下顎第二大臼歯が埋伏した2症例
application/pdfSummary An impacted mandibular second molar is rare. Here,we report two cases of such impacted molars.Case 1:A 22–year–old man presented at our university hospital upon referral from his family dental clinic.A panoramic image revealed horizontal impaction of the left mandibular second molar.Since the patient preferred surgical extraction,the left mandibular second molar was extracted. Case 2:A 54–year–old man presented at our health and dental checkup center.Based on the results of a panoramic image,a dentist from oral radiology diagnosed him with impacted bilateral mandibular second molars.He suggested conducting a complete examination;however,these molars were simply observed and no treatment was provided.journal articl
Comparison of oral and gut microflora in octogenarians and centenarians in relation to independence
松本歯科大学博士(歯学)2022甲第251号application/pdfABSTRACT This study compared the differences in salivary and fecal bacterial flora between 80- and 100-year-old people in different independent states to provide a reference for the bacterial composition of the digestive tract in older people of different ages. Saliva and fecal samples were collected from eight centenarians and 10 octogenarians whose Barthel index (BI) is less than 60,which means poor independence, as well as 10 independent octogenarians from nursing homes.A dentist counted the number of teeth, and evaluated the denture and nutritional status. Bacterial DNA was extracted and multiplex 16S rRNA (V3–V4)sequencing was performed using the Illumina MiSeq platform.Composition and diversity were analyzed by LEfSe,student’s t-test, weighted PcoA distance, and Anosim analysis. Older people who were more independent had significantly better nutritional intake than those who were less independent, and this difference was not reflected in age.The digestive tract of well independent seniors has more normal flora,which also reflects the link between general health and gut flora.While the saliva of less independent seniors contains more pathogenic bacteria,which may be a result of their inability to effectively clean their oral hygiene due to their poor functional independence.In addition, Akkermansia, a genus of bacteria commonly considered to be probiotics,was more frequently found in the stools of the less independent eighty-year-olds.There was no significant difference in the intestinal microbiota of 80-year-old and 100-year-old individuals with low functional independence in the β diversity that constitutes the microbiota.On the other hand,there was a significant difference in β diversity in the salivary flora.The reason for this was thought to be the difference in the number of remaining teeth between 80-year-old and 100-year-old subjects.In addition, there was a significant difference in β diversity in both the intestinal and salivary microbiota between the 80-year-old group with low functional independence and the normal 80-year-old group.From the above, it was clarified that functional independence affects the diversity that constitutes the salivary and gut microbiota.In addition, it was suggested that functional independence affects the diversity of salivary and intestinal microbiota more than age.doctoral thesi

application/pdfSummary A simulation practice of direct composite veneer restoration was newly started during the clinical training period of the 5th year conservatory course of Matsumoto Dental University since the AY 2020. This manuscript presents an overview of the program and examines its educational effects based on the evaluation of an anonymous questionnaire by students who participated in the program in 2020. 72 students in total participated in the 5th year clinical internship in AY 2020, and the program was conducted 9 times for 8 students each in the Department of Operative Dentistry. After a lecture and demonstration by the instructor, four pairs of two students each were assigned to practice direct composite veneer restoration without tooth reduction by applying the layering technique. After the practice, a lecture on the technique,materials and equipment used was given, followed by exercises on the relevant questions of the National Dental Examination that were asked in the past. After the program, an anonymous questionnaire was administered to the participants regarding their level of knowledge of direct composite veneer restoration before and after the program and their self–evaluation of the completion of their practice. The response rate of the questionnaires was 100%. After the program, 68% of the respondents answered that they “understood the direct composite veneer restoration very well” and 31% answered that they “understood it somewhat well. None of the respondents answered that they “did not understand much” or “did not understand at all. When asked whether the contents of the program deepened their knowledge of operative dentistry, 83% of the respondents answered “Very effective” and 15% answered “Somewhat effective”, and none of the respondents answered “Not very effective” or “Not at all effective”. These results suggested that the program was effective in deepening the knowledge level of operative dentistry.journal articl

松本歯科大学病院口腔外科における過去3年間の外来および入院患者の臨床統計学的観察
application/pdfSummary.The Matsumoto Dental University Hospital is located in the center of Nagano Prefecture and is the core hospital in the area.In this study,we carried out a clinicostatistical survey of outpatients and inpatients for the past 3 years.The number of outpatient and inpatients were 6,545 and 861,respectively,for the study period.The referral rate was 71.8%,which annually increased.The number of hospitalized patients by age was bimodal in their 20s and 70s.Dental diseases were most common in outpatients,followed by temporomandibular joint diseases,mucosal/skin diseases,and inflammatory diseases.Inpatients mostly had dental diseases,jaw deformities,inflammatory diseases,and cystic diseases.Moreover,410 cases used the operating room.Consequently,the number was annually increasing.Therefore,this study suggests that our department is functioning as a regional core hospital.However,building a medical care system that strengthens regional cooperation so that it can meet the various needs is necessary.journal articl

歯周組織におけるWnh/β-cateninシグナル陽性細胞の分布
松本歯科大学博士(歯学)2021甲第243号application/pdf[要旨]Wntは分泌性の糖タンパク質であり,近年Wntシグナルが骨形成に重要であることがわかってきた.歯周組織の維持や改造にもWntシグナルが関与することが報告されている.しかし,生理的状態において,Wnt/β-cateninシグナル陽性細胞が歯根膜を含む歯周組織のどこに分布するか,また,その分布が加齢により変化するかどうかは未解明な部分が多い.本研究では,Axin2CreERT2-tdTomatoマウス(4週齢,6週齢,30週齢,48週齢)を用いて,歯周組織におけるWnt/β-cateninシグナル活性化細胞を観察した.Wnt/β-cateninシグナルの活性化は,その標的遺伝子であるAxin2が発現すると赤色蛍光タンパク質であるtdTomatoが発現することを指標にした.タモキシフェン(TAM)投与後3日において,Axin2の発現は,4週齢および6週齢では根尖部歯根膜,根管歯髄,根間中隔部歯槽骨および歯肉上皮部に多く認められた.しかし,30週齢マウスでは,Axin2発現細胞は減少しており,48週齢マウスでは,その発現はわずかであった.TAM投与後30日では,Axin2の発現細胞がすべての週齢において増加したものの,48週齢マウスではAxin2発現細胞の増加をわずかに認めるのみであった.Axin2発現細胞は,細胞増殖マーカーであるKi67を発現した.つまり,Axin2発現細胞は細胞増殖活性が高いことを示唆する.加齢に伴いAxin2陽性かつKi67陽性細胞は減少した.一方,加齢に伴い,歯槽骨2におけるSclerostin陽性骨細胞が増加した.以上よりWnt/β-cateninシグナル陽性細胞は,歯根膜,歯肉上皮,歯槽骨といった歯周組織や歯髄に分布し,細胞増殖が旺盛であることが示唆された.また,加齢に伴い,歯骨でのSclerostin発現が増加するため,Wnt/β-cateninシグナル陽性細胞が減少する可能性が示唆された.若齢マウスでは,根尖部歯根膜と根尖部歯髄象牙芽細胞でWnt/β-cateninシグナルが強く活性化されていた.加齢に伴い,Wnt/β-cateninシグナル陽性細胞とその細胞増殖は顕著に減少したdoctoral thesi
