    Die vorliegende Arbeit analysierte 518 Fälle von Brustkrebs die neoadjuvant behandelten wurden. Es wurde eine neue Regressionsklassifikation identifizierte, die das progressionsfreie Überleben von Brustkrebspatientinnen nach neoadjuvanter Chemotherapie besser voraussagen kann als bisher gebräuchlichen Modellen. Das neue "Altonaer Regression Model" (ARM) erwies sich als überlegen, insbesondere in Bezug auf die Überlebensprognose über 5, indem es zusätzliche pathologische Faktoren wie Tumordichte, Zellform und umgebendes Stroma berücksichtigt. Die Studie zeigt, dass bestimmte Tumor- und Zellmerkmale, wie die Anzahl befallener Lymphknoten, Residualtumorgröße und Zellform, das progressionsfreie Überleben signifikant beeinflussen. Anhand eines Entscheidungsbaums wurden drei Risikogruppen mit unterschiedlicher Überlebenswahrscheinlichkeit identifiziert. Die höchste Überlebenswahrscheinlichkeit lag bei kleinen Residualtumoren ohne Lymphknotenbefall. Das ARM-Modell bietet nicht nur eine präzisere Klassifikation und damit eine bessere Vorhersagbarkeit der Prognose der betroffenen Patientinnen, sondern erleichtert basierend darauf ebenfalls die Entscheidungsfindung über das weitere therapeutische Vorgehen

    Clinical outcome analysis of medication-related osteonecrosis of the jaw (MRONJ) after surgical treatment from 2011 to 2019

    MRONJ is a rare condition in a large group of patients treated with antiresorptive medications. It severely reduces the quality of life and frequently presents with a refractory behavior after surgical treatment of stage II and III cases. Therefore, postoperative wound healing and recurrence of the diseases were defined as primary outcome parameters. Since the etiopathogenesis has not yet been conclusively elucidated, different mechanisms are hypothesized to impact the onset and progression of the disease. Antiresorptive agents and their route of administration are considered to influence the development of the disease, while the microorganisms seem to play a leading role in the pathogenesis of symptomatic necrosis in the jaw tissue. In this retrospective study of 148 patients from 2011 to 2019, medical history and therapeutic regimens were evaluated for their influence on surgical outcome parameters. Microbiological examinations of necrotic bone specimens and radiographic images were included in the descriptive and statistical analysis. Both healing (p=0.004) and recurrence (p=0.034) were negatively impacted by fungal colonization, while remaining unaffected by the number of species in the necrotic bone. The postoperative change in the antibiotic treatment (influence on healing: p=0.738; influence on recurrence: p=0.934) or an addition of an antifungal agent (influence on healing: p=0.701; influence on recurrence: p=0.747) did not improve the outcome in the affected patients as desired. Co-medications in cancer therapy also demonstrated a significant influence on the recurrence rates. Cases treated with additional chemotherapy compared to cases of other mAb therapies showed a negative influence with a p-value of 0.001 and the comparison of chemotherapy, hormonal therapy, and other mAb therapies showed a p-value of 0.005. To further substantiate these findings, it is suggested to investigate a larger population of non-oncologic participants and their associated medications. Future research on the possible impact of early fungal detection before the surgical intervention could also fundamentally improve the management of MRONJ.MRONJ ist eine seltene Erkrankung in einer großen Gruppe von Patienten, die mit antiresorptiven Medikamenten behandelt werden. Sie schränkt die Lebensqualität stark ein und zeigt sich häufig mit refraktärem Verhalten nach chirurgischer Behandlung von Fällen im Stadium II und III. Daher wurden die postoperative Wundheilung und Rezidivrate der Erkrankung als primäre Endpunkte definiert. Da die Ätiopathogenese noch nicht abschließend geklärt ist, werden verschiedene Mechanismen diskutiert, den Ausbruch und das Fortschreiten der Krankheit zu beeinflussen. Es ist anerkannt, dass die Art und Verabreichungsform der Antiresorptiva den Krankheitsverlauf beeinflussen, während