Case Report on Dental Treatment for Two Children with Liver Transplants from Living Donors
application/pdf部分生体肝移植を受けた小児患者2例の歯科治療を経験した.第1例は胆道閉鎖症のため7歳時にわが国3例目として手術を受け,最長期生存記録を更新中の女児である.10歳0か月で当科に紹介来院した.シクロスポリン100mg,プレドニゾロン1.5mgを長期連用中であり,体格はやや小柄ではあったが,毎日元気に通学しており,臨床検査結果でも著しい異常は認められなかった.主訴は上顎前歯部歯肉の著しい肥厚と中切歯の萌出障害で,主治医との連携の下に歯肉切除を行った.病理組織学的には著しい線維増殖は認められず,萌出性嚢胞と診断された.以後現在まで2年以上にわたってリコールによる歯科的健康管理を行っているが,全身的にも局所的にも良好な経過を得ている.第2例は胆道閉鎖症のため6歳時に肝移植を受けた男児で,上顎側切歯の萌出異常を主訴として,8歳3か月で当科に紹介来院した.シクロスポリン150mgなどを連用中であったが,肝機能障害が持続し,肝脾腫,黄疸,ムーンフェイスなどがみられた.C型肝炎も合併していた.体調不良と出血傾向とに加えて,恐怖心もきわめて強く,対応には苦慮したが,主治医との密接な連携の下にとりあえずの応急的な処置から徐々に治療を進め,乳歯抜歯4歯,修復10歯などをとくに異常なく終了した.歯科的健康管理に移行したが,入退院を反復していてきちんと受診ができないためか,口腔清掃状態がやや不良で歯肉増殖傾向がみられる.journal articl
Cytochemical Study on Guanylyl Cyclase Activity in Rabbit Taste Bud Cells : Effects of sodium azide, hydroxylamine and sodium nitroprusside
application/pdfSome characteristics of cytochemically-demonstrated guanylyl cyclase activity in rabbit foliate taste bud cells were studied. The enzymatic activity was exclusively localized in the apical portion of the cells, which was supportive of the hypothesis that cyclic GMP is involved in taste transduction. The enzymatic activity was observed mostly on the plasma membrane and was only slightly enhanced by sodium azide, hydroxylamine or sodium nitroprusside. Since these chemicals are thought to activate the soluble form of guanylyl cyclase through the formation of nitric oxide, the results seem to indicate that most of the enzymatic activity demonstrated in the present study represents membranebound guanvlvl cvclase activity.journal articl
平成4年における冠・架工義歯補綴に関する統計的観察その1 単独冠について
application/pdfA study was made of 1006 crowns which had been fabricated for patients at the Prosthodontic Clinic of Matsumoto Dental College during 1992. Some of results were as follows. 1) 42.8% of the patients were males and 57.2% were females. 2) 90.0% of the patients were between 20 and 69 years old. 3) Crowns of the upper abutment teeth were more numerous than those of lower abutment teeth. 4) 75.0% of the crowns were fabricated for nonvital teeth. 5) 49.2% of the crowns were fabricated as full cast crowns, 27.6% as facing crowns (6.3% as porcelain fused to metal crowns, 21.4% as res-in facing crowns), 3.6% as jacket crowns (3.6% as resin jacket crowns and no porcelain jacket crowns was fabricated), and 19.1% as partial coverage crowns, and no dowel crowns was fabricated.journal articl
ラットの皮下に自家移植した根管充塡歯に対する組織反応
application/pdfThe tissue reactions to a root canal filling material and transplanted teeth through the experimental models were studied. Tooth columns were made from lower incisors which were removed from ten-week-old Wistar rats anesthetized. Canals of the columns were enlarged and filled with calcium-hydroxide paste mixed with silicone-oil. These tooth columns and removed teeth for control were autotransplanted into subcutaneous tissues. At 3 to 28 days after the experiment, these materials with surrounding tissues were removed and examined histopathologically. Results are as follows: Granulation tissue with macrophages and multinuclear giant cells (MGCs) proliferated around the paste after 7 days. MGCs had already appeared close to the cut surfaces of the dentin mimicking the root apecies at 7 days, while there were few MGCs in the controls at this stage. After 14 days, MGCs formed resorption lacunae on dentin surfaces both in the experiments and the controls. Furthermore, in the experimental specimens at 28 days, a MGC was observed lying on both the dentin and the paste. In the control specimens at the same days, tooth pieces with resorption lacunae were encapsulated by thin fibrous tissue. In conclusion, this experimental model is thought to be useful to investigate tissue reactions to filling paste and to clarify the behavior of MGCs, because our experiment seems to be able to bring the same histopathological changes as that of the periapical tissue after root canal filling.journal articl
歯科用金属の安全性試験 : 金属イオンの細胞障害性について
