Travmaya sistemik aile terapisi yaklaşımı
Roy-Chowdhury (1997), sistemik terapinin ortaya çıkışına dair ilk önemli farklılığın 1950’lerde değerlendirmede, psikiyatrik tedavide ve psikoterapötik uygulamadaki psikanalize dayalı, bireyin psikopatolojisinin odak noktası olarak alındığı modellerin yerine sistemik aile terapisi alanının ilk öncülerinin, söz konusu odağı bireyden aileye kaydırarak hâkim yoldan ayrılmak sureti ile ortaya çıktığına değinmektedir
Investigating the dynamics of droplet spreading on a solid surface using PIV for a wide range of Weber numbers
The investigation of the underlying physical processes involved in the impact of droplets has various practical applications in engineering and science. In this research, the spreading velocities within droplet impingement on a sapphire glass were investigated for a wide range of Weber numbers using particle image velocimetry (PIV), which involves tracking the movement of polymeric fluorescent particles (6 µm) within the droplet. The experiments were carried out at room temperature, and the droplets had impact velocities ranging from 0.41 to 2.37 m/s, which corresponded to Weber numbers of 5–183. The results showed that the radial velocity was generally linear over a wide range of spreading radius but the velocity at the exterior radial positions became nonlinear over time due to the influence of capillary and viscous forces. This nonlinearity was more pronounced for lower Weber numbers because the viscosity effects in the droplet were more significant compared to the inertia forces. As the Weber number decreases, the spreading and receding of the droplets are completed faster, leading to different trends in the radial velocity profiles
Testing the performance of feature selection methods for customer churn analysis: Case study in B2B business
Churn analysis has recently become one of the favorite topics of marketing teams with the development of machine learning models. This study aims to discover the most suitable feature selection (FS) model for churn analysis by using the databases of BiletBank, a business-to-business (B2B) company. It was found that some categorical data such as agency type and currency used by customers, along with periodic flight sales data, are also meaningful features for churn analysis in the BiletBank customer portfolio. This feature selection study in the database will be a source for future churn analysis studies
Multicenter validation of FIB-6 as a novel machine learning non-invasive score to rule out liver cirrhosis in biopsy-proven MAFLD
Background and aims: We previously developed and validated a non-invasive diagnostic index based on routine laboratory parameters for predicting the stage of hepatic fibrosis in patients with chronic hepatitis C (CHC) called FIB-6 through machine learning with random forests algorithm using retrospective data of 7238 biopsy-proven CHC patients. Our aim is to validate this novel score in patients with metabolic dysfunction-associated fatty liver disease (MAFLD). Method: Performance of the new score was externally validated in cohorts from one site in Egypt (n = 674) and in 5 different countries (n = 1798) in Iran, KSA, Greece, Turkey and Oman. Experienced pathologists using METAVIR scoring system scored the biopsy samples. Results were compared with FIB-4, APRI, and AAR. Results: A total of 2472 and their liver biopsy results were included, using the optimal cutoffs of FIB-6 indicated a reliable performance in diagnosing cirrhosis, severe fibrosis, and significant fibrosis with sensitivity = 70.5%, specificity = 62.9%. PPV = 15.0% and NPV = 95.8% for diagnosis of cirrhosis. For diagnosis of severe fibrosis (F3 and F4), the results were 86.5%, 24.0%, 15.1% and 91.9% respectively, while for diagnosis of significant fibrosis (F2, F3 and F4), the results were 87.0%, 16.4%, 24.8% and 80.0%). Comparing the results of FIB-6 rule-out cutoffs with those of FIB-4, APRI, and AAR, FIB-6 had the highest sensitivity and NPV (97.0% and 94.7%), as compared to FIB-4 (71.6% and 94.7%), APRI (36.4% and 90.7%), and AAR (61.2% and 90.9%). Conclusion: FIB-6 score is an accurate, simple, NIT for ruling out advanced fibrosis and liver cirrhosis in patients with MAFLD
Nanoemulsions: New approaches in cancer therapy with herbal terpenes and essential oils