Mikroorganismen bei der Pathogenese der Nekrosen im Kiefergewebe eine bedeutende Rolle zu spielen scheinen. In dieser retrospektiven Studie von 148 Patienten aus den Jahren 2011 bis 2019 wurden die medizinische Vorgeschichte und der Behandlungsablauf auf ihren Einfluss auf die chirurgischen Ergebnisparameter untersucht. Mikrobiologische Untersuchungen von Proben des nekrotischen Knochens sowie bildgebende Verfahren wurden in die deskriptive und statistische Analyse einbezogen. Sowohl die Heilung (p=0,004) als auch das Rezidiv (p=0,034) wurden durch die Pilzbesiedlung negativ beeinflusst, während sie von der Anzahl der Spezies im nekrotischen Knochen unbeeinflusst blieben. Die postoperative Anpassung der Antibiotika (Einfluss auf Heilung: p=0,738; Einfluss auf Rezidiv: p=0,934) oder die Zugabe eines Antimykotikums (Einfluss auf Heilung: p=0,701; Einfluss auf Rezidiv: p=0,747) verbesserte die Ergebnisse der Betroffenen nicht wie erhofft. Die Begleitmedikation in der Krebstherapie führte ebenfalls zu einer signifikanten Auswirkung auf die Rezidivrate. Die Chemotherapie zeigte im Vergleich zu Fällen mit anderen mAb-Therapien einen negativen Einfluss (p-Wert von 0,001) und der Vergleich zwischen Chemotherapie, Hormontherapie und anderen mAb-Therapien ergab einen p-Wert von 0,005. Um diese Ergebnisse zu untermauern, wird angeraten eine größere Population mit nicht-onkologischen Diagnosen und der damit verbundenen Medikation zu untersuchen. Zukünftige Forschungen zu möglichen Auswirkungen einer frühzeitig erkannten Pilzbesiedelung vor dem chirurgischen Eingriff könnten außerdem das Management von MRONJ grundlegend verbessern

    Untersuchung der Stickstoffdynamik in küstennahen Sedimenten: Eine Studie über permeable Sande in der südlichen Nordsee

    Coastal systems are immensely impacted by mobile ground touching fisheries, global change related effects (e.g. increasing water temperatures, sea level rise and ocean acidification) and other human induced pressures (e.g. pollution and sand extractions). These stressors contribute to a worsening of the environmental status of the southern North Sea. To prevent further degradation, three marine protected areas have been established in the German Exclusive Economic Zone (EEZ). This thesis investigates benthic biogeochemical processes, with a focus on benthic oxygen and nitrogen dynamics in permeable sands of the southern North Sea and explores potential effects of the exclusion of mobile ground touching fisheries. First (chapter 2), various sediment characteristics (e.g. chlorophyll-a, phaeophytin-a, total organic carbon and mud content) and their application as proxies for benthic oxygen consumption and nitrogen turnover are explored. A parameterization framework is developed within this context to predict both net (oxygen, ammonium and nitrate) and gross (ammonium and nitrate) benthic reaction rates based on sedimentary predictor variables. The findings indicate that mud content and phaeophytin-a concentrations are among the most suitable proxies for assessing sediment reactivity. The following chapter (chapter 3) builds on a previously established pore water advection model and the parameterization framework introduced in chapter 2 to develop a process-driven biogeochemical model for permeable sands. This model utilizes the previously identified proxies to predict benthic fluxes of oxygen, ammonium, nitrate and dinitrogen. It examines how the vertical distribution of substrates within the sediment affects the magnitude of these fluxes, revealing that this vertical substrate distribution has minimal impact on the modeled benthic fluxes in permeable advection controlled sands. The fourth chapter extends the application of the benthic biogeochemical model to calculate total fluxes within the Natura 2000 MPAs of the German Bight. This forms the basis for integrating the sediment into a digital twin, enabling the exploration of "what-if" scenarios, such as the effects of excluding mobile bottom-contact fishing on the sedimentary organic matter inventory and sediment reactivity. The results show that excluding bottom trawling results in only slight increases in sedimentary total organic carbon content, leading to minor changes in the benthic fluxes of oxygen and nitrogen. Overall, this study provides a foundational understanding of sediment reactivity in coastal systems and provides crucial insights for refining management strategies to protect and sustain the southern North Sea

