Haploinsuficiência genômica decorrente da microdeleção envolvendo 13 genes localizados na região cromossômica 7q11.23 em paciente acometido pela síndrome de Williams-Beuren: relato de caso
Introduction: Williams-Beuren syndrome is a neurodevelopmental disorder that impacts multiple systems. The most frequent cause of Williams-Beuren syndrome is a submicroscopic deletion (0.2Mb to 2.5Mb) at 7q11.23 in the long arm of chromosome 7. The genetic condition hallmarking affected individuals is the genomic haploinsufficiency determined by the hemizygosity of contiguous genes. The genetic diagnosis cannot be done by banding G karyotyping because of the resolution limit (>5Mb) of the classic cytogenetic technique. The segmental rearrangement can be detected by in situ hybridization with DNA specific probes labeled with fluorochromes (FISH) or genotyping highly polymorphic microsatellite loci, and performing segregation analysis of alleles in nuclear families for verification of hemizygosity in the 7q11.23 region. Objectives: To report a case of Williams-Beuren syndrome with late diagnosis, emphasizing the advantages of the precise diagnostic confirmation by genotyping six microsatellite loci specific for the 7q11.23 chromosomal region, using Quantitative Fluorescence Polymerase Chain Reaction (QF-PCR).Methods: Case report and allelic genotyping using QF-PCR for six microsatellite loci at 7q11.23 region. Results: The phenotypic alterations led to the diagnostic hypothesis of Williams-Beuren syndrome. The child´s genotype exhibited loss of the expected heterozygosity in three informative microsatellite loci, which define a physical interval of 0.9 Mb, comprising 13 contiguous genes. The informative maternal alleles absent in the child´s genetic profile were D7SNUDIM3_227, D7SNUDIM6_303, D7SNUDIM11_242. The loss of the expected heterozygosity constitutes evidence of a microdeletion, characteristic in Williams-Beuren syndrome patients. Conclusions: Segregation analysis of parental alleles to the child revealed the presence of a segmental rearrangement involving a microdeletion of about one million base pairs in the chromosome 7 inherited from the mother, resulting in haploinsufficiency of at least 13 genes.Introdução: A síndrome de Williams-Beuren é um distúrbio do neurodesenvolvimento que impacta múltiplos sistemas. A causa mais frequente da síndrome de Williams-Beuren é a microdeleçãosubmicroscópica (0,2Mb a 2,5Mb) na região 7q11.23 no braço longo do cromossomo 7. A condição genética característica em acometidos é a haploinsuficiência genômica determinada pela hemizigose de genes contíguos. O diagnóstico genético não pode ser realizado por cariotipagem banda G devido ao limite de resolução (>5Mb) da técnica de citogenética clássica. O rearranjo segmental pode ser detectado por meio da técnica de hibridação in situ com sondas específicas de DNA marcadas com fluorocromos (FISH) ou por genotipagem de locos de microssatélites altamente polimórficos e a análise de segregação de alelos em núcleos familiares para verificação de hemizigose na região 7q11.23. Objetivos: Relatar um caso de diagnóstico tardio de síndrome de Williams-Beuren, enfatizando as vantagens de confirmação diagnóstica precisa pela genotipagem de seis locos de microssatélites específicos da região 7q11.23, utilizando a Reação Quantitativa Fluorescente em Cadeia da Polimerase (QF-PCR). Métodos: Relato de caso e genotipagem alélica utilizando a QF-PCR para seis locos de microssatélites localizados na região 7q11.23. Resultados: As alterações fenotípicas levaram à hipótese diagnóstica de síndrome de Williams- Beuren. O genótipo da criança apresentou perda da heterozigose esperada em três locos de microssatélites informativos, que definem um intervalofísico de 0,9 Mb, compreendendo 13 genes contíguos. Os alelos maternos informativos ausentes no perfil genético da criança foram D7SNUDIM3_227, D7SNUDIM6_303, D7SNUDIM11_242. A perda da heterozigose é clara evidência da microdeleção, característica em pacientes com síndrome de Williams-Beuren. Conclusões: Análise de segregação dos alelos parentais para o filho revelou a presença de rearranjo segmental envolvendo a microdeleção de cerca de 1 milhão de pares de nucleotídeos no cromossomo 7 herdado da mãe, resultando em haploinsuficiência de no mínimo 13 genes
