Effectiveness of symptom management after surgical fenestration of pineal cysts
Die klinische Behandlung von Pinealiszysten bleibt umstritten. Während die meisten Patienten asymptomatisch sind, berichten einige über unspezifische Symptome wie Kopfschmerzen, Schlafstörungen, Schwindel und Übelkeit. Eine chirurgische Entfernung wird häufig bei Patienten mit Hydrozephalus oder Parinaud-Syndrom durchgeführt, während bei unspezifischen Symptomen in der Regel ein konservativer Ansatz gewählt wird. Ziel dieser Studie war es, die Ergebnisse von Operationen bei Patienten mit Pinealiszysten zu bewerten und Faktoren zu identifizieren, die den chirurgischen Erfolg beeinflussen können.
Es wurden 58 Patienten untersucht, die zwischen 2005 und 2021 an der Universitätsmedizin Göttingen aufgrund von Pinealiszysten operiert wurden. Die Studie fokussierte sich auf prä- und postoperative Symptome, die Zufriedenheit der Patienten und Faktoren, die den chirurgischen Verlauf beeinflussen. Die Ergebnisse zeigten, dass die Zystengröße, Hydrozephalus und Parinaud-Syndrom nicht die einzigen Prädiktoren für symptomatische Zysten sind und, dass auch eine sorgfältige Auswahl von Patienten ohne diese Merkmale von einer Operation profitieren kann. Die meisten Patienten erlebten eine signifikante Symptomverbesserung und eine Reduktion der Schmerzmedikation, bei gleichzeitig hoher postoperativer Zufriedenheit.
Interessanterweise zeigte die Zystenfenestration vergleichbare Ergebnisse wie die Zystenresektion, obwohl in anderen Studien die Erfolgsraten der Resektion in der Regel höher waren. Die Studie legt nahe, dass Fenestration eine sichere und effektive Alternative sein kann.
Zusammenfassend zeigt die Studie, dass die chirurgische Entfernung von Pinealiszysten eine effektive und sichere Methode zur Symptomlinderung darstellt, wobei die Zystenfenestration eine praktikable Alternative bietet. Die Studie hebt die Bedeutung einer individuell abgestimmten Patientenauswahl hervor und legt nahe, dass beide Verfahren die Lebensqualität der Patienten signifikant verbessern können.The clinical management of pineal cysts remains controversial, with varying opinions regarding their pathological significance and the need for surgical intervention. While most patients are asymptomatic, some experience non-specific symptoms such as headaches, sleep disturbances, dizziness, and nausea. Surgical removal is commonly performed in cases with hydrocephalus or Parinaud’s syndrome, but for non-specific symptoms, a conservative approach is typically adopted. This study aimed to evaluate the outcomes of surgery in patients with pineal cysts, identifying factors that may influence surgical success.
A total of 58 patients who underwent surgery for pineal cysts at the University Medical Centre Göttingen between 2005 and 2021 were surveyed. The study focused on pre- and post-operative symptoms, patient satisfaction, and factors influencing surgical outcomes. Results indicated that cyst size, hydrocephalus, and Parinaud’s syndrome are not the sole predictors of symptomatic cysts, and a carefully selected group of patients without these features may also benefit from surgery. Most patients experienced significant symptom improvement and a reduction in pain medication, with high levels of post-operative satisfaction.
Interestingly, cyst fenestration produced results comparable to cyst resection, despite the generally higher success rates of resection reported in other studies. The study suggests that fenestration may be a safe and effective alternative, particularly when proximity to important vessels is a concern. However, complete fenestration is crucial to ensure effective drainage of cyst fluid.
In conclusion, surgical removal of pineal cysts is an effective and safe treatment for symptom relief, with cyst fenestration offering a viable alternative. In vivo study on the flow dynamics of cerebrospinal fluid in children and adolescents with cerebrospinal fluid circulation disorders - first clinical application of real-time phase contrast MRI
Durch den Einsatz von Echtzeit-Phasenkontrast-MRT konnte in den letzten Jahren ein enormer Zuwachs an Wissen hinsichtlich der Flussdynamik von Liquor-cerebrospinalis (CSF) gewonnen werden. Jahrzehntelange vorherrschende Lehrmeinungen, die von einem unilateralen Fluss von CSF aus den Hirnventrikeln bis in den Subarachnoidalraum ausgingen (bulk-flow-Theorie), wurden durch die Identifizierung der Atmung als wesentlicher Antrieb eines kranial gerichteten CSF-Flusses in Frage gestellt. Ebenfalls bietet die Methode die Möglichkeit zur genaueren Untersuchung von bisher nur unvollständig verstandenen Liquorzirkulationsstörungen, welche sich bereits im Kindesalter manifestieren.
