Inadequate fluid therapy is associated with an increased risk of postoperative complications. Perioperative goal-directed therapy to guide fluid administration can reduce mortality, postoperative complications and length of hospital stay. The fluid status of a patient can be estimated with variables such as pulse pressure variation (PPV) and cardiac output (CO). Capstesia is a mobile application that automatically calculates PPV and CO using pulse wave analysis of a snapshot of an arterial blood pressure waveform displayed on any patient monitor (PWAsnap) without any further advanced hemodynamic monitoring. We conducted a prospective method comparison study to evaluate the measurement performance of PPV and CO calculated by Capstesia (PPVPWAsnap, COPWAsnap) compared to an invasive internally calibrated pulse wave analysis (ProAQT/Pulsioflex-System, PPVPWAint, COPWAint) in adults having major abdominal surgery (operations with laparotomy in general anesthesia). We analyzed 190 paired PPV and CO measurements from 38 patients. The diagnostic accuracy (concordance) of PPVPWAsnap in comparison to PPVPWAint was evaluated according to three predefined PPV categories (< 9%, 9-13%, >13 %), by using Bland-Altman analysis and by calculating Cohen’s kappa coefficient. In addition, we determined the trending ability for COPWAsnap and COPWAint by using Bland-Altman analysis, the percentage error, linear regression and four quadrant plot/concordance rate analysis. The overall diagnostic agreement between PPVPWAsnap and PPVPWAint based on the three predefined PPV categories was 65% with a Cohen’s kappa coefficient of 0.45. The mean (± standard deviation) of the differences between PPVPWAsnap and PPVPWAint was -0.4 ± 3.7% (95%-LOA; -7.6 to 6.7%) and between COPWAsnap and COPWAint 0.6 ± 1.3 L/min (95%-LOA; -1.9 to 3.1 L/min) with a percentage error of 49% and a concordance rate of 45%. In adults having major abdominal surgery, PPVPWAsnap moderately agrees with PPVPWAint. The absolute and trending agreement between COPWAsnap with COPWAint is poor. Technical improvements must be realized before PWAsnap can be used as an alternative to invasive internally calibrated pulse wave analysis

In this thesis an atomistic, single flat particle-particle interface model is applied to investigate the fundamental properties of gold nanoparticle (GNP) assemblies. Using reactive force field molecular dynamics simulations and a tight-binding density functional theory approach, ligand distributions, interparticle distances, and conductance in GNP systems stabilized with various alkanedithiol and alkanemonothiol ligands are examined. The studies are conducted both in the absence of analyte molecules and after their adsorption. Additionally, the work explores the potential of organic radicals as alternative sensing ligands by analyzing their fundamental properties in single-molecule junctions through density functional theory calculations

Diversity and Self-Regulated Learning. An Exploratory Mixed-Methods Study Using Personal Learning Environments to Examine the Relationship between Student Teachers’ Diversity Experiences and Their Individual Self-Regulated Learning

