Дослідження впливу властивостей лікарських засобів та способу життя пацієнтів з ішемічною хворобою серця на прихильність до лікування
Get PDFAim: To investigate how lifestyle factors of patients with CHD and comorbid conditions, along with medicines properties, influence treatment adherence. Materials and Methods: include the results of a survey conducted among patients with CHD and comorbid conditions at the Department of Cardiology of the P.L. Shupyk National Healthcare University of Ukraine from June to September 2024 (n = 101). The study employed methods of analysis, synthesis, deduction, induction, comparison, statistical and bibliographic research.
Results: The study revealed a significantly low adherence rate of 13.9% [CI 95% 13.9 ± 0.002; p<0.0001] to risk factor modification and prescribed phar- macotherapy in secondary prevention of CHD. It confirmed a statistically significant influence of patient preferences with CHD and comorbid conditions on effective pharmacotherapy (χ2 = 3.350,232, p = 0.067). Patients receiving consultations from both doctors and pharmacists were 22 times more likely (OR = 22.67) to adhere to pharmacotherapy compared to those consulting only doctors. The study found that only 7.8% [CI 95% 7.8 ± 0.05; p<0.0001] of surveyed patients with CHD and comorbid conditions such as hypertension, diabetes mellitus and chronic kidney disease utilized the “Affordable Medicines” program. Conclusions: The decline in the socioeconomic status of the population during the war in Ukraine negatively affects adherence to clinical guidelines for risk factor modification and secondary prevention of CHD, as evidenced by observational studies EUROASPIRE IV and V. The study identified that 42.8% of respon- dents prefer original medicines when selecting medications, but financial constraints prevent prolonged use as part of prescribed pharmacotherapy, adversely affecting treatment adherence. The effectiveness of a multidisciplinary approach in enhancing treatment adherence among patients with CHD and comorbid conditions was confirmed, demonstrating a 22-fold higher adherence rate compared to consultations with doctors alone.Мета: Дослідити, як фактори способу життя пацієнтів з ІХС та супутніми захворюваннями, поряд із властивостями лікарських засобів, впливають на дотримання режиму лікування. Матеріали та методи: включають результати опитування, проведеного серед пацієнтів з ІХС та супутніми захворюваннями на кафедрі кардіології Національного університету охорони здоров'я України імені П.Л. Шупика з червня по вересень 2024 року (n = 101). У дослідженні використовувалися методи аналізу, синтезу, дедукції, індукції, порівняння, статистичного та бібліографічного дослідження.
Результати: Дослідження виявило значно низький рівень дотримання режиму лікування – 13,9% [ДІ 95% 13,9 ± 0,002; p < 0,0001] – до модифікації факторів ризику та призначеної фармакотерапії у вторинній профілактиці ІХС. Було підтверджено статистично значущий вплив уподобань пацієнтів з ІХС та супутніми захворюваннями на ефективність фармакотерапії (χ2 = 3,350,232, p = 0,067). Пацієнти, які отримували консультації як від лікарів, так і від фармацевтів, у 22 рази частіше (ВШ = 22,67) дотримувалися фармакотерапії порівняно з тими, хто консультувався лише з лікарями. Дослідження показало, що лише 7,8% [ДІ 95% 7,8 ± 0,05; p <0,0001] обстежених пацієнтів з ІХС та супутніми захворюваннями, такими як гіпертензія, цукровий діабет та хронічна хвороба нирок, скористалися програмою «Доступні ліки». Висновки: Погіршення соціально-економічного статусу населення під час війни в Україні негативно впливає на дотримання клінічних рекомендацій щодо модифікації факторів ризику та вторинної профілактики ІХС, що підтверджується обсерваційними дослідженнями EUROASPIRE IV та V. Дослідження показало, що 42,8% респондентів надають перевагу оригінальним препаратам під час вибору ліків, але фінансові обмеження перешкоджають тривалому використанню як частини призначеної фармакотерапії, що негативно впливає на дотримання режиму лікування. Було підтверджено ефективність міждисциплінарного підходу у підвищенні дотримання режиму лікування серед пацієнтів з ІХС та супутніми захворюваннями, що продемонструвало у 22 рази вищий рівень дотримання режиму лікування порівняно з консультаціями лише з лікарями

Particular attention is paid to the problems of the mismatch between the education received and the requirements of employers, insufficient interaction between educational institutions and business, as well as changes in the educational orientations of young people. The role of vocational (vocational and technical) education in meeting the post-war needs of the labor market
