    Peculiarities of Metabolism of Rats with Fructose-induced Insulin Resistance Against the Background of Congenital and Acquired Iodine Deficiency

    Animals following a high-fructose diet during eight weeks, have experienced changes in metabolism and the signs of insulin resistance have developed. Under such conditions, moderate hyperglycemia, hyperinsulinemia, hyperuricemia, an increase of the level of glycosylated hemoglobin in whole blood were observed. The significant role of the HOMA-IR index, as an early marker of carbohydrate metabolism disorders at the stage of pre-diabetes, has been confirmed. In experimental animals against the background of the high-fructose diet, the changes in the lipid spectrum of the blood were revealed: an increase of the total cholesterol level, low-density lipoproteins, triglycerides against the background of a high-density lipoproteins decrease. These disorders and a significant increase of the atherogenicity reflect the development of secondary dyslipidemia. In this case, the disorders of carbohydrate metabolism were combined with the degree of dyslipidemia. Males were found to have at increased risk of development the insulin resistance and comorbid pathology. Iodine deficiency, especially of congenital nature, is an aggravating factor of metabolic disorders. The obtained data can serve as a basis for extend of preventive measures and identification of the priority treatment schemes for type 2 diabetes mellitus in residents of endemic regions

    Use of Antifungal Creams in the Combined Treatment of Obliterating Diseases of Lower Extremities Arteries

    To perform clinical analysis of the effectiveness of the complex treatment of occlusive vascular diseases and fungal lesions of feet with addition of local antifungal medicines. Materials and Methods. The results of examination and treatment of five patients with obliterating diseases of the lower limb vessels and diabetes mellitus with dermatomycosis and onychomycosis of the feet were analyzed. Onychomycosis or mycosis of feet were treated with the help of the combination of the following antifungal creams was used in the treatment: miconazole, clotrimazole, tioconazole in a ratio of 1:1:6.5 contained in the combined preparation cream “XaTonic”. Results and discussion. The use of an antifungal mixture of creams after 7 days has led to the disappearance of the clinical manifestations of dermatomycosis of the feet in three patients, and in two patients with onychomycosis after 14 days there was the reduction of clinical symptoms. Conclusions. The combination of three local antifungal components: miconazole, clotrimazole, thioconazole is effective in treatment of dermatomycosis of the feet. In the treatment of fungal lesions of the feet by dermatologists in patients aged after fifty years, an angiologist/vascular surgeon’s consultation is required

    The long-term observations of leading endocrinologists, cardiologists, and gastroenterologists all over the world have shown that the quality of medical care and the level of the patient’s knowledge about alimentary-dependent diseases significantly affect their course and prognosis. As a part of the classes at the Children’s Health School, the interns of the 1st and 2nd years of study give interactive classes for the child patients and their parents on the prevention of diseases and the promotion of a healthy lifestyle, namely nutrition, physical activity, psychological health, work and rest regimes, personal hygiene, etc. Taking into account the conducted classes, such training can be argued to be useful and necessary both for children for the purpose of obtaining new educational knowledge and for interns in order to learn and develop communicative skills

    Ecstasy Intake Related Coagulopathy

    3,4-methylenedioxymethamphetamine (MDMA), substance known as "Ecstasy" among the people and used due to entertainment, euphoric and energy booster effect, is one of the famous synthetic stimulants.  22-year-old man was found as unconscious in the early morning. According to the expressions of his family and friends, it was learned that he drank alcohol until late in the previous evening and took ecstasy besides this before he died. It was reported that he was taken to intensive care unit with temperature of 41oC and death occurred 12 hours later. At autopsy in external examination, petechial and purpuric hemorrhages in purple and red color were detected on body. In internal examination, petechial hemorrhages on the surface of the heart and lungs, subendocardial hemorrhage in the heart, hemorrhage on the mucosal surface of the stomach were seen. In the toxicological analyses of the blood, Paracetamol (4870 ng/ml), MDMA (847 ng/ml), MDA (94,2 ng/ml), Lidocaine (23 ng/ml), Pantoprazole (10,5 ng/ml), Midazolam (1,83 ng/ml) were detected.     Substantiation of the Results of Morphological Examination of the Urinary Bladder Wall in Experimental Diabetic Cystopathy

