    Safety and effectiveness of controlled ovarian stimulation and oocyte retrieval during prepubertal and peripubertal period

    Purpose Is it safe and efective to perform controlled ovarian stimulation (COS) and oocyte retrieval (OR) in prepubertal and peripubertal patients? Methods In this retrospective cohort study, data of 20 pre-/peripubertal patients who underwent COS and OR for the purpose of oocyte cryopreservation (OC) between 2008 and 2023 were reviewed. Following COS, all OR procedures were performed transabdominally using a vaginal ultrasound probe. Ovarian reserve was assessed by serum FSH, LH, estradiol, AMH, and antral follicle counts (AFC) in all subjects. All mature oocytes were vitrifed. Results Mean age of the patients was 15.05±1.87, mean AMH was 0.84±0.8 ng/ml, mean FSH was 6.39±3.95 IU/L, mean estradiol was 61.6±51.9 pg/ml, mean LH was 4.69±3.46 IU/L, and mean AFC was 5.5±5.82. Among the patients, 12 had regular menstrual cycle, 5 had irregular menstrual cycle, whereas 3 patients still did not have their menarche yet. The indi cations for OC were as follows: primary ovarian insufciency (n=7), ovarian surgery for ovarian tumors (n=5) or ovarian torsion (n=1), mosaic Turner syndrome (n=2), acute lymphoblastic leukemia (n=1) anaplastic B-cell lymphoma (n=1), Ewing’s sarcoma (n=1), Noonan syndrome (n=1), and Thalassemia (n=1). The mean number of oocytes retrieved, MII oocytes frozen, and maturation rate were 5.11±5.0, 3.92±4.48, and 75.1±25.6%, respectively. Stepwise linear regression analysis demonstrated a positive correlation between AFC and number of total oocytes retrieved and number of MII oocytes. In the case diagnosed with Noonan syndrome, all 7 retrieved oocytes were MI and all frozen at MI phase. No patient had any complication related to COS or OR. Conclusion Even though number of the enrolled subjects is limited and mean AMH is lower in our cohort, we demonstrated that performing COS and OR is safe in pre-/peripubertal patients. If required, transabdominal route can be performed in this age group for OR. AFC appears as a prognostic factor for stimulation outcome in this age group. Pediatric patients or young adolescents at risk for primary ovarian insufciency should not be discouraged from utilizing OC

    Morphometric and morphological evaluation of the nasolacrimal groove in 150 dry bones in the Anatolian population

    Pupose In this study, we aimed to evaluate the anatomical features of the nasolacrimal groove in detail by providing a mor phological classifcation based on morphometric evaluations of the nasolacrimal groove. Methods A total of 150 sagittal dry bones in the Department of Anatomy, Faculty of Medicine, Istanbul University were evaluated. The length and the width at diferent points of the nasolacrimal canal were calculated. According to the widths of the nasolacrimal canal ten diferent morphological types were revealed. Results The length of the canal was found as mean 13.62±2.42 mm on the right and 12.44±2.68 mm on the left side. The entrance, the base, the upper and the lower thirds of nasolacrimal canal were 6.22±1.19 mm, 7.95±1.85 mm, 5.85±1.06 mm, 6.60±1.54 mm, on the right and 6.08±1.16 mm, 7.24±1.64 mm, 5.45±1.29 mm, 6.23±1.48 mm, on the left side, respectively. The width of the entrance of the nasolacrimal canal was the narrowest width compared to the base, upper and lower thirds in 7/10 types of 71/150 cranial bones. Conclusion This comprehensive morphological classifcation of the nasolacrimal groove sheds new light on its complex variations. We support that the fnding of this study has the potential to improve the precision of diagnostic assessments and guide specifc therapeutic interventions for patients with lacrimal drainage disorders

    Ultra fast liquid chromatographic analysis of nonsteroidal anti-inflammatory drugs with fluorimetric detection in tap water, urine, and pharmaceutical samples

    A novel analytical method based on ultra-fast liquid chromatography using fluorimetric detector was developed and validated for determination of non-steroidal anti-inflammatory drugs (NSAIDs) (ibuprofen (IBP), etodolac (ETD), dexketoprofen (DKP), sodium diclofenac (SDCF), and naproxen (NPX) in tap water, urine and pharmaceutical samples. Precolumn derivatisation of targeted NSAIDs was carried out with 4-bromomethyl-7-methoxy coumarin (BrMmC) using dibenzo-18-crown-6-ether as reaction catalyst leading to the formation of a fluorescent compound. The obtained fluorescent compound of NSAIDs were measured at excitation wavelength as 325 nm, and emission wavelength of 395 nm. Optimum analytical conditions were carefully studied and improved. C18 column, with the dimensions of 4.0 × 100 mm and 3 µm particle size, was used. Gradient elution with methanol: water 40:60; v/v (eluent A) and acetonitrile 100% (eluent B) were used as mobile phase and flow rate of 0.4 mL/min. The linearity range of the analytes were between 0.01–10.0 µg mL−1. Recovery values obtained from pharmaceutical preparations were found as 100.04%, 99.99%, 100.09%, 99.98% and 100.47% for IBP, ETO, DKP, SDCF, NPX, respectively. LOD values were found to vary between 0.00009 µg mL−1 and 0.00048 µg mL−1 in tap water, urine and pharmaceutical samples. The optimised technique was successfully applied for the determination of NSAIDs in tap water, urine, and pharmaceutical specimen. The specified NSAIDs were not found in real tap water samples

