Adult Cardiac Surgery During Covid-19 Pandemic
Coronavirus Disease 2019 (COVID-19), which is caused by the SARS Coronavirus 2 (SARS-CoV-2), has affected over 200 countries worldwide. First case of COVID-19 wasfound in Wuhan, China, precisely around December 2019. COVID-19, especially in thosewith underlying health conditions or comorbidities, has an increasingly rapid and severeprogression, often leading to death. This virus is a single-strand RNA coronavirus, binding the angiotensin-converting enzyme 2 (ACE2) receptor which enters human cells. Coronavirusdisease has been reported to affect cardiac and vascular organs. cardiomyocyte death andinflammation are results of a direct mechanism that involves viral infiltration into myocardialtissue. Some cardiovascular manifestations of myocardial injuries associated with COVID-19are arrhythmias, myocardial infarction, heart failure, and elevated biomarkers (cardiactroponin I and brain-type natriuretic peptide). Some of this manifestation requires immediate intervention or surgery. Criteria are needed for hospitals or institutions that mostly maintain cardiac surgery services and surgery-urgent patients depending on severity of the disease and hospital resources. These criteria also limit the risk of exposure to patients and healthcare workers and allocate resources appropriately to those in greatest need. This paper aims to share our discussion and give an overview of patients undergoing cardiac surgery, providing clinicians with recommendations to triage and plan these procedures during the COVID-19 outbreak effectively
The Comparison Of Antibacterial Effects On Cinnamomum Burmannii Water Extract With Penicillin Against Staphylococcus Aureus In Vitro
Introduction: Along with the development of science, many varieties of microorganisms are resistant to antibiotics. To avoid cases of resistance grew severe, researchers developed alternative medicines as a substitute for antibiotics. One of the ingredients of herbs researched that has antibacterial activity is Cinnamon (Cinnamomum burmannii).
Aim: This study aimed to compare antibacterial effects water extract of Cinnamomum burmannii with penicillin against Staphylococcus aureus
Methods: This an experimental studies witd non-equivalent control group design. Research using the bacteria S. aureus are divided into two groups, the control group and experimental group. The concentration of Cinnamomum burmannii and penicillin are used that is 80- 150mg/L. Antibacterial test used dilution method and a microplate reader for reading absorbance. This research conducted in the laboratory of Microbiology, Faculty of Pharmacy Widya Mandala Catholic University Surabaya for 2 weeks.
Result: Based on visual observations, Cinnamomum burmannii’s MIC is located at concentration 320- 600 mg/L while penicillin’s MIC is located at concentration 160- 300 mg/L. Based on the results of microdilution test, Cinnamomum burmannii’s MIC is located at concentration 160- 300 mg/L while penicillin’s MIC is located at concentration 640- 1200 mg/L.
Conclusion: The conclusion of this research is giving Cinnamomum burmannii water extract in Staphylococcus aureus has efficacy or inhibitory that is not significantly different with penicillin
Insomnia Levels In Elderly At Griya Santo Yosef And Panti Surya Before And After The Lavender Aromatherapy Treatment
Introduction: The process of aging that occurs in elders will decrease their physiology functions, such as their lack of sleep, both timewise and quality-wise. This situation will cause sleep problems in elders, such as insomnia. Insomnia could be treated by using both pharmacology and non-pharmacology. Lavender aromatherapy treatment is one of the nonpharmacology relaxation therapies that could help manage insomnia in older adults. Aim: Knowing the difference of insomnia levels before and after the lavender aromatherapy treatment in elders. Methods: This examination is using Quasi Experiment of the treatment group and the
control group. Furthermore, the idea of this experiment is to use a pre-test and post-test control group design approach. For sample earnings, consecutive sampling technique is used by picking a sample that is qualified for inclusion criteria, and that is not included in the exclusion criteria. Thus, the minimum sample could be fulfilled. The statistics test is using a Chi-Square test. Result: The result of insomnia level before and after aromatherapy treatment using the ChiSquare analysis (p=0,000) shows
that there isa difference that is due to the intervention. In the treatment group, there were 14 participants with mild insomnia and seven participants with severe insomnia. The result shows that 14 participants did not have insomnia, and seven other participants have mild insomnia. Meanwhile, the control group shows no difference in their insomnia, but decreasing in their score. Conclusion: Elders with lavender aromatherapy treatment for one week have shown declining insomnia level from severe to mild, and for the mild level of insomnia problems become cured, However, those who were given placebo shows no difference
Effectiveness Between Mango Honey and Kesambi Honey As Antibacterial Against Streptococcus Pyogenes
Introduction: Infectious disease is one of the biggest health problems in developing countries, including Indonesia. The development of the discovery of antibiotics was not balanced with the development of resistance that occurred after that Group A streptococcus β hemolytic causes various events of a severe degree of infection. Now many people use natural ingredients as an alternative to antibiotics, one of which is honey. Aim: Knowing the benefits of mango honey and kesambi honey as an antibacterial against Streptococcus pyogenes and knowing the difference in the effectiveness and potency to inhibit growth and kill Streptococcus pyogenes between the two honey. Method: Using the macro dilution method in determining the value of MIC and the streaking method to determine the value of MBC. Results: MIC mango honey at 15.625 µL, MIC kesambi honey at 15.625 µL, MBC mango honey at 15.625 µL, and MBC kesambi honey at 31.25 µL. Conclusion: There is a difference of MBC between mango honey and kesambi honey against Streptococcus pyogenes where mango honey at 15.625 µL and kesambi honey at 31.25 µL, there is no difference in MIC between mango honey and kesambi honey, and the effectiveness and potency of mango honey as an antibacterial against Sp higher than kesambi honey which was observed through differences in the value of MBC