    No full text
Self-regulatory competencies are of central importance for learning and study success (Perels et al., 2020). Cognitive and metacognitive learning strategies as well as strategies for regulating the self are components of self-regulated learning (Boekaerts, 1999) and can be learned in social contacts through observation and adaptation (Bandura, 1991; Dignath et al., 2008; Killus, 2018; Zimmerman, 2000). Especially among (university) students, the successful application of learning strategies is linked to learning success (Pelikan et al., 2022; Schneider & Preckel, 2017). While learning together with other learners, demographic, cognitive and subject-specific diversity emerges (Gaisch & Linde, 2020). This diversity can be experienced by the students (N. Bittner & Fiebig, 2018) and is generally understood as potential in the field of higher education (Hummrich, 2017; Linde & Auferkorte-Michaelis, 2021; Siegfried, 2019). Since there do not seem to be any studies available to date that address the potential of students' diversity experiences in connection with their individual self-regulation, the research interest of this dissertation is directed towards such connections and uncovering them in the specific case of student teachers. The pedagogical concept of the Personal Learning Environment (PLE), which describes an individual orchestration of learning elements by learners themselves (Fiedler & Väljataga, 2011), offers a theoretical and empirical observation framework for possible connections between demographic, cognitive and subject-specific diversity experiences of student teachers and aspects of their individual self-regulation when learning. The student teachers who participated in this study were guided to outline their PLEs, which reflect both facets of diversity and self-regulation in learning. Based on their outlined PLEs, episodic individual interviews were conducted with the participants, which constitutes the central instrument of this study. In addition, the participants answered the short scale of the questionnaire on learning strategies during studies (LIST-K) as well as a questionnaire to survey demographic and subject-specific diversity dimensions. In the mixed methods design resulting from the combination of the instruments, the results of the qualitative and quantitative instruments were integrated and condensed using an explicative content analysis of the interviews in the manner of Mayring (2022). The exploratory approach yields findings that can be interpreted in the direction of various connections between diversity experiences of the student teachers and their individual self-regulation when learning. For example, participants perceive diversity dimensions of cognitive diversity, such as ways of thinking or learning approaches, as a potential to make greater use of deep-level cognitive strategies such as elaboration or to expand their organizational strategies. In addition to cognitive diversity, connections between age diversity and the regulation of the self can also be assumed, since, for example, older students show a higher level of conscientiousness, which seems to be reflected in the management of resources such as effort and can be observed by younger students. In addition, subject-diversity plays a special role in teacher training, because different subjects prompt students to adopt different learning strategies. The participants describe conditions of joint learning such as openness, trust or reciprocity, without which students are less likely to learn together and consequently have fewer experiences of diversity. At the same time, student teachers have low-threshold opportunities to actively integrate other learners into their PLEs and thus enable themselves to experience diversity. Overall, this thesis taps into various diversity potentials to promote self-regulated learning, which can be experienced by student teachers in the context of their personal learning environments and from which the students can benefit. In higher education and especially in teacher training, corresponding experiences of diversity could be made accessible by means of the didactic and methodological design of courses. To this end, the study provides several starting points for further investigation of possible connections between diversity and self-regulation

Unnecessary operations and overcare - frequency of indications and implementation of surgical and interventional procedures

Unnecessary surgeries and overtreatment remain recurring issues in health research and public discourse. Various studies and analyses suggest that surgery rates in Germany are unusually high, both internationally and within the country. The factors at play are not solely medical needs; rather, findings indicate that structural and economic factors also influence treatment practices. These trends raise questions regarding patient safety and healthcare efficiency, underscoring the importance of evidence-based, indication-appropriate care planning. Objective: The aim of this study was to investigate how frequently surgical indications for certain procedures are uncertain or disputed. Additionally, it sought to examine the epidemiological variation in procedure frequency and assess which structural and economic factors might contribute to these differences.

"I'd like to lead a life in great honesty." Consensual non-monogamy and subjectification

In her doctoral thesis, the sociologist Gesa Mayer researches subjectification in the context of consensual non-monogamy and mononormativity. For this purpose, she analyses narrative interviews with people who live in polyamorous and open relationships. The theoretical background is set up by Michel Foucault’s theory of discourse, power, and subjectification in combination with additional elements of poststructuralist thought. The study proceeds according to Grounded Theory Methodology. By means of an Interpretative Subjectification Analysis, the author maps out discursive subject positions as well as self-positionings. Part one and two of the analysis show two different positionings regarding the question of how the subject gets at non-monogamy (or how non-monogamy gets at the subject): firstly, localisations within a subject position of identity and, secondly, narrations of becoming. The former construct non-monogamy as a biographical constant and an expression of the subject's inner truth. The latter in turn interpret non-monogamy as an unexpected event and non-monogamous subjectification as a discontinuous process which may require some work upon the self. The third and fourth part of the analysis examine consensual non-monogamy as a moral code and an ethic. The moral code places high demands on transparency and consent. It thereby incites positionings as a subject whose integrity is measured by its (striving for) honesty. At the same time, hierarchical subject positions, structural inequality, and power relations within the relationships do undermine the ideal of negotiation amongst equals. Constructions of consensual non-monogamy as a practice of freedom and an aesthetic of existence allow their subjects to position themselves as virtuous and non-normative. As a technology of the self, non-monogamy is supposed to enable modifications of subjectivity towards gaining (even more) competencies in self and relationship conduct. The conclusion discusses the results in relation to Foucault's theory of subjectification and to sociological diagnoses of the present such as the stated hegemony of a singularistic lifestyle. As an outlook, some questions and perspectives for further research are being sketched