Get PDFМета нашого дослідження полягала в оцінці ролі професійної (професійно-технічної) освіти України у забезпечені кадрових потреб ринку праці.Встановлено, що середній попит на ринку праці на всі спеціальності зростає приблизно на 6% кожний квартал. Вивчення темпів зміни індексу зміни зайнятих показало, що кількість кваліфікованих робітників майже на третину перевищує кількість некваліфікованих робітників. Виявлено, що кількість громадян України з вищою освітою, як сьогодні офіційно працевлаштовані за межами країни, на 89,9% перевищує кількість громадян з середньою освітою. Визначено, що попит на кваліфіковані кадри в за динамкою індексу зміни зайнятості, який для кваліфікованих робітників на 30% вищий, ніж для некваліфікованих.The purpose of our study was to assess the role of vocational (vocational and technical) education in Ukraine in meeting the personnel needs of the labor market. It was established that the average demand in the labor market for all specialties increases by approximately 6% each quarter. The study of the rates of change of the employment change index showed that the number of qualified workers exceeds the number of unskilled workers by almost a third. It was found that the number of Ukrainian citizens with higher education, as currently officially employed outside the country, exceeds the number of citizens with secondary education by 89.9%. It was determined that the demand for qualified personnel is in line with the dynamics of the employment change index, which is 30% higher for qualified workers than for unskilled workers
New Ukrainian School: modern content and practices of education of students with special educational needs
Get PDFУ статті висвітлено основні науково-методичні підходи щодо розроблення та реалізації змісту модельних навчальних програм для учнів 1-4 та 5-10 класів з порушеннями інтелектуального розвитку відповідно до реалізації Концепції Нової української школи. Акцентовано увагу на необхідності адаптації та модифікації змісту навчання до потреб учнів з особливими освітніми потребами. Проаналізовано низку законодавчих документів в освіті осіб з особливими освітніми потребами, спрямованих на підвищення якості навчання та забезпечення психолого-педагогічного супроводу.
Обґрунтовано проблеми, які потребують негайного розв’язання: забезпечення інноваційного змісту освіти шляхом розроблення модельних навчальних програм на основі компетентнісного підходу, створення освітнього середовища, що забезпечує доступність якісної освіти та успішну соціалізацію учнів з особливими освітніми потребами.
Автором окреслено методичні підходи та принципи побудови модельних навчальних програм, представлено їх структуру, яка охоплює зміст навчального предмета, види навчальної діяльності, очікувані результати навчання та напрями корекційно-розвивальної роботи. Охарактеризовано сучасні практики навчання учнів з порушеннями інтелектуального розвитку, що передбачають застосування індивідуального та диференційованого підходів під час викладання навчальних предметів та корекційно-розвивальних занять. Представлено узагальнений досвід та методичні рекомендації щодо організації спеціального освітнього середовища, використання спеціальних методів та прийоми навчання учнів з порушеннями інтелектуального розвитку.
Акцентовано увагу на важливості поєднання навчання учнів з порушеннями інтелектуального розвитку з корекційно-розвитковими заняттями з соціально-побутового орієнтування, розвитку мовлення, сенсорного та психомоторного розвитку, ритміки, лікувальної фізкультури, заняттями з психологом. Презентовано оновлене програмне забезпечення з корекційно-розвивального супроводу учнів 5-10 класів з порушеннями інтелектуального розвитку. Представлено перспективи подальшого дослідження з окресленої проблематики.The article highlights the main scientific and methodological approaches to the development and implementation of the content of model curricula for students in grades 1-4 and 5-10 with intellectual disabilities in accordance with the implementation of the Concept of the New Ukrainian School. The emphasis is on the need to adapt and modify the content of education to the needs of students with special educational needs. A number of legislative documents in the education of individuals with special educational needs, aimed at improving the quality of education and providing psychological and pedagogical support, are analyzed.
A number of problems that require immediate solutions are substantiated: ensuring innovative content of education by developing model curricula based on a competency-based approach, creating an educational environment that ensures the availability of high-quality education and successful socialization of students with special educational needs.
