Texture analysis with 3.0T MRI for Early Prediction of Response to Neoadjuvant Chemotherapy in Breast Cancer Patients

    No full text
The aim of our study was to investigate whether mathematical modeling with the use of texture features from MRI can predict pathologic complete response (pCR) to neoadjuvant chemotherapy (NAC) in patients with breast cancer. Between January 2012 and August 2017, this retrospective study included 136 patients (mean age, 47.9 yearsrange, 31-70 years) who underwent NAC and subsequent surgery for breast cancer. They were monitored with 3.0T MR imaging before (pretreatment) and after 3 or 4 cycles of NAC (midtreatment). Texture analysis was performed at pre- and midtreatment MRI with T2-weighted, contrast-enhanced T1-weighted, diffusion-weighted image (DWI) and apparent diffusion coefficient (ADC) maps by using a commercial research software. A random forest method was applied to build a predictive model to classify pCR responders with texture parameters. Diagnostic performance for predicting pCR was assessed by using the area under the receiver operating characteristic curve (AUC) was assessed. 40 (29.4%) of 136 patients achieved pCR after NAC. The random forest classifier showed the lowest diagnostic performance in ADC at pretreatment MRI (AUC, 0.5395% CI confidence interval: 0.44, 0.61) and the highest diagnostic performance in contrast-enhanced T1-weighted image at midtreatment MRI (AUC 0.8295% CI confidence interval: 0.74, 0.88) for predicting pCR among pre- and midtreament MRI with T2-weighted image (median AUC range: 0.53, 0.67) , contrast enhanced T1-weighted image (0.57, 0.82), DWI (0.57, 0.73) and ADC maps (0.53, 0.69). The prediction modeling using texture parameters of contrast-enhanced T1-weighted image at midtreatment MRI was valuable in the early prediction of pCR to NAC in patients with breast cancer.Docto

The prevalence and incidence epidemiology of pressure ulcer is still not clear in Korea. Hence, we aimed to determine the incidence, prevalence, mortality, and frequent comorbid conditions of pressure ulcer in the Korean population. Materials and Methods Using the health care insurance reimbursement claims from the South Korean National Health Insurance System (NHIS) database, all Korean patients in hospitals initially or additionally diagnosed with pressure ulcer [ICD-10: L89 + (dressing and/or flap operation)] between January 2002 and December 2016 were evaluated with a 4-year washout period (2002-2005). Results Incidence, prevalence, and survival rate of patients with pressure ulcers and risk factors related to survival rate were determined. The study population in 2006 was 36,195,121, but it changed yearly because of new entries of 20-year-old individuals and mortality cases. Standardized incidence rate decreased from 17.1 in 2006 to 14.9 in 2015 for every 10,000 people. Standardized prevalence also showed a slightly decreasing trend from 20.2 in 2006 to 18.9 in 2015 for every 10,000 people, except in 2007. Conclusions Incidence and prevalence were markedly increased with age after the seventh decade (above 60th). Pressure ulcer incidence was high in patients with pneumonia, cerebral infarction, sepsis, femur neck fracture, and malignant neoplasm of the bronchus. Among these diseases, patients with femur neck fracture and cerebral infarction showed higher survival than those with other high-risk comorbid conditions. Therefore, in these cases, a preventive and curative approach such as flap operation should emphasized, if the acute critical phase has passed.Maste

Among them, test case generation using model checker is one of the promising method. However, since a model checker is a tool for ensuring the quality of the program by examining all possible executions of a system, it can cause a state space explosion problem. In addition, there is a disadvantage of generating redundant test cases. Therefore, when deriving test cases using model checker, the method to limit the state space explosion and optimize test cases should be considered. This paper proposes a weight-based state space search method and a test suite optimization technique based on an execution path for test suite generation using a model checker and search-based algorithm. A weight-based state space search can solve the state space explosion problem by weighting the transition which is hard to pass through and selecting the space to search using adaptive random algorithm. The optimization technique distinguishes execution paths from the test suite based on the input parameters and deletes the path which does not ensure coverage using a greedy algorithm. The proposed method derives an optimized test suite which contains feasible test cases.Maste