Herbal remedies have become even more popular in recent years, as they are of natural origin and their therapeutic effects have been reported by scientific researches. Herbal origin substances and products can be preferred by many health authorities as an alternative to synthetic or semi-synthetic molecules in treatment of a lot of diseases. Terpenes, a type of natural bioactives, and essential oils rich in terpenes are valuable compounds and products especially in pharmaceutical industry. Nanoemulsions have been used for years in treatment. While conventional emulsions are suitable for topical and extraparenteral applications, nanoemulsions allow parenteral application of active substances thanks to their droplet size at nanometer level. These sophisticated drug delivery systems attract increasing attention of scientists and pharmaceutical companies due to their various advantages over conventional dosage forms. Nanoemulsions were first introduced to the pharmaceutical market as parenteral nutrition products. Intralipid® (intravenous fat emulsion for infusion) is the first nanoemulsion approved for parenteral nutrition in Europe. Then, research groups focused on drug incorporation into nanoemulsions and their benefits in treatment of various diseases. High payload of lipophilic substances and materials, stability during pharmaceutical processes (such as autoclaving, lyophilization, sterile filtration) and storage, intoxicity and biocompatibility, and effective therapy by targeting and/or modifying drug delivery have been reported as outstanding advantages of nanoemulsions. Pharmaceutical application of herbal remedies and extracts is limited due to their low solubility and permeability. Nanotechnology could offer smart solutions for these drawbacks by using drug carrier systems in nanometer size range. Nanoemulsions are reported as alternative delivery systems for herbal remedies and extracts. Moreover, droplet size of the nanoemulsions provides effective absorption for low permeable herbal bioactives. Basic principles of lipid nanotechnology and targeting strategies of nanoemulsions for delivery of lipophilic substances and materials for the treatment of cancer are mentioned in this chapter. Increasing therapeutic potential of plantderived terpenes and essential oils in cancer treatment by preparing their nanoemulsions is presented by evaluating the data collected from scientific studies
T-cut slotted quartz tube-atom trap strategy for the on-line preconcentration of thallium in well water samples
In this study, an on-line preconcentration system was developed for the quantitative determination of thallium in well water samples using hydrogen-assisted T-cut slotted quartz tube atom trapping (HA-T-SQT-AT) coupled to flame atomic absorption spectrometry (FAAS). The method is based on trapping the thallium atoms on the inner surface of the T-SQT for a short period using sample aspiration and lean flame conditions. Then, hydrogen gas as a reducing agent was released instantly to free the trapped analyte atoms, resulting in a sharp and narrow signal. The system analytical performance was enhanced by performing comprehensive optimization of several factors. The linear range and LOD/LOQ values of the optimized method were found to be 7.5–––200 ng/mL and 2.6/8.7 ng/mL, respectively. Accuracy/practicability of the presented method were verified using well water samples, and the percent recoveries calculated using the external calibration approach were all above 80%
Modeling the health economic burden of hepatitis C virus infection in Turkey: Cost-effectiveness of targeted screening
Background/Aims: In 2016, World Health Organization introduced global goals to eliminate hepatitis C virus by 2030. The aim of this study is to analyze the epidemiologic and economic burden of hepatitis C virus in Turkey and compare current practice (regular care) with a hypothetical active screening and treatment approach (active scenario). Materials and Methods: A Markov model was used to analyze and compare regular care with a scenario developed by experts including the screening and treatment of all acute and chronic hepatitis C virus infections between 2020 and 2050. General and targeted populations were focused. The model reflected the natural history of the disease, and the inputs were based on a literature review and expert opinions. Costs were provided by previous studies and national regulations. Results: The active scenario resulted in higher spending for all groups compared with regular care in the first year. Cumulative costs were equalized in the 8th, 12th, 13th, and 16th year and followed by cost-savings of 49.7 million, 1.1 billion, 288.6 million, and 883.4 million Turkish liras in 20 years for prisoners, refugees, people who inject drugs (PWID), and all population, respectively. In all groups, the mortality was found to be lower with the active scenario. In total, 62.8% and 50.6% of expected deaths with regular care in 5 and 20 years, respectively, were prevented with the active scenario. Conclusions: An active screening and treatment approach for hepatitis C virus infection could be cost-effective for PWID, prisoners, and refugees. Almost two-thirds of deaths in regular care could be prevented in 5 years’ time with this approach
