takes the role of government to educate and trained farmers how to use pesticide safely and the importance of personal protective equipment to reduce the poisoning level
Study on Policy and Implementation of Exclusive and Early Initiation of Breastfeeding in Indonesia
In Indonesia, the Ministry of Health has set an Exclusive Breast Feeding [EBF] target of 80%, which is considered asunrealistic, especially where the current trend of EBF is showing a decline. The aim of this paper is to review the implementation and the policy of EBF and Early Initiation of breastfeeding (EI) in Indonesia based on existing studies. The policy, as stated in Kepmenkes No. 237/1997, PP No. 69/1999, and Kepmenkes No. 450/2004, was analysed using content, context, process and actor models, and triangulated by an advocacy coalition framework. Review on implementation shows that EBF practice in Indonesia is still very low and midwives have not been facilitating EI optimally. Policies on EBF are not complete and not comprehensive. EI has not been included explicitly and several aspects of policy content should have been updated. The advocacy coalition framework analysis confirms the findings of earlier analysis by emphasizing weaknesses in the external system as well as policy sub-system in the development of EBF policy. It is suggested to update and renew the existing EBF policy as to be more relevant in terms of content, context, process, and actor. An EBF policy should always include an Early Initiation component. The new policy should also include sanction, reward, and monitoring and evaluation to strengthen the implementation of the policy in community
Inhibition of Bile Acid Accumulation Decreased the Excessive Hepatocyte Apoptosis and Improved the Liver Secretion Functions on Obstructive Jaundice Patients
Excessive hepatocyte apoptosisinduced by bile acid accumulation occurred in severe obstructive jaundice, and impair the liver secretion function. The objective of this study is to determine whether the inhibition of bile acid accumulation through bile duct decompression affect the excessive hepatocyte apoptosis and caused improvement the liver secretion functions on human model. In this study we use a before and after study on severe obstructive jaundice patients due to extra hepatic bile duct tumor was decompressed. Bile duct decompression was performed as a model of the role of inhibition of bile acid accumulation inhibition bile acid accumulation and excessive hepatocyte apoptosis. Bile acid and marker of liver secretion functions were serially measured. Liver biopsy pre and post decompression was performed for Hepatocyte apoptosis pathologic examination by TUNEL fluorescing, which measured by 2 people in double blinded system. Total bile acid, and liver secretion functions were measured by automated chemistry analyzer. The result of this study shows that twenty one severe obstructive jaundice patients were included. After decompression the hepatocyte apoptosis index decreased from an average of 53.1 (SD 105) to 11.7 (SD 13.6) (p < 0.05). Average of bile acid serum decreased from 96.4 (SD 53.8) to 19.9 (SD 39.5) until 13.0 (SD 12.6) μmol/L (p < 0.05) Total bilirubin decreased from 20.0 (SD 8.9) to 13.3 (SD 5.0) until 6.2 (SD 4.0) mg/dL (p < 0.05), while the phosphates alkaline (ALP) and γ-glutamil transpeptidase (γ-GT) activities also decreased significantly. In conclusion, bile acids accumulation and excessive hepatocyte apoptosis through bile duct decompression improve the liver secretion functions by inhibition mechanism
The Knowledge and Experience of Dengue Mosquitoes among Housewives
Dengue Hemorrhagic Fever (DHF) prevention programs in Semarang, were focused through controlling mosquito breeding sites (PSN), but the implementation of PSN was not become a habit in every household. The objective of this study was to explore knowledge and experience of dengue mosquitoes among housewives in the endemic villages.The research was using qualitative methods. Subjects of the study were 17 housewives which selected by purposive sampling. The data collection was carried in Sendangmulyo village, Semarang, through observation, focus groups discussions, and indepth interviews. The techniques used to test data validity were triangulation and member checking method. Data were analyzed using content analysis approached. The results showed that housewives classifying mosquito based on time occurrence whether the presence of mosquito in environment was perceived naturally. Unoptimalized PSN behavior was based on the lack of housewives knowledge on larvae development stages. Mosquito was not considered as a threatening because night mosquito biting was directly more disturbing rather than day mosquitoes’. Health promotion program could increase dasa wisma cadres knowledge and skill, particularly on mosquito life cycle and the correct stages of PSN behavior. This study did not distinguish the demographic characteristics of informants. Further reserch could explore it or develop media based on local knowledge and experience
The Relationship between Food Pattern and Hemoglobin Level in Pregnant Women at Maros, South Sulawesi
Food consumption pattern is known as a determinant factor for nutritional problems among pregnant mothers. This study was intended to assess food consumption and its relationship to anemia in Maros Districts, Indonesia. This study was conducted in two sub-districts and pregnant mothers was randomly selected (n = 200) and proportionally from both districts. Data was collected by train field workers including measurement of hemoglobin, height and weight, 24-hour recall and food frequency questionnaire. Multivariate analyses were performed to see the relationship between food consumption and anemia. It showed that anemia prevalence was 41% whereas mostly in mild and moderate levels (44% and 55% respectively). The most common pattern of food consumption was rice, fish, and some vegetables. However, vegetables and fruit mostly consumed only 3-6 time a week. Energy and protein intakes were only 59% to 72% recommended dietary allowance (RDA) or 1300 kcal and 48 gr respectively. Most vitamin was consumed only around 40% except for vitamin A (76%, 605 RE), folic acid (195%, 1170 ug), and Vitamin B12 (142%, 3,7 ug). However, iron and zinc intakes were only 6.1 gr (17.5% RDA) and 5.9 gr (44% RDA), respectively. Multivariate analyses showed that education duration of mothers, nutritional status, iron tablet intakes, vitamin C, and B6 consumption were significantly related to anemia of pregnant mothers in the study and accounted for 24% (p<0.05). We conclude that food consumption was relatively low and caused lack intakes for both macro and micro nutrients of pregnant mothers in the study. Education and nutritional status of the mothers contributed also to the anemia prevalence
Social Capital and the Use of Traditional Birth Attendant: Is It Relevant?