Linfoma Não-Hodking de Células do Manto: Relato de Caso
Introduction: Lymphomas are one group of malignancies originating in lymphoid tissue and the mantle cell lymphoma is a subtype of non-Hodgkin’s lymphoma (NHL) derived from small “virgin” B lymphocytes of the mantle zone. It corresponds to 6% of all NHL. Objective: To describe a case of non-Hodgkin´s lymphoma mantle cell. Method: Review of medical records. Case Report: J.N.A, 62 years old, male, with a history of painless left cervical lymphadenopathy, followed by the appearance of right and left axillary, bilateral inguinal and supraclavicular limphadenopathies, and painless enlargement of the scrotum. He reported night sweats, followed by asthenia and weight loss. On examination: firm lymph nodes adhered to deep planes, elastic consistency. Abdomen: distended, depressing, painless to superficial and deep palpation, presenting hepatosplenomegaly. Chest radiograph presenting bilateral mediastinal lymphadenopathy. The diagnoses was made by cervical lymph node biopsy: non-Hodgkin’s lymphoma of mantlecell, showing immunohistochemistry with positive CD20 and Cyclin D1. Iliac bone marrow biopsy on the right and left: diffuse infiltration by non-Hodgkin’s small cells, consistent with mantle cell lymphoma, representing 90% of the total cellularity. It was accomplished steroid pulse therapy and after, initiated chemotherapy with CHOP scheme. At this moment, he is in multidisciplinary follow. Conclusion: This case portrays an aggressive lymphoma with disseminated disease (stage IV of Ann Harbor) and prognostic index (IPI) with high scores indicating a worse prognosis. In these cases the treatments have been unsatisfactory, with complete remission achieved by a minority, with median survival of 3 to 5 years.
Introdução: Os linfomas constituem um grupo de neoplasias originárias do tecido linfóide, sendo o linfoma de células do manto um subtipo de Linfoma não-Hodgkin (LNH) derivado dos linfócitos B pequenos “virgens” da zona do manto. Corresponde a 6% de todos os LNH.Objetivo: Descrever um caso de linfoma não-Hodgkin de células do manto.Método: Revisão de prontuário médico.Relato de Caso: J.N.A, 62 anos, sexo masculino, com história de adenomegalias cervicais indolores à esquerda, seguidas pelo aparecimento de adenomegalias axilares à direita e esquerda; inguinais bilaterais; supra-claviculares e aumento indolor da bolsa escrotal. Relata sudorese noturna, acompanhada de astenia e emagrecimento. Ao exame: Linfonodos firmes, aderidos a planos profundos, de consistência elástica. Abdômen: globoso, depressível, indolor à palpação superficial e profunda, apresentando hepatoesplenomegalia. Radiografia de tórax apresentando adenomegalia mediastinal bilateral. O diagnóstico foi realizado por biopsia do linfonodo cervical: linfoma não-Hodgkin de células do manto, apresentando imunohistoquimica com CD20 e Ciclina D1 positivos. Biopsia da medula óssea ilíaca direita e à esquerda: Infiltração difusa por Linfoma não-Hodgkin de pequenas células, compatível com linfoma de células do manto,correspondendo a 90% da celularidade total. Feito pulsoterapia com corticóide e após, inicio da quimioterapia com esquema CHOP. No momento em acompanhamento multidisciplinar.Conclusão: Este caso retrata um linfoma agressivo, com doença disseminada (estágio IV de Ann Harbor) e índice de prognóstico (IPI) com escore elevado, indicando um pior prognóstico. Nestes casos os tratamentos têm sido insatisfatórios, sendo a remissão completa atingida por uma minoria, com sobrevida média entre 3 e 5 anos
Genotipagem do vírus do papiloma humano de alto risco como ferramenta útil em estudos epidemiológicos antes e após a vacinação para prevenção do câncer do colo de útero
