    高機能広汎性発達障害者における歯科診療場面の特性

    松本歯科大学博士(歯学)2013甲第159号application/pdf目的:120万人の自閉症者のうちアスペルガー症候群を含めた高機能自閉症の歯科場面における行動特性は明らかにされていない。そこで高機能広汎性発達障害者の歯科治療に対して配慮すべき事項などを検討する目的で、浸潤麻酔下の歯科治療場面での行動を観察し、各診療場面での特性について検討を行った。 対象および方法:調査対象者は平成25年3月から9月までに一般開業医に来院した患者のうち、浸潤麻酔下で歯科治療を行った高機能広汎性発達障害者(高機能自閉症、アスペルガー症候群)の16名である。比較対照は、同様な歯科治療を行った定型発達児40名である。本研究は、松本歯科大学倫理審査等検討委員会の承認(許可番号第0417号)を得たうえで代諾者を選定し、口頭と書面にて説明を行い、承諾を得た者を対象とし実施した。 調査方法は歴年齢、性別、受診回数、浸潤麻酔経験の有無、定期健診経験の有無、抜歯経験の有無を調査した。また、発達年齢は遠城寺式幼児分析的発達検査を用いて算出した。 対象者は、入室から歯科治療を受け退出までの行動をVTRにて撮影した。評価対象は、「①診療台に座る」から「⑫診療台から降りる」の12場面に区分した。各診療場面での判定は「適応」、「やや不適応」、「不適応」に分類して評価した。治療はすべて同一の歯科医師が行い、行動評価は治療に関与しない歯科医師および勤務する歯科衛生士の2名で行った。 分析方法は受診回数の比較は、Studentのt検定を用いた。各診療場面における適応性の比較検討は、Fisherの直接確率計算を用いた。またすべての診療場面において1回以上の「不適応」と評価された者を不適応、「不適応」と評価されずに1回以上「やや不適応」と評価された者をやや不適応、すべて「適応」と評価された者を適応として、高機能広汎性発達障害児群と定型発達児群との比較はχ2検定にて検討した。また、各診療場面の関係性を適応性を従属変数として患者背景、発達年齢、局所麻酔経験、定期検診の有無、受診回数、治療内容の13項目を独立変数として決定木分析を行った。 結果:高機能広汎性発達障害児群は、男児が13名、女児が3名、定型発達児群は、男児が16名、女児が24名であった。歯科診療における適応性は全ての診療場面において高機能広汎性発達障害児群と定型発達児群とは有意差を認めた。 37 決定木分析における適応要因は「診療台に座る」、「開口指示」、「口腔内診査」の適応性に関与する要因は移動運動の発達年齢で、4歳6ヵ月以上であれば適応性を示す可能性が示唆された。また適応性に最優先される診療場面は、「リクライニング位に倒す」、「説明」、「表面麻酔の塗布」、「表面麻酔奏効時間」、「浸潤麻酔」、「効果発現までの待ち時間」、「歯科処置」、「座位への変化」の8場面であった。また、高機能広汎性発達障害児が浸潤麻酔下での歯科治療において87.5%の者が歯科治療を妨げる体動がなく、歯科治療が実施できた。 考察:それぞれの診療場面で診療の妨げにならないが、体動などの行動がみられることが高機能広汎性発達障害の特性である。その行動には、原因があり、見通しが持てない、不安が原因の時は、診療場面ごとに絵カードで説明することや手鏡で口腔内を見せながら診療を進めるなどの対応を個々に行うことが歯科資料を円滑に進めることにつながると思われる。障害が疑われるか確定するに至っていない早期からの関わりは長期にわたって行動や発達を観察することで歯科医院に慣れさせることが可能となることが示された。 今回の調査において、患者の持つ能力を十分に評価し、十分な環境体制作りと関係者の理解を得ることによって地域の開業医での歯科治療は可能と考えられる。doctoral thesi