Using the services of traditional birth attendants (TBA) in childbirth is considered as one of the maternal mortality determinants in Indonesia. Researchers reported that mothers preferred to have the help of TBAs in childbirth because TBAs have such powers as prayers and mantras that help the delivery process. However, very little is actually known about the factors shaping their preference. This research investigates the role of social capital as to maternal preference for having TBAs in childbirth. A cross sectional data of Indonesia Family Life Survey (IFLS) 2007 was used. Social capital was measured by social cohesion and community trust. Maternal demographic factors were measured by marital status, employment status, and education. Chi-Square test was used to analyze statistical association. Finally, logistic regression was used to gaugetheir effects on the use of TBAs. The result showed that the existence of social cohesion and trust made a significant impact on the preference for choosing TBAs. In demographic factors, a comparable finding was found only at the educational level. The factors of having childbirth with the help of a traditional birth attendant are complex. An understanding of social context should be taken into consideration in making a serious effort to reduce the maternal mortality rate
Detection of Human Group A and C Rotaviruses in Pediatric Patients with Acute Gastroenteritis by Real Time RT-PCR Assay: A Preliminary Study
Rotavirus causes 25–55% of all hospital admissions for diarrhea and approximately 611.000 deaths every year in developing countries. Clinically, it is not possible to recognize the diarrhea caused by rotavirus and other infections. To know a causative agent of rotavirus gastroenteritis, availability of an accurate diagnosis assay is necessary. Therefore, we developed real time RT-PCR assay (rRT-PCR) assay for confirmation of infections of Group A or C rotaviruses simultaneously. A total of 54 stool samples obtained from pediatric patients (< 5 years old) was used in this study. All samples were tested for Group A rotavirus by Serological rapid test. Result of serological rapid test was compared with rRT-PCR assay to obtain the test accuracies of both assays. Result of this study showed that rates of positive testing for Group A rotavirus by serological rapid test and the rRT-PCR assay were 22.22% and 18.50%, respectively. Forty-two serology-negative specimens for Group A rotavirus were also PCR negative (100% specificity). Two serology-positive specimens for Group A rotavirus was rRT-PCR negative (confirmed by electrophoresis gel); therefore, rRT-PCR assay represents the decrease of 3.70% in the number of specimens that are positive for Group A rotavirus. For Group C rotavirus, all tested samples were no rRT-PCR positive and the results need to be confirmed in the future
Photoelectrochemical Detection of Dengue-Related Oligonucleotide Sequence using Anthocyanin as an Intercalating Agent and Electrochromic Material
World Health Organization (WHO) presupposes a confirmation of dengue virus infection diagnosis with two criteria, i.e. clinical and laboratory criteria. One of the basic methods used by most laboratories to diagnose dengue virus is to detect oligonucleotide sequence using a DNA amplification technique. In this research, the measurement of denguerelated oligonucleotide was conducted by photoelectrochemical method. The presence of oligonucleotide sequence in target DNA can be detected by DNA probe that is immobilized on TiO2 electrode. The DNA hybrid is then bound to electrochromic substance like anthocyanin that generates current when it is subjected to light. The photocurrent is directly proportional to the number of target DNA. The aim of this research is to obtain photoelectrochemical system that has sensitivity and high responsiveness toward the change in oligonucleotide concentration, especially the applicability of anthocyanin as a electrochromic substance and intercalating agent. Linearity (R2) generated from the change of current in response to concentration changes of target DNA (in the concentration range of 0.75–3.00 nM) is 0.9611. Thus, this method has the potential to be developed to detect the presence of dengue virus in biological sample