Introduction: Persistent infection by some genotypes of the human papillomavirus (HPV) in women is a recognized risk factor for development of intraepithelial cervical neoplasia. Cervical cancer is the fourth cause of death in women in Brazil. Vaccination against HPV is an effective public health measure to prevent cervical cancer. Objective: To develop a molecular test specific for HPV-16, HPV- 18, HPV-31 e HPV-45 types, which together account for 85% of cases of cervical cancer.Methods: Detection of type-specific viral DNA by the technique of quantitative fluorescent polymerase chain multiplex reaction (QFPCR). Clinical samples of vaginal swabs were genotyped from 28 patients, undergoing screening for low and high-grade cervicovaginal lesions, and attending the Ambulatory of Gynecology of the Hospital Escola Álvaro Alvim.Results: A type-specific cervico-vaginal HPV multiplex assay was developed for the simultaneous detection of HPV-16, HPV-18, HPV- 31 e HPV-45.Fifteen (53.6%) samples were positives for at least one HPV high-risk type, HPV-16 being the most prevalent (50%); four samples tested positive for HPV-16 and HPV-31, indicating co-infection. Only one (3.6%) sample tested positive for both HPV- 16 and HPV-18. None of the samples tested positive for HPV-45 type.Conclusions: The multiplex assay developed is a sensitive and rapid test, useful in molecular epidemiology studies before and after vaccination against HPV.Introdução: A infecção persistente por determinados genótipos do vírus do papiloma humano (HPV) em mulheres é um reconhecido fator de risco para o desenvolvimento de neoplasia intra-epitelial cervical. O câncer cervical é a quarta causa de morte em mulheres no Brasil. A vacinação contra o HPV é uma medida efetiva de saúde pública para prevenção de câncer cervical. Objetivo: Desenvolver um teste molecular genótipo-específico para HPV-16, HPV-18, HPV- 31 e HPV-45, que em conjunto são responsáveis por 85% dos casos de câncer cervical. Métodos: Detecção de DNA viral genótipo-específico pela técnica da reação multiplex quantitativa por fluorescência em cadeia dapolimerase (QF-PCR). Foram genotipadas amostras clínicas de esfregaço vaginal de 28 pacientes atendidas no Ambulatório de Ginecologia do Hospital Escola Álvaro Alvim, incluídas pela triagem de lesões cérvico-vaginais de baixo ou de alto grau. Resultados: Foi desenvolvido um ensaio multiplex para a detecção simultânea dos genótipos HPV-16, HPV-18, HPV-31 e HPV-45. Quinze (53,6%) amostras foram positivas para pelo menos umenótipo de HPV de alto risco, sendo o HPV-16 o mais prevalente (50%); quatro (14,3%) amostras foram positivas para o HPV-16 e HPV-31, indicando co-infecção. Apenas uma (3,6%) amostra foi positiva para ambos os genótipos HPV-16 e HPV-18. Nenhuma amostra foi positiva para o genótipo HPV-45. Conclusões: O ensaio multiplex desenvolvido é um teste sensível e rápido, útil em estudos de epidemiologia antes e após a vacinação contra o HPV
TUMOR CARCINÓIDE PULMONAR: RELATO DE CASO
Background: Carcinoid tumors are rare neuroendocrine neoplasms of slow growth. In most reported cases in the literature, they are diagnosed between the fifth and sixth decades of life, manifested byrecurrent pulmonary infections, cough, hemoptysis, or wheezing. It can be classified into typical and atypical and the optimal treatment is surgical resection.Objective: To report a case of Pulmonary Carcinoid Tumor.Methods: Review of medical records.Case Report: Patient, female, 46 years old, asymptomatic, referred to the Department of Thoracic Surgery due to an incidental finding on chest radiograph of a mass located in the middle third of the left lung. Pulmonary lobectomy was performed as surgical treatment, with a satisfactory outcome. The diagnosis of pulmonary carcinoid tumor was confirmed by immunohistochemistry.Conclusion: This report emphasized that the best treatment for pulmonary carcinoid tumors is surgical