Overcoming challenges in time reversal for passive seismic source localization

Passive seismic source localization plays a crucial role in understanding geodynamics, monitoring geological activities, forecasting geological hazards, and managing geological fracturing processes. This doctoral thesis presents three consecutive approaches to enhance the robustness and accuracy of time-reversal source localization. The proposed methods address challenges such as source imaging artifacts, low-resolution source images, sparse and small-aperture seismic data acquisitions, and unknown seismic velocity models. In the first approach, the focus is on refining time-reversal imaging. Conventional time-reversal source imaging methods like autocorrelation imaging or grouped crosscorrelation imaging often suffer from source imaging artifacts due to the use of low-quality seismic data or less constrained velocity models. These artifacts typically degrade the quality of the image and can lead to subsequent misinterpretation, resulting in false source location estimation. To overcome this, the Gaussian-weighted crosscorrelation imaging condition is proposed. Each time step in this method includes dividing the back-propagated wavefield, weighting seismic amplitudes using Gaussian functions, and using a zero-lag crosscorrelation. This process effectively minimizes source imaging artifacts, resulting in high-resolution, low-noise source images. Numerical examples of complex models and field examples illustrate the method’s performance, demonstrating its effectiveness in identifying sources, even within clusters and under conditions of noisy and sparse-sampled data. The second approach addresses another challenge posed by sparse and small-aperture seismic data acquisition in time-reversal imaging techniques. Such acquisitions often lead to false wave focusing due to insufficient wave illumination. To address the issues, the maximum-amplitude path method is introduced for source localization using maximum-amplitude paths. The paths are constructed from back-projected wavefields using receiver patches selected from the acquisition. They include the maximum amplitudes of the back-projected wavefronts for each considered time step. The proposed method exploits the continuity of the maximum amplitudes of the back-projected wavefields. The point of closest proximity (or crossing point) of the paths is the source location and the corresponding time is the source time. The maximum-amplitude path method successfully overcomes the acquisition problems and provides accurate source location and source excitation time even in challenging scenarios. In the final approach, a data-driven hybrid workflow is presented to address the challenge of the lack of velocity model in time-reversal localization methods including the proposed first and second time reversal localization approaches. The proposed workflow can simultaneously invert the source location, excitation times, and velocity model using wavefront attributes of passive seismic data. By combining wavefront tomography and time-reversal methods, the workflow eliminates the need for detailed prior information, making it particularly applicable in practical scenarios. The proposed workflow comprises the following steps. First, a set of user-defined vertical gradient velocity models is designed. Time reversal is then used to estimate the source excitation times for each model. After that, these source times and gradient models are used in wavefront tomography. The second step uses an optimization procedure to refine the velocity model and the source excitation time. Each iteration of the optimization involves a sequential application of time reversal and wavefront tomography. In the final step, the source location is refined using the optimal velocity model and the Gaussian-weighted crosscorrelation imaging condition. The proposed workflow overcomes the limitations of time reversal in the absence of a velocity model, providing good velocity models and fairly accurate source locations and excitation times

