    application/pdfWe treated an ITI hollow cylinder implant F-type (ITI・F) that had been removed from a patient due to breakage thereof and obtained the following conclusions.1. The possibility of breakage must be stated as one of the risks of dental therapy when using ITI・F.2. ITI・F osseointegrates with bone.3. If radiolucency extends to the perforations, the tissue inside the cylinder might be degenerated; therefore, ITI・F could be a candidate for removal.4.     application/pdfIn a conventional dental service system, a patient used to visit a dental office, and receive treatment on a dental chair. Recently, however, it has been promoted extensively that dentists visit home-bounded patients for the following reasons. 1) In light of the growing population of the aged, patients who have limited mobility and can not easily visit an ordinary dental clinic, so called bed-bounded elderly and so on, are on the increase rapidly. 2) The general public has recognised "the pleasure of eating" as a major factor for the quality of life, and are taking a growing interest in dental health. 3) The authorities concerned are promoting visiting health and medical care extensively. For instance, the Ministry of Health and Welfare has newly established the section of visiting medical care in the social insurance system. 4) People have become aware of the effects of the visiting dental service through the achievements of advanced dental surgeons. It should be indicated that the promoting of the visiting dental service brings several advantages to the dental profession which are listed as follows. 1) It would widely expand the dental service which has been limited to a dental office in the community up until now. 2) The dental profession would gain a better understanding of the necessity of the turnabout from conventional "repairing dentistry" to "comprehensive dentistry". 3) The dental profession would realize that their objects are "human", and that they are not only working to treat "bad teeth", but also serve patient's total health and the quality of life. 4) It would reinforce the idea of teamwork between the dental profession and other health, medical and welfare workers. However, there are several serious problems in today's visiting dental service. These problems have to be solved for the development of tomorrow's dentistry to continue. 1) The medical problems: Objects of the visiting dental service are often high risk patients. A unprepared treatment might bring the patient into danger. 2) The dental problems: Equipment is restricted by portability. Delicate procedures are difficult without dental chair and adequate lighting. Easy low quality treatment would be apt to be justified in such a situation. 3) The social problems: The visiting dental service is not a paying business under the social insurance system. And yet, community health care systems have not been established for adequate dental service. Hospital dentistry which has back-up ability for primary care by general practitioners is absent in almost all communities.journal articl

    application/pdfThe purpose of this study was to investigate the size of the primary tooth crowns, primary dental arches, standard values and frequency distribution of primary occlusion in Chinese children. With the cooperation of a kindergarten in Shijiazhuang-city, China, the dental plasters from 55 children (36 boys and 19 girls, age range from 3-6 years old) with normal primary occlusion were collected. Because of different growth rates of the children, these children were divided into two groups, one comprising children less than 5 years old and the other of 5 years old or older. According to the method that the Japanese Society of Pediatric Dentistry reported, the data were statistically analyzed and compared to Japanese children. The results were summarized as follows: 1. Except lower primary lateral incisors, the values of the mesio-distal width of primary crowns of each tooth were significantly larger for boys than for girls in all tooth types. Also the Chinese children were inclined to show smaller mesio-distal tooth crown width than those of the Japanese children in all types of teeth. 2. Compared with the Japanese children, the Chinese children were inclined to show smaller primary dental arch length both in the maxilla and the mandible. But in the senior group, the Chinese girls were inclined to show smaller primary dental arch width than their Japanese female counterparts. On the contrary, the Chinese children tended to show larger dimensioned primary dental arch height than those of the Japanese children. 3. Though spaces between teeth (primate and growth spaces) were found in the Chinese children, the frequency was lower than that of the Japanese children. 4. The frequency of terminal plans in the Chinese children was 41.8% in the vertical type, 6.4% in the distal-step type and 51.8% in the mesial step type. 5. The primary canines occlusal relationship in the Chinese children was 63.1% in type I, 13.6% in type II and 23.7 in type III.journal articl