A Systematic Overview Of Abdominal Aortic Aneurysm (AAA)
An abdominal aortic aneurysm (AAA) is defined as an abnormal dilatation of the abdominal aorta more than 50% of its diameter. The aortic wall continues to weaken and become unable to hold the forces of the luminal blood pressure, resulting in progressive dilatation and rupture. The rupture of the aneurysm wall is influenced by several factors, such as the aneurysm size, expansion rate, continued smoking, and persistent hypertension. The AAA is a common degenerative condition with high mortality in older people. The AAA increases these last two decades. It is occurring in 7-8% of men at the age of over 65 years old. The disease prevalence is six times higher in men than in women. It is also increasing dramatically by the presence of the following factors: age older than 60 years old, smoking, hypertension, and ethnicity. Repair of large or symptomatic AAA by open surgery or endovascular is recommended, but not significant in the small aneurysm
Blood Sugar Levels With Neutrophil-Lymphocyte Ratio As a Marker of Diabetes Mellitus In Elderly
Introduction: Diabetes mellitus is a chronic metabolic disease, characterized by high blood sugar levels due to abnormal insulin secretion by the pancreas, insulin action, or both. Based on the Basic Health Research results in 2018, the elderly group dominates patients with diabetes mellitus. The diagnosis of diabetes mellitus requires an expensive examination that is not available in all laboratories, especially primary care facilities. Some research suggested that the neutrophil-lymphocyte ratio can be an alternative marker for establishing a diagnosis of diabetes mellitus. Purpose: To determine the correlation between blood sugar levels and lymphocyte ratio as a marker of diabetes mellitus in the elderly. Method: This study was an observational analytic study with a cross-sectional approach. Data was collected by conducting interviews, physical examinations, and laboratory examinations. Hypothesis testing in this study used the Spearman correlation test. Results: A total of 53 respondents were included in this study, with a proportion of 32 female subjects (60.4%). The mean age of respondents was 76.0 ± 7.29 years. An insignificant correlation was obtained between fasting blood sugar levels and the ratio of neutrophil lymphocytes in the elderly (p = 0.161; r = 0.195). An insignificant correlation was found between blood glucose levels after two hours of postprandial with the neutrophil-lymphocyte ratio in the elderly (p = 0.595; r = 0.075). HbA1c level and the lymphocyte ratio in the elderly also showed a non-significant correlation (p = 0.873; r = -0.023) Conclusion: There is no significant correlation between blood sugar level and neutrophillymphocyte ratio as a marker of diabetes mellitus in the elderly
Mother's Knowledge Level About Growth And Development With The Growing Development In Infants Aged 6-12 Months
Introduction: The age of an infant is an essential age for growth. Its development is highspeed, follows a particular pattern, and varies between individual. An infant is still vulnerable to various health problems. The infant's development will be optimal if the mother's knowledge level is right and appropriate social interaction with the needs of the baby at its development stage. Purpose: This study aims to analyze the relationship between mother's knowledge levels about growth development with the compatibility of growing development in infants aged 612 months. Method: This study used an observational analytic method with a cross-sectional design. Statistical analysis used the Chi-Square test to determine the correlation followed by a contingency coefficient. Samples were taken according to the criteria of age 6-12 months as much as 55 respondents. data on the mother's knowledge level was collected using a questionnaire. The research evaluate growth using a baby scale of height, weight, head circumference, and to evaluate development progress using a prescreening developmental questionnaire (KPSP). This study was conducted on July 5 - August 1, 2019, at the Gotong Royong Pratama Clinic in Surabaya. Results: The relationship of mother's knowledge level about growth development with the compatibility of growing development in infants aged 6-12 months obtained significant results with a p-value of p = 0,000 (p <0.05). Contingency coefficient test results with a value of 0.436 which shows that the strength of the relationship is strong enough. the relationshipbetween mother's knowledge level about growing development with the compatibility ofgrowing development in infants aged 6-12 months obtained significant results with a p-valueof p = 0.007 (p <0.05 ). Contingency coefficient test results with a value of 0,343, whichshows that the strength of the relationship is strong enough. Conclusion: There is a significant relationship between the mother's knowledge levels about growing development with the compatibility of growing development in infants aged 6-12 month
Thoracic Surgery Preparation, Patient Election, and Its Management During Covid-19 Pandemic: Surabaya Experiences
A novel coronavirus pneumonia outbreak began in Wuhan, Hubei Province, in December 2019. It has spread rapidly from China to worldwide. The COVID-19 pandemic of 2020 posed a historic challenge to health care systems around the world, healthcare systems needed to consider providing clinical services to other patients in need. The specialty of general thoracic surgery includes the management of serious diseases such as chest wall congenital anomaly, lung cancers, esophageal cancers, mediastinal tumor, chest wall infection, and trauma. Thoracic surgery is one of high risk procedure in this pandemic, however, a high level of evidence supports the surgical management of potential patients with thoracic disease and anomaly is still lacking. Critical determinants of robust thoracic surgery service provision are pre-existing plans for epidemic response. "Flatten the curve" as an aggressive action is needed. Prioritizations of thoracic surgery cases are needed to differ between elective and emergency cases to limit any contamination. Before surgery, important pre-operative assessments were conducted aims to identify patients' high risk and adjust the procedure. example of a recommendation, if urgent / emergency surgery with symptoms clear clinical pneumonia or rapid test (+). The lessons learned can apply to the other areas during this pandemic, and the world, in preparation for the next one