    Measurement of consumed water and daily diuresis proved the pathomorphological manifestations of streptozotocin-induced cystopathy in the experiment on rats according to the results of biochemical studies of blood and urine. It is argued that the desquamation of cells in the transitional epithelium, its atrophy, stratification violation, and baring of the basal membrane are caused by a large volume of urine, which excessively stretches the urinary bladder, destroying the intercellular contacts of the urothelial layer. It is proved that primary hyperglycemia leads to widening of the lumen of the arterioles and moderate thickening of the basal membrane of the micro-hemovessels, and high chronic hyperglycemia – directly triggers the whole cascade of pathomorphological changes: on the 42nd day of the experiment it causes vasoconstriction of the arterioles, and at the later terms – the secondary expansion of the arterioles and venules of the microcirculatory bed of UB (urinary bladder); is the cause of dystrophic changes of endothelial cells, further thickening and lamellar transformation of the basal membrane and plasma permeation of the perivascular connective tissue; causes the appearance of dark involutional myocytes with few organelles and sarcoplasm sequestration. Hydropic dystrophy of smooth myocytes has been found to be associated with the hydration of blood plasma as a result of excessive polydipsia in diabetic animals, and vacuole dystrophy of urothelial cells, enlargement of their size and interstitial edema – with low specific urinary density due to the multiple fast increase of diuresis. It has been established that prolonged high glucosuria and decreased diuresis lead to a decrease in urothelial cell size, compaction of their cytoplasm and ultrastructural readjustment. The increase of the content of glycosylated hemoglobin during the experiment justified the appearance and increase of the sludge-syndrome

    Improvement of Magnesium and Calcium Deficiency in Patients with Gastroesophageal Reflux Disease Associated with Undifferentiated Connective Tissue Disease

    The objective of the research was to study the effectiveness of course application of magnesium and calcium-containing drugs on the background of conventional therapy on the changes in the content of macronutrients in red blood cells and the activity of matrix metalloproteinase - 9 in the patients with a co-existence of undifferentiated connective tissue disease and gastroesophageal reflux disease. Materials and Methods. All 75 patients with gastroesophageal reflux disease associated with undifferentiated connective tissue disease were divided into three groups: Group I included 25 patients who received Magne-B6 at a dose of 2 tablets 3 times a day in addition to the standard background therapy with “Panocid” at a dose of 40mg once a day; Group II comprised 25 patients who received “Ca-D3 Nycomed” at a dose of 1 tablet 3 times a day in addition to the standard background therapy; Group III included 25 patients who received a combination of “Magne-B6” and “Ca-D3 Nycomed” in addition to the standard background therapy.  Results. In Group I, the proposed complex therapy resulted in a significant increase in Mg2+ by 40.9%. Mg2+ dynamics was less significant in Group II; this index reached the level of 17.9 ± 2.04 µg/g (р"˃0.05); in its turn, Ca2+ level increased to 80.6 ± 2.12 μg/g (p"<0.05) in these patients. Mg2+ and Ca2+ indices showed the most significant positive dynamics in Group III: 21.52 ± 2.47 μg/g (р"<0.05) and 102.7±1.37 µg/g, respectively. After combination treatment, we traced the dynamics of changes in the level of matrix metalloproteinase - 9: it decreased by 39.45% in Group I, by 35.3% in Group II, by 53.18% in Group III and reached the level of 1689.266 ± 14.89 pg/ml, approaching the indicators of the control group. Сonclusions. Based on the data obtained, it can be argued that simultaneous addition of “Magne-B6” and “Ca-D3 Nikomed” to the standard therapy for gastroesophageal reflux disease in the patients with undifferentiated connective tissue disease contributed to a significant decrease in the level of matrix metalloproteinase-9 resulting in reduced degradation and increasing synthesis of new connective tissue components

