Healthcare service accessibility path planner: Unveiling a new era of intelligent appointment management systems based on outpatient prioritizing
In light of increased constraints on healthcare systems, particularly as a result of the pandemic, the importance of directing patients to the appropriate healthcare departments for individualized treatment based on their health conditions has been emphasized. Numerous healthcare institutions currently employ an online booking system that enables patients to schedule appointments. However, because patient requests are the main driving force behind this process, appointments with inappropriate departments or the bypassing of primary care facilities like general practice clinics frequently occur. Many studies proposed the use of AI-based chatbots and machine learning algorithms in healthcare systems to improve clinic operations, reduce patient wait times, and predict outpatient appointment no-show rates. This paper describes the conception and implementation steps of an innovative (mhealth app) that uses open AI tools to prioritize and classify outpatients based on their symptoms. Our AI-based appointment scheduling app will decide for the outpatient either to schedule appointments with primary care facilities or direct them to the appropriate healthcare department in hospitals only when absolutely necessary, thereby nurturing a more efficient, patient-centered healthcare service
Effect of Aza-BODIPY-photodynamic therapy on the expression of carcinoma-associated genes and cell death mode
Background: Breast cancer is the most common cancer affecting women worldwide.Photodynamic therapy(PDT) has now proven to be a promising form of cancer therapy due to its targeted and low cytotoxicity to healthy cells and tissues.PDT is a technique used to create cell death localized by light after application of a light-sensitive agent.Aza-BODIPY is a promising photosensitizer for use in PDT. Our results showed that aza-BODIPY-PDT induced apoptosis, probably through p53 and caspase3 in MCF-7 cells. Future studies should delineate the molecular mechanisms underlying aza-BODIPY-PDT-induced cell death for a better understanding of the signaling pathways modulated by the therapy so that this novel technology could be implemented in the clinic for treating breast cancer. Aim: In this study,we aimed to determine the change in the expression levels of 88 carcinoma-associated genes induced by aza-BODIPY-PDT were analyzed so as to understand the specific pathways that are modulated by aza-BODIPY-PDT. Material method: In this study,the molecular basis of the anti-cancer activity of aza-BODIPY-PDT was investigated.Induction of apoptosis and necrosis in MCF-7 breast cancer cells after treatment with aza- BODIPY derivative with phthalonitrile substituents (aza-BODIPY) followed by light exposure was evaluated by Annexin V 7- Aminoactinomycin D (7-AAD) flow cytometry. Results: Aza-BODIPY-PDT induced cell death in MCF-7 cells treated with aza-BODIPY-PDT; flow cytometry revealed that 28% of the cells died by apoptosis. Seven of the 88 carcinoma-associated genes that were assayed were differentially expressed -EGF, LEF1, WNT1, TCF7, and TGFBR2 were downregulated, and CASP3 and TP53 were upregulated - in cells subjected to aza-BODIPY-PDT.This made us think that the aza-BODIPY-PDT induced caspase 3 and p53-mediated apoptosis in MCF7 cells. Conclusion: In our study,it was determined that the application of aza-BODIPY-PDT to MCF7 cells had a negative effect on cell connectivity and cell cycle.The fact that the same effect was not observed in control cells and MCF7 cells in the dark field of aza-BODIPY indicates that aza-BODIPY has a strong phodynamic anticancer effect

Combination of St. John’s wort oil and neem oil in pharmaceuticals: An effective treatment option for pressure ulcers in intensive care units