The author outlines methodological approaches and principles for building model curricula, presents their structure, which covers the content of the subject, types of educational activities, expected learning outcomes and areas of correctional and developmental work. Modern practices of teaching students with intellectual disabilities are characterized, which involve the use of individual and differentiated approaches when teaching subjects and correctional and developmental classes. Generalized experience and methodological recommendations are presented on the organization of a special educational environment, the use of special methods and techniques for teaching students with intellectual disabilities. The emphasis is on the importance of combining the education of students with intellectual disabilities with correctional and developmental classes on social and everyday orientation, speech development, sensory and psychomotor development, rhythmics, therapeutic physical education, classes with a psychologist. Updated software for correctional and developmental support of students in grades 5-10 with intellectual disabilities is presented. Prospects for further research on the outlined issues are presented

Adherence to treatment among patients with coronary heart disease with comorbid conditions: the impact of socio-economic factors during martial law in Ukraine (first message)
Get PDFМета. Визначити впливу соціально-економічних факторів на прихильність до лікування пацієнтів на ІХС з коморбідними захворюваннями в Україні під час війни.
Матеріали і методи. На базі кафедри кардіології Національного університету охорони здоров'я України імені П.Л. Шупика було проведене дослідження за допомогою опитування пацієнтів на ІХС та супутніми артеріальною гіпертензією, цукровим діабетом 2 типу, хронічною хворобою нирок за допомогою онлайн-анкетування з використанням Google Forms з червня по вересень 2024 року. Використано методи систематизації, узагальнення, дедукції та індукції, статистичні, бібліографічні.
Результати. Визначено, що чоловіки більш прихильні до лікування ніж жінки (χ2=0,271, p=0,000 та φ=0,271; p=0,006). Чоловіки та жінки однаково часто відмовляються від лікування у разі покращення (χ2=0,468, p=0,000 та φ=0,468, p=0,000) або погіршення (χ2=0,393, p=0,000 та φ=0,393, p=0,000) стану самопочуття. Пацієнти, що хворіють на ІХС з коморбідними станами із одним/двома соціально- економічними факторами ризику (76,2% [76,2±0,04, p<0,0001]) мають найменшу прихильність до лікування.
Висновки. Прихильність до лікування пацієнтів на ІХС з коморбідними захворюваннями в Україні під час війни залежить від гендерних ознак: чоловіки більш прихильні до лікування. Чоловіки та жінки відмовляються від лікування однаково часто при покращенні/погіршенні стану здоров’я після розпочатого прийому лікування. Визначено статистичну достовірність впливу одного-двох соціально-економічних факторів ризику. Запропоновано для розв’язання питань зменшення помилок у прийомі фармакотерапії пацієнтів на ІХС із коморбідними станами та подальшого покращення впливу на прихильність до лікування створювати мультидисциплінарні команди з участю фармацевтів.Purpose. To assess the impact of socio-economic factors on treatment adherence among patients of coronary heart disease (CHD) with comorbid conditions in Ukraine during wartime.
Materials and methods. The study was conducted at the Department of Cardiology of the P.L. Shupyk National University of Health Care of Ukraine. Patients with CHD with concomitant arterial hypertension, type 2 diabetes mellitus, and chronic kidney disease were surveyed using an online questionnaire via Google Forms between June and September 2024. Methods employed included systematization, generalization, deduction, induction, statistical analysis, and bibliographic review.
Results. The study revealed that men demonstrated higher adherence to treatment compared to women (φ = 0.271, p=0.006). However, both genders equally tended to discontinue treatment when their condition either improved (φ = 0.468, p=0.000) or worsened (φ = 0.393, p=0.000). Patients of CHD with comorbid conditions facing one or two socio-economic risk factors exhibited the lowest adherence levels (76.2% [76.2 ± 0.04, p<0.0001]).
Conclusions. Adherence to treatment among patients of CHD with comorbid conditions in Ukraine during martial law is influenced by gender, with men showing greater adherence. Nonetheless, both men and women frequently discontinue treatment upon experiencing changes in health status, either positive or negative. The study confirmed the significant impact of socio-economic risk factors on adherence. To address these challenges, the creation of multidisciplinary teams, including pharmacists, is recommended to reduce medication errors and improve adherence to pharmacotherapy in patients of CHD with comorbid conditions