Artificial intelligence technology is applied to various fields such as autonomous driving, healthcare, voice recognition and translation. In order to cope with this situation, an electronic system capable of high-speed and large capacity of information processing is required. Currently, the semiconductor technology has been improving the performance of the electronic system through the miniaturization of electronic devices, but due to the difficulties of manufacturing process caused by physical limitations, a new solution is necessary today. Stochastic computing is a one of digital computing methods that use probabilistic bit-stream as basic unit of data. Compared with existing digital computing method using floating point, stochastic computing has advantages of error-tolerance and ability to perform complex arithmetic operations at low cost used in artificial intelligence operation. However, a large number of electronic devices are still required for hardware implementation of stochastic computing, especially in the process of converting data to probabilistic bit-stream, which is still a fundamental challenge. Neuromorphic architecture mimicking a biological neural network, also called spiking ix neural network, has been believed to solve the problems in current computing system based on the von Neumann architecture by reducing power consumption and area. Since the spike signal used as basic unit of data in neuromorphic architecture is analog but one spike signal is digital (spike 1 and no spike 0), the spike train can be expressed as probabilistic bit-stream. Therefore, it is a suitable approach to implement the stochastic computing with neuromorphic architecture for the realization of electronic systems for efficient information processing. In this thesis, we constructed stochastic computing system with neuromorphic architecture using magnetic tunnel junction (MTJ) which is a non-volatile spintronic device. First, the magnetization switching characteristics of MTJ (tunneling magnetoresistance, coercivity, switching current for spin-transfer torque) are evaluated. Tunable telegraphic switching characteristics are investigated to generate probabilistic bit-stream and a device macro-model is created based on the electrical measurement data. Furthermore, we propose practical circuit design to use the bit-stream signal in stochastic computing system. (Ch. 2) Next, as an application of the bit-stream signal, stochastic computing system is presented for pattern recognition of MNIST data set. By using probabilistic bit-stream signal generated from telegraphic switching of MTJ, the recognition rate for MNIST digit pattern in the stochastic computing system composed of AND gates and multiplexers is 89.1% which is close with existing software-based system (89.7%). (Ch. 3) A spiking neural network is constructed with artificial neuron generating rate-coded spike signal based on MTJ bit-stream generator and synaptic device using memeristive switching characteristics of MgO barrier in MTJ. The pattern recognition of MNIST data set is carried in the spiking neural network and the accuracy is improved to 89.4% compared with the proposed stochastic computing system. We also investigated a performance of the spiking x neural network for practical applications under input noise, digitalized synaptic weight, synaptic device conductance variation and present practical circuit for operating the proposed hardware spiking neural network. (Ch.4) This result indicates that the MTJ bit-stream generator studied in this thesis can be used for stochastic computing system as well as artificial neuron in neuromorphic architecture for pattern recognition, which shows the high performance closed with conventional software method. It is expected that this study helps stochastic computing and neuromorphic architecture researches for artificial intelligence technology.Docto

Americans and Europeans were allies for decades. This traditional alliance which was conceived on the eve of the Cold War, is being threatened by the Trump administrations trade war. By withdrawing from numerous international agreements and imposing new bilateral agreements, the Trump administration has not only increased tensions among its rivals, but also among its partners. One could ask whether it is in the best interest of the U.S. to act more aggressively towards the EU, when it should seek a strong partnership to face China. I argue in this thesis that the current relationship cannot be fully explained by Trumps vision but rather, reflects a shift from geo-politics to geo-economics. Attacking allies reveals the inability for the U.S. to protect an international order they created, and collaterally impulse their disengagement from a multilateral world where the U.S. interest, which tends to be more economic, are not anymore advantaged. Hence, the hegemonic struggle with the economic threat of China and the political economic framework has shaped its relationship with the EUMaste

The rapid emergence of China is changing the geopolitical circumstances of international society. Along with its enhanced economic status, China is strengthening its military power through enormous military expenditure and its expansion of military power with a focus on the navy, raising nerves among neighbouring countries as well as the US, the hegemon. So far, Europe and the US have been the axis of change in international society as the center of the world. During the cold war, there was a balance of power between the US and Russia. However, after the end of the cold war, there has been a single hegemony led by the US. But as China has rapidly emerged over the last 20 or more years to become the largest exporting country, the hegemony led by the US is being challenged. China's challenge has formed a de facto dual power system and is changing the center of the world from the west to asia. China, recognized at least as the most powerful country in the region, is sharing the continental shelf and Exclusive Economic Zone with the Philippines, Vietnam, Malaysia, Brunei, and Taiwan. The south china sea, surrounded by China and its neighboring countries, is an important area as a sea line of communications, in addition to its abundant fishery stocks and natural resources. China is building a naval base in the south china sea via policies of 'The String of Pearls' and expanding its maritime dominion in the south china sea in accordance with the 'One Belt, One Road' strategy. The expansion of chinese influence in the ocean has caused disputes with neighboring countries, while its defeat by the Philippines at the ITLOS (International Tribunal for the Law of the Sea), where it claimed its maritime dominion based on a U-shaped Line, reinforced the importance of international norms. Meanwhile, the Philippines and Vietnam, which were under cooperation or an alliance with the US during the course of the maritime dominion disputes, are holding China in check, utilizing the US. These countries are dealing with the disputes directly with China as well as trying to solve the maritime dominion issues by enhancing cooperation with the US at the same time. The US is enhancing cooperation not for the interests of other countries, but for the interests of the US and to reinforce its hegemony in the East Asian region. China, which is rapidly emerging, is a country, along with Russia, that needs a close watch in terms of both economic and military regards, as a potentially hostile power that may threaten the US. Mutual distrust between the US and China is providing both countries' expansion of military spending and the enhancement of military power. The US, hoping for a status quo, is pressing China via Freedom of Navigation Operational assertions and China wants a status change by acquiring maritime power with its reinforced naval force and building artificial islands. The dispute of maritime dominion in the South China Sea is expanding into a hegemonic competition between the US and China. The conflict in the south china sea between the US and China needs to be considered from a range of perspectives. Korea should contemplate the differences and common grounds it has with China and, being semi-enclosed, reinforce its naval power to secure the safety of its sea lines of communications. It needs to focus more on the economic value that functions as core national interests and to make efforts and preparations in the areas of international law to cope with dominion issues. Also, as the maritime hegemonic competition between the US and China intensifies, there is a need for seeking compromise and cooperation for national survival. It should be noted that there is always a possibility of the Korean peninsula falling into a warfield, along with the intensifying strategic competition between the US, the maritime power, and China, the continental power. However, the possibility of a military collision between the US and China is very low and even if it happens, it is very unlikely that the collision would develop into an international war like a third world war. Lastly, Korea needs to reinforce its diplomatic efforts via enhancing alliances and cooperation with the US, Japan, etc. and multilateral security cooperation organizations.Docto