Background and Objectives: Phytotherapeutically, various herbal remedies, such as St. John’s wort oil, have been introduced as wound care options. Recently, Neem oil has been considered a herbal option for the management of superficial wounds. Wound care is a complex process that involves several factors including the patient, caregiver, and medications. Herbal combinations could be an alternative to the chemical counterparts in the wound care area. This report includes an investigation of the possible supportive impacts of the St. John’s wort and Neem oil containing ointment (W Cura G Plus ®) in the management of pressure ulcers (PUs) in three intensive care unit (ICU) patients. Materials and Methods: The ointment was administered to individuals once daily for 42 consecutive days. The status of individuals was macroscopically monitored by measuring the PU area and histopathological assessment of the tissue sections taken on the first and last days of wound treatment. Results: The outcomes of the macroscopic and histopathological techniques exhibited that St. John’s wort and Neem oil containing ointment provided a remarkable supportive impact on the patients that suffered from PUs in the ICUs. Conclusions: The combination of St. John’s wort and Neem oil could be suggested as an efficient active phytoconstituent for the management of PUs. The herbal ointments may be suggested as an alternative for the patients that have PUs in the ICUs

A global study on the abortion views and knowledge of trainee obstetrician-gynecologists
Objective: To assess the knowledge and views of trainee obstetrician-gynecologists (ObGyn) on abortion. Methods: A cross-sectional study of trainee ObGyn was conducted by the World Association of Trainees in Obstetrics and Gynecology. A study-specific questionnaire designed using Google Forms® was utilized for the study. The questionnaire was distributed electronically to study participants, to gather information on their sociodemographic characteristics, opinions, knowledge, and training on abortion. Collected data were analyzed using the IBM SPSS, version 25. Results: Most (140, 74.8%) trainee ObGyn reported that abortion was legal in their countries and most (171, 91.4%) supported the legalization of abortion. Eleven (5.9%) trainees who did not support the legalization of abortion cited religious concerns as their most common reason (5/11, 45.5%). Nearly three-quarters (133, 71.1%) of the trainees would perform an abortion on request. Most of the trainees reported that abortion education was included in the medical school curricula (131, 70.1%) and the ObGyn residency training programs (155, 82.9%) in their countries. Notwithstanding, 36.4% (68) either did not know or did not correctly know the legal status of abortion in their countries. Conclusion: There is some disparity between ObGyn trainees' knowledge of safe abortion and their awareness of the legality of the same in their countries. There is a need for all countries to include abortion education in the curricula of medical schools and all ObGyn residency programs should offer abortion training to all residents. There is a need for increased advocacy for the legalization of abortion in countries where abortion remains criminalized
Microbiota and childhood urinary tract infections
İdrar yolunun mikrobiyomu, mesane üzerindeki etkisiyle sağlığın korunmasında önemli bir rol oynar. Ürobiyom, ürotelyal bütünlüğün korunmasında ve idrar yolu enfeksiyonunun (İYE) önlenmesinde ve ayrıca lokal bağışıklık fonksiyonunun desteklenmesinde büyük önem taşır. Bu bölgedeki disbiyoz, artmış İYE riski, nefrolityazis ve alt üriner sistem disfonksiyonu ile ilişkilendirilmiştir. Çocuklarda İYE’ye sebep olan üropatojenler, hedeflenen terapötik yaklaşımlara izin vermek için kesin olarak tanımlanmalıdır. Bu aynı zamanda, yaşam boyu disbiyozla sonuçlanabilecek mikrobiyom homeostazının yok edilmesini önlemeye de yardımcı olabilir. Bakterilerin artan intrinsik antibiyotik direnci nedeniyle yeni tedavi yaklaşımları ve probiyotiklerle yeniden kolonizasyon gereklidir.The microbiome of the urinary tract plays an important role in maintaining health through its effect on the bladder. The urobiome is of great importance in maintaining urothelial integrity and preventing urinary tract infection (UTI), as well as supporting local immune function. Dysbiosis in this region has been associated with an increased risk of UTI, nephrolithiasis, and lower urinary tract dysfunction. The uropathogens that cause UTI in children must be precisely identified to allow for targeted therapeutic approaches. This may also help prevent the destruction of microbiome homeostasis, which can result in lifelong dysbiosis. New treatment approaches and recolonization with probiotics are required due to the increasing intrinsic antibiotic resistance of bacteria
Yılansı robotun tasarımı, statik, dinamik ve modal analizi
