La invención se refiere a un método de configuración de redes sensibles al retardo, TSN, basadas en planificadores con conformación de tráfico asíncrono, ATS, de una pluralidad de flujos de tráfico, donde el retardo de encolado de paquete para un flujo dado viene determinado por un criterio de ordenación para calidad de servicio determinista. Ventajosamente, dicho método comprende realizar, de forma iterativa en niveles de prioridad creciente: ordenar los retardos de encolado de paquete para la pluralidad de flujos de tráfico; asignar un mismo nivel de prioridad a la pluralidad de flujos; calcular el retardo de encolado de paquetes en el peor de los casos para el nivel de prioridad asignado; seleccionar el flujo con el requisito de retardo de encolado de paquete más estricto para el nivel de prioridad; y verificar que dicho retardo de encolado de paquetes en el peor de los casos es igual o inferior al retardo de encolado de paquete.Universidad de Granad
Meningococcal Vaccination in High-Risk Patients: A Systematic Approach to Evaluating Coverage and Patient Catch-Up Through Healthcare Databases
The publication costs were funded by the research group CTS-295 in Preventive Medicine and Public Health, University of Córdoba.Background: Invasive meningococcal disease (IMD) can lead to severe and fatal outcomes. Vaccines against meningococcus (serogroups B, MenB; or ACWY, MenACWY) are recommended for patients at high risk of developing IMD. Our aim was to identify high-risk patients through a systematic search of medical codes and to evaluate vaccination coverage by high-risk group. Methods: An observational retrospective study was conducted in patients discharged at Reina Sofía University Hospital (Cordoba, Spain) from 1 January 2000, to 31 December 2023. Selection of high-risk patients was conducted through national administrative and clinical databases and vaccination coverage was determined through the Andalusian electronic vaccine database. Vaccine coverages of MenB and MenACWY were calculated within risk groups. Finally, bivariate analyses were conducted to assess the potential association between coverage, sex, and the year of admission. Results: A total of 2689 patients with 2710 high-risk conditions for IMD were identified from the databases searched. Of the 1755 requiring MenB vaccination, only 624 (35.6%) had received one dose and 558 (31.8%) two doses. Of the 2710 requiring MenACWY vaccination, only 784 (28.9%) had received one dose and 520 (19.2%) two doses. Patients with properdin-complement deficiencies showed the lowest vaccination rates (<10%). For the rest of the high-risk groups, vaccination coverages were significantly lower when the condition was diagnosed before the immunization guideline recommendations (p < 0.001). Conclusions: The identification of high-risk patients through databases using R-coded algorithms is both feasible and effective for identifying and catching-up patients for vaccination. The population at risk of IMD lacks adequate meningococcal vaccination coverage. Our methodology can serve to identify patients in other regions and for different vaccines.University of Córdoba CTS-29
Organic macromolecules transport a significant proportion of the calcium precursor for nacre formation

Between them lies the surface membrane, a thick organic structure that covers the mineralization chamber and the forming nacre. The surface membrane is a dynamic structure that grows by the addition of vesicles on the outer side and recedes by the formation of interlamellar membranes on the inner side. By using a combination of electron microscopy imaging and spectroscopy, we have monitored the journey of the vesicles from the mantle epithelium to the mineralization chamber, focusing on the elemental composition of the organic structures at each stage. Our data reveal that transport occurs in lipid bilayer vesicles through exocytosis from the outer mantle epithelium. After release into the surface membrane, chitin undergoes a process of self-assembly and interaction with proteins, resulting in progressive changes of the internal structure of the surface membrane until the final structure of the interlamellar membranes is acquired. Finally, these detach from the inner side of the surface membrane. The high prevalence of VD deficiency in the population requires stronger preven- tive measures. The aim was to characterize the dietary and lifestyle determinants of VD levels in blood and of VD deficiency to further develop predictive models of these two outcomes. A total of 63,759 participants from the UK Biobank study with available data on dietary intake of VD, assessed via 24-hour recalls, and with measurements of serum 25(OH)D levels were included. Linear and logistic regression models were applied to identify factors associated with VD levels and VD deficiency outcomes, and to evaluate the influence of covariates on the association between VD in serum and VD in the diet. Predictive models for both VD outcomes were constructed using classical regression models and machine learning methods based on penalized likelihood methods. Approximately 10% of the participants had VD deficiency (VD < 25 nmol/L), and 38.9% were at risk of VD inadequacy (VD 25–49 nmol/L). The dietary intake of VD was significantly lower in the VD deficient group. This latter group showed lower engagement in physical activity (22.1%) compared to the non-deficient group (13.4%; P < .001). Also, overweight and obesity (vs normal weight) were related to a greater likelihood of VD deficiency (OR = 1.18 and 1.96, respectively). A similar odds of VD deficiency was observed for abdominal obesity (OR = 1.83). A weaker association was observed between dietary VD intake, based on participant reports, and VD levels. With regard to sunlight exposure, darker skin tones (OR dark vs fair skin = 3.11), season (OR winter vs autumn = 3.76) and less outdoor time activities (OR per 1 h increase = 0.96) were also related to VD deficiency. Predictive models for both classical regression and machine learning, showed good accuracy (AUC = 0.8–0.9 for VD deficiency). In conclusion, while a rich diet in VD boosts its levels, sun exposure plays a more significant role particularly in populations from the UK or Northern Europe. Physiotherapist Online Assessment in Patients with Stroke: Protocol for a Systematic Review and Meta-Analysis

