Clinical Applicability of the Periotest^[○!R] for Implants
application/pdfIt is necessary to establish objective criteria for determining the optimal time to connect the super structure to the implant. Several studies have examined the usefulness of the Periotest^[○!R] for determining this timing. However, few investigators have studied the usefulness of this test, based on analysis of the reproducibility of the periotest^[○!R] values (PTV). We are studying the clinical applicability of the Periotest for POI 2-Piece implants,^[○!R] using a post specially designed for this test. The results obtained were as follows. 1. It seems possible that the Periotest^[○!R] allows surgeons to predict the prognosis of implants based on an analysis of the time course of PTV for the same type of implants and to determine the optimal time to connect the super structure to the implant. In our department, a PTV below +9 is regarded as indicating the optimal timing of super structure connection. 2. It seems possible that the Periotest^[○!R] can be used in judging whether or not a 2-piece implant^[○!R] should be connected to natural teeth or that the optimal post can be selected based on an analysis of the PTV of natural teeth. 3. The Periotest^[○!R] seems to allow surgeons to check that an implant has been bicortically inserted. This check seems to be most useful when it is done immediately after implant insertion. In practice, however, the use of the Periotest^[○!R] for this check immediately after implant insertion is not possible because the accuracy of drilling differs among different surgeons. The Periotest^[○!R] may therefore be used in making such a check at the time of super structure application and in predicting long-term prognosis after super structure application. Although the Periotest^[○!R] seems to be clinically applicable to these purposes, it seems indispensable to pay attention to the reproducibility of the PTV and to standardize the conditions for PTV measurement.journal articl
延長ブリッジ装着時の咬合面垂直荷重に対する下顎骨の力学的反応
application/pdfExtension-bridge has been accepted as the substitute for removable partial dentures for patients missing most distal teeth due to the fact that removable partial dentures cause discomfort for patients. On the other hand, it is pointed out that the cantilever force of the extension-bridge causes periodontal disease with absorption of bone and gingivitis of the abutment teeth. In this study, we attached the three-unit-extension-bridge to the dried mandibular bone of an adult human and force corresponding to 5 kg was applied to the bridge. The stress of various areas on the buccal surface of bone was measured using a strain gauge. On the other side, the same force was applied to natural teeth and the strain was measured as a control. In the control, the stress was revealed to be dispersed widely throughout the bone, which could be the result of a change in direction of the strain in response to the magnitude. In the experimental side, a different response was observed between two abutments; stress was concentrated in the abutment adjacent to a missing tooth as a result of the unchanged direction of the strain, and because the direction of strain changed only slightly, stress was not concentrated around the anterior abutment tooth.journal articl

application/pdfThe number, district distribution and ethnological cause of new patients during twenty years from 1974 to 1993 were studied statistically. The results were as follows: (1) The total number of new patients showed a gradual yearly increase except in 1984 and 1988. The number of the new patients from the area outside Shiojiri and Matsumoto Cities remarkably increased from 1989 to 1993. (2) On the etiological cause of patients, the pericoronitis of wisdom teeth and apical periodontitis were the major diseases. The number of patients with dysfunction of temporomandibular joint showed a increasing tendency in recent years. (3) On the age of patients with dysfunction of the temporomandibular joint, the number of patients in the teens and thirties was numerous while aged patients were scarce. (4) The number of the aged patients (more than 65 years old) remarkably increased in the last three years.journal articl
電気歯髄診断器無関電極の接触位置変更による測定値の変動について
application/pdfToday, it is necessary for dentists to wear laxtex gloves for cross infection control in general dental practices. However, some problems have arisen because of the use of latex gloves. One of these such problems has occurred during electric pulp tests. When electric pulp testing occurs, the circuit is completed by the dentists placing one hand on the electrode handle and one or two fingers in contact with the patient's cheek or lip. The circuit was severed by the wearing of latex gloves. To solve this problem, the patient grasped the metal pole that was connected to the electrode handle with an electric cord. This clinical study was carried out for the purpose of knowing whether or not the measured this new electric pulp testing method differed from those of the usual method. The results obtained were as follows. 1. There were no different measured values between the new and usual methods. These values were also confirmed statistically. 2. It was recognized that the measured values had much difference among individuals.journal articl
