    Transient Increase of Retinal Nerve Fiber Layer Thickness after Vitrectomy with ILM Peeling for Idiopathic Macular Hole

The purpose of this study was to determine the long-term changes in the circumpapillary retinal nerve fiber layer (RNFL) thickness following macular hole surgery with internal limiting membrane (ILM) peeling combined with phacoemulsification. Methods. Thirty-eight eyes of 37 patients who had pars plana vitrectomy (n = 36) between 2010 and 2014 were studied. The average thicknesses of the global and the six sectors of the RNFL were determined before and at 1, 3, 6, 12, and 24 (n = 22) months (M) after the surgery by spectral-domain optical coherent tomography. The postoperative mean RNFL thickness at each time was compared to that before the surgery by paired t-tests. Results. The RNFL of the operated eyes was significantly thicker at 1 month (1 M) and 3 M in all but the inferior-nasal sectors. The significant increase remained until 12 M in the superior-temporal and superior-nasal sectors. In addition, the RNFL was also significantly thicker in the temporal-inferior sector at 12 M based on the findings in 38 eyes. Conclusions. The postoperative RNFL was thicker in all but the nasal-inferior sector for at least 12 M after surgery.     Atelocollagen enhanced osteogenesis in a geometricstructured beta–TCP scaffold by VEGF induction.

    application/pdfIn order to establish the convertibility of a host for bone augmentation, we herein developed a new honeycombshaped β-tricalcium phosphate (37H) using atelocollagen as a scaffold, which exhibited unique geometric properties for in vitro and in vivo studies.Human mesenchymal stem cells (MSC) were cultured with 37H or atelocollagen-coated honeycomb-shaped β-tricalcium phosphate (Col37H), and their osteoblastic differentiation was then analyzed. Atelocollagen promoted cell adhesion and formation of vessel-like structures in the tunnels of scaffolds of cultured MSC. The mRNA expression levels of type I collagen, osteocalcin, and VEGF were greater in MSC cultured with Col37H than with 37H. Bone generation with Col37H in the rat calvaria was greater than with 37H, and this was attributed to early vessel construction. A large number of blood vessels invaded tunnels from the periosteum and existing bone surface. A strong VEGF signal was detected immediately before the new bone surface in the tunnels of Col37H. These results indicate that the addition of atelocollagen to Col37H has potential in the construction of functional artificial bone.journal articl

    The effect on salivary flow of lip trainers and sonic toothbrushes in older Japanese patients

It also causes oral bacterial imbalance and increases risk of caries. Therefore, it is important to examine the effect of stimulation of the saliva flow by devices in an aged population with complaint of oral dryness sensation. The aim of this study was to improve and maintain the saliva flow and the oral wetness in Japanese elderly patients by facial mimetic muscle training using the lip trainer device or by using the sonic electric tooth brushing. Methods: Twenty-six patients were referred to the Department of Periodontology, Matsumoto Dental University Hospital, with complaints of oral dryness were enrolled in this study. All the patients had 20 or more teeth and were aged 60 years and above. They were randomly divided into two groups of 13 subjects each. In group P, subjects used the Patakara® lip trainer for 3 minutes, 3 times per day for 6 months. In group S, subjects used the Sonicare® sonic toothbrush for 2 minutes, 3 times per day for 6 months. Unstimulated and stimulated salivary flow rates, oral wettability and lip closing force were measured at baseline, and after 1 and 6 months. Clinical periodontal parameters: probing depth (PD), clinical attachment level (CAL) and bleeding on probing (BOP) were checked at baseline and 6 months. Results: Both of the groups showed increased timulated and unstimulated salivary flow rates, with group P showing better results than group S at 1 and 6 months. Group P also recorded better results for wettability than group S. Lip closing force was improved by facial muscle mimetic training at 1 and 6 months in group P.     application/pdfWe report a case of intramuscular lipoma of the left buccal region, along with a review of the relevant literature. Magnetic resonance imaging showed a spindle–shaped mass extending from the canine region to the anterior portion of the masseter muscle in the buccal–alveolar portion of the left mandible. The inner part was visualized as homogeneous high–signal intensity on both T1– and T2–weighted images, and a partial linear region with the same signal intensity as muscle was also observed. The removed tumor measured 35×40×15mm in size, and histopathological findings showed an intramuscular lipoma composed of adipose, muscle and connective tissues. Intramuscular lipomas account for 1.7%–2% of all lipomas, and they very rarely occur in the oral and maxillofacial region. Twenty–three cases with intramuscular lipomas in the oral and maxillofacial region, including our case, have been reported in Japan. The postoperative healing process is generally good. We followed–up our patient after explaining to him the importance of careful, long–term follow–up. It has been only 2 months, to date, since surgery but the outcome appears to be good.journal articl