    Optimization of the Prediction of Arterial Hypertension Development in the Patients with Type 2 Diabetes Mellitus on the Background of Undifferentiated Connective Tissue Dysplasia

    In recent years, disorders of connective tissue metabolism that are involved in the pathogenesis of many diseases have attracted increasing attention. They are of particular importance in the presence of connective tissue dysplasia. Undifferentiated forms, being a frequent component of comorbid pathology, are quite widespread among the general population. Among comorbidities, arterial hypertension, diabetes mellitus and undifferentiated connective tissue dysplasia in various combinations are more often observed.   The objective of the research was to determine the prognostic significance of undifferentiated connective tissue dysplasia in the development of arterial hypertension in the patients with type 2 diabetes mellitus. Materials and methods. To determine the prognostic significance of undifferentiated connective tissue dysplasia in the development of arterial hypertension, there was conducted an analysis of hospital discharge reports received by the patients with type 2 diabetes mellitus duration of at least 2 years who were treated two to five years ago. The predictive significance of the factors selected for the analysis was determined through applying the regression analysis using a logistic regression model, the Wald test. Results. The test for coincidence of the predicted and observed values revealed that the specificity of the regression model was 87.2%, while its sensitivity was 89.7%. The overall predictability was 88.5%. Conclusions. These are quite high indicators that allow us to apply the proposed model to detect the patients with type 2 diabetes mellitus being at high risk of arterial hypertension

    Argonoperitoneum Effect on Pain Syndrome Intensity in Laparoscopic Cholecystectomy

    The objective of the research was to compare the objective and subjective indicators of intra- and postoperative pain and surgical stress intensity.Materials and Methods. There were examined 360 patients suffering from gallstone disease. There were used the intraoperative monitoring of nociception index by means of Analgesia Nociception Index  - monitor, the determination of serum cortisol concentration, the assessment of pain syndrome in the postoperative period by means of the Numerical Rating Scale in accordance with life quality assessment and the number of injections at the patient’s request. Results. The analysis conducted indicated the reduction in pain irritation, low indices of pain syndrome and postoperative stress intensity under the effect of argonperitoneum. Considerable reduction in the need for analgesics in the study groups served as evidence to this predicted pattern. The assumption that the noticed phenomenon can be explained by the absence of acid irritation of the peritoneum due to argon chemical neutrality being distinct from carbon dioxide was substantiated.Conclusions. The results proved the effectiveness of argonperitoneum application in laparoscopic surgeries

    Psychological Peculiarities of Professional Burnout of Teachers of High Medical Educational Institutions

    In the article the relevance of the outlined problem in the context of the entrance of Ukraine in the world medical educational space is covered. The essence of the professional burnout of teachers of the higher medical educational institutions (HMEI), which is characterized by the gradual loss of life tone by the subject under influence of prolonged stress in the professional teaching activity and is manifested by symptoms of chronic fatigue, general exhaustion, depersonalization, reduction of professional achievements, etc., is determined too. The objective of the study – to substantiate reasons, peculiarities, objective and subjective factors of the professional (emotional) burnout of teachers of the HMEI. In the investigation the historical-systematic, comparative method and objective-specific method are used. It is emphasized that the syndrome of professional burnout can lead to general decrease of the effectiveness of the professional activity of the teacher, deformation of interpersonal relationships as well as decrease of activity in the creative professional self-improvement, self-development.     Aortic dissection is the most common catastrophic event affecting the aorta, which is characterized by complex clinical manifestations, and high missed and delayed diagnosis rate. Acute aortic dissection has the importance of medical emergency and is associated with a high mortality. The presented case was, 55 year-old-man who was found as dead in the shed. At autopsy on internal inspection; heart examination revealed dissection 1 cm above aortic valve surrounding full thickness and intact adventitia, also hematoma making pressure on the descending branch of left coronary artery after 1 cm from proximal resulting from dissection and full- thickness dissection in the origin of the right coronary artery and intact adventitia were observed. We aimed to discuss this interesting aortic dissection case in the aspects of medico legal literature