    application/pdfThe name 'Tomes' is known to every dentist and dental student because of the dental 'fibrils' to which his name has been attached. He accomplished many achievements in dentistry, his first work was an article on the dental forceps in the London Medical Gazette on 4 June 1841. Then he described for the dental forceps in his landmark works, A Course of Lectures on Dental Physiology and Surgery in 1848, and A System of Dental Surgery in 1859. I will review Tomes's points of view for the dental forceps that has been described in the above three works. They are as follows: All forceps whatever should embrace the tooth they are used to extract at the neck; the neck being that part which is between the termination of the enamel and the free edge of the alveoli, and which is covered by gum. In order to arrive at this part of the tooth without difficulty, or unnecessary pain to the patient, the jaws must present an inclined plane terminating in an edge. The external surface of the jaws of forceps when closed, should present something like a cone, or parts of several cones, with the apex or apices cut off; and a perpendicular section should present an inclined plane, terminating in an edge, but more or less curved as may be suitable to the particular instrument. The length from the joint to the edge of the jaws should on no account be greater than will be necessary to allow sufficient space for the reception of the crown and neck of the tooth, so that no strength may be lost. The fangs of all teeth having a general conical form, forceps when well made and applied should be but as a lengthening of the cone towards its base. For removing teeth which are not decayed down to the gums, the ends of the jaws should be square, but when nothing but the fang remains of a tooth rounded ends are the more convenient, as with that shape they are readily introduced between the fang and enclosing alveolus. Instruments for extracting stumps should be made altogether lighter, the jaws thin and sharp at their convex ends, so that they may be made to cut rather than to tear the membrane connecting the fang with the adjoining tissues. Forceps should be constructed and used upon the principle of lengthening the tooth for the extraction of which they are intended; thus enabling the operator to move it from side to side, or rotate it if the fang be single, and of a shape admitting of such motion. After these lateral movements have been produced the tooth may, unless the fangs have some peculiar position or shape, be raised in a perpendicular direction, leaving as little injury from its removal as the operation can admit. I would sum up the position held by Tomes in relation to the development of the dental forceps by writing that, though he was not the first to think of or to publish the idea of forceps made to fit the necks of the individual teeth, he certainly thought of the idea independently, and put forward the best form of forceps which had up till then been devised, a form which has served as pattern for succeeding types up to the present time.journal articl

    application/pdfThe silver impregnation method for nucleolar organizer regions (Ag-NORs) was developed by Howell and Black (1980), and their one-step method is thought to be quiet useful as a marker of proliferation activities of cells. However it had a tendency to cause contamination due to non-specific absorption and precipitation of silver dots on the surface of the specimens. Therefore, the purpose of the present article is to evaluate the best method for the demonstration of Ag-NORs proteins using two materials; one is hyperkeratosis, and the other is squamous cell carcinoma. The original and modified methods used and the staining results are as follows: The original method was performed according to the Howell and Black method. AgNORs staining solution was prepared by dissolving gelatin in 1% aqueous formic acid at a concentration of 2%, this solution was mixed, 1:2 volumes, with 50% aqueous silver nitrate solution (this solution was freshly made at each time, filtered through a 0.02μm Millipore filter, Millex-GV^[○!R]: JAPAN MILLIPORE, Ltd., Tokyo), to create the final working solution. This was poured over the tissue sections and left for 25 minutes under safetylight conditions. These specimens were washed off for 15 minutes with running water. Counter staining was not used, and they were dehydrated and mounted. Staining results were difficult to discriminate construction like-mass from microscopic silver dots. However this method had a tendency to contaminate much of the surface of the specimens. The fixing method was used the same Ag-NORs solution as the original one. Tissue sections had the Ag-NORs solution poured upon them and left for 25 minutes under safetylight conditions. After washed off with running water, they were dipped into a 5% sodium thiosulfate pentahydrate solution and left for 5 seconds. Finally they were washed off for 15 minutes with running water, dehydrated and mounted. According to the results, it was considered possible to observe correct Ag-NORs by eliminating excessive absorption and precipitation of silver dots by 5% sodium thiosulfate pentahydrate solution. The next method, called the substitution method, substituted gold chloride acid tetrahydrate for silver nitrate. Therefore tissue sections had the same Ag-NORs solution poured on them just specimens as the former and were left for 25 minutes. After being washed off with running water, these were dipped into 0.1% gold chloride acid tetrahydrate solution and left for 5 seconds. After wards, these were washed with running water, dehydrated and mounted. These procedures were carried out under safetylight conditions. The results of this method are as follows. Althogh it is comparatively easy to observe microscopic silver dots, this staining contaminates the surface of many specimens, and this tendency was very similar to the original method. The fixing-substitution method is as follows; after fixation the tissue sections had the same Ag-NORs solution poured them as on the former specimens and left for 25 minutes. After washed off with running water, these were dipped for 5 minutes into 5% sodium thiosulfate pentahydrate solution and washed off with running water, and dipped for 5 seconds into 0.1% gold chloride acid tetrahydrate solution. Then, these were washed off for 15 minutes with running water, dehydrated and mounted. We suspected that this method was able to discriminate between microscopic silver dots and construction like-mass. No contaminations were observed on the surface of these specimens. It may be concluded that this method may prove to be better than other three methods.journal articl