Teknolojinin ilerlemesiyle robotların hayatımıza girmesi ciddi bir şekilde hızlanmıştır. Neredeyse her alanda bu insan yapımı akıllı makinelerden istifade ediyoruz. Arama kurtarma çalışmalarından sanayiye kadar her alanda kullandığımız bu robotlar üzerinde, çalışabilecekleri ortamların zorlu şartlarına fiziki olarak nasıl tepki vereceklerini öğrenmek için gerçek imalattan önce belli analizlerin yapılması önem arz etmektedir. Bu çalışmada yılansı bir robot üzerine çalışılmış, yılansı robotun tasarımı gerçekleştirilmiş ve yılansı robot üretilmeden önce belli analizlere tabi tutulmuştur. Yılansı robot 7 adet eklemden oluşup her eklemi hareketlidir. Sistem tasarımı SolidWorks paket programında gerçekleştirilip sistemin montajı yapılmıştır. Oluşturulan tasarımın fiziksel özellikleri ve parametreleri kullanılarak Matlab ortamında, DenavitHertenberg parametrelerinden yararlanılarak yılansı robotun ileri kinematik sonuçları bulunmuştur. Daha sonra Ansys paket programında üç adet analize tabi tutulmuştur. Bu analizler statik analiz, doğal frekans analizi ve rijit dinamik analizidir. Dar ve pürüzlü yollarda, arama kurtarmada, araştırmalarda kullanılan bu robotların sınır şartları belirlenerek hareketlerinin analiz edilmesi amaçlanmıştır. Bunun sonucunda gerekli parametrelerle analiz gerçekleşmiş ve maksimum çalışma şartları belirlenmiştir
Evaluation of shaping ability of different glide path instruments: A micro-computed tomography study
Background: This study aimed to compare the shaping ability of different instruments, TruNatomy Glider (TRN-G), WaveOne Gold Glider (WOG-G), and ProGlider (Pro-G) using micro-computed tomography (micro-CT). Methods: The mesial canals of 27 mandibular molars with two separate mesial canals and moderate curvature were included in this study [n = 27 mesiobuccal (MB) and mesiolingual (ML) root canal]. According to the manufacturer's instructions, the glide path was created with TRN-G, WOG-G, and Pro-G glide path instruments (n = 9 MB and ML root canal in each group). Micro-CT scanning was performed before and after preparation. Mesiodistal (MD) and buccolingual (BL) transportation and the centering ratio were measured at three levels within the canal (3, 5 and 7 mm). A three-way robust ANOVA was used to compare the parameters. Results: TRN-G showed significantly greater transportation in the MD direction than the other instruments throughout the root canal (overall root canal) (p 0.05). Whether the root canal was MB or ML did not affect centering or transportation (p > 0.05). Conclusions: Glide path instruments can be used to shape moderately curved canals with minimal apical transportation and better centering ability. Experiences lived by international nurses concerning professional integration and interpersonal relationships in the workplace: qualitative research
Purpose – The recruitment of international nurses has been used for a long time to address the global nurse shortage. In 2012, the employment of international nurses was released in Turkey. Cultural differences can hinder interpersonal relationships, and fostering strong interpersonal relationships among nurses in the workplace is known to have a positive impact on productivity, job satisfaction and the quality of care provided. This study aims to explore the lived experiences and perceptions of international nurses working in Turkey regarding their professional integration and interpersonal relationships in the workplace. Design/methodology/approach – It is a qualitative descriptive study. Face-to-face interviews were conducted with 19 international nurses using a semi-structured form. Data were analyzed using thematic analysis. Findings – Data were presented into two themes: (1) Interpersonal relationships in the workplace and (2) Professional integration. Participants reported positive and negative experiences regarding interpersonal relationships in the work environment, such as warm and supportive approaches, or exclusionary and discriminatory behaviors. The existence of different nursing practices, positive and negative factors concerning orientation programs and the gain of experience and professional satisfaction were mentioned by the participants. Originality/value – This is the first study to explore the lived experiences and perceptions of international nurses working in Turkey. The study highlights the unique needs and challenges faced by international nurses during workplace adaptation and provides practical recommendations to facilitate interpersonal relationships in the workplace and other aspects related to professional integration from hiring to the end of the adaptation period
