    Neuron–Glia Crosstalk and Neuropathic Pain: Involvement in the Modulation of Motor Activity in the Orofacial Region

    application/pdfNeuropathic orofacial pain (NOP) is a debilitating condition. Although the pathophysiology remains unclear, accumulating evidence suggests the involvement of multiple mechanisms in the development of neuropathic pain. Recently, glial cells have been shown to play a key pathogenetic role. Nerve injury leads to an immune response near the site of injury. Satellite glial cells are activated in the peripheral ganglia. Various neural and immune mediators, released at the central terminals of primary afferents, lead to the sensitization of postsynaptic neurons and the activation of glia. The activated glia, in turn, release pro-inflammatory factors, further sensitizing the neurons, and resulting in central sensitization. Recently, we observed the involvement of glia in the alteration of orofacial motor activity in NOP. Microglia and astroglia were activated in the trigeminal sensory and motor nuclei, in parallel with altered motor functions and a decreased pain threshold. A microglial blocker attenuated the reduction in pain threshold, reduced the number of activated microglia, and restored motor activity. We also found an involvement of the astroglial glutamate–glutamine shuttle in the trigeminal motor nucleus in the alteration of the jaw reflex.     Characteristics of Clinical and Imaging Findings of Epidermoid Cysts under the Skin of the Mental Region

    application/pdfEpidermoid cyst often arises in the fl oor of oral cavity; however, it is relatively rare to arise subcutaneously in the mental region. We reported characteristics of clinical and imaging fi ndings of epidermoid cysts under the skin of the mental region. The patient was a 53-year-old male. He complained of a subcutaneous mass without pain in the left mental region. A palpable soft and movable thumb-sized mass without pain was observed at the fi rst examination. Ultrasound examination revealed a well-demarcated mass with heterogenous hyper-echoic internal echo and posterior echo enhancement. On power Doppler sonography, a small amount of blood supply was found around the periphery of the mass. Plain CT scan revealed a globular soft-tissue mass with peripheral high and central low CT values area. The mass was removed under local anesthesia using intraoral approach. A histopathologic diagnosis was epidermoid cyst. Recurrence is not observed 2 years after surgery. There were only 3 reports of epidermoid cysts in the mental region, including our case in Japan, clinically characteristics of age ranging 20 to 66 years of age and the mean age of 46 years, with 2 males and 1 female. The size was from 20 mm to 12 mm in diameter and from the tip of a thumb up to the tip of a small fi nger. Ultrasound images of the inside of epidermoid cysts that we experienced this time showed enhancement of relatively uniform hyperechoic image and posterior echo, which was characteristically slightly diff erent from fi ndings of epidermoid cysts generally reported.journal articl