application/pdfToxic effects of four metal cations released generally from dental casting alloys were evaluated by corrosion test and in vitro cytotoxicity. Corrosion tests were conducted on four pure metal plates (Cu, Zn, Ag, Pd), and immersed in human resting saliva for 3 weeks at 37℃. Quantitative analysis of the metal cations released into the saliva was performed by the I.C.P. (inductively coupled plasma emission spectrometry) method. In vitro cytotoxicity was examined using L-929 cell lines. The cell line to be tested was adjusted to 2×10^4cells/ml in growth medium containing cupper and zinc ions at various concentrations. Cytotoxicity was determined to have occurred if a 50% inhibitory concentration was found when compared to controls (IC_). The results obtained were as follows. 1. The amount of cupper ion released into human saliva was 274μg/cm^2. This was equal with the amount of nickel ions released from some Ni-Cr alloys. 2. The amount of zinc ions released was 14μg/cm^2. The release was restrained by white matter deposited on the surface of the metal plate, 3 days later. 3. Silver and Palladium were excellent for the inhibition of corrosion to human saliva. 4. The IC_ of the copper ion was 1.28ppm (20.2μM) and of the zinc ion was 10.6ppm (159μM) in the case that the surface area of metal restration was 1cm^2, and the quantity of saliva was 1L/day. 5. The amount of copper ions to the IC_ reached immediately after metal restration was set, and all cells died in a day. 6. The amount of zinc ions to the IC_ reached in 2~3 days. However, no increase of concentration occurred after that, and did not reach the concentration at which all cells died.journal articl
授乳方法と乳歯齲蝕との関連について
application/pdfThough the relationship between nursing method and dental caries has been discussed by many researchers, definite conclusions are still not fully able to be made. The purpose of this study was to investigate the relationship among timing of weaning, nursing method before weaning and the incidence of dental caries. In the present study, the correlation between answers from an actual nursing methods questionnaire survey and the severity of dental caries was analyzed. However, the materials were devided into two groups. The high caries severity index group (82.7%) consisted of 93 children with severe dental caries who were subjected to intensive dental treatment under general anesthesia at Matsumoto Dental College. Another group of 91 children (caries severity index, 27.4%) who live in Asahi Village were selected as a control group. The following results were obtained: 1. Nursing method. Compared with the control group, the high caries severity index group tended to show a high percentage of breast-feeding, on the contrary, there was no difference between the percentage of artificial milk nursing between the two groups. 2. Weaning. In terms of mean month-age of weaning, the high caries severity index group tended to be later than the control group. 3. Weaning of mother's milk 55.1% of the high caries severity index group were not yet weaned until 16 months of age, on the contrary, only 3% of the control group were not weaned until 16 months of age. 4. Regularity of nursing. The high caries severity index group tended to nurse more irregularly than the control group.journal articl
Isolation and Properties of an Extracellular Gelatinase from a Strain of Prevotella intermedia Which Coelaborates Elastase
application/pdfGelatinase in the culture supernatant of Prevotella intermedia was purified to homogeneity by the combined procedure of ethanol precipitation and three steps of chromatography. The enzyme was a cystein protease with a molecular mass of 45 kDa. The activity was inhibited by divalent metal chelators and its inhibition was recovered by the addition of Ca^. The optimum pH for activity was 7.0-7.5 and the enzyme was inactivated by heating at 60℃ for 10 min. It hydrolyzed actively azocoll and hide powder besides gelatin. Hydrolysis of type IV collagen, if not strong, was also observed.journal articl