In dieser Arbeit wurden erstmalig in-vivo Messungen von CSF- sowie Blutfluss in Kindern und Jugendlichen mit Liquorzirkulationsstörungen unter Zuhilfenahme von flusssensitiven Echtzeit-Phasenkontrast-MRT durchgeführt und die Ergebnisse der Flussmessungen, mit denen von gesunden Kindern und Jugendlichen verglichen. Die Messungen wurden im prä- und postoperativen Verlauf nach einem neurochirurgischen Eingriff durchgeführt. Es erfolgten quantitative Fluss- und Frequenzmessungen in 3 Patienten mit dem Krankheitsbild des Hydrozephalus, 4 Patienten mit spinalen Pathologien und teilweise resultierender Syringomyelie sowie 13 gesunden Kindern und Jugendlichen. Die region-of-interest der Flussmessungen wurden abhängig von der zugrundeliegenden Pathologie an verschiedenen Stellen der inneren und äußeren Liquorräume platziert. Aufgrund der hohen zeitlichen sowie räumlichen Auflösung und Unabhängigkeit von physiologischen-gating konnten die Flussmessungen unter Rücksichtnahme mehrerer physiologischer Faktoren, insbesondere der Atmung, durchgeführt und ausgewertet werden.
Gegensätzlich zur bulk-flow-Theorie, konnte erstmals auch in Kindern und Jugendlichen ein kranialer CSF-Fluss im zervikalen-Subarachnoidalraum sowie im Aqueductus mesencepahli nachgewiesen werden. CSF fließt sowohl in der untersuchten Patientengruppe als auch in gesunden Kindern und Jugendlichen, vor allem während forcierter Atmung, in kranialer Richtung. Die Atmung hatte neben dem Herzschlag einen messbaren Einfluss auf den Fluss von CSF. Zeitgleich konnte ebenfalls ein direkter Einfluss der Respiration auf den venösen, spinalen-Blutfluss in Patienten- und Probanden-Gruppe nachgewiesen und quantitativ gemessen werden. In allen Patienten waren postoperativ Änderungen der Flussdynamik von CSF messbar.
Die Ergebnisse zeigen, dass es nach den durchgeführten neurochirurgischen Eingriffen zu Krankheits- und Patienten spezifischen Veränderungen der Flussdynamik in den äußeren und inneren Liquorräumen kommt, welche sich durch Echtzeit-Phasenkontrast-MRT nachweisen und quantifizieren lassen. Diese Veränderungen korrelierten in dieser Studie nicht in allen Fällen mit einer klinischen Besserung der Symptomatik. Hinsichtlich möglicher pathophysiologischer Erklärungsansätze ließen sich vor allem in den Patienten mit kommunizierendem Hydrozephalus ein deutlich erhöhter CSF-Fluss im Aqueductus mesencephali nachweisen. Der im Vergleich zur Probanden-Gruppe doppelt so hoch

gemessene, hyperdynamische bilaterale CSF-Fluss bietet mögliche Erklärungsansätze abseits von den weiterhin verbreiteten Lehrmeinungen.
Die gewonnen Erkenntnisse zur Dynamik von CSF- und Blutfluss in gesunden Probanden sowie in Kindern- und Jugendlichen mit Liquorzirkulationsstörungen kann in der Zukunft neue Erkenntnisse hinsichtlich möglicher pathophysiologischer Erklärungsansätze bieten und die Basis für die Entwicklung personalisierter Therapieverfahren darstellen.In recent years, the use of real-time phase-contrast MRI has led to an enormous increase in knowledge regarding the flow dynamics of cerebrospinal fluid (CSF). Decades of prevailing doctrine, which assumed a unilateral flow of CSF from the cerebral ventricles into the subarachnoid space (bulk-flow theory), have been called into question by the identification of respiration as the main driver of a cranially directed CSF flow. The method also offers the possibility of a more precise investigation of cerebrospinal fluid circulation disorders that were previously only incompletely understood and that already manifest themselves in childhood.
In this study, for the first time, in-vivo measurements of CSF and blood flow were performed in children and adolescents with CSF circulation disorders using flow-sensitive real-time phase-contrast MRI and the results of the flow measurements were compared with those of healthy children and adolescents. The measurements were performed in the pre- and postoperative course after a neurosurgical procedure. Quantitative flow and frequency measurements were performed in 3 patients with hydrocephalus, 4 patients with spinal pathologies and partially resulting syringomyelia and 13 healthy children and adolescents. The region-of-interest flow measurements were placed at different locations in the inner and outer CSF spaces depending on the underlying pathology. Due to the high temporal and spatial resolution and independence from physiological gating, the flow measurements could be performed and analyzed taking into account several physiological factors, especially respiration.