    application/pdfBackground: Salivary histatins are bioactive peptides related to the innate immune system associated with antimicrobial activities. However, very little is known about the physiological and biological functions of histatins against host cells or their role in oral cell inflammation. Histatin 3 binds to heat shock cognate protein 70 (HSC70, a constitutively expressed heat shock protein (HSP)). It is unclear whether HSC70 is involved in the inflammatory response in oral cells. Injured oral cells release some intracellular proteins including HSC70. It is possible that released HSC70 induces toll-like receptor (TLR) activation, just as extracellular HSP70 (a stress inducible HSP) does, and that histatin 3 affects this process. Therefore, we tested the hypothesis that HSC70 activates TLR signaling and histatin 3 inhibits this activation and inflammatory cytokine production. Methods: A nuclear factor (NF)-κB-dependent luciferase reporter plasmid was transfected into HEK293 cells stably expressing TLR2 with coreceptor CD14 (293-TLR2/CD14 cells) or stably expressing TLR4 with CD14 and the accessory molecule MD2 (293-TLR4/MD2-CD14 cells). The cells were stimulated with HSC70 in the presence or absence of histatin 3, and examined using luciferase assays. We also stimulated human gingival fibroblasts (HGFs) with HSC70 with or without histatin 3. Then, we analyzed the levels of inflammatory cytokines (interleukin (IL)-6 and IL-8) in the culture media. Cell proteins were analyzed using enzyme-linked immunosorbent assay and Western blotting with antibodies of mitogen-activated protein kinases and NF-κB inhibitor IκB-α, respectively. Histatin 3-bound form of HSC70 was analyzed using limited V8 protease proteolysis. Results: HSC70 induced NF-κB activation in a dose-dependent manner in 293-TLR2/CD14 and 293-TLR4/MD2-CD14 cells, and histatin 3 inhibited this process and when histatin 3 binding to HSC70 was precluded by 15-deoxyspergualin, which augmented NF-κB-triggered activation. In HGFs, histatin 3 also inhibited HSC70-induced inflammatory cytokine production, extracellular signal-regulated protein kinase phosphorylation, and degradation of IκB-α. Moreover, HSC70 in the presence of histatin 3 was relatively resistant to digestion by V8 protease compared with HSC70 in the presence of control peptide. Conclusions: Histatin 3 may be an inhibitor of HSC70-triggered activation of TLR signaling and inflammatory cytokine production and may be involved in inflammation processes noted in oral cells.journal articl

    Preventive Effects of a Kampo Medicine, Kakkonto, on Inflammatory Responses via the Suppression of Extracellular Signal-Regulated Kinase Phosphorylation in Lipopolysaccharide-Treated Human Gingival Fibroblasts

    application/pdfPeriodontal disease is accompanied by inflammation of the gingiva and destruction of periodontal tissues, leading to alveolar bone loss in severe clinical cases. The chemical mediator prostaglandin E2 (PGE2) and cytokines such as interleukin- (IL-)6 and IL-8 have been known to play important roles in inflammatory responses and tissue degradation. In the present study, we investigated the effects of a kampo medicine, kakkonto (TJ-1), on the production of prostaglandin E2 (PGE2), IL-6, and IL-8 by human gingival fibroblasts (HGFs) treated with lipopolysaccharide (LPS) from Porphyromonas gingivalis. Kakkonto concentration dependently suppressed LPS-induced PGE2 production but did not alter basal PGE2 levels. In contrast, kakkonto significantly increased LPSinduced IL-6 and IL-8 production. Kakkonto decreased cyclooxygenase- (COX-)1 activity to approximately 70% at 1mg/mL but did not affect COX-2 activity. Kakkonto did not affect cytoplasmic phospholipase A2 (cPLA2), annexin1, or LPS-induced COX-2 expression. Kakkonto suppressed LPS-induced extracellular signal-regulated kinase (ERK) phosphorylation, which is known to lead to ERK activation and cPLA2 phosphorylation. These results suggest that kakkonto decreased PGE2 production by inhibition of ERK phosphorylation which leads to inhibition of cPLA2 phosphorylation and its activation. Therefore, kakkonto may be useful to improve gingival inflammation in periodontal disease.journal articl