    Uncertainty about the past, present, and future, is a psychological fact of life since the beginning of times, springing from different sources. The self and the communities have developed and adopted numerous ways and structures to address its multifaceted discomforts in space and time, sharing many commonalities. Due to hustling globalization and European integration processes, we are facing relatively new forms of uncertainty, which need to be addressed. This thesis endeavors to revisit the role and meaning of law in the 21st century, by holding firmly to the stand that: despite its urge to keep pace with the societal progress and development, law should maintain its functional role and protect the essence – the human need for certainty - and argues that the principle of legal certainty, the only principle to guarantee a state of optimal certainty, shall be enhanced to a constitutional level. The thesis is conceived as a contribution to the ongoing debate on the harmonization of laws in Europe, especially harmonization of family law

    The Impact of a Non-Ideal ILC Helical Undulator on the Photon Beam Parameters and Protection of the Undulator Wall Against Synchrotron Radiation

    The helical undulator-based positron source is considered for the International Linear Collider (ILC) baseline design. The scheme works by passing a multi-GeV (> 100 GeV) electron beam through a long superconducting helical undulator to produce a multi-MeV circularly polarized photon beam. It is then directed to a thin target to produce the electron-positron (e−e+) pair. The collected positron beam is longitudinally polarized. Since photons in the undulator are produced with an opening angle, a part of the photon beam will hit the superconducting undulator wall. The acceptable level of the incident power at the wall is 1 W/m. Computer simulations have been performed in order to determine the incident power at the ILC superconducting undulator walls as well as the photon beam distribution and polarization at the target. For the ILC-250 parameters, the maximum incident power at the wall of the ideal undulator is 21 W/m. This thesis proposes a mask system for undulator protection to keep the incident power at the undulator wall below the acceptable level. The proposed mask has a 30 cm length and a 0.44 cm inner diameter. Three various material candidates for masks are studied: iron, copper, and tungsten. The incident power at the undulator walls due to the secondary particles is also studied. This system reduces the maximum incident power at the ideal undulator wall by approximately two orders of magnitude to ≈ 0.22 W/m for the ideal case of ILC-250. This thesis takes into account the non-ideal (realistic) helical undulators and considers all ILC energy options: the ILC-250, ILC-350, ILC-500, and GigaZ options. The realistic study is based on the measured B-fields of the helical undulator prototypes that were demonstrated at Daresbury Laboratory and Rutherford Appleton Laboratory. In the realistic case for ILC-250, the maximum incident power increases to 0.26 W/m with copper material for masks. The proposed mask system with any of the material candidates can keep the incident power at the undulator walls below the acceptable level for both ideal and realistic undulators and for both nominal and luminosity upgrade parameters of all ILC energy options. The incident power at the photon masks is studied. It is shown that the energy deposition, instantaneous temperature rise, and resulting stress in the masks are below the recommended limits even for long cyclic loads in the materials. The effects of placing masks along the undulator line on the power and polarization of the photon beam at the target plane are also studied. Studies show that placing masks will reduce the power of the photon beam by ≈ 8 % and slightly increase the photon beam polarization at the target plane for both ideal and realistic undulator cases. Compared with the photon beam polarization produced by the ideal undulators, the study shows that in the case of non-ideal undulators, the photon beam polarization decreases. Therefore, in order to achieve an efficient polarization upgrade of the positron beam using a photon collimator, the undulator parameters as well as those of the photon collimator should be re-optimized taking into account the realistic undulator as well as the updated OMD performance