Contrary to the bulk-flow theory, cranial CSF flow in the cervical subarachnoid space and in the aqueductus mesencepahli could also be detected in children and adolescents for the first time. CSF flows in a cranial direction both in the examined patient group and in healthy children and adolescents, especially during forced breathing. In addition to the heartbeat, breathing had a measurable influence on the flow of CSF. At the same time, a direct influence of respiration on the venous, spinal blood flow could also be demonstrated and quantitatively measured in the patient and volunteer groups. Postoperative changes in CSF flow dynamics were measurable in all patients.
The results show that disease- and patient-specific changes in the flow dynamics in the outer and inner CSF spaces occur after the neurosurgical procedures performed, which can be detected and quantified by real-time phase-contrast MRI. In this study, these changes did not correlate with a clinical improvement in symptoms in all cases. With regard to possible pathophysiological explanations, a significantly increased CSF flow in the mesencephalic aqueduct was detected, particularly in the patients with communicating hydrocephalus. The insights gained into the dynamics of CSF and blood flow in healthy subjects as well as in children and adolescents with CSF circulation disorders may provide new insights into possible pathophysiological explanations in the future and form the basis for the development of personalized therapy methods.2025-02-1
The role of CHD1 depletion in prostate cancer during epithelial to mesenchymal transition and osseous metastasis
In the treatment of advanced prostate cancer with androgen deprivation, almost all patients develop castration resistance after some time. The tumor growth continues despite ongoing therapy. In these castration-resistant tumor cells an increased depletion of the chromatin modulator CHD1 was observed. A link between CHD1 and androgen resistance can therefore be assumed. The aim of this study was to investigate the possible influence of CHD1 depletion on prostate carcinoma cells and the influence of androgen deprivation on the CHD1-depleted cells. CHD1-depleted LNCaP cells were analyzed at the protein and RNA level using various methods. A dependence of mesenchymal markers on the CHD1 depletion and on androgen deprivation was found. CHD1-depleted cells migrate more frequently and typical EMT markers are upregulated. This can already be observed in standard medium and is further enhanced in androgen deprivation. As prostate carcinoma cells often metastasize to the bone, a possible change in the tumour cells to osteotropic cells was also investigated. No change in knockdown was found here, but a clear upregulation of osteotropic markers in the CHD1-depleted cells in androgen deprivation. The results indicate that CHD1 depletion in cells is responsible for their more aggressive growth and increased metastasis. Androgen deprivation further enhances these properties, and androgen deprivation stimulates osteomimicry in the cells. It remains to be investigated how far these changes go and whether the CHD1 depleted tumor cells may begin to metastasize under androgen deprivation, which would mean that androgen deprivation would be contraindicated in patients with CHD1 depletion.2025-02-2
Langlebigkeit wiederbefestigter Zahnfragmente im Vergleich zu Kompositrestaurationen zur Therapie von Schmelz-Dentin-Frakturen
Die adhäsive Wiederbefestigung von Zahnfragmenten (Reattachment) stellt eine Therapieoption bei unfallbedingten Schmelz-Dentin-Frakturen dar. Liegt das Fragment nicht vor oder kann nicht wiederbefestigt werden, kann die Fraktur mit einer Kompositrestauration versorgt werden. Ziel dieser retrospektiven Studie war es, das Überleben von Reattachments und Kompositrestaurationen zur Versorgung von Schmelz-Dentin-Frakturen zu vergleichen und mögliche Einflussfaktoren für ein restauratives und/oder biologisches Versagen zu ermitteln.
