    Comunicado de Seguridad de Farmacovigilancia Nº 37-2023. Inyecciones intravítreas de anti-VEGF y efectos adversos sistémicos

    Los inhibidores del factor de crecimiento endotelial vascular (anti-VEGF) constituyen un tratamiento fundamental para el manejo oftalmológico de la retinopatía diabética proliferativa (RDP), edema macular diabético (EMD), degeneración macular relacionado a la edad (DMRE), entre otras patologías oculares, mejorando el deterioro de la visión y la calidad de la vista del paciente. EsSalud dispone de los siguientes anti-VEGF: aflibercept y bevacizumab, aprobados bajo dictamen aún vigente. En el caso del bevacizumab se le ha dado un uso fuera de etiqueta para la administración intravítrea, debido a su bajo costo en comparación con su homólogo ranibizumab, con el que comparte un mecanismo de acción semejante

    El síndrome de Stevens-Johnson (SSJ) y la Necrólisis Epidérmica Tóxica (NET) son reacciones adversas graves que comprometen severamente la piel y mucosas, además de otros órganos y sistemas. Pertenecen a una misma entidad patológica que se caracteriza por la necrosis epidérmica generalizada y se diferencian por el porcentaje de compromiso en la superficie corporal afectada (SSJ < 10 %, NET ≥ 30 % y la sobreposición de SSJ/NET 10 - 30%)

    En el último estudio de Carga de Enfermedad y Lesiones en la población asegurada de EsSalud -2023, se realizó la estimación de 149 subcategorías diagnósticas, se actualizaron indicadores para la estimación de la discapacidad y se incorporó como fuente de datos al ESSALUD Servicios de Salud Inteligente ESSI. Las estimaciones realizadas brindan una fuente de información valiosa para la identificación de los problemas de salud en la población asegurada, valorando de manera integral el efecto de un daño o lesión, considerando las pérdidas mortales y no mortales de padecer un daño o lesión desde un punto de vista de limitación física, social y bienestar, asimismo permite evaluar el impacto de intervenciones sanitarias y son el insumo para la determinación de prioridades sanitarias institucionale

    Characteristics and survival of adults with differentiated thyroid cancer in a peruvian hospital

    El propósito del estudio fue describir las características clínicas, anatomopatológicas, tratamiento y supervi-vencia de los pacientes con cáncer de tiroides diferenciado. Se realizó un estudio de cohorte retrospectiva con datos de 150 pacientes de un hospital peruano entre los años 2010 al 2020. Se describieron las características y supervivencia (método de Kaplan-Meier). La media de edad fue 48,3 años, 130 (86,7%) fueron mujeres y el tipo histológico más frecuente fue el papilar 94,6%. El 74,2% tenían estadio TNM I, 70,7% tiroidectomía total y 68,7% recibió yodo radiactivo. La supervivencia global a los 5 años fue 89,3%, siendo menor en aquellos con estadio TNM IV y mayor en los que usaron yodo radiactivo. En conclusión, en un hospital de Cusco, el cáncer diferenciado de tiroides fue más frecuente en mujeres y la supervivencia fue menor en comparación con reportes de otros países.This study aimed at studying the clinical and anatomopathological characteristics, treatment and survival of patients with differentiated thyroid cancer. A retrospective cohort study was conducted with data from 150 patients from a Peruvian hospital between the years 2010 to 2020. Characteristics and survival (Kaplan- Meier method) were described.     Evaluation of the G-8 test as a screening tool for comprehensive geriatric assessment in elderly patients with cancer