    Emerging infectious diseases (EIDs) are an increasing threat to the human population. In the past two decades, three coronaviruses (CoVs) have spilled over to humans and caused outbreaks of the following respiratory diseases: The severe acute respiratory syndrome (SARS), the Middle East respiratory syndrome (MERS), and the coronavirus disease 2019 (COVID-19), caused by infection with SARS-CoV, MERS-CoV, and SARS-CoV-2, respectively. Vaccination is an important cornerstone in pandemic preparedness against pathogens, in particular concerning the WHO-prioritized CoVs. Vaccine development can be accelerated by the employment of adaptable vaccine platforms, such as mRNA technology and viral vectors, as exemplified by the COVID-19 pandemic. Within a year of the emergence of SARS-CoV-2, one ChAdOx1 viral vector-based and two mRNA-based COVID 19 vaccines were licensed. While vaccine-induced antibodies can provide protection against infection, T cell responses are important for the clearance of virus-infected cells and provide long-term protection against disease. The aim of this thesis was a comprehensive, longitudinal analysis of the T cell response following vaccination against SARS-CoV-2 and MERS-CoV in humans. Vaccine candidates based on the recombinant Modified Vaccinia virus Ankara (rMVA) viral vector were developed against MERS (MVA-MERS-S) and COVID-19 (MVA-SARS-2-S and MVA-SARS-2-ST) and investigated in phase 1 clinical trials at the University Medical Center Hamburg-Eppendorf between 2017 and 2023. Frequent blood sampling in these phase 1 clinical trials provided the unique opportunity to study T cell immunity in detail. Blood samples were collected from trial participants before and longitudinally after vaccination, and compared to blood samples from healthy and immunocompromised individuals, who were vaccinated with licensed vaccines. The magnitude, function, and antigen-specificity of T cell responses were investigated in various assays. The comparative study of the COVID-19 vaccines revealed a higher T cell immunogenicity of MVA SARS-2-ST encoding a prefusion-stabilized SARS-CoV-2 spike protein, compared to MVA-SARS 2 S encoding the native spike protein. While both vaccine candidates were less immunogenic in SARS CoV 2 naïve individuals compared to the licensed vaccine regimens, in heterologous vaccination they showed promising results: MVA-SARS-2-S could prime a polyfunctional CD8+ response when given before mRNA-based immunization. MVA-SARS-2-ST as a third vaccination following mRNA-based immunization could boost the T cell response in individuals with low residual immunity. In contrast to healthy individuals who generated a robust T cell response following prime-boost immunization with licensed COVID-19 vaccines, immunocompromised patients with chronic lymphocytic leukemia, liver cirrhosis or liver transplant had a reduced response. Notably, a third booster dose and heterologous regimens combining mRNA- and viral vector-based vaccines enhanced the T cell response in these patients considerably. The analysis of the MVA-MERS-S vaccine candidate revealed that using a lower vaccine dose and a prolonged time interval between prime and boost immunization enhanced the T cell immunogenicity. Furthermore, CD8+ T cells against a newly identified immunodominant epitope were shown to have a long-lived, polyfunctional phenotype. In summary, these findings underline that vaccine characteristics, vaccination regimen, as well as host factors, can shape the vaccine-induced T cell response. These findings provide important insights for the development of more effective immunization strategies which aim