    Straight Back Syndromeの麻酔管理

    application/pdfChest wall deformities may produce the mimic signs of organic heart disease. The straight back syndrome (SBS) is well recognized as "a pseudoheart disease" in this category. Although clinical identification of this syndrome was once thought sufficient to withdraw from any further investigation, many retrospective studies, conducted in the late seventies, pointed to a substantially higher incidence of SBS in patients with mitral valve prolapse syndrome. This association has clinical importance in that mitral prolapse syndrome is linked with serious cardiac events such as sudden death, infective endocarditis, dangerous ventricular arrythmias and congestive heart failure. Patients with SBS or skeletal abnormalities, even asymptomatic, have to be estimated clinically by careful auscultation of cardiac sounds, and chest X-rays, phonocardiograms and echocardiograms are necessary to confirm the latent valval abnormalities. A case of a 18 year-old male patient with SBS who underwent the surgical operation under general anesthesia was reported. The patient was screened by careful examinations to exclude mitral prolapse. The anesthetic and post-operative courses were uneventful. Problems for the management of SBS, including its association with mitral prolapse syndrome, as a dental patient were discussed.journal articl

    application/pdfIt is our daily requirement to restore the various mandibular position by the prosthodontic intervention in a partially dentate mouth. The most basic mandibular position is the intercuspal position, composed of four segmental contacts in both premolar and molar teeth. These key contacts in restoring the intercuspal mandibular position are called as "the Supporting Zone" of the mandible. The purpose of these series of studies is to clarify how effective the supporting zone of the mandible will be restored by the various prosthetic designs and procedures. The author has analyzed the following problems and has observed the following results. The Problems were; (1) how the various retainers would restore the mandibular position in the free-end R.P.D.'s, (2) how the various methods of impression procedures of the free-end R.P.D.'s would affect the supporting ability of the dentures, (3) how the preprosthetic treatments on the abutment restorations would affect the supporting ability of the dentures and (4) how alignment of the artificial teeth would affect the supporting ability of the dentures. The results obtained were; (1) The more rigid the retainer was that meant the good support and a well designed bracing action, the more effective the suppporting ability of the dentures was, (2) The mucocompressive impression technique was more effective in restoring the support of the free-end saddles than any other methods, (3) The genuine mesio-distal parallelism in the abutment crowns and the R.P.I. retainers were effective in restoring the support of the free-end dentures, (4) In the free-end saddle, it was advisable not to align the second molar tooth, in case the antagonists did not exist, or the antagonists were also the free-end saddles for the reason of reducing the force to the bounded abutments and saddles.journal articl