In die Studie wurden Patient*innen, die im Zeitraum zwischen Januar 2000 und Dezember 2018 aufgrund einer unfallbedingten Schmelz-Dentin-Fraktur mit oder ohne Pulpabeteiligung der bleibenden Dentition in der Poliklinik für Präventive Zahnmedizin, Parodontologie und Kariologie der Universitätsmedizin Göttingen behandelt und entweder mit einem Reattachment oder mit einer Kompositrestauration versorgt wurden. Das Überleben beider Versorgungsarten wurde anhand der digitalen und papier-basierten Aktendokumentation über einen Beobachtungszeitraum von bis zu fünf Jahren auf restaurative und biologische Komplikationen untersucht und Einflussfaktoren auf das Überleben analysiert. Die statistische Analyse wurde unter Verwendung der Kaplan-Meier-Statistik durchgeführt. Die mittleren jährlichen Verlustraten wurden für zwei und fünf Jahre berechnet. Darüber hinaus wurde der Einfluss potenzieller Risikofaktoren auf das Überleben bewertet. Log-Rank-Tests und univariate Cox-Regressionsmodelle (Likelihood-Ratio-Tests) wurden verwendet, um den univariaten Effekt aller Variablen von Interesse zu bewerten. Variablen mit einem p-Wert ≤ 0,1 wurden in ein multivariates Cox-Regressionsmodell mit gemeinsamer Korrelation (p < 0,05) aufgenommen.
Insgesamt wurden 235 Zähne von 164 Patient*innen mit in die vorliegende Studie aufgenommen. Das Überleben von Reattachments und Kompositrestaurationen betrug bis zum Ende des Beobachtungszeitraums von durchschnittlich 1,6 ± 2,5 Jahren 59,3 % bzw. 77,8 % und unterschied sich signifikant (p = 0,002). Restaurative und biologische Versagensmuster konnten häufiger bei Reattachments als bei Kompositrestaurationen beobachtet werden. In der Gruppe der Kompositrestaurationen wurde das Überleben signifikant beeinflusst von der Lokalisation des Zahnes im Kiefer und dem Vorhandensein begleitender Dislokationsverletzungen. In der Gruppe der Reattachments traten 50 % aller Misserfolge innerhalb des ersten Jahres nach der Behandlung auf, während in der Gruppe der Kompositrestaurationen 50 % aller Misserfolge innerhalb der ersten eineinhalb bis zwei Jahre beobachtet wurden.
In der vorliegenden Studie zeigten Kompositrestaurationen ein höheres Überleben bei der Versorgung von Schmelz-Dentin-Frakturen im Vergleich zu Restaurationen mittels Reattachment. Ein Vitalitätsverlust des Zahnes ist am ehesten zu beobachten, wenn der traumatisierte Zahn zusätzlich zur Kronenfraktur auch eine begleitende Dislokationsverletzung erleidet. Bei beiden Versorgungsarten – Reattachment und Kompositrestauration – traten die meisten Komplikationen innerhalb der ersten 24 Monate nach einem Unfall auf. Dies verdeutlicht die Notwendigkeit engmaschiger Nachkontrolltermine in den ersten Jahren nach einem dentalen Trauma der bleibenden Dentition.The adhesive reattachment of a tooth fragment is a treatment option for traumatic enamel-dentin-fractures in permanent teeth. If the fragment is not available or cannot be reattached, the fracture can be treated with a composite resin restoration. The aim of this retrospective study was to analyze the survival rates of reattachments and composite restorations for the treatment of an enamel-dentin-fracture and to identify possible factors influencing (restorative and/or biological) survival.
Patients who were treated in the Department of Preventive Dentistry at the University Medical Centre Göttingen between January 2000 and December 2018 due to an enamel-dentin fracture with or without pulpal involvement of the permanent dentition and either treated with a reattachment or with a composite restoration were included in the study. Survival rates of both types of restoration were examined for restorative and biological complications over an observation period of up to five years based on digital and paper-based dental records.
Furthermore, factors influencing survival rates were analyzed. Statistical analysis was performed using Kaplan-Meier statistics. Mean annual failure rates were calculated for two and five years. In addition, the impact of potential risk factors on survival was assessed. Log-rank tests and univariate Cox regression models (likelihood ratio tests) were used to assess the univariate effect of all variables of interest. Variables with a p-value ≤ 0.1 were included in a multivariate Cox regression model with common correlation (p < 0.05).
A total number of 235 teeth from 164 patients were included in the present study. The survival of reattachments and composite restorations was 59.3 % and 77.8 % until the end of observation period of 1.6 ± 2,5 years, respectively, and differed significantly (p = 0.002). Restorative and biological failures were observed more frequently in cases with reattachment than with composite restoration. Survival of composite restorations was significantly affected by the location of the tooth in the jaw and the presence of concomitant dislocation injuries. For reattachments, 50 % of all failures occurred within the first year after treatment, while for composite restorations 50 % of all failures were observed within the first 18 up to 24 months.