    Objectives: Human estrogen receptor alpha (ERα) is considered an important target, especially in the treatment of breast cancer, as it has a vital role in cancer development. ERα-targeted therapies generally target the ligand binding domain (LBD) of ERα. However, over time, cells develop resistance to this mechanism alternative approaches to inhibit ERα activity target ERα–DNA or ERα–cofactor in teractions. Inhibitors of ERα–cofactor interactions are designed by targeting the hydrophobic hollow region of the receptor box LXXLL motif. Methods: In this context, helix-stabilized cyclic peptides (SPs) designed with in silico approaches were obtained by solid phase peptide synthesis. The effects of SPs on MCF-7 cells were examined with MTT and ATP, and qPCR and flow cytometry were used for further analysis. Results: Our results demonstrated that the SPs were effec tive only in MCF-7 cells expressing ERα. In addition, cyclic peptide combinations (SPCs) showed anti-proliferative and toxic effects on MCF-7 cells. The impact of SPCs with the highest inhibitory effect in MCF-7 cells on ERα-related genes and markers of apoptosis was revealed. Moreover, the flow cytometry analysis result used to examine apoptotic cells proved the apoptosis of SPCs in MCF-7 cells. Conclusions: These findings suggest that our novel SPs, which inhibit coactivator interactions of ERα, induce apoptosis of MCF-7 cells. Thus, considering this strong effect of SPs in the inhibition of receptors, it is pointed out that they can be further developed as an alternative to current clinical treatments or as an auxiliary approach in the generating of new targeted peptide-based therapies.Health Institutes of Turkey (TUSEB) -- 1216

    Effects of circuit exercise program on cognitive function, functional exercise capacity, mobility, depression, and quality of life in women with gestational diabetes

    Objectives: The purpose of this study was to investigate the effects of circuit exercise programs applied in addition to diet therapy on cognitive function, functional exercise capacity, mobility, depression, and QoL in women with GD. Materials and Methods: 43 women with GDM aged 18-35 years old were included and 43 were analyzed. A parallel, single-blind trial with participants allocated (1:1) to one of two groups the Exercise Group and the Diet Group. Personalized diet programs were received by all participants. In the Diet Group, only a diet program was performed. In the Exercise Group, a circuit exercise program was applied to 40-minute sessions, 3 days a week over 6 weeks in addition to the diet program. Participants were assessed at baseline, after 6 weeks for general cognitive function, attention, selective attention, functional exercise capacity, mobility, depression, and quality of life. Results: Significant differences were found in terms of MoCA, Stroop ve SF-36 Physical Functioning in both groups (p<0.05). In Time*Group evaluations, there was a significant difference in the WMS Backward Digit Span in favor of the Diet Group, and in TUG in favor of the Exercise Group (p<0.05). Conclusion: In conclusion, personalized diet programs may enhance cognitive functions, and circuit exercise programs in addition to the personalized diet program may enhance mobility and cognitive functions in women with GD

    Investigating leptin gene variants and methylation status in relation to breastfeeding and preventing obesity

    Objective: We investigated whether the results of leptin gene (LEP) 2548G/A (rs7799039) and leptin receptor gene (LEPR) 668 A/G (rs1137101) variants, as well as the methylation analysis of CpG regions at nucleotides −31 and −51 of the LEP gene, showed any differences between breastfed and non-breastfed children in this study. Materials and Methods: The cross-sectional study included 100 children aged 2–5 years who were attending nursery and kindergarten and had been accepted to the Department of General Paediatrics. Infants who were exclusively breastfed for the first six months after birth constituted the study group, and those who were not only breastfeed constituted the control group. Methylation percentages at CpG islands of the LEP gene were compared between exclusively breastfed and non-exclusively breastfed infants, and the statistical significance was analyzed by looking for changes in LEP −31 and −51 nt methylation and LEP 2548G/A ve LEPR 668 A/G variants. Results: Both groups were compared by feeding, and the association of LEPR and LEP gene polymorphisms and −51 nt and −31 nt methylations were analyzed. There were no significant differences between the groups regarding genotype and allele frequency for the LEPR 668 A/G, LEP 2548 G/A gene variant, −31 nt methylation, and −51 nt methylation status. Similarly, there was no significant difference in genotype and allele frequency for the LEPR 668 A/G gene variant in terms of duration of exclusive breastfeeding, total breastfeeding, body mass index, family obesity, and satiety status. However, maternal support from family elders and physical activity increased the 51 nt methylation, but this methylation was not significantly affected by BMI, age, or satiety status. Conclusions: Maternal support from family elders and physical activity were associated with increased 51 nt methylation, but this methylation was not significantly affected by BMI, age, or satiety status. However, there are not enough studies in this area to reach a definitive conclusion, and further research is needed