    音楽が疼痛閾値に与える影響

    application/pdf要旨【背景と目的】痛みは組織の障害や疾患を認識させる重要な感覚であるが,不快感や機能障害をもたらすことが多くQOL(Quality of Life)の低下につながる.しかしそのような痛みは,スポーツ等他の事に熱中している時は認知が低下するという事例が多数報告されているため,痛みの認知は行動や情動と密接な関係があることが示唆される.そこで,多様な条件を加えた時の侵害刺激に対する認知程度をVAS(Visual Analog Scale)を用いて調べた結果,音楽を聞いている時は痛みの認知程度が低下する事がわかった.そこで本研究では,音楽を聞くことにより疼痛閾値はどの程度変化するか,また侵害刺激に反応していた帯状回の神経活動は音楽を聞くことにより変化するかを検討した.【方法】被験者45名を対象に前腕内側と足首内側に電極 を貼り知覚・痛覚定量分析装置(Pain VisionRPS–2100N:ニプロ株式会社)を用いて,無条件時と3 種類の音楽(ポップス・バラード・クラシック)を聞かせたときの知覚閾値(最小感知電流値)と疼痛閾値(痛み対応電流値)を測定し比較検討した.さらに,口腔内測定用の電極を舌・頬粘膜・上顎歯肉・下顎歯肉に置き,上記と同様に無条件時と3 種類の音楽を聞かせたときの疼痛閾値を測定し比較検討した.また,被験者8 名を対象にPain Visionから流れる電流80μAを侵害刺激として足首内側に与えた時の帯状回の神経活動を機能的磁気共鳴装置(fMRI)で調べ,無条件時と3 種類の音楽を流している時の活動状態を比較した【結果と考察】 前腕の知覚閾値では無条件と3 種類の音楽による4 条件下での有意差は認められなかったが,前腕の痛覚閾値,足首の知覚閾値,足首の痛覚閾値では4 条件下での有意差が認められた(Friedmantest:順にp< 0.001,p< 0.05,p< 0.01).また口腔内4 箇所においても4 条件下での有意差が認められた(Friedman test:p<0.01).各部位のそれぞれの2 条件をWilcoxon signed–rankstestを用いて比較した場合,前腕知覚閾値ではポップスとバラード,ポップスとクラシックの間に有意差が認められ,痛覚閾値ではクラシックと他3 条件の間に有意差が認められた( 2 条件間のうち後者が前者に比較して閾値が上昇).足首知覚閾値では無条件とポップス,無条件とクラシック,バラードとクラシックの間に有意差が認められ,痛覚閾値では無条件とバラード,無条件とクラシック,ポップスとバラード,ポップスとクラシックとの間に有意差が認められた.舌の疼痛閾値は無条件とバラード,無条件とクラシック,ポップスとクラシックの間に有意差が認められ,頬粘膜の疼痛閾値は無条件とバラード,無条件とクラシック,ポップスとバラード,ポップスとクラシックの間に有意差が認められ,上顎歯肉では無条件とバラード,無条件とクラシック,ポップスとバラードの間に有意差が認められ,下顎歯肉では,無条件とバラード,無条件とクラシック,ポップスとクラシックの間に有意差が認められた.fMRIの実験では,侵害刺激に反応を示した帯状回での神経活動がポップスを聞くことにより2 名,バラードを聞くことにより1 名,クラシックを聞くことにより2 名の被験者において減弱した.これらの結果より,バラードやクラシックのようなスローテンポの曲を聞くことは疼痛緩和に非常に有効であることが示唆された.これは音楽の気分や感情に与える心理的作用と痛覚伝導系への抑制作用によるものだと考えられた.journal articl