resection, with an excellent prognosis.Introdução: Os tumores carcinóides são neoplasias neuroendócrinas raras de crescimento lento. A maioria dos casos registrados na literatura é diagnosticada entre a quinta e sexta décadas de vida e se manifesta através de infecções pulmonares recorrentes, tosse, hemoptise ou chiado no peito. Classificam-se em típicos e atípicos e o tratamento ideal é a ressecção cirúrgica.Objetivo: Relatar um caso de Tumor Carcinóide Pulmonar.Método: Revisão de prontuárioRelato de Caso: Paciente, sexo feminino, 46 anos, assintomática, encaminhada ao serviço de Cirurgia Torácica devido a um achado incidental na radiografia de tórax de uma massa localizada em terço médio do pulmão esquerdo. Submetida a lobectomia pulmonar como tratamento cirúrgico, apresentando evolução satisfatória. O diagnóstico de Tumor Carcinóide Típico Pulmonar foi confirmado através da imunohistoquímica.Conclusão: O presente relato enfatizou que o tratamento de escolha para tumores carcinóides típicos pulmonares é a ressecção cirúrgica, apresentando um excelente prognóstico
DOENÇA DO ARMAZENAMENTO DE GLICOGÊNIO TIPO Ib: RELATO DE CASO
Introduction: Glycogen Storage Disease is an autosomal recessive genetic disease, being the incidence of type I with one per 100,000 live births. The initial signs and symptoms usually appear within 28 days of life. The glycogenosis type Ib is associated with liver, kidney and intestine damage. Treatment is primarily nutritional.Objective: To report a case of glycogen storage disease type Ib, emphasizing his clinic, and the importance of early diagnosis for better patient management.Method: Revision of medical records.Case Report: A 3-month-old infant with a history of sister affected by glycogenosis Ib presenting hepatomegaly and remaining physical examination without modification with excepting of growth retardation. Laboratorial examination showed hypoglycemia, hypertriglyceridemia, hyperlactatemia and metabolic acidosis.Conclusion: It is necessary to have high clinical suspicion and that laboratory tests are performed early for confirmation and management, when a case of glycogenosis is suspected. The definitive diagnosis may be done with liver biopsy and genetic study. The treatment is based on nutritional therapy.Introdução: A Doença do Armazenamento de Glicogênio é uma patologia genética autossômica recessiva, sendo o tipo I com incidência de 1/ 100.000 nascidos vivos. Os sintomas e sinais iniciais surgem geralmente nos primeiros 28 dias de vida. A Glicogenose tipo Ib está associada a lesão hepática, renal e intestinal. O tratamento é basicamente nutricional.Objetivo: Relatar um caso de doença do armazenamento de glicogênio tipo Ib, enfatizando sua clínica, e a importância de um diagnóstico precoce para um melhor manejo do paciente.Método: Revisão de prontuário.Relato de Caso: Lactente de 3 meses com história de irmã portadora de glicogenose Ib, apresentando hepatomegalia e restante do exame físico sem alterações, à exceção de retardo no crescimento. Os exames laboratoriais evidenciaram hipoglicemia, hipertrigliceridemia, hiperlactatemia e acidose metabólica.Conclusão: É necessário que se tenha alta suspeita diagnóstica e que exames laboratoriais sejam realizados precocemente para confirmação e conduta apropriada diante de um caso suspeito de glicogenose. O diagnóstico definitivo pode ser feito com biópsia hepática e estudo genético. O tratamento é baseado em terapia nutricional
Instabilidade meiótica da expansão patogênica (CAG)n=38 no gene HTT em caso familiar de doença de Huntington