    Objetivo: evaluar la sensibilidad y especificidad del test G8 en el tamizaje de adultos mayores con cáncer para la realización de la valoración geriátrica integral (VGI). Materiales y métodos: el presente estudio observacional y retrospectivo se realizó en el Servicio de Geriatría del Hospital Almenara de Lima, Perú. Se revisaron los informes de VGI en las historias clínicas electrónicos de adultos mayores (> 60 años) con cáncer, ambulatorios y hospitalizados, durante noviembre de 2022 y julio de 2023. Los pacientes se clasificaron según los criterios SIOG-1 (Sociedad Internacional de Oncología Geriátrica), formando dos grupos: pacientes aptos y pacientes no aptos o unfit (vulnerables + frágiles + muy enfermos). Del test G8 se estimó la sensibilidad, especificidad y valor predictivo positivo, área bajo la curva característica operativa del receptor (AUC). Resultados: ingresaron al estudio 201 pacientes, 91 mujeres (45,3%) y 110 (54,7%) varones, la media de la edad fue de 76,2 ± 7,4 años. Las neoplasias más frecuentes fueron colorrectal, estómago, próstata y vías biliares. La prevalencia de pacientes aptos y no aptos (unfit) fue del 23,4 y 76,6%, respectivamente. Cuando el puntaje de la prueba G8 fue ≤11, la sensibilidad, especificidad, valor predictivo positivo y AUC fueron 73,4% (intervalo de confianza al 95%: 65,7-80,2%), 91,5% (79,6%-97,6%), 96,6% (91,7-98,6%) y 89% (84-93%), respectivamente. Conclusiones: el test G8 con puntaje ≤11 tendría una alta sensibilidad y especificidad, para identificar adultos con cáncer vulnerables o frágiles, que podrían beneficiarse de la VGI.Objective: To evaluate sensitivity and specificity of the G8 test in screening older adults with cancer who may benefit from a Comprehensive Geriatric Assessment (CGA). Material and methods: This observational retrospective study was carried out in the Geriatrics Service of the Guillermo Almenara Hospital in Lima, Peru. CGA reports were reviewed in the electronic medical records of older adults (> 60 years) with cancer, both outpatients and inpatients, between November 2022 and July 2023. Patients were classified according to the SIOG-1 (International Society of Geriatric Oncology) criteria into two groups: fit and non-fit patients (vulnerable + frail + too sick). Sensitivity, specificity, and positive predictive value, area under the receiver operating characteristic curve (AUC), were estimated for the G8 test. Results: 201 patients entered the study, 91 women (45.3%) and 110 (54.7%) men; their mean age was 76.2 ± 7.4 years. The most frequent neoplasms were colorectal, stomach, prostate, and bile ducts. The prevalence of eligible and unfit patients was 23.4% and 76.6%, respectively. When the G8 test score was ≤11, sensitivity, specificity, positive predictive value, and AUC were 73.4% (95% Confidence Interval: 65.7- 80.2%), 91.5% (79.6%-97.6%), 96.6% (91.7-98.6%), and 89% (84-93%), respectively. Conclusions: The G8 test with a score ≤11 would have high sensitivity and specificity for identifying vulnerable or frail patients with cancer who could benefit from the CGA

    Clinical practice guideline for the treatment of hormone-sensitive metastatic prostate cancer in the Peruvian Social Security (EsSalud)

    Introducción: Este artículo resume la guía de práctica clínica (GPC) para el tratamiento del cáncer de próstata metastásico hormonosensible en el Seguro Social del Perú (EsSalud). Objetivo: Proveer recomendaciones clínicas basadas en evidencia para el tratamiento de adultos con adenocarcinoma de próstata metastásico hormonosensible con presentación de novo o recurrente y de alto o bajo volumen metastásico en EsSalud. Métodos: Se conformó un grupo elaborador de la guía (GEG) que incluyó médicos especialistas y metodólogos, el cual formuló preguntas clínicas. Se realizaron búsquedas sistemáticas de revisiones sistemáticas y –cuando fue considerado pertinente– estudios primarios en PubMed durante el 2022. Se seleccionó la evidencia para responder cada una de las preguntas clínicas planteadas. Se evaluó la certeza de evidencia usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). En reuniones de trabajo periódicas, el GEG usó la metodología GRADE para revisar la evidencia y formular las recomendaciones. La GPC fue revisada por expertos externos antes de su aprobación. Resultados: La GPC abordó 6 preguntas clínicas, divididas en 2 temas principales: consideraciones sobre la terapia de deprivación androgénica (TDA) y TDA combinada con otras terapias. En base a dichas preguntas se formularon 10 recomendaciones (4 fuertes y 6 condicionales) y 6 puntos de buena práctica clínica. Conclusión: Se emitieron recomendaciones basadas en evidencia para el manejo de pacientes con esta patología.Introduction:This article summarizes the clinical practice guideline (CPG) for the treatmentof metastatic hormone-sensitive prostate cancer in the Social Security of Peru (EsSalud). Objective:To provide evidence-based clinical recommendations for the treatment of adults with hormone-sensitive metastatic adenocarcinoma of the prostate with de novo or recurrent presentation and high or low metastatic volume in EsSalud. Methods:A guideline development group (GDG) was formed, which included specialist physicians and methodologists, who formulated clinical questions. Systematic searches were conducted for systematic reviews and -when deemed relevant -primary studies in PubMed during 2022. Evidence was selected to answer each of the clinical questions posed. The certainty of evidence was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. In periodic working meetings, the GEG used the GRADE methodology to review the evidence and formulate recommendations. The CPG was reviewed by external experts before its approval. Results:The CPG addressed 6 clinical questions, divided into 2 main topics: considerations on androgen deprivation therapy (ADT) and ADT combined with other therapies. Based on these questions, 10 recommendations were formulated (4 strong and 6 conditional), and 6 points of good clinical practice. Conclusion:Evidence-based recommendations were issued for the management of patients with this pathology