In the present study, composite restorations showed higher survival rates for the treatment of enamel-dentin fractures compared to restorations using reattachment techniques. A loss of vitality of the tooth is most likely to be observed when the traumatized tooth suffers a concomitant dislocation injury in addition to the crown fracture. For both types of restoration - reattachment and composite restoration - most complications occurred within the first 24 months after a traumatic dental injury. These findings underline the need for frequent follow-up appointments in the first few years after dental trauma in the permanent dentition.2025-02-1
Assessing the stratification and the prognostic value of SARS-CoV-2 routine data in the context of advanced hospitalization rate 2.0 (HR 2.0)
Background: Machine learning is becoming increasingly popular in the field of infection prevention and control. This is particularly evident in the context of surveillance, where screening clinical prediction rules (CPR) can be used with routine patient data.
Aim: Several machine learning algorithms as CPR were evaluated based on their performance metrics on the example of the so-called Hospitalization Rate 2.0 (HR 2.0), which aims to differentiate cases admitted due to COVID-19 from incidental SARS-CoV-2 positive cases.
Materials and methods: A monocentric, retrospective, cross-sectional study design was used for model development and model comparison. Patients with a positive SARS-CoV-2 RT-PCR test at hospital admission were included for model development (01/01/2022 - 06/30/2022). Patients were characterised as primary case (admitted due to COVID-19) versus incidental case (SARS-CoV-2 positive but admitted for another reason) using clinical reasoning and the six-eye principle. 6 models were applied using2026-01-2
Feasibility study on implementing formative key feature assessments in dentistry
Diese Studie untersucht die Implementierbarkeit und Optimierung von formativen Key-Feature-Fällen in der zahnmedizinischen Ausbildung. Ziel war es, zu prüfen, ob dieses Prüfungsformat geeignet ist, um kritische Schlüsselstellen der zahnärztlichen Behandlung abzubilden und das klinische Denken der Studierenden zu fördern. Im Wintersemester 2017/2018 nahmen 39 Zahnmedizinstudierende an fünf formativen E-Fall-Prüfungen sowie einer abschließenden Prüfung teil. Einige Items wurden mehrfach wiederholt, um den Einfluss eines wiederholten Testens auf den Lernerfolg zu untersuchen. Die Prüfungen wurden sowohl quantitativ als auch qualitativ ausgewertet. Die quantitativen Ergebnisse zeigten eine signifikante Leistungssteigerung bei jeder weiteren Exposition gegenüber denselben Prüfungsitems, was den positiven Effekt von wiederholtem Testen auf den Lernerfolg belegt. In den Fokusgruppendiskussionen wurden der praxisnahe Bezug und die Förderung nachhaltigen Lernens besonders hervorgehoben, während Bedenken bezüglich der Zeitintensität und der detaillierten Fragestellungen geäußert wurden. Die erfolgreiche Implementierung des Prüfungsformats stellt einen wichtigen Schritt in der Weiterentwicklung der zahnmedizinischen Ausbildung dar. Die Analyse unterstützt die Zuverlässigkeit und Validität des Formats und liefert wertvolle Erkenntnisse für eine mögliche, weiter optimierte Einführung in das Curriculum.This study examines the feasibility and optimization of formative key feature cases in dental education. The aim was to determine whether this examination format is suitable for capturing critical key aspects of dental treatment and fostering students' clinical reasoning. In the winter semester of 2017/2018, 39 dental students participated in five formative e-case examinations as well as a final assessment. Certain items were repeated multiple times to investigate the effect of repeated testing on learning outcomes. The examinations were analyzed both quantitatively and qualitatively. The quantitative results demonstrated a significant improvement in performance with each additional exposure to the same test items, confirming the positive impact of repeated testing on learning success. In focus group discussions, the practical relevance and promotion of sustainable learning were particularly emphasized, while concerns were raised regarding the time intensity and detailed nature of the questions. The successful implementation of this examination format represents an important step in the further development of dental education. The analysis supports the reliability and validity of the format and provides valuable insights for a potentially further optimized integration into the curriculum.2025-05-1
Bone Morphogenetic Protein signaling in the planarian flatworm Schmidtea mediterranea – evolutionary, cellular, and spatial patterns
In early development, morphogen gradients orchestrate the establishment of body axes. Perturbations of such a fundamental process are often damaging to embryonal development, making it difficult to study the role of body patterning factors later during development or even in adult animals. The planarian flatworm Schmidtea mediterranea provides a valuable model for addressing this challenge: Its pluripotent stem cell population does not only allow it to regenerate and reestablish its body axes after injury but is also required for dynamic tissue turnover and body plan maintenance. Therefore, by introducing perturbations of body axis patterning by RNA-interference we can study the remodeling of the body plan and the functions of patterning factors in an adult animal. A specifically interesting body plan determinant is Bone morphogenetic protein (BMP), a member of the TGFβ signaling family, which has been shown to not only pattern the dorso-ventral axis in S. mediterranea, but also influences anterior-posterior and mediolateral patterning. It is expressed on the dorsal side in a mediolateral gradient with the highest expression on the midline and fading intensity towards the body edge. However, how this mediolateral bmp expression gradient is formed is poorly understood. Moreover, the spatial distribution of BMP signaling activity through the planarian body remains unstudied so far.