Introduction: Huntington\u27s disease (HD) is an autosomal dominant neurodegenerative disorder, with progressive loss of striatal neurons, characterized by choreic movements, cognitive deterioration and psychiatric disturbances. HD is caused by the pathogenic expansion of the unstable trinucleotide repeat (CAG)n in exon 1 of the huntingtin HTT gene. The symptoms generally manifest at late age (35-50years), and there is a significant inverse correlation between the threshold number of CAG repeats and the time of onset of symptoms.Objectives: To screen for alleles with expanded (CAG)n repeats in a nuclear family with clinical suspicion of HD, by meiotic segregation analysis, and to assist genetic counseling.Methods: Four adults (father, daughter 1, son and daughter 2) were included in the study, being father and son referred because of clinical findings suggestive of HD and the daughters asymptomatic.The (CAG)n alleles were determined by a specific quantitative fluorescent polymerase chain reaction assay.Results: Genotyping of (CAG)n alleles showed that the father, son and both daughters carried pathogenic alleles with 38, 45, 40 e 41 CAG repeats, respectively.Conclusion: Segregation analysis of (CAG)n alleles revealed paternal carrier transmission of the pathogenic unstable expansion (CAG)n=38 to the son and both daughters. Aiming at early and adequate genetic counseling in the daughters, the genetic test allowed identifying the pathogenic alleles in a rapid and precise manner.Introdução: A doença de Huntington (DH) é um distúrbio autossômico dominante, neurodegenerativo com perda progressiva de neurônios estriatais, caracterizado por movimentos coreiformes, declínio cognitivo e distúrbios psiquiátricos. Ela é causada pela expansão patogênica da repetição trinucleotídica instável (CAG)n no éxon 1 do gene HTT para huntingtina. Os sintomas geralmente se manifestam tardiamente (35-50 anos), havendo uma correlação inversa significante entre o limiar do número de repetições CAG e a idade em que os sintomas se iniciam.Objetivos: Rastrear alelos com expansões da repetição (CAG)n em um núcleo familiar com suspeita de DH, por meio de análise de segregação meiótica e assistir ao aconselhamento genético.Métodos: Foram incluídos na pesquisa quatro indivíduos (pai, filha 1, filho e filha 2), maiores de idade, sendo pai e filho com achados clínicos sugestivos de DH e as duas filhas assintomáticas. Os alelos (CAG)n foram determinados por meio de ensaio específico da reação em cadeia da polimerase quantitativa por fluorescência.Resultados: A genotipagem dos alelos (CAG)n demonstrou que o pai, o filho e as duas filhas eram portadores de alelos patogênicos com 38, 45, 40 e 41 repetições CAG, respectivamente.Conclusão: Análise de segregação dos alelos (CAG)n revelou a transmissão paterna da expansão patogênica instável (CAG)n=38 para os três filhos. Diagnóstico molecular da distrofia muscular Duchenne
Duchenne muscular dystrophy is the most frequent recessive X-linked genetic disease in humans, affecting 1 in 3500 born males. It is caused by mutations in the DMD gene, localized in the Xp21.2- Xp21.1 chromosomal region, which codes for dystrophin, a cytoskeleton protein found in the inner surface of muscle fibers. Pathogenic mutations are heterogeneous in nature and a considerably large number has been described. Approximately 2/3 of index cases are hereditary and the remaining 1/3 is due to de novo mutations. Direct molecular diagnosis of the etiology of the disease involves sequencing of the 79 exons for determination of mutations in affected males. This method is laborious and expensive, imiting its broad application, particularly in tracking mutation in women carriers. The identification of mutation carriers can be done indirectly by linkage analysis of microsatellites. Currently only dinucleotide microsatellites are known in 29 introns; 15o of which are used and they exhibit heterozigosity rates varying from 40 to 84% in various populations. The main drawback of genotyping dinucleotide microsatellites is the occurrence of stutter products, which differ in size by multiples of the repeat unit from the true allele. There is a need to invest in development of tetranucleotide and pentanucleotide microsatellites that result in significant reduction of stutter products and that enables accurate allele designation, diminishing the possibilities of error of diagnosis.A distrofia