    application/pdfWe retrospectively evaluated the results of mandibular setback by sagittal splitting ramus osteotomy performed by our department between 1989 at 2005. Two hundred-one patients were grouped into three categories; nonrigid fixation using intraosseous wiring with a mean intermaxillary fixation (IMF) period of 38.9 days (52 cases), rigid fixation consisting of positioning screw (18 cases, mean IMF period; 21.7 days) and mini-plate (131 cases, mean IMF period; 21.4 days) were reviewed. Regarding functional disturbance after surgery, a higher incidence of neurosensory deficiencies of mental nerve and TMJ disorders were observed in cases in which positioning screw or mini-plate were used, in comparison with wire osteosynthesis cases. To determine skeletal relapse within 12 months after surgery, lateral cephalograms were employed for cases in which the amount of R-, L- set back differences were less than 3mm. The relapse was minimal in positioning screw cases, while the mini-plate fixation showed a greater amount of relapse compared to wire fixation, suggesting that three-dimensional repositioning of temporomandibular joint and longer term intermaxillary fixation were useful to obtain skeletal stability.journal articl

As a result, we were able to achieve a good arrangement of occulusion without root resorption. The in superposition of the Cephaologram trace on a three-dimensional shape measuring system using a dental cast at initial diagnosis was useful for planing orthodontic treatment with visual and numerical prediction of tooth movement.journal articl

    application/pdfGranular cell tumor, also known as Abrikossoff's tumor, is a relatively uncommon benign neoplasm with a histogenesis that is still uncertain. This report describes a case of granular cell tumor that originated at the left lateral border of the tongue in a 21-year-old man. Surgical resection with surrounding tissue was performed under local anesthesia. As a result, the patient has remained free of disease for two years after surgery to date. Histopathological examination of the excised specimens demonstrates strands or sheets of large polygonal tumor cells with eosinophilic granular cytoplasm, with no cellular atypia or anomalous mitotic activity. Immunohistochemically, tumor cells showed reactivity to S 100 protein and neuron-specific enolase (NSE), suggesting Schwann cell origin of the lesion.journal articl

    application/pdfPorphyromonas gingivalis is a major etiological agent implicated in adult periodontitis. The survival of this organism in the periodontal pocket would require an ability to overcome oxidative stress induced by reactive oxygen species generated by neutrophils or exposure to free oxygen from blood. An antioxidant enzyme that may contribute to the survival strategy of this organism is superoxide dismutase (SOD). P. gingivalis has a characteristic SOD which is called "cambialistic SOD" from the Latin cambialis, suggesting change and denoting enzymes capable of making a cofactor substitution. We prepared a mutant P. gingivalis SOD that possessed a mutation of the amino acids near the active-metal by site-directed mutagenesis, and discussed the structural basis of the metal-specific activity. SODs distribute in three primary domain, Bacteria, Eucarya, and Archaea, then the survival strategy of P. gingivalis seems to be an evolutional strategy of early life in remote ages.journal articl

A male of 11 years old showed Angle Class I malocclusion of upper anterior crowding and missing lower lateral incisors. Upper lateral incisors extracted to reduce the arch length discrepancy and to make in our treatment plan upper canines move into the lateral incisors position. As a result, we were able to do shortening of treatment periods and to get excellent good alignment without root resorption. It was useful for diagnosis of position relation of apical root to use panoramic tomography and dental micro X-ray tomography (3DX).journal articl

    application/pdfWe reported a rare case of cemento-ossifying fibroma on the right jaw of an elderly person. A 66-year-old woman complained of gingival swelling in the right first and second small molar region. Radiographic examinations demonstrated a relatively well-demarcated radiolucent lesion that was continuous with the roots of the teeth and, contained small radiopaque spots. We enucleated the lesion under general anesthesia, and the final diagnosis was cemento-ossifying fibroma. We further discuss the mechanism underlying the development of this tumor in an elderly person.journal articl

    application/pdfFor the treatment of jaw deformity with facial asymmetry, it should be important to achieve both functional and esthetic improvement. We report two cases of surgical correction of facial asymmetry with surgically assisted rapid palatal expansion (RPE) with Le fort I cortiotomy, prior to sagittal splitting ramus osteotomy (SSRO). Rapid palatal expansion in these cases attempted to achieve a sufficient setback range, to improve facial symmetry. Le fort I cortiocotomy was performed unilaterally on the unshifted side under general anesthesia, to reduce the physical and psychological stress of the patient. Combination of RPE and SSRO was a more effective procedure for the correction of facial asymmetry, and decrease the duration of orthodontic treatment.journal articl

    application/pdfThis article presents the characteristics and roles of dental hygienists in the Department of Orthodontics at Matsumoto Dental University. The presence of a fixed orthodontic appliance makes tooth cleaning more difficult and facilitates plaque build-up. Orthodontic patients are thus counseled and treated by hygienists before placement of the orthodontic appliance and are instructed in tooth brushing techniques during orthodontic treatment. In our clinic, additional roles of hygienists are to provide tooth brushing instructions (T.B.I.) and myofunctional therapy (MFT). Hygienists practice T.B.I, with various devices. Utilization of MFT to correct adverse oral habits during the treatment of malocclusion can be of value to the practicing orthodontist. This approach is also useful for achieving more complete treatment results and preventing post-orthodontic relapse. We also describe the contents ofMFT as practiced by hygienists in our clinic.journal articl