The overall goal of my thesis was to probe the mechanistic basis of BMP gradient formation in S. mediterranea. In the first part, I systematically annotated the TGFβ signaling pathway components including BMP in the S. mediterranea genome and reconstructed their phylogeny in the context of other metazoans. Next, I established a Western blotting assay for the quantification of BMP signaling activity in response to the knockdown of potential BMP signaling factors. Furthermore, I established a whole mount immunostaining assay for the activated BMP signaling transducer phospho-Smad1 to reveal the spatial patterns of BMP signaling activity. With these tools at hand, I was able to quantify BMP signaling activity and visualize its spatial distribution throughout the planarian body. In the last part of the thesis, I utilized these methods to gain understanding of the mechanisms that establish the mediolateral expression profile of bmp in S. mediterranea.
I found previously undescribed TGFβ ligands and identified candidates for putative regulators of BMP4 signaling activity in an RNAi screen. Furthermore, I was able to visualize the spatial distribution of BMP signaling activity in S. mediterranea wild type animals and found that BMP signaling is not only active in the sites of dorsal bmp4 expression, but also occurs in other tissues like the gut. Moreover, my findings indicate that BMP signaling is likely involved in gut branching. By assessing the expression pattern of bmp4 and the spatial distribution of BMP signaling activity, I found indications for autoregulation of BMP signaling in S. mediterranea, possibly by a patterning organizer on the midline. Moreover, the metalloproteinase Tolloid is possibly necessary for long range BMP signal distribution from the midline to the body edge and influences muscle development.
In summary, my work provides an overview of the TGFβ signaling components of S. mediterranea and their phylogenetic relation to components of other metazoans, new assays to assess BMP signaling activity in S. mediterranea, and insights into the mechanisms of the mediolateral distribution of BMP signaling activity.2026-01-2
Cell-Substrate Distance Fluctuations and their Impact on Wound Healing in Epithelial Tissues
Cellular movements are fundamental to a wide array of physiological processes essential for an organism's development, maintenance, and repair. Understanding the underlying mechanisms that regulate these movements provides valuable insights into both normal cellular function and the pathological alterations associated with disease. Central to cellular motility are mechanical forces predominantly driven by the actomyosin cytoskeleton. The generation and regulation of these forces rely on intricate signaling cascades and diverse proteins, all of which are influenced by internal factors and external stimuli, such as interactions with neighboring cells and the extracellular matrix. Within a confluent epithelial cell layer, cells are interconnected by various junctions that not only maintain structural integrity but also serve as critical sensors for signal transduction. However, how do cells respond to the loss of a specific junction type? How do epithelial cells reorganize their actomyosin cortex to compensate for the disruption of cell-cell connections? And what are the consequences for the mechanical properties and rigidity of the cell layer?