    Autologous ovarian tissue transplantation: Preoperative assessment and preparation of the patient

    Importance: Ovarian tissue cryopreservation (OTC) is an innovative and established fertility preservation method. More than 150 live births have been reported worldwide to date with the use of this strategy. OTC is one of the options to preserve fertility in prepubertal girls and for women who have time constraints and/or contraindications for ovarian stimulation for oocyte/embryo freezing before cancer treatment. The success rate of the ovarian tissue transplantation (OTT) depends on many interrelated factors. Therefore, preoperative evaluation and preparation of the candidate patients for the procedure are of paramount importance. Objective: In this review, our aim was to provide a guide for the clinicians, which demonstrates step-by-step assessment and preparation of the patients and ovarian tissue samples for transplantation. Evidence Acquisition: We searched for published articles in the PubMed database containing key words, such as OTT, OTC, preoperative assessment, primordial follicle density, and cancer, in the English-language literature until May 2024. We did not include abstracts or conference proceedings. Results: OTT is still a developing method as an effective fertility preservation approach. It is essential to perform a thorough preoperative evaluation of the patient to improve the success rates of transplantation. Conclusions and Relevance: Preoperative evaluation and preparation of women for ovarian transplantation surgery should include safety management to prevent reimplantation of malignant cells, transplanting ovarian tissue with minimum follicle loss and the decision of the best transfer site

    Evaluation of brain electrical activity of visual working memory with time-frequency analysis

    The term visual working memory (VWM) refers to the temporary storage of visual information. In electrophysiological recordings during the change detection task which relates to VWM, contralateral negative slow activity was detected. It was found to occur during the information is kept in memory and it was called contralateral delay activity. In this study, the characteristics of electroencephalogram frequencies of the contralateral and ipsilateral responses in the retention phase of VWM were evaluated by using time-frequency analysis (discrete wavelet transform [DWT]) in the change detection task. Twenty-six volunteers participated in the study. Event-related brain potentials (ERPs) were examined, and then a time-frequency analysis was performed. A statistically significant difference between contralateral and ipsilateral responses was found in the ERP. DWT showed a statistically significant difference between contralateral and ipsilateral responses in the delta and theta frequency bands range. When volunteers were grouped as either high or low VWM capacity the time-frequency analysis between these groups revealed that high memory capacity groups have a significantly higher negative coefficient in alpha and beta frequency bands. This study showed that during the retention phase delta and theta bands may relate to visual memory retention and alpha and beta bands may reflect individual memory capacity.Istanbul Üniversitesi -- 4920

    Nurses’ perceptions of nursing management: A metaphorical analysis

    Background Nursing management encompasses actions such as planning, organising, commanding, coordinating, and controlling, all of which directly impact staff satisfaction and the quality of care provided in healthcare institutions. Therefore, the experience and qualifications of nurse managers are indispensable. Despite nursing management being taught in nursing schools through both theoretical and practical classes, gaps in the field are still evident, and even conceptualising the process can be challenging. Aim To describe nurses’ perceptions of nursing management through metaphors. Methods This is a descriptive study with a qualitative approach that involved a total of 84 participants (47 staff nurses and 37 nurse managers). Data were collected online in Brazil between February 10, 2024, and March 25, 2024, through Google Forms. The metaphor analysis technique was used. Findings Technical, interpersonal, and conceptual skills, as well as basic management functions, were identified through the metaphors of staff nurses and nurse managers. The categories of metaphors for ‘nurse manager’ are as follows: ‘Leadership and guidance’, ‘Coordination and balance’, ‘Adaptation and flexibility’, ‘Support and empathy’, and ‘Vision and vigilance’. The categories of metaphors for ‘nursing management’ are as follows: ‘Obstacles and strategic planning’, ‘Coordination and harmony’, and ‘Balance and control’. Discussion Nurse managers should act as role models for the other members of their teams. The lack of certain skills in nurse managers and failures in some functions of nursing management indicate that the selection and education of nurse managers deserve special attention. Conclusion Both staff nurses and nurse managers are aware of the important role played by nursing management in the well-being of the team and the quality of care, consequently, in the success of healthcare institutions. Healthcare institutions must be rigorous in choosing their nurse managers. Additionally, educational institutions must pay attention to the theoretical teaching and practical education of nurses to effectively perform nursing management
