STUDI PENGGUNAAN OBAT ANTI TUBERKULOSIS PADA PASIEN TB-HIV/AIDS DI RSUP SANGLAH DENPASAR TAHUN 2009
Background and Objective: A retrospective descriptive studyof anti-tuberculosis drugs in patients with TB - HIV/AIDS whowere hospitalized Nusa Indah Sanglah Hospital 2009 aims todetermine suitability of the use of anti-tuberculosis (OAT) tothe National Treatment Guidelines 2008 which consists of typesof drugs, combination drug and dosage, also to identify druginteractions that do occur based on the level of significance.Results and Conclusions: The results showed that the useof OAT category 1 has the largest percentage of 61.7% andthen use of a combination of streptomycin with ethambutolwhich is 12%, then use of isoniasid and rifampin which has apercentage of 6%. The percentage of the correct dose 60.9%,8.7% is not appropriate, and not identified was 30.4%. Usageof the appropriate OAT that is equal to 74% and 26% did notdeserve. Drug interactions occurred in 23 patients with asignificant percentage of drugs that interact with a percentageof 11.1%, the importance of 2 namely 33.3%, the importanceof 3 namely 0%, significance 4 namely 16.7%, and thesignificance was 5 of 55, 6%.Keywords: tuberculosis, TB-HIV/AIDS, suitabilit
PENGOBATAN TRADISIONAL, UPAYA MEMINIMALKAN BIAYA KESEHATAN MASYARAKAT DESA DI JAWA
Background: Traditional medicine is frequently perceived asnegative and even though many people still practice. It the useof traditional medicine is mainly due cause to tradition. Thevillager tends to use traditional medicine as primary healing.This article tried to identify frequent diseases among Javaneseand economic capability to afford health cost.Methods: Ethnography study using observation and in-depthinterviewamong 48 informants and 6 key informant of peasantcommunity in Sleman, DIY and fisherman in Rembang, CentralJava, during 2007-2008. Phenomenology approach used duringdata collection and analysis.Result: Masuk angin (wind illness) is a disease whichfrequently occur among the villager. Traditional medicine wasapplied because it is inexpensive, easy, effective and suitablewith the cognitive related to the harmony (equilibrium). Theprinciple of coining was binary opposition such as: hot x cold;loose x tight; angin masuk x angin keluar; better x awful andthe equilibrium is the basic rational of traditional medicine.Recommendation: Traditional medicine must be recognizedby the government and should be in equal position to modernmedicine in order to reduce negative opinion, so the rational ofit is recognized by others.Keywords: holistic, traditional, masuk angin, effective, chea
CITRA RUMAH SAKIT BALIMÉD DENPASAR
Background: Hospital industry competition in Indonesia isgetting more intense. Identifying image position and positiveimage building of new hospital become one of importantstrategies in anticipating competitive situation and consumers’characteristics that are getting more critical in the choice ofhealth service. Positive image building of hospitals encouragepatients to pay more for health services they get and helps todecide purchase of the community.Objective: To identify perception of patients and visitors aboutimage of BaliMéd Hospital Denpasar, and to identify effect ofrespondent demography characteristics toward perceptionabout image of BaliMéd Hospital Denpasar.Method: The study used cross sectional survey design withquantitative as well as qualitative method as supplement.Primary data were obtained through questionnaire distributedto 200 patients and visitors as respondents at BaliMéd HospitalDenpasar. Qualitative method was applied through in-depthinterview to 4 patients and 3 visitors of BaliMéd HospitalDenpasar. Data were analyzed descriptively, usingindependent t-test and U Mann – Whitney test followed bymultiple regression analysis to identify the effect ofrespondents’ characteristics to perception about image ofBaliMéd Hospital Denpasar.Result: Average perception of patients (150.12) about theimage of BaliMéd Hospital Denpasar showed positive imagewhereas average perception of visitors (144.68) showedneutral image. The result of independent t-test with p=0.003(p<0.05) showed there was significant difference in perceptionabout the image of BaliMéd Hospital Denpasar between thepatients and the visitors. R=5.5% indicated low contribution ofeffect of variables of education, income, and frequency ofhealth service utilization to perception about the image ofBaliMéd Hospital Denpasar. Only the variable of frequency ofhealth service utilization at regression coefficient 1.658 thathad significant effect p=0.016 (p<0.05) to perception abouthospital image.Conclusion: Perception of patients about the image of BaliMédHospital Denpasar was positive image but visitors had neutralimage. In general, patients and visitors had significantdifference in perception about the image of BaliMéd HospitalDenpasar. Only the frequency of health service utilization thathad positive and significant effect to the image of BaliMédHospital Denpasar. Education, income and frequency of healthservice utilization all together did not have significant effect tothe process of perception building on the image of BaliMédHospital Denpasar.Keywords: image of hospital, perceptio

EVALUASI BESARAN PREMI TERHADAP KESESUAIAN PAKET PELAYANAN KESEHATAN PADA JAMINAN PEMELIHARAAN KESEHATAN DAERAH
Background: The concept of community health care insuranceis presently being adopted by the local government of SinjaiDistrict known as Local Health Care Insurance of Sinjai managedby an operating council. This program provides health servicepackages for inpatient, outpatient and other particulartreatments at health centers and local hospitals of Sinjai. Thepremi presently imposed is used to pay operational cost oflocal health care insurance program and more than 80%-90%of claims from health service providers have to be supportedby local government. If the amount of fund required to payclaims is continuously increasing and not supported by relevantamount of premi, the local government will consequently havegreater burden. Therefore some aspects related to real premi,normative utilization of premi and benefit package have to beevaluated by considering the ability and willingness to pay ofthe community.Objective: The objective of the study was to identify theamount of real premi and normative utilization of standard premi,assess the ability and willingness to pay premi of the participantsand analyze perception of the participants and stakeholdersabout the relevance of benefit package, as well as identify theperception of stakeholders about the existence and solution tofinancial problems of Sinjai Local Health Care Insurance.Method: This descriptive study used both quantitative andqualitative approaches. Data of claims and administration wereobtained from the operational council of local health careinsurance; ability and willingness to pay of the community andperception about relevance of benefit package of local healthcare insurance were obtained from questionnaires distributedto 96 respondents of participants and in depth interview with7 stakeholders from the government.Result: Real premi in 2006 was Rp2,923.86/capita/month andin 2007 was Rp2,453.59/capita/month; premi of normativeutilization in 2006 was Rp10,101.97/capita/month and in 2007was Rp9,857.13/capita/month; ability to pay (ATP) 1 of eachparticipant was Rp11,246, ATP2 was Rp66,178/capita/monthand willingness to pay was Rp3,104/capita/month; 97.92% ofparticipants perceived benefit package relevant with theirexpectation whereas stakeholders perceived that benefitpackage greatly varied, the existence of local health careinsurance had to be sustained and financial problems could besolved by increasing premi and optimizing premi billing, providingsubsidy for poor communities and generating other financialsources.Conclusion: In order to sustain the existence of local healthcare insurance the real premi should be increased fromRp10,000,00/family/month to Rp12.500,00 – Rp15.000,00 perfamily/month as compensation for relevance of benefit packageof Sinjai Local Health Care Insurance.Keywords: real premi, premi of normative utilization, perceptionof stakeholders, benefit package, cost of claims, ability to pay,willingness to pa