This thesis addresses these questions, offering a comprehensive analysis of the mechanical behavior of epithelial cells (MDCK-II) lacking tight junctions, with a focus on the dynamics of the basolateral actomyosin cytoskeleton during cellular movements in confluent layers and wound healing processes.2025-03-2
The role of Hedgehog signaling in Ependymoma
Im Rahmen dieser Arbeit wurde die Aktivität der Hedgehog (HH) Signalkaskade in Ependymomen untersucht. Dabei erfolgte zunächst der Nachweis von Sonic Hedgehog (SHH) an Gewebeschnitten mittels immunhistochemischer Färbungen (IHC). Dafür wurden 86 Paraffinschnitte analysiert. Die meisten Ependymome wiesen eine Expression von SHH auf. Anhand dieser Untersuchungen zeigte sich die höchste SHH Expression in Grad II und III Ependymomen. Tumore der hinteren Schädelgrube wiesen ein etwas höheres SHH Expressionsniveau auf als supratentorielle und spinale Ependymome. Interessanterweise zeigte sich in Abhängigkeit der prognostisch ungünstigen Fusion des RELA-Protoonkogenes, NF-kB Untereinheit (RELA) kein Unterschied in fusionspositiven und -negativen Ependymomen. Neben Ependymomen wurde auch eine SHH Expression in Ependymzellen nachgewiesen, die als Ursprungszellen für Ependymome gelten. Darüber hinaus wurde die Aktivität des Signalweges anhand von quantitativer real time-PCR (qRT-PCR) von Zielgenen an RNA untersucht, die aus 25 tiefgefrorenen Gewebeproben von Ependymomen isoliert wurde. So konnten eine erhöhte Expression des glioma-associated oncogene (GLI) - GLI1 und eine verringerte Expression von HHIP im Vergleich zu normalem Kontrollgewebe gezeigt werden. Zusammengefasst weisen diese Ergebnisse auf eine aktive HH Signalkaskade in Ependymomen hin. Schließlich konnte die Expression von GLI1 Transkripten auch exemplarisch an Gewebeschnitten von Ependymomen mittels RNAscope nachgewiesen werden. Des Weiteren wurde in dieser Arbeit mittels IHC an Paraffinschnitten erstmalig gezeigt, dass SHH in Astroblastomen, Pineozytomen und Pineoblastomen exprimiert wird. Ebenso ließ sich SHH in Glioblastomen, Astrozytomen und Neurozytomen nachweisen. Zusammengefasst stellen diese Daten eine wichtige Grundlage für weiterführende Analysen im Hinblick auf mögliche neue Therapieoptionen von Ependymomen dar. Eine Vielzahl von verschiedenen Inhibitoren der HH Signalkaskade, die an unterschiedlichen Zielmolekülen ihre Wirkung entfalten, wird bereits in der Therapie anderer Tumore verwendet. Somit könnten auf der Grundlage dieser Arbeit in der Zukunft alternative Behandlungsoptionen überprüft und umgesetzt werden.In this thesis the activity of Hedgehog (HH) signaling in ependymoma was analyzed. To this end, immunohistochemical stainings were performed on 86 paraffin sections to examine the expression of Sonic Hedgehog (SHH). The results demonstrated that the majority of ependymomas exhibited positive staining for SHH. The highest levels of expression were observed in grade II and III ependymoma. A slightly higher expression level was observed in tumors located in the posterior fossa compared to those in supratentorial or spinal regions. Notably, tumors exhibiting the unfavorable fusion of the protooncogene RELA, which is a subunit of NF-kB, demonstrated no significant variation in SHH expression when compared to fusion-negative ependymoma.
Ependymal cells, regarded as the cells of origin for ependymoma, also exhibited positive staining for SHH.
Furthermore, the activity of HH signaling was analyzed by means of quantitative real-time PCR (qRT-PCR) using RNA isolated from 25 frozen tissue samples of ependymoma. These analyses revealed an increased expression of the glioma-associated oncogene (GLI) and reduced expression of hedgehog interacting protein (HHIP) compared to control tissue without tumor.
The collective findings of these analyses suggest the presence of active HH signaling in ependymoma.
Finally, GLI1 transcripts were detected in a few representative paraffin sections by RNAscope.
Furthermore, this study provides the initial documentation of positivity for SHH by immunohistochemistry in astroblastoma, pineocytoma, and pineoblastoma. Glioblastoma, astrocytoma, and neurocytoma also demonstrate SHH expression.
In summary, these findings provide a significant foundation for further analyses, particularly regarding the exploration of novel therapeutic approaches for ependymoma. Several different inhibitors of HH signaling that interact at various levels of the signaling cascade are already used for treatment of other tumor entities. Consequently, this study provides a foundation for the future investigation and potential implementation of alternative therapeutic options that target HH signaling in ependymoma.2025-04-1
Die Wirksamkeit eines 6-wöchigen hochfrequenten, standardisierten Sportprogramms auf die Erkrankungsschwere in drei Gruppen psychiatrischer Störungen
Depressionen, Abhängigkeitssyndrome und Schizophrenie stellen drei der häufigsten
schweren psychiatrischen Störungen dar. Für alle drei Erkrankungen gibt es etablierte
Therapieregime und S3-Leitlinien zur Diagnostik und Behandlung. Die hohen Prävalenzen
führen zu einer gesellschaftlich hohen Krankheitslast und die weitere Erforschung besserer
und additiver Therapien wie der Bewegungs- und Sporttherapie ist zur Ergänzung der
bisherigen Literatur notwendig. Insbesondere in Bezug auf Symptome der Depression, aber
auch bei anderen psychiatrischen Störungen, haben sich Sporttherapien vorrangig im
aeroben Belastungsbereich als wirksam erwiesen.