Since the first phenomenological descriptions of psychosis, severe alterations in sensory processing and perception have been assumed as one key factor at the root of the disorder. Studies on sensory processing in psychosis have repeatedly observed impairments in information processing within individual sensory systems such as vision or audition. It has been discussed that psychosis might be associated with pronounced deficits in multisensory processing, i.e. the processing of stimuli conveyed via multiple senses. Dysfunctions in multisensory integration (MSI) might lead to fragmented or distorted perception and facilitate the development of psychotic disorders via maladaptive perceptual learning mechanisms. Previous studies on MSI in psychosis have observed deficits such as reduced behavioral benefits during processing of congruent crossmodal stimuli or inappropriate integration. Impairments have repeatedly been observed in temporal, sensorimotor, and linguistic processes. However, findings in emotional MSI have been inconsistent, with reports of deficient, intact, and excessive integration. Only a few earlier studies suggested intact spatial MSI in psychosis and their findings require replication. No published study investigated if crossmodal recalibration, i.e. the short- and long-term adaptation to changes in crossmodal characteristics, is altered and as such might contribute to altered perception in psychosis. Further, little is known about the developmental trajectory of multisensory processes in psychosis and no convincing mechanism has been proposed, which describes how impaired multisensory processing might contribute to psychosis. To address these gaps in the literature, this dissertation project aimed to investigate different aspects of multisensory processes in psychosis. We examined emotional MSI in psychosis proneness as well as MSI and crossmodal recalibration in the temporal and spatial domains in patients with the diagnosis of a psychotic disorder. First, we addressed the question, if impaired MSI might reflect a global underlying deficit or might be rather specific for some domains such as temporal and social information processing. Second, we examined crossmodal spatial and temporal recalibration in order to investigate if both processes might be altered in psychosis. Third, we aimed to contribute to the question if altered MSI might be a consequence of disorder manifestation or if it might play a role in the development of psychosis. Finally, we addressed the issue of a lack of disorder model-based research and aimed to provide initial evidence for a potential mechanism, via which altered multisensory processing might contribute to psychosis. To investigate these questions, we conducted three different experimental studies. In the first study, we investigated emotional MSI in psychosis proneness by means of an emotion categorization task with affective facial expressions and vocal prosody, expecting impaired emotional MSI in subjects with high psychosis proneness. In the second study, we investigated temporal MSI and crossmodal temporal recalibration in psychosis using an audiovisual (AV) simultaneity judgement (SJ) paradigm. We expected to replicate previous findings on impaired temporal MSI and provide first evidence for impaired crossmodal temporal recalibration in psychosis. In the third study, we conducted a paradigm on the spatial ventriloquist effect and aftereffect to investigate spatial MSI and crossmodal spatial recalibration in psychosis. We expected to replicate previous findings on intact spatial MSI in psychosis and provide first results on crossmodal spatial recalibration in psychosis. Results showed that high and low proneness subjects did not differ in their emotional categorization performance in unimodal, bimodal emotionally congruent and bimodal emotionally incongruent stimuli. This indicates typical emotional MSI in psychosis proneness. Further, patients with the diagnosis of a psychotic disorder and healthy controls showed comparable performance in judging the synchrony of AV stimuli and similarly adjusted their responses to changes in stimulus asynchrony. This indicates typical temporal MSI and crossmodal temporal recalibration in psychosis. Finally, patients and controls showed similar ventriloquist effects and aftereffects, suggesting intact spatial MSI and crossmodal spatial recalibration in psychosis. In all studies, no correlation between measures of multisensory processes and psychotic symptoms could be observed. While our findings on emotional MSI in psychosis proneness and temporal MSI in psychosis are in contrast with previous findings in patient samples, we successfully replicated earlier findings on intact spatial MSI. The lack of group differences in all three of our studies neither supports the hypothesis of a global underlying impairment nor of a domain-specificity of multisensory processing dysfunctions in psychosis. Further, our results indicate intact crossmodal recalibration in the spatial and temporal domain, suggesting that patients with the diagnosis of a psychotic disorder are able to adapt to changes in crossmodal spatial and temporal characteristics. Moreover, our findings do not offer support for the assumption that deficient emotional MSI plays a role in the development of psychosis. Taken together, our findings could suggest that altered perception in psychosis might not be generally driven by deficits in multisensory processes