to build up a long-lived T cell memory for protection against disease. This is of particular relevance for immunocompromised patients.Neu auftretende Infektionskrankheiten stellen eine zunehmende Bedrohung für die Bevölkerung dar. In den vergangenen zwei Jahrzehnten sind drei Coronaviren (CoV) auf den Menschen übergesprungen und haben Ausbrüche der folgenden Atemwegserkrankungen verursacht: Das severe acute respiratory syndrome (SARS), das Middle East respiratory syndrome (MERS), und das coronavirus disease 2019 (COVID-19), die durch die Ansteckung mit SARS-CoV, MERS-CoV bzw. SARS-CoV-2 verursacht werden. Impfungen sind ein wichtiger Grundpfeiler der Pandemievorsorge, insbesondere in Bezug auf die von der WHO als prioritär eingestuften Coronaviren. Die Impfstoffentwicklung kann durch anpassungsfähige Impfstoffplattformen wie die mRNA-Technologie und virale Vektoren beschleunigt werden, wie das Beispiel der COVID-19-Pandemie zeigte. Innerhalb eines Jahres wurden ein ChAdOx1-Vektor-basierter und zwei mRNA-basierte COVID-19-Impfstoffe zugelassen. Während impfstoffinduzierte Antikörpertiter einen Schutz vor einer Infektion bieten können, ist die T-Zell-Antwort entscheidend für die Beseitigung von virusinfizierten Zellen und den langfristigen Schutz vor einer Erkrankung. Ziel dieser Arbeit war eine umfassende, longitudinale Analyse der T-Zell-Antwort nach Impfung gegen SARS-CoV-2 und MERS-CoV. Impfstoffkandidaten, die auf rekombinanten Modifizierten Vacciniaviren Ankara (rMVA) basieren, wurden gegen MERS (MVA-MERS-S) und COVID-19 (MVA-SARS-2-S und MVA SARS-2-ST) entwickelt und zwischen 2017 und 2023 in klinischen Phase-1-Studien am Universitätsklinikum Hamburg-Eppendorf untersucht. Regelmäßige Blutentnahmen in diesen klinischen Phase-1-Studien boten die besondere Möglichkeit, die T-Zell-Immunität im Detail zu untersuchen. Es wurden Blutproben von Studienteilnehmern vor und longitudinal nach Impfung genommen und mit Blutproben von gesunden und immungeschwächten Personen verglichen, die mit zugelassenen Impfstoffen geimpft wurden. Die Stärke, die Funktion und die Antigenspezifität der T Zell-Antwort wurden in verschiedenen Assays untersucht. Die vergleichende COVID-19 Studie ergab eine höhere T-Zell-Immunogenität des Impfstoffkandidaten MVA-SARS-2-ST, der ein präfusionsstabilisiertes SARS-CoV-2-Spike-Protein kodiert, im Vergleich zu MVA-SARS-2-S, der das native Spike-Protein kodiert. Während beide Impfstoffkandidaten bei SARS CoV-2-naïven Individuen im Vergleich zu den zugelassenen Impfstoffen weniger immunogen waren, erwiesen sie sich bei der heterologen Impfung als vielversprechend: MVA-SARS-2-S konnte eine polyfunktionelle CD8+-Antwort auslösen, wenn es vor der mRNA-basierten Immunisierung verabreicht wurde. MVA-SARS-2-ST als Auffrischungsimpfung nach einer mRNA-basierten Immunisierung konnte die T-Zell-Antwort bei Personen mit geringer Restimmunität verstärken. Im Gegensatz zu gesunden Personen, die nach der Grundimmunisierung mit zugelassenen COVID-19-Impfstoffen eine robuste T Zell-Antwort entwickelten, zeigten immungeschwächte Patienten mit chronischer lymphatischer Leukämie, Leberzirrhose oder Lebertransplantation eine geringere Antwort. Insbesondere eine dritte Auffrischungsimpfung und ein heterologes Impfschema aus mRNA- und vektorbasierten Impfstoffen, verstärkten die T-Zell-Antwort bei diesen Patienten deutlich. Die Untersuchung des MVA-MERS-S-Impfstoffkandidaten ergab, dass eine niedrigere Impfstoffdosis und ein längeres Zeitintervall zwischen den ersten beiden Impfungen die T-Zell-Immunogenität verbesserte. Darüber hinaus wurde gezeigt, dass CD8+ T-Zellen gegen ein neu identifiziertes immundominantes Epitop einen langlebigen, polyfunktionalen Phänotyp aufweisen. Zusammenfassend verdeutlichen diese Ergebnisse, dass Impfstoffcharakteristika, Impfschema sowie individuelle Faktoren die durch den Impfstoff ausgelöste T-Zell-Antwort beeinflussen können. Diese Ergebnisse liefern wichtige Erkenntnisse für die Entwicklung von effektiveren Immunisierungsstrategien, die den Aufbau eines langlebigen T-Zell-Gedächtnisses als Schutz vor Krankheit zum Ziel haben. Dies ist besonders für immungeschwächte Patienten von Bedeutung