In der dieser Dissertation zugrundeliegenden SIWAS-Studie haben voll- und teilstationär in
den offen geführten psychiatrischen Stationen der Universitätsmedizin Göttingen
aufgenommene Patient:innen aus den drei Erkrankungsgruppen unipolare Depression,
Abhängigkeitssyndrom und Schizophrenie zusätzlich zur Standardbehandlung an einem
sechswöchigen Sportprogramm teilgenommen. An fünf Tagen pro Woche wurde ein
standardisiertes einstündiges, hauptsächlich ausdauerorientiertes Programm angeboten. An
verschiedenen Zeitpunkten erhobene Daten wurden im Anschluss an die Studie ausgewertet,
um eine mögliche Wirksamkeit zu ergründen.
Die SIWAS-Studie reiht sich in viele weitere bisher durchgeführte Studien im Bereich der
Bewegungs- und Sporttherapie ein, da sie eher klein, monozentrisch, nicht randomisiert und
nicht kontrolliert durchgeführt wurde. Auch deshalb können aus ihren Ergebnissen keine
eindeutigen Kriterien für die Gestaltung einer erfolgreichen Sportintervention abgeleitet
werden. Dennoch stehen die Studienergebnisse weitgehend im Einklang mit den bisherigen
Erkenntnissen über Bewegungs- und Sporttherapie bei psychiatrischen Störungen. So zeigen
sich potenziell Wirkungen der Sportintervention bei der gesamten Teilnehmendengruppe auf
den Erkrankungszustand und die allgemeine Funktionsfähigkeit innerhalb der ersten
Wochen nach Therapiebeginn. Dies ist ein Hinweis darauf, dass Sporttherapie auch initial
zur Therapie vor Wirkbeginn der Psychotherapie und Pharmakotherapie unterstützend
wirksam sein könnte.
Insbesondere der frühe Einsatz von Sporttherapie bei akut-stationären Patient:innen sollte
aufgrund dieser potenziell überbrückenden Wirkung noch weiter erforscht werden.
Möglicherweise
kann die Wirkung von Sport initial einen Beitrag zur
Krankheitsverbesserung leisten, während Psychopharmaka und Psychotherapie ihre
Wirkung noch nicht voll entfaltet haben. Um diese und weitere Hypothesen zu überprüfen,
sollten insbesondere hochqualitative, groß angelegte und multizentrische Studien zu
Sporttherapie bei psychiatrischen Erkrankungen durchgeführt werden. Dadurch kann
sichergestellt
werden, dass die nebenwirkungsarme und krankheitsspezifisch
vielversprechende Option der Bewegungs- und Sporttherapie adäquat eingeschätzt und
entsprechend flächendeckend angeboten werden kann.Depression, addiction syndromes and schizophrenia are three of the most common severe psychiatric disorders. For all three disorders, there are established treatment regimes and guidelines for diagnosis and treatment. The high prevalence rates lead to a high social burden of disease. Further research into better and additive therapies such as exercise and sports therapy is necessary to supplement the currently available literature. Particularly with regards to symptoms of depression, but also for other psychiatric disorders, sports therapies have proven to be effective.
In the SIWAS study on which this dissertation is based, full and part-time inpatients in psychiatric wards of the University Medical Centre Göttingen from the three disease groups unipolar depression, dependency syndrome and schizophrenia took part in a six-weeks exercise program in addition to treatment as usual. Five days a week, a standardised one-hour, mainly endurance-oriented program was offered. Data collected during the study were analysed to determine possible effectiveness.
The SIWAS study suffers the fate of many other studies conducted to date in the field of exercise and sports therapy, as it was rather small, monocentric, non-randomised and uncontrolled. This is one reason why no specific criteria for the design of a successful sports intervention can be derived from its results. Nevertheless, the results of the study are largely in line with previous findings on exercise and sports therapy for psychiatric disorders. They show potential positive effects of the sports intervention on the entire group of participants on the state of illness and general functional ability within the first few weeks after the start of therapy. This is an indication that sports therapy can be supportive in the early treatment of psychiatric disorders especially before the effects of psychotherapy and pharmacotherapy unfold.
In particular, the early implementation of sports therapy for acute inpatients should be further researched due to this potentially bridging effect. It is possible that the effect of exercise can initially contribute to improve the patient's condition while psychotropic drugs and psychotherapy have not yet fully developed their effect. In order to test this and other hypotheses high-quality, large-scale and multi-centre studies should be conducted on sports therapy for psychiatric disorders. This could ensure that the promising effects of exercise and sports therapy can be assessed adequately.2025-12-0