muscular Duchenne (DMD) é a doença genética com padrão de herança recessiva ligada ao sexo mais frequente em humanos, afetando 1 a cada 3500 meninos nascidos vivos. É causada por mutações no gene DMD, localizado na região cromossômica Xp21.2-Xp21.1, que codifica a distrofina, uma proteína do citoesqueleto encontrada na superfície interna das fibras musculares. As mutações patogênicas são de natureza heterogênea e um grande número tem sido descrito. Aproximadamente 2/3 dos casos são hereditários e 1/3 esporádicos causados por mutações de novo. O diagnóstico molecular direto da etiologia da doença envolve o sequenciamento dos 79 éxons do gene DMD para determinação de mutações em homens afetados. Este método é laborioso e dispendioso, o que limita a sua ampla aplicação, em particular no rastreio de mulheres portadoras. A identificação de portadoras pode ser feita de maneira indireta mediante análise de ligação de microssatélites pelo teste de DNA. Atualmente são conhecidos só microssatélites do tipo dinucleotídeo em 29 íntrons; 15 desses são utilizados e exibem taxas de heterozigose variando de 40 a 84% em diversas populações. O principal problema da genotipagem de microssatélites dinucleotídeos é a ocorrência de produtos stutter, que diferem em tamanho por múltiplos da unidade de repetição do alelo verdadeiro. Há necessidade de investir no desenvolvimento de microssatélites tetranucleotídeo e pentanucleotídeo que resultam em significativa redução da amplificação de produtos stutter e que possibilitam uma melhor designação alélica, diminuindo as possibilidades de erro de diagnóstico
SÍNDROME DE TURNER:RELATO DE CASO
lntroduction: Turner syndromeis a chromosomopathy that affects women, it is distinguished by the partial or complete absence of the X chromosome, accompanied by a combination of characteristic phenotypic features including the presence of short stature, sexual infantiilsm, ovarian dysgenesisand major and minor malformations. Objective: To describe a case of Turner syndrome.Method: Review of medical record.Case report: A 2-year-old girl with recurrent ear infections history and hypertrophy of adenoid,slymphedema of extremities at birth, beyond the following physical characteristics: growth retardation, low posterior hairline, pterigium coli, short neck, micrognathia, inner canthal folds and widely spaced nipples. The diagnosis was confirmed by the quantitative fluorescent polymerase chain reaction. Thereafter the patient was evaluated for the presence of associated cardiac and renal malformations that were excluded. At thistime she was in multidisciplinary monitoring.Conclusion: This report is an opportunity for discussion of clinical manifestations, diagnostic methods, follow up, and therapeutic strategies for Turner syndrome.Introdução: A síndrome de Turner é uma cromossomopatia que acomete o sexo feminino, definida por uma ausência completa ou parcial do segundo cromossomo X, acompanhada por uma combinação de aspectos fenotípicos característicos, que incluem a presença de baixa estatura, infantilismo sexual, disgenesia ovariana e malformações principais e secundárias.Objetivo: Descrever um caso de Síndrome de Turner.Método: Revisão de prontuário médico.Relato de caso: Paciente de 2 anos de idade com história de otites de repetição e hipertrofia de adenóide,slinfedema em membros inferiores ao nascimento e as seguintes características físicas: hipodesenvolvimento pondoestatura,limplantação posterior baixa do cabelo, pterigium coli, pescoço curto, micrognatismo, pregas epicânticas e hipertelorismo mamilar. O diagnóstico foi confirmado pela reação em cadeia da polimerase fluorescente quantitativa. Em seguida a paciente foi avaliada quanto à presença de malformações cardíacas e renais, que foram afastadas. No momento em acompanhamento multidisciplinar.Conclusão: O presente relato ensejou a discussão das manifestações clínicas, métodos diagnósticos , seguimento, bem como das estratégias terapêuticas para a Síndrome de Turner
