骨格性下顎前突に対してチンキャップタイプの上顎前方牽引装置を使用し,側貌変化に留意し,上下顎小臼歯抜歯による矯正治療を行った一症例
application/pdfSummary This case was a 10–years and 10–month–old girl who visited the hospital with a complaint of opposite anterior bite. Facial findings showed no anterior asymmetry, but lateral features showed a concave type. The molar relationship was Angle Class III on both sides,and the left side showed a more prominent class III relationship. Anterior cross bite and labial inclination of both upper and lower side anterior teeth were observed. The diagnosis was a case of skeletal mandibular protrusion with functional factors. It was decided to use a chin–cap type anterior traction device of the upper jaw that can promote the growth of the upper jaw and suppress the growth of the lower jaw. In the phase of adult treatment, it was decided to extract the maxillary right first premolar, the maxillary left second premolar,and the mandibular bilateral first premolar to establish Angle Class I. At the end of the dynamic treatment, the molar relationship showed Angle Class I on both the left and right sides, and a tight and stable cusp fitting was maintained. Although ANB did not change on the lateral cephalogram, labial inclination was observed on both upper and lower anterior teeth. The dynamic treatment period was 2 years and 11 months, and the end age was 18 years and 1 month. No problem was observed in the occlusal state 4 years after the device was removed, but it will be carefully observed in the future.journal articl
成長発育に伴って生じる歯列状態の変化がマウスガードの維持力に及ぼす影響
application/pdfSummary The use of mouthguards is one of the main strategies that are considered effective in preventing sports–related trauma.Although mouthguards are recommended for use in children during sports activities,little is known as to how growth and developmental changes in the oral cavity, jaw, and dentition affect the retention of mouthguards. In the present study, we designed mouthguards for primary,mixed(4 types),and permanent detention stages and performed tensile testing to determine the effects of dental growth and development on their retention.We also designed mouthguards for the left maxillary second milk molar,left maxillary second premolar, and left maxillary first molar to determine how the shape of the teeth affects their retention.Data was analyzed statistically,and the following results were obtained:1.Mouthguards for permanent detention stage had a significantly higher level of retention than those for primary and mixed detention stages.2.Mouthguards for primary detention stage had a significantly lower level of retention than those for mixed and permanent detention stages. 3.The retention of mixed detention stage mouthguards became poorer with the number of teeth requiring relief.4.Differences in the shape of the teeth(left maxillary second milk molar, left maxillary second premolar, left maxillary first molar)had no significant effect on the retention of mouthguards.These findings indicate that the level of mouthguard retention is lower in children at primary and mixed detention stages than in adults and children with complete eruption of central incisor to second molar.This difference may be attributed to the differences in the coverage areas of mouthguardsjournal articl
Macrophages promote bone regeneration through the activation of LepR (+) cells
松本歯科大学博士(歯学)2022甲第249号application/pdfABSTRACT Macrophages not only clean up damaged tissues but also promote the proliferation and differentiation of stem cells during the regeneration process in various tissues such as bone and muscle. However,the mechanisms by which macrophages promote bone regeneration has remained to be elucidated.We have shown here that macrophages promote the differentiation of an osteogenic progenitor Leptin Receptor (LepR)(+)lineage cells during the bone regeneration process through Wnt/β‑catenin signals.The cortical bone of tibiae was punctured in LepR‑Cre;Rosa26‑tdTomato(tdTomato)mice and then the clodronate liposome(Clo‑lip)or control liposome(Ctrl‑lip)was intraperitoneally injected into those mice to deplete macrophages.F4/80(+) macrophages was largely depleted in Clo‑lip‑treated mice. Micro CT analysis revealed that the newly formed bone volume in Clo‑lip‑treated mice was significantly lower than that in Ctrl‑lip‑treated mice in the bone injury site at 7 days post injury (dpi).LepR‑Cre labeled bone marrow stromal cells were markedly increased in Ctrl‑lip‑treated but not in Clo‑lip‑treated mice in the bone injury site at 4 dpi and the percentage of proliferating Ki67(+)LepR(+) cells is lower in Clo‑lip‑treated mice.Most of Sp7(+) osteoblasts in the injury site at 4 and 7 dpi were labeled by LepR‑Cre, suggesting that osteoblasts are differentiated from LepR‑Cre‑labeled lineage cells.To clarify the involvement of Wnt signals in the differentiation of LepR(+) cells into osteoblasts,the bone regeneration was examined using tamoxifen‑treated Axin2‑CreERT2;tdTomato mice in which cells with activated Wnt/β‑catenin signals express red fluorescent protein tdTomato. Clo‑lip administration decreased the percentage of Axin2‑ tomato(+) Sp7(+) cells in the bone injury site at 7 dpi.Together, macrophages promote Wnt signals in LepR(+) cells and then facilitate bone regeneration.doctoral thesi