    Severe hypoglycemia in a 14-year-old adolescent with negative imaging tests and inconclusive intra-arterial calcium stimulation test

    Introducción: El insulinoma es la causa más frecuente de hipoglicemia adquirida. Su localización con el uso de imágenes auxiliares puede constituir un reto diagnóstico. Reporte de caso: Adolescente de 14 años con convulsión y glucosa de 22mg/dl; insulina en 62UI/ml, péptido C: 9.4ng/ml, compatibles con hipoglicemia hiperinsulinémica. Tomografía, resonancia magnética de páncreas y ecoendoscopía son negativas. Se realiza test de estimulación intraarterial con calcio (TEIC) que muestra elevación de insulina menor al doble en arteria esplénica. Ante resultado no concluyente se decide pancreatectomía subtotal cuerpo-cola. Anatomía patológica informa: tumoración de 0.7cm compatible con insulinoma. Discusión: Insulinoma debería considerarse siempre como posibilidad diagnostica en toda hipoglicemia hiperinsulinémica. No existe un protocolo definido a seguir en caso de TEIC no concluyente; hacer uso de otros marcadores como el péptido C, repetir este, analizar el número de veces que se eleva insulina respecto al basal sería una alternativa.Introduction: Insulinoma is the most frequent cause of acquired hypoglycemia. Tumor localization with auxiliary images is possible in many patients, and it is necessary to resort to other methods in the rest.Report: 14-year-old adolescent with seizure and glucose 22mg/dl; insulin at 62UI/ml, C-peptide: 9.4ng/ml, compatible with hyperinsulinemic hypoglycemia. Tomography, magnetic resonance imaging of the pancreas, and echoendoscopy are negative. An intra-arterial stimulation test with calcium (TEIC) was performed, which showed less than adouble elevation of insulin in the splenic artery. Given the inconclusive result, a body-tail subtotal pancreatectomy was decided. Pathological anatomy reports: 0.7cm tumor compatible with insulinoma.Discussion:Insulinoma should always be considered as a diagnostic possibility in all hyperinsulinemic hypoglycemia. There is no defined protocol to follow in case of inconclusive TEIC; making use of other markers such as C-peptide, repeating this, analyzing the number of times that insulin rises with respectto baseline would be an alternative

    The impact of clinical genome sequencing in a global population with suspected rare genetic disease

    There is mounting evidence of the value of clinical genome sequencing (cGS) in individuals with suspected rare genetic disease (RGD), but cGS performance and impact on clinical care in a diverse population drawn from both high-income countries (HICs) and low- and middle-income countries (LMICs) has not been investigated. The iHope program, a philanthropic cGS initiative, established a network of 24 clinical sites in eight countries through which it provided cGS to individuals with signs or symptoms of an RGD and constrained access to molecular testing. A total of 1,004 individuals (median age, 6.5 years; 53.5% male) with diverse ancestral backgrounds (51.8% non-majority European) were assessed from June 2016 to September 2021. The diagnostic yield of cGS was 41.4% (416/1,004), with individuals from LMIC sites 1.7 times more likely to receive a positive test result compared to HIC sites (LMIC 56.5% [195/345] vs. HIC 33.5% [221/659], OR 2.6, 95% CI 1.9–3.4, p < 0.0001). A change in diagnostic evaluation occurred in 76.9% (514/668) of individuals. Change of management, inclusive of specialty referrals, imaging and testing, therapeutic interventions, and palliative care, was reported in 41.4% (285/694) of individuals, which increased to 69.2% (480/694) when genetic counseling and avoidance of additional testing were also included. Individuals from LMIC sites were as likely as their HIC counterparts to experience a change in diagnostic evaluation (OR 6.1, 95% CI 1.1–∞, p = 0.05) and change of management (OR 0.9, 95% CI 0.5–1.3, p = 0.49). Increased access to genomic testing may support diagnostic equity and the reduction of global health care disparities