occur in people under 50 years of age. The clinical management of neurological disorders depends on reliable diagnostic tools to identify impairments and aid in the early
and accurate detection of disease. The objective of this study is to present a systematic
review protocol for identifying the scientific evidence on the use of tele-assessment compared with in-person assessment delivery by physiotherapists for stroke patients. This
protocol was registered on the International Prospective Register of Systematic Reviews
(PROSPERO) database (CRD42024613552). Methods: Original studies of any design in
which physiotherapy tele-assessment using videoconferencing compared with face-to-face
assessment for patients with stroke conditions will be included. The research will be carried
out in PubMed/Medline, Cochrane Library, PEDro (Physiotherapy Evidence Database),
and NICE. The risk of bias will be assessed using the Quality Appraisal Tool for studies of
diagnostic Reliability (QAREL) and the Quality Assessment of Diagnostic Accuracy Studies
(QUADAS). Results: The screening, selection, and analysis process will be conducted by
two independent researchers and reviewed by a third evaluator to resolve any potential
disagreements. The feasibility of conducting a meta-analysis for quantitative data will be
evaluated based on the homogeneity analysis of the selected studies. Conclusions: We hope
that this systematic review protocol will provide scientific evidence for tele-assessment as a
physiotherapeutic assessment strategy for stroke patients and that it will be available as a
complementary tool to face-to-face physiotherapeutic assessments for specific situations
Sentido espacial en las evaluaciones escritas de Uruguay
En este trabajo se analizan las componentes del sentido espacial que se manifiestan en la
resolución de tareas de evaluación propuestas por PISA, en el contenido espacio y forma.
Para ello, se describen las características de las tareas de evaluación tanto de PISA como
de aula relacionadas con contenido geométrico; se determinan las expectativas de
aprendizaje en cuanto a competencias matemáticas, procesos cognitivos y componentes
del sentido espacial. Además, se identifican las componentes del sentido espacial en las
respuestas a las tareas de evaluación de PISA, de 122 estudiantes de 15-16 años que
cursan primer año de bachillerato uruguayo; se categorizan los errores que manifiestan
estos estudiantes cuando las resuelven; y se relaciona el rendimiento alcanzado en las
tareas de aula con la manifestación del sentido espacial en las tareas de PISA.
La investigación tiene un enfoque cualitativo de carácter descriptivo. Su diseño
corresponde a la aplicación de una metodología propia de la didáctica de la matemática,
el análisis didáctico. El análisis aborda las características de las tareas, las competencias
matemáticas, los procesos cognitivos y las componentes del sentido espacial como
expectativas de aprendizaje. Además, a partir del análisis se obtienen relaciones entre el
rendimiento y las habilidades de visualización y el manejo de conceptos geométricos en
algunas de las tareas.
Los resultados muestran semejanzas en las características de las tareas: ante la
complejidad cognitiva y en la tipología de tareas; las diferencias vienen dadas en el
contexto de las tareas y en el formato de las respuestas. Otras semejanzas se manifiestan
en las competencias matemáticas de comunicar y representar, en el proceso cognitivo de
comprender, en el manejo de conceptos geométricos y en la habilidad de percepción de
las relaciones espaciales.
De las conclusiones obtenidas se destaca que el alto rendimiento en las tareas de PISA
garantiza alto resultado en las tareas de aula, pero no al revés, el buen rendimiento en las
tareas de aula no implica un alto resultado en las tareas de PISA; y el bajo rendimiento
en el aula conlleva bajo resultado en PISA. Además, muestran que los errores asociados
al sentido espacial más frecuentes responden a la incorrecta interpretación de enunciados
y de gráficos y se vinculan más a un bajo desarrollo de las habilidades de visualización

Genomics yields biological and phenotypic insights into bipolar disorder
The Psychiatric Genomics Consortium, PGC has received major funding from the US National Institute of Mental Health (PGC4: R01 MH124839, PGC3: U01 MH109528, PGC2: U01 MH094421 and PGC1: U01 MH085520).Bipolar disorder (BD) is a leading contributor to the global burden of disease 1 . Despite high
heritability (60-80%), the majority of the underlying genetic determinants remain unknown 2 . We
analysed data from participants of European, East Asian, African American and Latino
ancestries (n=158,036 BD cases, 2.8 million controls), combining Clinical, Community, and Self-reported samples. We identified 298 genome-wide significant loci in the multi-ancestry meta-
analysis, a 4-fold increase over previous findings 3 , and identified a novel ancestry-specific
association in the East Asian cohort. Integrating results from fine-mapping and other variant-to-gene mapping approaches identified 36 credible genes in the aetiology of BD. Genes prioritised
through fine-mapping were enriched for ultra-rare damaging missense and protein-truncating
variations in BD cases 4 , highlighting convergence of common and rare variant signals. We
report differences in genetic architecture of BD depending on the source of patient
ascertainment and on BD-subtype (BDI and BDII). Several analyses implicate specific cell types
in BD pathophysiology, including GABAergic interneurons and medium spiny neurons.
Together, these analyses provide novel insights into the genetic architecture and biological