    歯科用多目的超音波治療器エアフローSⅡⓇの歯面為害性

    松本歯科大学博士(歯学)2016甲第196号application/pdf近年,歯面の着色除去あるいはProfessional Mechanical Tooth Cleaning(PMTC)時にグリシン粉末がエアアブレーションに使用されている。今回,グリシン粉末によるエアアブレーションが歯面に為害性があるか,従来法と比較検討した。本研究に使用した機器は,1)エアフローSⅡⓇ(EMS社, スイス):新規法,2)クィックジェットⓇM(株式会社ヨシダ, 東京):従来法,3)ピエゾンマスターⓇ700 (EMS社, スイス):超音波,4)ラバーカップ(プロフィーカップPro ソフトタイプ(カボデンタルシステムズジャパン株式会社, 東京),回転数M+メルサージュファイン(株式会社松風, 京都):PMTCの4種類単独ならびにPMTCとの組合せとした。基礎的検討として,便宜抜歯した抜去歯を台座に頬側面を上面に固定し,新規法,従来法,超音波は歯面から3mm離して先端を固定,エアー圧0.5MPaにて5,10秒噴射した.超音波も5,10秒間操作した.それぞれ,セメントエナメル境(Cement Enamel Junction: CEJ)から根尖寄りセメント質,CEJから歯冠寄りエナメル質部に施行した.処置後に歯面を水洗乾燥し,走査型共焦点レーザ顕微鏡(OLS3000, LEXT, 東京)による非接触計測と表面形状の撮影をし,表面粗さ(Ra)を計測した。その結果,新規法はセメント質面において噴射時間を長くしても表面粗さは有意に増加せず,PMTC処置後に近い表面粗さであることが示された.この傾向は,エナメル質面においても同様であった.また,新規法は従来法と比較して,10秒処置後のエナメル質面で表面粗さは有意に小さかった。次に,臨床的検討を行った.対象者は,動的歯周治療終了後1年以上経過した,プラークコントロールが安定している歯周維持療法(Supportive Periodontl Therapy; SPT)期の患者で,かつ,歯面着色が認められるものとした.対象部位は着色が認められる上下顎前歯部頬側面とした.なお,歯周疾患に関連する全身疾患を有するもの,喫煙者等は除外した。処置は,新規法,従来法,PMTCの各々単独,各処置のPMTCとの併用処置とした.研究開始時に口腔内写真撮影と歯周組織検査を施行した。その後,各種の処置を施行,所要時間を計測した.術後1,3ヶ月に口腔内写真撮影と同時に,着色指数と知覚過敏等の為害性発現の有無を調査した。その結果,新規法により歯面清掃時間が短縮され,歯面着色については PMTC併用と同様の結果が得られた。また,研究期間中には歯周ポケット深さ(Probing Pocket Depth: PPD)や歯肉炎指数に変化はなく,知覚過敏等の為害性は確認されなかった。一方,従来法は歯面着色について新規法よりも多く認められる症例が多く,知覚過敏症状が発現することがあった.すなわち,臨床的検討によって、エアフローSⅡⓇの為害性の発現は確認されず,使用時の安全性,有効性が示唆された。doctoral thesi

    Association between number of teeth present and mandibular cortical erosion in Japanese men and women aged 40 years and older: A cross-sectional study

Additionally, many reports have demonstrated an association between decreased number of teeth present and osteoporosis. However, whether mandibular cortical erosion is associated with a decreased number of teeth remains unclear. The purpose of this study, therefore, was to clarify the association between mandibular cortical erosion and number of teeth present in Japanese men and women aged 40 years and older. Among patients who visited our university hospital and underwent dental panoramic radiography for the diagnosis of dental diseases, 839 patients (293 men and 546 women) aged 40e89 years (mean [SD], 63.7 [10.6] years) participated in this study. Multiple regression analysis revealed that mildly to moderately eroded cortex (p ¼ 0.007) and severe eroded cortex (p < 0.001) were significantly associated with a decreased number of teeth present. Analysis of covariance adjusted for covariates revealed a significant association between mandibular cortical erosion category and number of teeth present (p < 0.001). Subjects with a severely eroded cortex had significantly fewer teeth present than those with a normal cortex (mean [SE], 20.7 [0.5] vs. 23.4 [0.3], p < 0.001) or mildly to moderately eroded cortex (22.2 [0.4], p ¼ 0.04). Subjects with a mildly to moderately eroded cortex had significantly fewer teeth present than those with a normal cortex (p ¼ 0.033).     Effects of Placental Extract on Cell Proliferation, Type I Collagen Production, and ALP Secretion in Human Osteosarcoma Cell Line Saos-2

    application/pdfPorcine placenta extract (P-placenta) is widely applied in medicine and cosmetics. However, few studies have examined the effect of the extract on the cellular behavior of the osteoblastic cell line Saos-2. Here, we demonstrated that P-placenta enhances the proliferation, collagen type I production, and alkaline phosphatase (ALP) secretion of Saos-2 in vitro. Proliferation of Saos-2 was assessed by MTT and DNA synthesis assays. Type I collagen production and ALP secretion were evaluated using enzyme-linked immunosorbent assay and ALP assays. The cells were treated with/without 20, 200 and 2000 g/ml of P-placenta for 24 h. We found that 200 g/ml P-placenta significantly induced the proliferation of Saos-2 and enhanced type I collagen production and ALP secretion. The results indicate that P-placenta controls the cellular behavior of osteoblasts, resulting in the secretion of early bone-related biomarkers.journal articl
