    A Clinical Result of 5,092 Implants Consecutively Placed in Both Maxilla ans Mandible

    application/pdfThe purpose of this retrospective study was to designate an outline of criteria for the clinical evaluation of patients treated with dental implants. In this paper, the contents of our survey are also reported briefly. A total of 2,261 patients (957 males and 1,304 females), ranging in age from 10 to 88, participated in this study. The patients in their 40s and 50s represented 62.1% of the total number of subjects. The number of implants examined was 5,092, which had been placed in 2,969 cases. The numbers of cases and implants placed during the 1990s were higher by 2.7 times and 3.6 times compared with the 1980s, respectively. The mean number of implants was 2.3 for males and 2.2 for females. Those who had systemic health problems accounted for 11.7% of the total number of patients, of which 49.5 percent were suffering from some form of circulatory disease, out of the total number of systemic diseases. Cigarette smokers, habitual drinkers, and in particular drug users, were very few, but there was a gender difference in these habits. Many implants were planted in the mandible, free end missing parts, partially endentulous dental arches and the molar region. The prostheses supported by implants were mostly cement-retained bridges for retention and implant/implant bridges for connection. Regarding the implant design, screw-type implants were frequently used. The surface of most implants was coated with hydroxyapatite. Many implants were 10-15 mm long and 3 mm in diameter. Bone quality and bone quantity in both the maxilla and the mandible were medium. The mean number of years after implant placement was 5.5 years. As for the present condition, 85.4% of the cases and 87.8% of placed implants were favorable.     Krabbe病患者の全身麻酔経験

    application/pdfWe present a 5-year-old boy with late infantile form of Krabbe disease who required general anesthesia for dental treatment. He was diagnosed with Krabbe disease at 1 year and 6 months old, and bone marrow transplantation was performed to prevent the neurological deterioration at 2 years old. He presented severe neurodegeneration and quadriplegia, and could not move actively except for tossing about from laterel to supine position. A diazepam syrup (2 mg) was administered 90 minutes before the start of anesthesia. After the establishment of a venous route, anesthesia was induced with thiopental sodium (50 mg), and nasotracheal intubation was facilitated with vecuronium bromide (1.4 mg). The anesthesia was maintained with N_2O (3.0l/min), O_2 (1.5l/min) and sevoflurane (1.0~2.0%). Pulpectomy and dental prosthesis for every 20 deciduous teeth was performed, and the anesthesia was completed uneventfully after 5 hours and 50 minutes.journal articl

    application/pdfWe investigated the circulatory changes during impression taking and the effect of posture for the purpose of establishing guidelines for taking impressions more safety in elderly patients. The subjects were 31 elderly patients aged over 64 who visited the Matsumoto Dental University Hospital Special Patients Clinic or lived in a nursing home for the aged. Blood pressure was significantly higher at the insertion of the impression material compared to at rest in some patients. There was no significant influence of posture. Some elderly patients with systemic disease exhibited significant changes in circulatory conditions, even during impression taking, which suggests the importance of monitoring.journal articl

    application/pdfIn the previous reports (the 1st, 2nd, 3rd and 4th), 235 publications regarding the biography of Dr. Hideyo Noguchi have been described (Matsumoto Shigaku, 13 : 1~34, 1987 ; 15 : 217~231, 1989 ; 20 : 80~99, 1994 ; 23 : 194~210, 1997). In this paper 19 books and journals were added. Among them, two books, "The Hideyo Noguchi Memorial Association (ed) : Collection of Hideyo Noguchi's Letters III Ditto Association, Tokyo, 1998" and "Ha-mako Kogure et al. (ed) : Now Again Hideyo Noguchi, Aibun Shorin, Tokyo, 2000", are very important. In total, 254 publications have been introduced to the Matsumoto Shigaku over the past 13 years.journal articl

    application/pdfSurgical orthodontic treatment by means of costochondral graft was performed on a 15-year-old female with severe micrognathia and hypoplasia of the condyle and ramus. This patient had undergone arthroplasty at bilateral temporomandibular joint-ankyloses. With regard to the pretreatment, other orthodontic findings included skeletal openbite, steep antero-posterior inclination of maxillary occlusal plane, and crowding. Presurgical orthodontic treatment was accomplished by the extraction of 4 first premolars over about 2 years. Costochondral graft, double-stepped genioplasty and Le Fort I osteotomy were performed for reconstruction of condyle-ramus units as well as restoration of the temporomandibular joint function, advancement of the mandible and counter-clockwise rotation of the maxilla. These procedures produced marked improvement of the severe skeletal deformities. After surgery, however, a small amount of relapse was observed which seemed to be attributable to both backward rotation in the tension of the soft tissue and the instability of replaced condyles. During the postsurgical orthodontic treatment, the mandible was mainly stretched toward the upward and forward directions by using class II and up & down elastics for 23 months. In this treatment, although the costochondral graft was carried out after the adolescent growth period, lengthening of calcified images of bilateral transplanted grafts on lateral cephalograms was recognized. As results of this therapy, the occlusion and profile in this patient were significantly improved, and the restored conditions have been maintained during the 1 year after the beginning of retention.journal articl
